Mortality and risk factors associated with death in patients with obesity hypoventilation syndrome and of predominantly Afro-Caribbean origin in Martinique.

BACKGROUND: Obesity hypoventilation syndrome (OHS) is a respiratory complication of obesity. Mortality from OHS is non-negligible. Risk factors for OHS-related mortality are not well established and vary in the literature. This study aimed to assess mortality from OHS in patients in Martinique and to identify risk factors for death. METHODS: We performed a single-centre, observational study from 1 January 2019 to 31 December 2024, including patients with OHS. RESULTS: Among 167 included patients, 44 (26.4%) died as of 31 December 2024. 5-year mortality was 12.6%.In multivariable Cox regressi

Placental morphometric and hemodynamic parameters as predictors of early neonatal adaptation in dogs.

BACKGROUND: Placental dysfunctions are associated with fetal restriction growth and unfavorable neonatal outcome, with reduced uteroplacental blood flow being one of the most frequent causes of placental insufficiency. This study compared fetoplacental Doppler ultrasonography throughout gestation and placental morphology of neonates presenting either high or low vitality at birth. METHODS: Puppies born by elective c-section were assigned to: High Vitality Group (vitality score ≥ 7, n = 10 and 7 bitches) and Low Vitality Group (<7, n = 12 and 8 bitches). Pregnant bitches were retrospectively e

False Passage During Single-Lung Ventilation in a Tracheostomized Patient: Intraoperative Recognition and Rescue.

BACKGROUND Airway management in tracheostomized patients may become particularly challenging in the presence of recent stomas, local infection, single-lung ventilation, and restricted intraoperative airway access. False passage ventilation is an uncommon but potentially catastrophic complication requiring immediate recognition and rescue. CASE REPORT We report the case of a 63-year-old man with a recent surgical tracheostomy performed after emergent cricothyrotomy for upper airway obstruction secondary to cervical cellulitis with mediastinal extension. The patient underwent urgent thoracotomy

Understanding emotional and practical challenges of initiating oxygen therapy in pulmonary fibrosis: insights from a patient-centered survey.

BACKGROUND: Pulmonary fibrosis (PF) causes respiratory insufficiency due to progressive lung scarring and impaired gas exchange. Although no curative treatment exists, pharmacological therapies may slow progression, while oxygen therapy alleviates hypoxemia, improves functional capacity, and enhances well-being. This study explored psychosocial and practical barriers to oxygen initiation, and their prevalence, among patients with PF. METHODS : We conducted a cross-sectional mixed-method survey in 2024, following the Consensus-Based Checklist for Reporting of Survey Studies (CROSS), co-develope

Plasma Exchange for Hantavirus Pulmonary Syndrome: A Mechanistic Case for an Untested Intervention.

Hantavirus pulmonary syndrome (HPS) carries a case fatality rate of 35%-40% and has no approved pharmacologic treatment. Its pathophysiology is rooted in the plasma compartment: cytokines, viral antigens, immune complexes, and platelet-binding glycoproteins circulate as drivers of immunopathologic endothelial injury, noncardiogenic pulmonary edema, and cardiogenic depression. Therapeutic plasma exchange (TPE) achieves bulk clearance of plasma-borne mediators and is established by the American Society for Apheresis (ASFA) for numerous indications that share an analogous mechanistic rationale. Utilization and Feasibility of a Wearable Device in Patients With Sedative Effects of Drugs: Protocol for a Prospective Observational Study for the Advanced Respiratory Monitoring Events in Drug Toxicity (ARM-ED) Study.

BACKGROUND: Drug-related deaths worldwide are most commonly attributed to opioids. Opioids and other sedative drugs can cause respiratory depression and airway compromise, leading to hypoxia and death. Device technology and artificial intelligence used to detect drug overdose has the potential to improve outcomes. PneumoWave Ltd has developed a small chest-worn respiratory monitoring device to detect concerning breathing patterns and alert an emergency response. OBJECTIVE: The aim of this study is to investigate the feasibility of using the PneumoWave device in hospital patients at risk of re

Potentiation of fentanyl-induced respiratory depression by alcohol is not fully reversed by naloxone.

The high frequency of opioid overdose deaths often involves co-use of alcohol, which is reported in approximately 30% of fentanyl fatalities. Both substances depress respiratory function, and their combined effects can be lethal. The present study investigated physiological parameters of respiratory-depressant effects of fentanyl when coadministered with alcohol and their sensitivity to naloxone reversal using whole-body plethysmography in male and female Long-Evans rats. Administration of a high, sedative-like dose of alcohol alone or fentanyl alone resulted in no mortality, but fentanyl plus

Prediction of Long-Term Mortality in Acute Hypercapnic Respiratory Failure with Use of Low-Flow Veno-Venous Extracorporeal CO2 Removal: A Retrospective Single-Center Study.

UNLABELLED: <p>Introduction: Hypercapnic respiratory failure is associated with high morbidity and mortality. Low-flow extracorporeal CO2 removal (ECCO2R) has been shown to facilitate lung-protective ventilation or spontaneous breathing. However, three multicenter randomized trials have failed to show benefit which could potentially be a result of patient selection. In this study, we aimed to characterize prognostic scores developed for extracorporeal membrane oxygenation therapy which could potentially assist with the selection of patients for ECCO2R. METHODS: A total of 70 patients admitted

Effectiveness of high-flow nasal cannula in the management of acute hypercapnic respiratory failure: A meta-analysis of randomized controlled trials.

BACKGROUND: Acute hypercapnic respiratory failure (AHRF) commonly complicates chronic respiratory diseases and is traditionally managed with noninvasive ventilation (NIV), although tolerance and interface-related limitations may reduce adherence. High-flow nasal cannula (HFNC) has been proposed as an alternative, but comparative efficacy in AHRF remains uncertain. METHODS: We conducted a systematic review and meta-analysis in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. PubMed, Cochrane Library, Web of Science, and Embase were searched from inception

The role of capnography in improving respiratory safety during pediatric endoscopic procedures: An observational study.

Endoscopic procedures in pediatric patients are increasingly performed under sedation in non-operating room anesthesia settings, where early detection of respiratory compromise is essential. While capnography is recommended for identifying hypoventilation, its clinical impact remains debated. This study aimed to evaluate whether end-tidal carbon dioxide (ETCO2) monitoring reduces the incidence of hypoxia during sedated pediatric endoscopic procedures. This observational study included 135 children aged 1 to 17 years undergoing elective upper gastrointestinal endoscopy or combined endoscopy and

Intranasal Naloxone Reversal of Opioid-induced Respiratory Depression in Opioid-naive Individuals and Self-reported Daily Opioid Users.

BACKGROUND: Since current opioid overdose deaths occur mainly from potent synthetic opioids with high affinity for the opioid receptor, such as fentanyl and carfentanil, it is important to determine the efficacy of naloxone, particularly the intranasal formulation, in reversing opioid-induced respiratory depression. This study evaluated effectiveness of 4 mg intranasal naloxone (Narcan; Adapt Pharma Inc., USA) in reversing moderate respiratory depression induced by fentanyl and sufentanil in opioid-naive individuals and self-reported daily opioid users. Sufentanil was compared to fentanyl beca

[Clinical scale of ventilatory failure risk in patients with amyotrophic lateral sclerosis].

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease that causes atrophy and paralysis of skeletal muscles, including respiratory muscles. The development of ventilatory failure determines the prognosis. The primary outcome was to determine which common clinical variables can be predictors of daytime hypercapnia and develop a risk model of ventilatory failure. Secondary outcome was to determinate the survival rate of high-risk patients with and without hypercapnia. MATERIALS AND METHODS: Retrospective study. Patients with ALS without mechanical ventilat
