Adaptive support ventilation with IntelliSync versus pressure support ventilation for non-invasive ventilation in acute exacerbation of COPD: A feasibility randomized trial.

BACKGROUND: While non-invasive ventilation (NIV) is recommended for managing acute exacerbations of chronic obstructive pulmonary disease (AECOPD), it has a 30-40% failure rate when delivered using pressure support ventilation (PSV). Adaptive support ventilation with IntelliSync (ASVi) synchronizes breath initiation and cycling to match patient's requirements. We conducted a feasibility study to inform design of a larger trial and compared NIV failure rates using PSV or ASVi in patients with AECOPD. MATERIALS AND METHODS: We conducted a single-center, randomized controlled trial from December

High-flow nasal cannula versus noninvasive ventilation in patients with hypoxemic respiratory failure: a prospective cohort study.

High-flow nasal cannula (HFNC) and non-invasive ventilation(NIV) are commonly used for hypoxemic respiratory failure, but their comparative efficacy remains unclear. This prospective cohort study enrolled 259 non-hypercapnic hypoxemic respiratory failure patients (PaO₂ <60 mmHg, PaCO₂ <50 mmHg, pH ≥ 7.30) in a Chinese ICU. Patients were allocated to HFNC (n = 128) or NIV (n = 131) based on physician judgment. Primary outcome was 28-day intubation rate; secondary outcomes included 28-day mortality, treatment withdrawal, and ICU/hospital stay duration. Baseline characteristics were similar excep

Muscle transcriptome profiling reveals novel molecular pathways and biomarkers in laminin-α2 deficient patients.

Merosin-deficient congenital muscular dystrophy (LAMA2-RD) is a neuromuscular disorder caused by mutations in the LAMA2 gene, coding for the α2 subunit of laminin-211 (merosin). LAMA2 mutations leading to complete laminin-211 absence result in a severe clinical phenotype with profound muscle weakness and respiratory insufficiency, whereas mutations allowing the production of a partially functional protein are often associated with milder phenotypes. While several dysregulated pathways linked to LAMA2-RD have been reported, an in-depth characterization of muscle gene expression in patients with

How to Interpret Ventilator Waveforms Using the Taxonomy for Modes of Mechanical Ventilation.

Mechanical ventilation is a complex yet essential aspect of critical care. This article provides a brief review of historical developments related to descriptions of ventilators, the derivation of the foundational equation of motion for the respiratory system and its derivatives, and a short overview of the taxonomy for modes of ventilation. These topics are then applied in a step-by-step procedure for interpreting patient-ventilator interactions through waveform analysis.

Successful Bilateral-Lung Transplantation With Long Survival Time for Multicentric Castleman Disease With Fatal Pulmonary Involvement.

We present a rare case of multicentric Castleman disease (MCD) complicated by severe respiratory failure, which was successfully managed through bilateral lung transplantation. A 31-year-old male patient exhibited progressive dyspnea attributed to MCD associated with bronchiolitis obliterans. Pretransplant evaluations indicated severe hypercapnia and markedly impaired pulmonary function. Despite the administration of steroid therapy, the patient's respiratory status continued to decline. Consequently, he underwent bilateral lung transplantation, which resulted in significant alleviation of hyp

Outcomes and outcome measures in studies of pulmonary alveolar proteinosis: a scoping review.

BACKGROUND: Pulmonary alveolar proteinosis (PAP) is a rare lung disease characterised by progressive accumulation of surfactant-derived lipoproteinaceous material within alveoli, impairing gas exchange and causing respiratory insufficiency. Despite therapeutic advances such as whole lung lavage, substantial heterogeneity persists in outcome selection and reporting across studies, limiting comparability and evidence synthesis. OBJECTIVE: To systematically map and categorise outcomes and outcome measures reported in studies of PAP treatments and establish a foundation for developing a standardi

The diagnostic yield of a structured comorbidity workup in a real-life outpatient population with obstructive sleep apnea: a cross-sectional study.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with multiple systemic complications. However, evidence-based recommendations for comorbidity screening remain sparse. This study aimed to evaluate whether a structured comorbidity workup could yield clinically meaningful diagnostic gains in OSA patients. METHODS: In this single-center cross-sectional observational study, patients diagnosed with OSA underwent a standardized comorbidity assessment including laboratory tests, electrocardiogram, echocardiography, abdominal ultrasound, ambulatory blood pressure monitoring, spirometry, and va

Airway clearance techniques - a proposed classification based on definitions?

INTRODUCTION: Airway clearance techniques (ACT) are a longstanding modality of treatment in many chronic respiratory diseases. Their aims are to counteract airway obstruction by mobilizing airway secretions and/or by limiting the airway collapse. AREAS COVERED: This review first outlines the physiological mechanisms underlying ACT. In a second part, it categorizes the ACT into seven distinct groups. These groups include both instrumental and non-instrumental ACT. Strategies such as physical activity and upper airway clearance techniques are also considered as ACT due to their complementary ro

Efficacy of positive expiratory pressure in the prevention and treatment of postoperative pulmonary complications following thoracic and abdominal surgery. A systematic review and meta-analysis.

Postoperative pulmonary complications (PPCs), including atelectasis, pneumonia, and respiratory failure, are common after thoracic and upper abdominal surgery and are associated with increased morbidity, longer hospital stays, and higher costs. This systematic review and meta-analysis investigated whether positive expiratory pressure (PEP) devices reduce PPCs after thoracic or upper abdominal surgery compared with continuous positive airway pressure (CPAP), usual care/no intervention, or other non-CPAP respiratory treatments. We searched major databases and included randomized controlled trial

Harmonizing patient-reported outcome measures for nasal complaints using traditional and machine learning methods.

BACKGROUND: Nasal obstruction measurement instruments are widely used in the field of nasal surgery. There are various scales that measure nasal obstruction and they differ regarding the number of items, their wording, and the type of response options. In order to pool the data and analyze it together, it is necessary to harmonize it so that we can compare participants' nasal obstruction scores irrespective of instrument they filled in. Data harmonization is still not used in the field of nasal obstruction assessment. THE AIM: The aim of this study was to find the best harmonization method in

Hypercapnia at admission, regardless of acidosis, may worsen the outcome of hospitalised patients with chronic obstructive pulmonary disease exacerbations.

In patients with exacerbation of chronic obstructive pulmonary disease (ECOPD), the presence of respiratory acidosis is considered "a marker of severity", identifying the need for ventilation treatment. The evaluation of ECOPD patients with hypercapnia, even without acidosis, may provide information on patients not commonly defined as "at risk". We retrospectively assessed 407 hospitalised patients with ECOPD, divided into three groups at admission: patients with normocapnia (N = 176), hypercapnia (PaCO2 > 45 mmHg and pH ≥ 7.35, N = 126), and acidosis (PaCO2 > 45 mmHg and pH < 7.35, N = 105).

Consensus on identifying and ranking ventilator asynchronies in invasively ventilated ICU patients: a modified Delphi study (SYNAPsE).

PURPOSE: Despite extensive research, it remains unclear which patient-ventilator asynchronies are reliably detectable in clinical practice, most clinically relevant, and how they rank in severity. METHODS: Multiple-choice questions and 5-point Likert-scale statements were used in iterative Delphi rounds. Feedback was incorporated until stable consensus or dissensus was reached for all items. First series of rounds focused on identifying and classifying patient-ventilator asynchronies detectable from ventilator waveforms, second series assessed their associations with outcomes in three patient

Impact of elexacaftor/tezacaftor/ivacaftor on gastrointestinal outcomes, inflammation, exocrine pancreatic function and fat malabsorption: Report of PROMISE pediatric substudy.

BACKGROUND: CFTR mutations likely contribute to inflammation, dysmotility and dysbiosis in the CF intestine. While CFTR modulators such as Elexacaftor/Tezacaftor/Ivacaftor (ETI) have shown pulmonary benefit, their efficacy on gastrointestinal (GI) outcomes is less clear. The PROMISE pediatric sub-study examines GI symptoms and fecal biomarkers in children with CF 6 to <12 years of age who recently initiated ETI. METHODS: Patient-Reported Outcome Measures (PROMs) from 3 validated GI symptom questionnaires (PAGI-SYM, PAC-SYM, PAC-QOL) and a stool-specific questionnaire (SSQ) were obtained. Stoo
