Epidemiology of Sudan ebolavirus disease cases in Uganda, January-April, 2025.

OBJECTIVES: On January 30, 2025, the Ministry of Health, Uganda (MoH), announced an outbreak of Sudan ebolavirus disease (SVD). We describe the epidemiological characteristics and transmission dynamics of the SVD outbreak in Uganda during January-April, 2025. METHODS: SVD cases were identified using MoH case definitions and were classified as confirmed if positive on RT-qPCR or probable if an SVD-related death was reported with epidemiological linkage but without laboratory confirmation. Cases were identified through contact tracing, community and health facility alerts, and active search. Da

Non-neutralizing antibody responses control acute Crimean-Congo hemorrhagic fever virus infection in naïve mice.

Crimean-Congo hemorrhagic fever virus (CCHFV) is the cause of a sometimes-severe viral hemorrhagic fever, CCHF. Severe cases of CCHF often have little-to-no CCHFV-specific antibody responses prior to death. However, it is unclear if the failure to mount an antibody response is the cause of a poor outcome or a correlate of a failed host response. Here, we used a mouse-adapted strain of CCHFV (MA-CCHFV) to investigate the requirement of humoral immunity in control of acute CCHFV infection in immunocompetent mice. We found that mice infected with MA-CCHFV develop a rapid non-neutralizing antibody

Climate-informed surveillance for climate-sensitive infectious diseases in Somalia: early warning systems, policy gaps, and preparedness priorities.

Climate variability is reshaping infectious disease risk through ecological, environmental, social, and health-system pathways. However, its operational implications remain insufficiently defined in fragile and conflict-affected settings. Somalia is a critical case because recurrent droughts and floods intersect with displacement, food insecurity, weak water and sanitation infrastructure, disrupted immunisation, livestock dependence, and constrained health-system capacity. This narrative review synthesises evidence on climate-sensitive infectious disease risks in Somalia, examines existing sur

Longitudinal Trajectories of Platelet Indices Across Dengue Severity in Children: A Linear Mixed-Model Analysis.

Background Dengue has a high disease burden, and some affected children progress to severe dengue (SD), making early identification of deterioration a clinical priority. Automated platelet indices such as mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT) are generated at no additional cost by routine hematology analyzers. However, they are usually interpreted as static measurements and cannot distinguish peripheral platelet destruction from impaired bone marrow production. Linear mixed modeling (LMM) is well suited to correlated repeated measurements but has

Development and internal analytical validation of plaque reduction neutralization test for clinical trial application of Kyasanur forest disease virus vaccine.

Kyasanur Forest Disease (KFD) is a tick-borne viral hemorrhagic fever endemic in the Western Ghats of India. The previous KFD vaccine was discontinued in 2022 due to waning immunity and suboptimal protection. A new vaccine candidate developed by the Indian Council of Medical Research in partnership with Indian Immunologicals Ltd. has progressed to clinical development, necessitating accurate measurement of virus-specific neutralizing antibodies. This study outlines the development, optimization, and internal analytical validation of a plaque reduction neutralization test (PRNT) for KFD virus u

Impact of Wolbachia-mediated incompatible and sterile insect techniques on dengue incidence: a large-scale cluster randomised controlled trial.

BACKGROUND: Wild-type female Aedes aegypti which mate with Wolbachia-infected male Aedes aegypti (Wolbachia-Aedes) produce non-viable eggs due to cytoplasmic incompatibility. Sustained releases of Wolbachia-infected male Aedes aegypti can suppress wild-type mosquito populations, but robust evidence of its efficacy in reducing dengue incidence is needed. METHODS: We conducted a large-scale cluster-randomised controlled trial involving releases of male wAlbB-infected Ae. aegypti mosquitoes (wAlbB-SG) for dengue control in Singapore. Mosquitoes were irradiated to sterilise potential residual fem

Viral Dynamic Model-Informed Dose Selection of Mosnodenvir Prophylaxis against DENV-3 in a Controlled Human Infection Model.

Dengue virus (DENV) infection is a significant global health threat. Mosnodenvir (JNJ-64281802) is a pan-serotype small molecule antiviral blocking DENV replication. A Phase IIa study (NCT05048875) evaluated the antiviral activity, safety, and pharmacokinetics of mosnodenvir as prophylaxis against DENV-3 in a controlled human infection model (CHIM). A combined population pharmacokinetic (PopPK) and viral dynamic model (VDM) was developed to estimate the prophylactic effect of mosnodenvir and to inform dose selection. The PopPK model is a two-compartment model based on plasma pharmacokinetic da

Isolation and molecular characterization of West Nile Virus with evidence of vertical transmission in the Coastal region, Kenya.

West Nile virus (WNV) is a mosquito-borne flavivirus of global public health importance, maintained in an enzootic cycle between birds and mosquitoes, with humans and other mammals as incidental hosts. Although WNV has been documented in Kenya, surveillance remains inconsistent, geographically fragmented, and largely dependent on serological methods. This limits understanding of circulating lineages, vector diversity, and transmission dynamics, particularly in coastal regions. This study aimed to address these gaps through systematic arboviral surveillance of mosquito populations across four e

Dual readout RT-LAMP for rapid on-site dengue virus detection and serotyping.

UNLABELLED: Dengue is the fastest-growing mosquito-borne disease, with over 14 million reported cases globally in 2024, rendering dengue a major public health issue of global concern. Fast and reliable serotype-specific diagnosis of dengue is critical for patient risk management and epidemiological surveillance. While real-time PCR currently remains the gold standard for dengue virus detection, it requires thermal cyclers and trained personnel in an established laboratory infrastructure, limiting its use in low-resource settings. Loop-mediated isothermal amplification (LAMP) offers a promising

Barriers in operating proactive source reduction informed by an early warning system for dengue prevention: A qualitative study from end-user perspectives in Vietnam.

BACKGROUND: Dengue is a major vector-borne disease, with rising incidence driven by climate change, rapid urbanization, and increased international travel. Although early warning systems (EWSs) have shown promising predictive performance in endemic settings, limited evidence on implementation barriers from local end users hinders their integration into routine proactive vector control. METHODS: A qualitative study with in-depth interviews was conducted with local end users, including local authorities, health practitioners, and health volunteers. A deductive approach utilizing a predefined co

Emergence of a Bundibugyo virus variant in the 2026 outbreak in the Democratic Republic of the Congo and Uganda.

In May 2026, an outbreak of Ebola disease caused by Bundibugyo virus (BDBV, species Orthoebolavirus bundibugyoense) was declared in the Democratic Republic of the Congo (DRC), with cases originating from DRC and locally transmitted cases reported in Uganda. Bundibugyo virus disease (BVD) outbreaks were previously recorded in 2007-2008 in Bundibugyo District, Uganda, and in 2012 in Isiro, DRC. Here, we generated 22 genomes from samples obtained from individuals with BVD in DRC and Uganda. These genomes form a well-supported phylogenetic cluster separate from BDBV variants associated with the 20

Post-Exposure Prophylaxis with a monoclonal antibody cocktail Following Bundibugyo Ebolavirus Exposure in Adults and Children.

Five individuals from a single-family cluster, one rVSV-ZEBOV-vaccinated adult and four unvaccinated children aged 1-7 years, received investigational MBP134 as post-exposure prophylaxis (PEP) 5-6 days after high- to intermediate-risk occupational or household exposure to Bundibugyo ebolavirus (BDBV). MBP134 is a broadly neutralizing monoclonal antibody cocktail that has conferred protection against BDBV in non-human primates but to our knowledge has not previously been evaluated as PEP in humans. Given the absence of approved preventive interventions against BDBV and the substantial case fata

Evaluation of inactivated and live attenuated, tetravalent dengue vaccine prime-boost strategies in a randomized, open-label, phase I study: implications for understanding immune correlates of protection.

UNLABELLED: Vaccines that elicit balanced, protective immunity to all four dengue virus (DENV) types are needed to reduce the global burden of dengue disease. Achieving this goal has been a challenge for dengue vaccine developers, particularly when immunizing dengue immunologically naïve individuals. In this randomized, phase I, open-label study of 40 healthy adults, we evaluated a heterologous, tetravalent purified inactivated vaccine (PIV) prime and a tetravalent live attenuated vaccine (LAV) boost. We compared a 0, 90 day PIV/LAV prime-boost schedule to a 0, 180 day schedule. These two PIV/

Prevalence and factors associated with cardiac involvement in hospitalized patients with dengue: a cross-sectional study.

BACKGROUND: Cardiac involvement is an underrecognized but potentially life-threatening complication of dengue fever. We aimed to estimate the prevalence and identify predictors of cardiac involvement among hospitalized patients with dengue. METHODS: We conducted a cross-sectional study including 919 patients with confirmed dengue. Cardiac involvement was defined based on clinical features (e.g. chest pain, dyspnoea, syncope), electrocardiogram (ECG) changes, elevated cardiac biomarkers (high-sensitivity troponin I [hs-troponin I] or N-terminal pro-hormone of brain natriuretic peptide) or echo

Comprehensive conservation profiling enables a robust multiplex RT-qPCR assay for simultaneous pan-detection and serotype discrimination of dengue virus.

Dengue virus (DENV) remains a critical global health threat, necessitating rapid pan-detection and precise serotyping. In this study, we developed a multiplex real-time RT-qPCR assay for simultaneous detection and identification of DENV1-4. Guided by a systematic genomic conservation analysis of 4,754 sequences, we targeted the highly conserved 5' untranslated region (5' UTR) to design one pan-DENV and four serotype-specific primer-probe sets. The assay demonstrated robust analytical performance, with the limit of detection (LOD) ranging from 15.8 to 1.58 × 103 copies/mL and the lowest LOD (15

Clinical severity and health-seeking behaviors of cases with hemorrhagic fever with renal syndrome: A comparative study across spatial clusters in Jiangxi Province, China.

BACKGROUND: Hemorrhagic fever with renal syndrome (HFRS) is a significant public health concern in Jiangxi Province, China, where both Hantaan (HTNV) and Seoul (SEOV) co-circulate. Previous studies have identified three distinct clusters of HFRS epidemic in Jiangxi Province, characterized by geographical separation and heterogeneous incidence levels. We hypothesized that clinical severity and healthcare-seeking behaviors may vary across these clusters, and thus conducted a retrospective study to examine this hypothesis. METHODS: We retrospectively analyzed 4,614 HFRS cases (13-year period) fr

A multiplex serological assay for quantification of arbovirus antibody kinetics and neutralizing antibodies.

Arboviruses such as dengue virus (DENV) and chikungunya virus (CHIKV) pose major global health threats. To support serological surveillance and study cross-reactivity, an in-house multiplex bead-based immunoassay was developed to measure IgG responses against a wide panel of orthoflavivirus and alphavirus antigens. Samples from Senegal, France, the Philippines, and Bangladesh enabled analyses to be carried out in endemic and non-endemic regions. IgG kinetics following DENV1 and CHIKV infections revealed virus-specific temporal profiles and identified antigens, such as CHIKV E2 and VLP, as well
