The distinct contribution of sternotomy to the systemic inflammatory response during children's heart surgery.

BACKGROUND: Sternotomy provides access to the mediastinum, heart and great vessels for congenital cardiac surgery in children. The contribution of this incision to the systemic inflammatory response during open-heart surgery, particularly in combination with the complement-mediated response to cardiopulmonary bypass (CPB), is unknown. This study aimed to characterize the inflammatory mediator profile of sternotomy and contrast that with CPB-associated inflammation. METHODS: This study is a post-hoc analysis of a single-arm prospective clinical study (NCT05154864) of 40 pediatric patients unde

Upfront surgery for clinical N1 stage II non-small cell lung cancer.

PURPOSE: To identify the high-risk subgroup of outcomes following upfront surgery for clinical (c)-N1 stage II non-small cell lung cancer (NSCLC). METHODS: Between 2009 and 2022, 140 patients underwent upfront lung resection of more than one lobe with mediastinal lymphadenectomy for cN1 stage II NSCLC. The subjects of this retrospective study were 129 of these patients, whose cN1 node was confirmed pathologically as metastasis after surgery. “Continuous cN1” was an N1 node directly involving the primary tumor. Disease-free survival (DFS) and overall survival (OS) were estimated using log-rank

Approaches to single port mediastinal surgery.

Single Port Video-Assisted Thoracic Surgery (VATS) is now the standard of care for many pulmonary operations. The adoption of Single Port VATS for mediastinal surgery has been considerably slower. Nonetheless, intercostal, subxiphoid, and robot-assisted single-port surgery approaches for mediastinal conditions have been explored in recent years. This narrative review looks at the variety of techniques described and offers an appraisal of the clinical evidence surrounding them.

Endobronchial ultrasound-guided mediastinal biopsies for the diagnosis of mediastinal diseases: A network meta-analysis.

BACKGROUND: Clinical guidelines recommend endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) as the initial diagnostic tool for lung cancer staging. However, despite the availability of mediastinal forceps biopsy and cryobiopsy, the optimal diagnostic approaches for other mediastinal conditions remain unclear. METHODS: We searched multiple databases and sources up to February 21, 2025, and employed single-arm, pairwise, and network meta-analytical approaches to comprehensively evaluate EBUS-based biopsies for mediastinal diseases in terms of efficacy and safety. Early outcomes of subcostal four-arm robotic pulmonary resection and mediastinal lymph node dissection for non-small cell lung cancer.

We report the early outcomes of subcostal robot-assisted thoracic surgery (subRATS) for non-small cell lung cancer (NSCLC). We conducted a prospective, single-arm cohort study at Seoul National University Bundang Hospital beginning in June 2022 to evaluate the feasibility of subRATS for pulmonary resection in NSCLC. This interim analysis assessed outcomes in consecutive patients from June 2022 to June 2025. The operative times for a subgroup of patients who underwent lobectomy were also analyzed using cumulative sum (CUSUM) analysis to assess procedural reproducibility. A total of 102 patients

Spontaneous hemorrhage from a functionally well-controlled, non-enlarged but hyperplastic parathyroid gland due to secondary hyperparathyroidism: a case report.

Chronic kidney disease causes secondary hyperparathyroidism (SHPT) with hyperplastic enlarged parathyroid glands. Deep cervical spontaneous hemorrhage has been occasionally reported to be caused by rupture of an enlarged parathyroid gland under functionally uncontrolled SHPT. We encountered an unusual case of life-threatening spontaneous hemorrhage from a parathyroid gland in a patient undergoing hemodialysis. A 40-year-old man with end-stage renal disease received renal replacement therapy for 12 years. He had SHPT, which was functionally well controlled using multiple medications. The parath

Primary mediastinal mucormycosis presenting with hoarseness: a case report.

BACKGROUND: Mucormycosis is a rare, rapidly progressive fungal infection caused by molds of the order Mucorales, most commonly Rhizopus species. While it predominantly affects immunocompromised patients, mediastinal mucormycosis is exceptionally rare, with fewer than 50 cases reported, and has a high mortality rate. Diagnosis is challenging owing to nonspecific clinical and radiologic findings, often mimicking malignancies such as lymphoma or thymoma. This study aims to report a rare case of primary mediastinal mucormycosis presenting with hoarseness and to highlight the diagnostic challenges

Necrotizing fasciitis of the neck and emphysematous osteomyelitis of sternoclavicular joint: a rare case and peculiar imaging findings.

Emphysematous osteomyelitis accompanied by necrotizing fasciitis (NF) is a rare life-threatening condition requiring immediate recognition and aggressive management. We report a case of a 47-year-old male who initially presented to the Emergency Department (ED) with fever, shoulder pain, and leukocytosis. The patient was discharged with analgesics for presumed musculoskeletal pain after an unremarkable chest radiograph. Over the following days, he developed a progressive chest and neck erythema, prompting his return to the ED. Bloodwork revealed a progression of leukocytosis. Contrast-enhanced

Thoracoscopic repair of Boerhaave's syndrome in the prone position: A novel left-sided minimally invasive approach.

Boerhaave's syndrome, a spontaneous transmural rupture of the esophagus, is associated with high mortality and requires left thoracotomy. Minimally invasive surgery (MIS) is an alternative. Although most reports on thoracoscopic repair describe the procedure being performed in the lateral decubitus position, there are few reports of repairs conducted in the prone position. We hereby describe a left-sided thoracoscopic technique for primary esophageal repair, in the prone position. Patients are positioned prone and procedure is conducted under general anesthesia with carbon dioxide pneumothorax

Personalized organ dose assessment from CT imaging during proton therapy for classical Hodgkin lymphoma patients.

BACKGROUND/PURPOSE: Proton therapy (PT) improves treatment precision for classical Hodgkin lymphoma (cHL), with computed tomography (CT) being essential for treatment planning and verification. This study evaluates the individual organ doses from CT imaging in these patients treated with PT using free-breathing (FB) or deep inspirational breath hold (DIBH) techniques. MATERIAL/METHODS: A cohort of 38 patients treated at the Skandion Clinic was studied. Organ doses from each planning and verification scan were estimated using VirtualDose software, based on CT parameters extracted from DICOM da

Comparison of EEG features between patients with and without reduced 123I-MIBG myocardial scintigraphy uptake: A 6-year retrospective study of 230 patients with suspected dementia.

OBJECTIVE: Dementia with Lewy bodies (DLB) remains a diagnostic challenge due to its clinical overlap with other dementia-related diseases. Although 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy provides high specificity for DLB and is one of the indicative biomarkers, its application is constrained by substantial cost and limited accessibility. Electroencephalography (EEG), by contrast, is inexpensive and widely available, but currently serves only as a supportive biomarker. The aim of this study is to investigate the association between these biomarkers. METHODS: This retrospe

Surgical Approaches and Perioperative Outcomes in Mediastinal Paragangliomas: A 20-Year Comprehensive Systematic Review.

BACKGROUND: Mediastinal paragangliomas (MPs) are rare, highly vascular neuroendocrine tumors whose surgical resection is the gold standard but carries a high risk of perioperative complications due to the MPs' proximity to major cardiovascular structures with potential life-threatening hemorrhage. Due to their rarity, the literature consists primarily of case reports. Our systematic review aims to synthesize the evidence from the last two decades to propose a standardized, multidisciplinary approach to the diagnosis and surgical management of MPs. METHODS: A systematic literature review was c

Long-Term Response Without Immune-Related Adverse Events to Atezolizumab Treatment in TMB-High Thymoma: A Case Report from the KOSMOS-II Study.

Background: Thymic epithelial tumors (TETs), including thymic carcinomas and thymomas, are rare malignancies originating in the mediastinum. Therapeutic options remain limited for patients experiencing disease progression following platinum-based chemotherapy. High tumor mutational burden (TMB) is uncommon in thymic malignancies but may predict response to immunotherapy. We report a patient with TMB-high TET who participated in the KOSMOS-II study in South Korea and achieved a durable response to atezolizumab without developing immune-related adverse events (irAEs). Case presentation: A 73-yea

Advancing mediastinal sampling in non-metastatic lymphadenopathy.

Despite advances in PET/CT, tissue confirmation remains essential for mediastinal lymphadenopathy. EBUS-TBNA is minimally invasive but limited by cytology, particularly in benign disease and lymphoma. Emerging EBUS-guided techniques, notably transbronchial mediastinal cryobiopsy, improve diagnostic yield where tissue architecture is required. The multicenter randomized study1 strengthens evidence for its added value, while underscoring the need for selective application and standardized, indication-specific trials.

Unicentric Castleman Disease of the Mesentery Mimicking a Gastrointestinal Stromal Tumor: A Case Report.

Unicentric Castleman disease (UCD) is a rare benign lymphoproliferative disorder commonly found in the mediastinum and retroperitoneum, with mesenteric involvement being exceptionally uncommon. We present a case of a 21-year-old male with a history of constipation, fatigue, and recurrent right iliac fossa pain for six months. Initial imaging suggested a gastrointestinal stromal tumor (GIST); however, the final diagnosis of UCD hyaline vascular (HV) variant was made based on immunohistochemical (IHC) analysis and histopathological reports. Surgical excision led to complete resolution of symptom
