Thymic hyperplasia with lymphoepithelial sialadenitis-like features A single-institution series of 21 new cases.

Thymic hyperplasia with lymphoepithelial sialadenitis-like features (TH-LESA) is a rare tumor-like condition of the thymus gland with less than 50 cases reported in the literature. Named after its resemblance to lymphoepithelial sialadenitis (LESA) of the salivary glands, TH-LESA is a distinct form of thymic hyperplasia characterized by simultaneous epithelial and lymphoid hyperplasia. Recently, TH-LESA has been linked with autoimmune diseases (AID) and a potential for developing thymic lymphoma. We report 21 additional cases of surgically resected TH-LESA in 15 women and 6 men (mean age 53 ye

A case of phlegmonous esophagitis complicated by pharyngeal infection: so-called descending necrotizing esophagitis.

BACKGROUND: Descending necrotizing mediastinitis (DNM) is a fatal disease caused by odontogenic or pharyngeal infection extending into the mediastinum along the deep fascial surface, retropharyngeal space, or peritracheal space. Although the esophagus is generally excluded from the infectious spillover route, in extremely rare cases deep neck infections can cause phlegmonous esophagitis. Herein, we report a case of phlegmonous esophagitis that was difficult to differentiate from DNM. CASE PRESENTATION: A 61-year-old man was diagnosed with deep neck infection and DNM based on radiological find

Very late extragonadal recurrence of testicular seminoma presenting as a bladder mass: a case report.

Very late relapse of testicular seminoma is rare and typically occurs at conventional metastatic sites such as the retroperitoneum, mediastinum, or lungs. Extragonadal recurrence involving the urinary bladder is particularly uncommon and may pose diagnostic challenges. We report the case of a 52-year-old male who presented with painless visible hematuria 16 years after undergoing radical inguinal orchidectomy for stage I testicular seminoma. Imaging revealed a large infiltrative mass arising from the left posterolateral wall of the urinary bladder, with extension into the prostate and distal u

SEPAR-SECT Recommendations for Perioperative Invasive Mediastinal Staging of Non-small Cell Lung Cancer.

Accurate mediastinal nodal staging is essential for the management of non-small cell lung cancer (NSCLC), as it refines prognosis, guides multimodal treatment, and improves survival. This joint consensus document from the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR) and the Spanish Society of Thoracic Surgeons (SECT) updates and expands the 2011 SEPAR guidelines, incorporating the 9th edition of the TNM classification and extending recommendations to the entire perioperative setting, including both preoperative and intraoperative mediastinal staging. The recommendations were dev

Early timepoint 99mTc-DPD whole body scintigraphy and quantitative SPECT/CT imaging for diagnosis of cardiac ATTR amyloidosis.

BACKGROUND: Cardiac transthyretin amyloidosis (ATTR-CM) is a progressive myocardial disease ultimately leading to heart failure. Standard diagnostic workup includes 99mTc-DPD scintigraphy performed after 2.5-3 h. The purpose of this study is to compare early (1 h after injection) to late imaging of 99mTc-DPD scintigraphy and SPECT for the detection of ATTR-CM. Early imaging could improve patient comfort and examination efficiency. RESULTS: 50 patients undergoing 99mTc-DPD scintigraphy and SPECT for suspected ATTR-CM were included. Imaging was performed at both 1 h and 2.5-3 h post-injection.

Type A Aortic Intramural Hematoma Without Identifiable Intimal Tear Presenting With Minimal Initial Findings.

Aortic intramural hematoma (IMH) is a subtype of acute aortic syndrome characterized by hemorrhage within the aortic media in the absence of an identifiable intimal tear. Although IMH represents a less frequent presentation of acute aortic syndrome, involvement of the ascending aorta (type A) carries significant morbidity and mortality comparable to classic aortic dissection. Type A IMH has been reported to progress to acute aortic dissection and is associated with an increased risk of aortic rupture due to thinning of the outer aortic media. We report a 77-year-old female with hypertension pr

Triangle-like lumen in fibrosing mediastinitis-induced pulmonary vein stenosis: a case report.

BACKGROUND: Fibrosing mediastinitis (FM) is a rare chronic fibrotic disease of the mediastinum, characterized by diffuse fibrous tissue proliferation in the mediastinal space that can compress adjacent pulmonary vessels bilaterally. This fibrous proliferation is the core pathological basis for simultaneous stenosis of pulmonary arteries (PAs) and pulmonary veins (PVs) in affected patients, with adhesion as a secondary pathological manifestation of FM rather than the primary cause of vascular stenosis. Fibrous tissue proliferation compresses the pulmonary veins, causing stenosis, obstructing pu

Unicentric Adrenal Castleman Disease Mimicking an Adrenal Tumour.

Castleman disease (CD) is an uncommon lymphoproliferative disorder of undetermined origin, also known as giant lymph node hyperplasia or angiofollicular lymph node hyperplasia. It is broadly classified into unicentric and multicentric forms, each exhibiting distinct clinical manifestations, pathological features, and prognostic implications. While the mediastinum remains the most frequently involved site, CD may occur in a variety of nodal and extranodal locations. Adrenal involvement is exceptionally uncommon and can pose a significant diagnostic challenge by mimicking primary adrenal neoplas

Kaposiform hemangioendothelioma: Diagnosis and treatment.

Kaposiform hemangioendothelioma (KHE) is a rare borderline vascular tumor that occurs primarily during infancy and childhood. The tumor typically originates in the skin and exhibits invasive growth into deeper tissues, often manifesting as a firm, poorly defined purplish-red mass. Cases of KHE involving visceral organs, including the bone, retroperitoneum, or mediastinum, have also been reported; however, their clinical features are often nonspecific, which may lead to delayed diagnosis. Importantly, these tumors are frequently associated with Kasabach-Merritt phenomenon, a consumptive coagulo

From Unicentric Castleman Disease to Lymphoma: A Rare Case Highlighting a Diagnostic and Therapeutic Challenge.

Unicentric Castleman disease (UCD) is a rare lymphoproliferative disorder treated with surgical resection. However, its association with lymphoma, particularly nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL), is rare and not well characterized. We report a case highlighting the potential for malignant transformation in an otherwise asymptomatic patient. The patient was a 28-year-old male with a history of UCD (hyaline vascular type) in the mediastinum who presented for routine follow-up of previously enlarged lymph nodes. Surveillance chest CT revealed a new posterior mediastinal lymph

Refining oncological resectability for intrahepatic cholangiocarcinoma: Radiological assessment of mediastinal lymph node enlargement does not reflect clinical prognosis after curative resection.

INTRODUCTION: With advances in surgical techniques, resectability of intrahepatic cholangiocarcinoma (ICCA) is now largely determined by oncological criteria. Distinguishing localized disease from early systemic spread remains a major challenge in interdisciplinary decision-making. This study aimed to evaluate the prognostic significance of radiologically enlarged mediastinal lymph nodes (emLN) in patients with resectable ICCA. MATERIALS AND METHODS: Clinical data from all consecutive patients who underwent liver resection for ICCA between 2008 and 2018 at a single high-volume center were pro

Narrative review of descending necrotizing mediastinitis, focusing on the etiology, classification-guided surgical management, treatments, and prognosis.

BACKGROUND AND OBJECTIVE: Descending necrotizing mediastinitis (DNM) is a rare, life-threatening infection characterized by the rapid spread of deep neck infection (DNI) along the continuous fascial planes into the mediastinum. Although diagnostic and therapeutic advances have improved patient outcomes, the prognosis remains unsatisfactory in advanced cases or in the absence of timely and appropriate treatment, mainly due to delayed diagnoses and difficulty in selecting an appropriate surgical approach. This review summarizes the current knowledge on the pathophysiology, anatomical classificat

Evolving paradigms in thymoma surgery and multidisciplinary care: a narrative review.

BACKGROUND AND OBJECTIVE: Thymic epithelial tumors (TETs)-including thymomas and thymic carcinomas-are rare anterior mediastinal malignancies characterized by variable biology, frequent paraneoplastic syndromes, and high recurrence potential. Recent advances in pathologic classification, staging systems, imaging, surgical techniques, and systemic therapies have led to significant changes in clinical management. Complete R0 surgical resection remains the cornerstone of curative treatment; however, the increasing adoption of minimally invasive approaches, refinement of multimodal therapy for adv

Acute mediastinitis, mediastinal granuloma, and fibrosing mediastinitis: a narrative review.

BACKGROUND AND OBJECTIVE: Acute mediastinitis, mediastinal granuloma (MG), and chronic fibrosing mediastinitis (FM) are distinct disease processes affecting the mediastinal structures. These diseases have variable etiologies, management strategies, and outcomes. The purpose of the manuscript is to present a narrative review of these distinct, albeit rare conditions affecting the mediastinum. METHODS: We searched various databases such as PubMed and Google Scholar from the last 10 years for the various benign mediastinal disorders we wanted to discuss. Relevant articles that were written in En

Salvage resection via a trans-manubrial approach after definitive chemoradiotherapy for initially unresectable thymic carcinoma in the superior mediastinum: a case report.

BACKGROUND: Initially unresectable thymic carcinoma poses significant management challenges, and the role of salvage resection after definitive chemoradiotherapy has limited evidence. CASE DESCRIPTION: We report a successful case of complete salvage resection via a trans-manubrial approach. A 48-year-old man presented with hoarseness and was diagnosed with thymic squamous cell carcinoma originating from the superior mediastinum, invading the aortic arch and arch vessels, and metastasizing to a left supraclavicular lymph node (LN; T4N2M0). He underwent definitive chemoradiotherapy with two cyc
