Loss of Inspiratory Augmentation as a Predictive Factor for the Development of Gastroesophageal Reflux Disease After Peroral Endoscopic Myotomy in Patients With Achalasia.

BACKGROUND/AIMS: : Gastroesophageal reflux disease (GERD) after peroral endoscopic myotomy (POEM) is a significant challenge. Disruption of the lower esophageal sphincter during POEM and dysfunction of the crural diaphragm, which contributes to esophagogastric junction pressure, may lead to GERD after POEM. We aim to identify predictors of GERD after POEM and focus on physiological parameters of esophagogastric junction pressure in patients with achalasia before POEM. METHODS: : We retrospectively analyzed a prospectively collected database of patients who underwent POEM between July 2016 and

Platypnoea-orthodeoxia from kyphoscoliosis and diaphragmatic dysfunction without intracardiac shunt on transthoracic echocardiography.

Platypnoea-orthodeoxia syndrome (POS) is a rare cause of posture-dependent hypoxaemia, usually linked to intracardiac right-to-left shunts. We report an elderly woman with progressive dyspnoea and recurrent falls whose oxygen saturation fell from 95% supine to 84% upright. Imaging showed kyphoscoliosis with partial right hemi-diaphragmatic dysfunction. Supine and upright contrast transthoracic echocardiography demonstrated delayed bubble appearance, excluding an intracardiac shunt and suggesting an intrapulmonary mechanism. CT pulmonary angiography and Tc-99m macroaggregated albumin scintigrap

Role of diaphragmatic ultrasound in the assessment of disease severity in stable interstitial lung disease patients.

Assessment of disease severity in interstitial lung disease (ILD) is usually performed using lung function tests, exercise testing, and chest imaging. Each modality has its own benefits and drawbacks. Ultrasound (USG) examination of the diaphragm is a non-invasive imaging modality that has been found to be effective in evaluating diseases like chronic obstructive pulmonary disease and asthma. However, its role in the assessment of stable ILD has been scarcely evaluated. We conducted a cross-sectional study to evaluate the role of diaphragmatic USG in the assessment of disease severity in 55 st

MYRF controls mesothelium specification, signaling, and plasticity in lung development.

The mesothelium is a squamous monolayer that ensheathes internal organs and lines the body cavities. Aside from facilitating tissue sliding, its additional functions remain poorly understood. Here, we study the mesothelium through investigating myelin regulatory factor (Myrf), a transcription factor expressed in the mesothelium and a top mutated gene in congenital diaphragmatic hernia (CDH), a developmental disorder that affects the lung and diaphragm. In mice, inactivation of Myrf early in embryogenesis resulted in CDH and defective mesothelium specification, compromising its role as a signal

An unexpected finding of large bowel obstruction from colonic diaphragm disease following prolonged use of indomethacin.

Colonic stricture leading to large bowel obstruction is an uncommon presentation to the emergency department. Strictures are commonly established secondary to diverticular disease, inflammatory bowel disease, malignancy or ischaemic/infective colitis. The formation of a colonic stricture secondary to the chronic use of non-steroidal anti-inflammatory drugs (NSAIDs) is an unusual finding. We present a case of a 48-year-old lady who presented with a large bowel obstruction secondary to NSAID-induced right colonic stricture in a patient who was initially thought to have an index presentation of c

Seeing readiness: a pilot diagnostic accuracy study integrating diaphragm and lung ultrasound to predict extubation outcomes in the NICU.

UNLABELLED: Predicting extubation readiness in the neonatal intensive care unit (NICU) remains difficult. Prolonged mechanical ventilation may contribute to ventilator-induced diaphragmatic dysfunction, whereas premature extubation increases morbidity. We evaluated whether diaphragm ultrasound (DUS) parameters, alone or integrated with lung ultrasound aeration scoring, predict extubation outcomes in ventilated neonates. In this prospective, single-center pilot diagnostic accuracy study, 72 mechanically ventilated neonates underwent a standardized spontaneous breathing trial (SBT) on a Hamilton

Therapeutics in hepatobronchial fistula caused by microwave ablation for the treatment of recurrent hepatic carcinoma: A case report.

Hepatobronchial fistula is an uncommon but potentially life-threatening complication that can occurs following microwave ablation for hepatocellular carcinoma. The complication is associated with a significant mortality rate. We report the case of a patient with alcoholic cirrhosis and recurrent hepatic carcinoma who developed a delayed pyogenic liver abscess and hepatobronchial fistula and was treated using an uncommon treatment approach. The abscess in segment VIII, adjacent to the diaphragm, progressed to hepatobronchial fistula due to infection caused by Citrobacter freundii. Despite septi

Retroperitoneum and Pelvic Extraperitoneum: Anatomic Landmarks, Imaging Features, and Patterns of Disease Spread.

The retroperitoneum and pelvic extraperitoneum are complex anatomic spaces that contain numerous vital structures such as major vessels, lymphatics, nerves, and abdominopelvic organs. These regions are divided by fascial planes into distinct compartments with defined boundaries and contents, which guide the spread of disease within and beyond the abdominopelvic cavity. The retroperitoneum represents the abdominal part of the extraperitoneum located posterior to the parietal peritoneum and extends from the diaphragm superiorly to the pelvic brim inferiorly. The pelvic extraperitoneum, also refe

Dual-Mode Ventilation Promotes Diaphragm Function Recovery in Rats With Ventilator-Induced Diaphragmatic Dysfunction by Upregulating Insulin-Like Growth Factor-1.

INTRODUCTION: Ventilator-induced diaphragmatic dysfunction (VIDD) is associated with diaphragm atrophy and decreased contractility, leading to difficult weaning. Respiratory muscle training could prevent diaphragmatic dysfunction. IGF-1 is correlated with the enhancement of muscle mass and strength. The aim of this study was to investigate the effect of dual-mode ventilation (DMV) on diaphragm function recovery in VIDD rats, and to explore the role of insulin-like growth factor-1 (IGF-1) as an underlying mechanism. METHODS: Adult male Sprague-Dawley rats were used to establish the VIDD model

Comparison of Interscalene Brachial Plexus, Anterior Suprascapular Nerve, and Costoclavicular Brachial Plexus Blocks in Arthroscopic Shoulder Surgery: A Prospective Observational Study.

Background: Interscalene brachial plexus block (ISB) remains the gold standard anesthesia method in shoulder surgery. However, risk of diaphragm paralysis is a major concern among anesthesiologists. Recent studies on anterior suprascapular nerve block (ASB) and costoclavicular brachial plexus block (CCB) have given promising results for preventing diaphragm paralysis and providing sufficient analgesia. Methods: Forty-six patients who underwent arthroscopic shoulder surgery under one of three regional anesthesia techniques, including ISB (n = 15), ASB (n = 15), and CCB (n = 16), were included i

Neuromuscular and neuromechanical assessments of respiratory performance in the mdx mouse model of Duchenne muscular dystrophy across the natural history of disease.

Duchenne muscular dystrophy (DMD) is a severe life-limiting X-linked neuromuscular disorder characterised by progressive skeletal muscle degeneration and respiratory failure. The mdx mouse, lacking dystrophin, is the most widely used preclinical model of DMD, yet the trajectory of respiratory dysfunction in this model remains incompletely defined. We evaluated neural respiratory drive (NRD), neuromechanical efficiency (NME), tension-time index (TTI), inspiratory drive rate and electromyographic (EMG) frequency spectrum parameters in the diaphragm, external intercostal and parasternal muscles a
