[Pathophysiological mechanisms and multimodal management of acute respiratory distress syndrome following return of spontaneous circulation after cardiac arrest].

Epidemiological data indicate that cardiac arrest is one of the three leading causes of death, and its high case fatality and disability rates pose a major threat to human health. Early after return of spontaneous circulation (ROSC), patients with cardiac arrest frequently develop post-cardiac arrest syndrome (PCAS), which may lead to multiorgan dysfunction. Acute respiratory distress syndrome (ARDS) occurs in 50%-70% of these patients and is an important prognostic factor. This article systematically reviews the pathophysiological mechanisms of post-cardiac arrest ARDS and approaches to its e

Obstructive sleep apnoea severity and insulin resistance in children with Prader-Willi Syndrome: the SPISE index as a candidate metabolic marker.

UNLABELLED: We compared two surrogate markers of insulin sensitivity - the single-point insulin sensitivity estimator (SPISE) and HOMA-IR - and evaluated their associations with OSA severity in children with genetically confirmed PWS and a symptomatic non-syndromic referral group (NSR) with available polysomnography (PSG) and fasting biochemical data. In this single-centre retrospective cross-sectional study, OSA severity was indexed by the obstructive apnoea-hypopnoea index (oAHI), central AHI (cAHI) and oxygen desaturation index (ODI), and caregiver-reported symptoms by the PSQ-SRBD scale. Remimazolam ED95 for Preoperative Sedation in Children with Obstructive Sleep Apnea, with Obesity or a Reference-Range BMI: A Biased-Coin Dose-Finding Study.

BACKGROUND: Obesity may alter total-body-weight dosing of remimazolam, whereas obstructive sleep apnea narrows the respiratory safety margin. We estimated the ED95 for protocol-defined preoperative sedation in children with obesity or a reference-range body mass index (BMI). METHODS: In this prospective, single-center sequential dose-finding study, 80 children aged 3-17 years with a clinical OSA diagnosis undergoing adenoidectomy and/or tonsillectomy entered study-defined obesity (BMI-for-age >95th percentile) or reference-BMI (25th-84th percentile) groups (40 each). Force feedback and vocal cord dysfunction after robotic esophagectomy: a comparison of the da Vinci 5 and Xi systems.

The da Vinci 5 (DV5) is the first robotic platform to integrate force feedback (FFB), which may limit excessive traction and tissue injury. Its clinical impact in robotic transthoracic esophagectomy remains unclear. We compared perioperative outcomes between the da Vinci Xi and DV5 systems. This retrospective study included consecutive patients who underwent robotic transthoracic esophagectomy for esophageal squamous cell carcinoma by a single high-volume surgeon between December 2024 and July 2026. Patients treated with the Xi system without FFB (n = 126) and the DV5 system with FFB (n = 37)

Plasma p-tau217 and neurodegeneration-related biomarkers predict incident cognitive impairment in obstructive sleep apnea: a prospective cohort study.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with cognitive decline, but biomarkers for identifying cognitively unimpaired patients at increased future risk remain limited. We evaluated the prognostic value of plasma biomarkers associated with neuroaxonal injury and Alzheimer's disease-related biology in OSA. METHODS: In this prospective cohort study, 286 cognitively unimpaired adults with polysomnography-confirmed OSA underwent baseline measurement of plasma NfL, GFAP, p-tau181, p-tau217, Aβ42, and Aβ40 and were followed for incident cognitive impairment. Combined Large and Small Airway Obstruction Is Associated With Increased Exacerbation Frequency and Poorer Symptom Control in Persistent Asthma.

BACKGROUND: Spirometry remains central to the assessment of asthma but may fail to detect small airways dysfunction (SAD), which contributes to considerable symptoms and exacerbation risk. OBJECTIVE: We investigated whether integrating oscillometry with spirometry improves physiological phenotyping and risk stratification in asthma. METHODS: In this retrospective, multicenter study, data from 1497 adults with asthma were analyzed from the Oscillometry Asthma Registry. SAD was defined using the ratio of resistance heterogeneity between 5 and 20 Hz to resistance at 5 Hz ≥19% and airflow obstru

Nasal Obstruction Is Reduced 5-7 Years After Radiofrequency Turbinate Reduction: Results From a Retrospective Observational Cohort Study.

INTRODUCTION: Nasal obstruction, often caused by structural and inflammatory conditions, is prevalent and significantly impacts health-related quality of life. Radiofrequency Turbinate Reduction (RFTR) is a widely employed treatment for inferior turbinate hypertrophy, yet evidence of its long-term effectiveness remains limited. This study aimed to assess the patient-perceived long-term outcomes of RFTR in patients with nasal obstruction, focusing on quality of life as measured by the Sinonasal Outcome Test-22 (SNOT-22) and to identify baseline factors predicting surgical outcome. METHODS: A c

Continuous Positive Airway Pressure Effects on Pulmonary Hemodynamics and Left Ventricular Function in Obstructive Sleep Apnea and Obesity Hypoventilation Syndrome: A Systematic Review and Meta-Analysis.

BACKGROUND: Obstructive sleep apnea (OSA) and obesity hypoventilation syndrome (OHS) are strongly linked to pulmonary hypertension and left ventricular abnormalities, driving cardiovascular issues. OHS, with daytime hypercapnia, affects 10%-20% of OSA patients. Continuous positive airway pressure (CPAP) is the first-line therapy, but its cardiac effects are inconsistent. METHODS: We conducted a systematic review and meta-analysis adhering to the PRISMA 2020 statement. Risk of bias was assessed using RoB 2 (RCTs) and the Newcastle-Ottawa Scale (NOS; cohort studies). Transcutaneous Carbon Dioxide Monitoring in Amyotrophic Lateral Sclerosis: A Scoping Review.

BACKGROUND: Respiratory failure is the leading cause of death in amyotrophic lateral sclerosis (ALS). Conventional pulmonary function tests are effort-dependent and may fail to detect early nocturnal hypoventilation. Transcutaneous carbon dioxide (PtcCO2) measurement is a non-invasive method for assessing ventilation, but its role in ALS remains unclear. We performed a scoping review to map the current evidence. METHODS: PubMed, Web of Science, Scopus, and Google Scholar were searched from inception to April 2026. Original studies evaluating PtcCO2 monitoring in ALS were included. Due to hete

Respiratory-swallowing Measures and the Impact of Bolus Hold in Individuals with Parkinson's Disease.

Respiratory and swallowing impairments are common in Parkinson's disease (PD), potentially disrupting respiratory-swallow coordination. This study aimed 1) to examine respiratory-swallowing phase patterns and respiratory cessation duration in individuals with PD across varying bolus consistencies and swallow tasks, and 2) to investigate the influence of the 'bolus hold' verbal cue on these measures. Twenty individuals with PD (mean age = 71.7 years, 6 female) completed 14 swallow trials with varying bolus consistencies (thin, nectar, honey, pudding, solid) and tasks (5mL, 20mL) with each trial

A Sedation Protocol for Endoscopic Baton-Plate Adjustment in Neonates With Pierre Robin Sequence.

BACKGROUND: Robin sequence (RS) is characterized by upper airway obstruction leading to life-threatening desaturations in neonates. Procedural sedation in this population presents unique challenges due to anatomical airway anomalies, young patient age, and associated comorbidities. The pre-epiglottic baton-plate (PEBP) procedure is a non-invasive, endoscopically guided intervention that alleviates obstruction, facilitates oral feeding, and promotes mandibular growth. Sedation for PEBP adjustment requires the maintenance of spontaneous respiration while ensuring patient comfort and procedural t

Acute Respiratory Distress Syndrome and Corticosteroids: Reconciling Conflicting Evidence.

Acute respiratory distress syndrome (ARDS) is a life-threatening form of acute inflammatory lung injury in which loss of alveolar-capillary membrane integrity produces protein-rich pulmonary edema, refractory hypoxemia, and reduced lung compliance, typically within 6-72 hours of a precipitating insult. The 2012 Berlin criteria defined ARDS by acute onset within one week of a known insult, bilateral opacities not fully explained by effusion, atelectasis, or cardiac failure, and a PaO₂/FiO₂ ratio of 300 mmHg or less with a minimum PEEP of 5 cmH₂O. The 2023 global definition expands this framewor
