Coblation intracapsular tonsillectomy outcomes in paediatric obstructive sleep disordered breathing: A Singapore study.

INTRODUCTION: Coblation intracapsular tonsillectomy (ICT) may offer a more favourable post-operative morbidity profile compared with extracapsular tonsillectomy for paediatric obstructive sleep-disordered breathing (oSDB). Data from the Singapore paediatric population remain limited. This study examines the outcomes of coblation ICT in children with oSDB at a tertiary centre in Singapore. METHOD: This was a single-centre prospective cohort study of 102 consecutive children aged 21 months to 15 years undergoing coblation ICT for oSDB (April-October 2023). Concurrent adenoidectomy was performed

Therapeutic Treatment with a Clinically Relevant Dose of Dexamethasone Reduces Pulmonary Vascular Leak in a Mouse Model of ARDS.

PURPOSE: Acute Respiratory Distress Syndrome (ARDS) is a life-threatening acute lung injury with great unmet medical needs. Lung edema resulting from increased vascular leak is a hallmark of the disease. Recently corticosteroids were recommended for ARDS patients by the American Thoracic Society. Here, we use the influenza infection mouse model to study vascular leak induced by viral lung injury and the effect of dexamethasone on vascular leak and mortality. METHODS: Near-infrared fluorescence (NIRF) imaging was used to quantitatively measure vascular leak in mice infected with influenza PR8,

Efficacy and safety of erythrocytapheresis in chronic mountain sickness: a randomised controlled trial protocol (ESCAPE-CMS).

INTRODUCTION: Chronic mountain sickness (CMS) is a clinical syndrome affecting more than 4 million individuals living at high altitudes, primarily characterised by excessive polycythaemia. Current CMS consensus recommends therapies targeting haematocrit (HCT), such as phlebotomy or isovolemic haemodilution. However, the clinical benefit of HCT-reduction strategies in CMS has yet to be established. This trial aims to evaluate whether erythrocytapheresis, regarded as one of the most precise and effective approaches for HCT-reduction, could improve clinical outcomes in patients with CMS. METHODS

Risk Factors for Pediatric Acute Chest Syndrome Utilizing Machine Learning Techniques.

Acute chest syndrome (ACS) causes significant morbidity and mortality in both adult and pediatric patients with sickle cell disease (SCD). Despite known risk factors for developing ACS, this complication is often unexpected. We hypothesized that a machine learning model could predict progression to ACS in children. A random forest model was utilized using the Scikit-Learn Python library. Prediction performance was assessed using the C-statistic, sensitivity, and specificity. The features included are demographics, vital signs, laboratory values, medication orders, and additional comorbidities.

Clinical Diagnostic Value of miR-142-3p in Pediatric Acute Respiratory Distress Syndromes and Its Effects on Human Lung Epithelial Cells.

Pediatric acute respiratory distress syndrome (PARDS) is a major cause of morbidity and mortality in critically ill children. microRNAs (miRNAs) were discovered to be involved in regulating the progression of numerous disorders, including PARDS. We aim to explore the diagnostic value and relevant mechanisms of miR-142-3p in PARDS. Real-time quantitative PCR (RT-qPCR) was used to detect the levels of miR-142-3p and myristoylated-alanine rich C-kinase substrate (MARCKS) in the serum of PARDS patients or in A549 cells. Receiver operating characteristic (ROC) curve analysis predicted the diagnosti

Honokiol regulates Acute respiratory distress syndrome (ARDS)-associated pulmonary fibrosis via the miR-19a-3p/POSTN axis.

BACKGROUND AND PURPOSE: Acute respiratory distress syndrome (ARDS) and subsequent pulmonary fibrosis are associated with high mortality and limited treatment options. Periostin (POSTN) is a profibrotic mediator predicted to be regulated by microRNA-19a-3p (miR-19a-3p), but the relevance of this axis in ARDS-associated pulmonary fibrosis remains unclear. Honokiol (HKL), a phytochemical derived from Magnolia officinalis, possesses antioxidant and anti-inflammatory properties. This study investigated whether HKL modulates the miR-19a-3p/POSTN axis in ARDS-associated lung injury and fibrosis. EXP

Piezoelectric Ceramic Sensor Array Based Obstructive Sleep Apnea Event Detection.

Obstructive sleep apnea (OSA) is one of the major sleep disorders, which has been demonstrated to be a high-risk factor for cardiovascular disease, hypertension, and motor vehicle accidents. Pressure sensors in a contactless manner are a promising way to monitor sleep conditions outside of the hospital. However, previous studies mainly based on limited sensors are often subjected to noise contamination and constrained by the sleeper position to pressure sensors. The acquired pressure signals are of poor quality or even lost, which are not appropriate for the downstream task of OSA event detect

Pyogenic Granuloma in the Sinonasal Cavity: A Potential 38-Year Latency Following Foreign Body Entrapment.

BACKGROUND: Lobular capillary hemangioma (LCH), also known as pyogenic granuloma (PG), is a benign vascular lesion typically associated with minor trauma or irritation, predominantly occurring in the oral cavity and in females. Its sinonasal occurrence, particularly in males, is rare, and an association with remote childhood trauma remains a speculative hypothesis. Clinically, it must be differentiated from other vascular sinonasal lesions such as angiofibroma, hemangioma variants, and malignant mimics. CASE PRESENTATION: This rare case report descripts a highly unusual case of an inflammator

[Nucleoside analogues in etiotropic therapy of hantavirus fevers].

Hantavirus fevers (HF) are caused by pathogenic orthohantaviruses and are classified into two clinical forms: hemorrhagic fever with renal syndrome (HFRS), reported in Eurasia, and hantavirus pulmonary syndrome (HPS), reported in the Americas. HF are characterised by an acute course with predominant involvement of internal organs: HFRS is mainly associated with kidney damage, while HPS is characterised by severe lung injury. Both diseases may also affect the central nervous and endocrine systems. In addition, hantavirus fevers are marked by severe complications during the acute phase and a pro
