Background: Diffuse alveolar haemorrhage (DAH) is a rare but potentially life-threatening pulmonary complication of systemic autoimmune diseases. Although interstitial lung disease (ILD) is a hallmark of antisynthetase syndrome (ASyS), DAH has been only exceptionally reported in this setting. Methods: We present two patients with ASyS who developed DAH confirmed by bronchoalveolar lavage. Results: The first case involved a 52-year-old woman initially diagnosed with rheumatoid arthritis, later reclassified as rheumatoid arthritis-ASyS overlap, who developed DAH in the context of progressive ILD

Background/Objectives: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is a major contributor to morbidity and mortality in RA, yet early recognition remains challenging in routine care. The study aimed to identify clinical factors associated with hrCT-confirmed RA-ILD using a CT-verified case-control design. Methods: A single-center retrospective case-control study was designed to include RA patients who underwent chest hrCT in routine care. Cases were patients with ILD on index hrCT (n = 79) and controls were RA patients with hrCT negative for ILD (n = 59). Data were manua

AIM: This study investigated whether systolic phase imaging improves cardiac magnetic resonance (CMR) mapping by reducing artifacts compared with diastolic phase analyses. We evaluated T1 and T2 relaxation times and extracellular volume (ECV) in healthy individuals, early rheumatoid arthritis (ERA), and aortic stenosis (AS) patients.
MATERIALS AND METHODS: A total of 114 participants (54 healthy adults, 30 ERA and 30 AS patients) underwent 1.5 T CMR with T1, T2, and ECV mapping in both diastolic and systolic phases. Imaging artifacts were categorized as minor or major, and inter-observer agre

OBJECTIVES: Chimeric antigen receptor (CAR)-T cell therapy recently emerged as a possible new treatment option in refractory systemic autoimmune diseases (AID). With growing evidence in B cell-mediated AID, the question arises as to its potential application in difficult to treat RA. Building on positive experience in a case of overlapping seropositive RA and SSc treated with CD19 CAR-T cells at our centre, we reviewed the available evidence on the efficacy and safety of CAR-T cell therapy in RA.
METHODS: The patient successfully received a single infusion of autologous CD19 CAR-T cells (KYV-

OBJECTIVES: The diagnosis of systemic sclerosis (SSc) relies on specific autoantibodies, but the clinical value of non-specific and other immune-mediated inflammatory disease (IMID)-associated antibodies in patients without overlap features is unclear. This study determines the frequency and clinical relevance of a broad autoantibody panel in this well-defined SSc cohort.
METHODS: This is a cross-sectional study including 263 consecutive SSc patients without overlap syndromes. A panel was tested for anti-Scl70, anti-centromere, anti-RNApol-III, anti-PM-Scl, anti-dsDNA, anti-Sm, anti-Ro52, ant

Rheumatoid arthritis (RA) may present with extra-articular pulmonary manifestations, including rheumatoid lung nodules. Leflunomide is an effective disease-modifying antirheumatic drug, but increasing case reports suggest an association with accelerated pulmonary nodulosis. We describe a 61-year-old woman with long-standing seropositive RA who developed left-sided lower chest pain, generalized weakness, fatigue, and loss of appetite after switching from methotrexate to leflunomide 20 mg/day. CT imaging demonstrated multiple bilateral cavitary nodules predominantly in the lower lobes. Extensive

OBJECTIVE: Arthralgia, an early manifestation preceding definite rheumatoid arthritis (RA), represents a critical window to identify high-risk individuals and implement timely interventions. However, the immunopathologic mechanisms underlying the transition from arthralgia to established RA remain incompletely defined.
METHODS: We employed a multiomics strategy integrating peripheral immune cell phenotyping, serum proteomics, and autoantibody profiling to investigate the immunopathologic continuum from preclinical to established RA. A prospective cohort of 346 patients with recent-onset arthr

Rheumatoid arthritis (RA) is a systemic autoimmune disease with substantial extraarticular morbidity and mortality, particularly from rheumatoid arthritis–associated interstitial lung disease (RA-ILD). Although advances in disease-modifying therapies have improved joint outcomes and overall survival, RA-ILD remains a major contributor to premature death. Emerging evidence supports the mucosal origins hypothesis, which proposes that RA-related autoimmunity may be initiated at extraarticular sites, particularly the lungs following inhalational exposures. This narrative review synthesizes the cur

Rheumatoid arthritis associated interstitial lung disease (RA-ILD) is a serious extra-articular manifestation, being the second cause of death in patients with RA. Usual interstitial pneumonia is the most frequent form of ILD with several factors resembling idiopathic pulmonary fibrosis. Early recognition of ILD through screening could change treatment strategies and prognosis in patients with RA. In this sense achieving articular activity remission included in the treat to target strategy becomes one of the most important factors not only to diminish the risk for developing ILD but also disea
