Dental Pulp Stem Cells Orchestrate Macrophage Polarisation in Pulpitis via Mitochondrial Transfer.

AIM: Pulpitis represents a prevalent clinical condition in dentistry. Macrophages play pivotal roles in pulpitis immunopathology, while dental pulp stem cells (DPSCs) serve as key effectors in pulp tissue repair and immune regulation. Although mesenchymal stem cells are known to regulate immunity through mitochondrial transfer, this mechanism remains unexplored in pulpitis. This study investigated how mitochondrial transfer influences pulpitis progression and resolution. METHODOLOGY: To investigate DPSC-macrophage mitochondrial transfer and its role in polarisation of macrophages, Lipopolysac

Microneedling (MN) combined with albumin gel and liquid platelet-rich fibrin mixtures (Alb-PRF) promotes periodontal soft tissue regeneration in SD rats.

We evaluated the efficiency, as a nonsurgical method, of a combination of microneedling (MN) and a blood-derived biomaterial albumin-platelet-rich fibrin (Alb-PRF) in a rat gingival recession (GR) model for periodontal soft tissue augmentation. Rat blood was used to prepare an Alb-PRF mixture, which was used in combination with MN for periodontal soft tissue regeneration therapy. Twenty-two SD rats, 9-10 weeks of age, were randomly selected; eight were used for blood collection and preparation of Alb-PRF mixtures for use as injectable drugs; three, tested for their ability to release growth fa

Sporadic Late-onset Nemaline Myopathy Presenting with Dropped Head Syndrome and Complicated with Sjögren's Syndrome.

Sporadic late-onset nemaline myopathy (SLONM) is a rare neuromuscular disorder often associated with monoclonal gammopathy of undetermined significance (MGUS). However, reports of SLONM without MGUS are limited, particularly in cases complicated by Sjögren's syndrome. We herein report the case of a 62-year-old woman with SLONM without MGUS, associated with Sjögren's syndrome. The patient presented with dropped head syndrome. A muscle biopsy confirmed SLONM, and Sjögren's syndrome was diagnosed. Treatment with intravenous immunoglobulin and corticosteroids improved her muscle strength. This cas

Evaluation of Oxidative Stress Marker 4-hydroxynonenal in Oral Submucous Fibrosis: An Immunohistochemical Study.

BACKGROUND: Oral submucous fibrosis (OSF) is a chronic and progressive fibrotic condition affecting the oral cavity and oropharynx, with a reported malignant transformation (MT) rate ranging from 1.9% to 10%. It is characterized by subepithelial fibrotic changes accompanied by epithelial alterations. The primary etiological factors include betel quid chewing and tobacco use. Habitual use of tobacco results in sustained exposure to reactive oxygen species, leading to oxidative stress (OS), which plays a crucial role in the pathogenesis and progression of OSF. AIM: The objective of this study w

Effect of immediate provisionalization using natural crowns with fiber splints on gingival contour and esthetic outcomes after immediate implant placement: a randomized clinical trial.

BACKGROUND: Immediate implant placement (IIP) requires sufficient primary stability to permit implant-supported immediate provisionalization, which supports peri-implant soft tissue contour during healing, particularly in the anterior esthetic zone. This exploratory study aimed to evaluate the effect of using natural crowns combined with fiber-reinforced composite (FRC) splints on gingival contour and esthetic outcomes following IIP in the esthetic zone dental trauma over a 4–5-month follow-up period. METHODS: A total of 20 participants who underwent IIP in the anterior maxilla were enrolled i

Oral submucous fibrosis: Regulatory mechanism of collagen metabolism and management.

Oral submucous fibrosis (OSF) is a chronic and insidious potentially malignant disorder characterized by progressive fibrosis of the oral submucosa. It is recognized to be an irreversible disease of multifactorial origin, with limited therapeutic options due to incomplete understanding of its pathogenesis. Increased collagen synthesis or decreased collagen degradation is vital for the development of OSF. This review mainly introduces the key regulatory mechanism of collagen metabolism. Prolonged use of areca nut is arguably the major causative agents for OSF, which is vital for the myofibrobla

The Clinical Significance of Attached Gingiva in the Natural Dentition.

Background: The attached gingiva (AG) is the portion of the gingiva firmly bound to the underlying alveolar bone and root cementum, rendering it immobile during functioning. Its dense connective tissue attachment contributes to resistance against mechanical challenges, stabilization of the gingival margin, and dissipation of forces transmitted from the alveolar mucosa. Histologically, AG is characterized by a keratinized epithelium supported by dense collagen fiber bundles, which provide structural integrity to the dento-gingival unit. Clinically, the buccal and lingual width of AG is estimate

Periodontally accelerated osteogenic orthodontics combined with i-PRF for the treatment of gingival recession: a case report with 5-year follow-up.

Orthodontic treatment can significantly impact periodontium, particularly in dentitions with a thin periodontal phenotype. Under such vulnerable anatomical conditions, orthodontic tooth movement may lead to iatrogenic complications, including the development or exacerbation of bony dehiscences and fenestrations, which can manifest as loss of periodontal support and gingival recession.Periodontally accelerated osteogenic orthodontics offers the potential to reduce overall orthodontic treatment duration while enhancing hard tissue outcomes in the dentition. This case report presents the outcomes
