Comparative Histopathological Spectrum of Periapical Lesions in Root Canal-Treated and Non-root Canal-Treated Teeth.

INTRODUCTION: Periapical lesions represent a spectrum of inflammatory and pathological changes associated with endodontic infection. The histopathological characteristics may differ according to the previous root canal treatment status of the associated tooth. This study aimed to compare the histopathological spectrum and predominant inflammatory patterns of periapical lesions in root canal-treated (RCT) and non-root canal-treated (non-RCT) teeth. MATERIALS AND METHODS: A comparative cross-sectional histopathological study was conducted on 120 periapical tissue specimens comprising 60 RCT and

Appliance-Supported Decompression for Radicular Cysts Associated With Primary Molars: A Pediatric Case Series.

INTRODUCTION: Radicular cysts associated with primary teeth are uncommon in pediatric patients but may cause extensive bone destruction and displacement of developing permanent tooth germs. Conventional surgical enucleation may increase the risk of damage to adjacent structures; therefore, conservative treatment approaches such as marsupialization and decompression are preferred. Appliance-supported decompression systems have been increasingly used to maintain continuous drainage while preserving the eruption potential of permanent teeth. CASE SERIES: This case series presents three pediatric

Integrative bioinformatics informed by network toxicology and machine learning elucidates the carcinogenic mechanisms of benzo[a]pyrene-induced breast cancer.

BACKGROUND: Benzo[a]pyrene (BaP) is a recognized mutagen and carcinogen, yet epidemiological links to breast cancer (BC) remain inconclusive. METHODS: We integrated network toxicology, machine learning, and bioinformatics. BaP targets (ChEMBL, PharmMapper, SEA, GeneCards, OMIM) were intersected with BC genes, followed by Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment. Core regulatory genes were further screened using diverse machine-learning algorithms, and their expression levels, diagnostic performance, and associations with the tumor immune microenvironmen

Development of a Preliminary Diagnostic Tool for the Segmentation of Benign Jaw Lesions in CBCT Images Using nnU-Net v2: An Artificial Intelligence-Based Approach.

Accurate segmentation of jaw lesions on cone beam computed tomography (CBCT) images is essential for diagnosis and treatment planning. While previous studies have investigated automated segmentation, most were limited to 2D data, and the performance of newer architectures like nnU-Net v2 remains unexamined. This study evaluated the nnU-Net v2 algorithm for 3D segmentation of jaw lesions and compared its speed to manual expert segmentation. Due to significant class imbalance in the dataset, training a single multiclass segmentation model could have led to dominance of the majority class and red

Histopathological Spectrum of a Calcifying Odontogenic Cyst With Uncommon Findings.

Calcifying odontogenic cyst (COC) is an uncommon lesion accounting for less than 1% of all odontogenic cysts. This pathology commonly affects the anterior jaw and is prevalent in the second to third decades of life, with no sex predilection. The lesion presents with a variable clinical and radiographic presentation, which can simulate other common jaw pathologies. Therefore, histopathological confirmation stands to be diagnostic in COC. The fifth edition classification for jaw tumors of the head and neck by the World Health Organization has summarized the essential diagnostic criteria of COC a

Global prevalence of odontogenic cysts: a systematic review.

BACKGROUND: Odontogenic cysts constitute a common group of lesions affecting the jaws and represent a relevant clinical and epidemiological burden in oral and maxillofacial practice. Their occurrence varies across populations, and understanding global prevalence patterns is essential for improving diagnosis, treatment planning, and public health strategies. However, an updated synthesis of prevalence data across different regions remains limited. METHODS: A systematic review was conducted following a predefined methodology, with a comprehensive search of PubMed, EBSCOhost, and Web of Science

Surgical Management of a Large Radicular Cyst in a Maxillary Incisor Following Failed Regenerative Endodontics and Apical Plug in an Adolescent: A Case Report.

This case report describes the surgical treatment of a large radicular cyst in a maxillary incisor of an adolescent, following an unsuccessful regenerative endodontic procedure (REP) and an apical plug. It highlights minimally invasive, age-appropriate decision-making. A 14-year-old boy presented with an acute abscess (pain, swelling, and fever) in the right maxilla. Tooth #11 was nonvital, with apical resorption, an open apex, and a huge radiolucent lesion involving Tooth #12, confirmed by CBCT. Initial treatment included incision/drainage, calcium hydroxide dressing, and antibiotics, which r

Dental Odontogenic Lesion CBCT and Histopathology Integrated Dataset for Benchmarking Deep Learning Algorithms.

Accurate diagnosis of odontogenic lesions requires pre-operative cone-beam computed tomography (CBCT) and post-operative histopathological confirmation, a workflow that is time-consuming and reliant on clinical expertise. With the rise of artificial intelligence (AI) and deep learning, automated diagnostic solutions have shown great promise. However, progress in deep learning for odontogenic lesions has been hindered by the lack of publicly available paired datasets that combine radiological and histopathological data. To address this gap, we present the Dental Odontogenic Lesion CBCT and Hist

Different Approaches to the Treatment of Radicular and Related Cysts Associated with Nasal Mucosa in the Maxilla: A Case Series.

Radicular cysts (RCs) represent the most frequent inflammatory cystic lesions of the jaw, typically arising from non-vital teeth. While standard management via enucleation is well-documented, complex cases involving the anterior maxilla present significant surgical challenges due to their proximity to the nasal cavity floor (NCF) and the maxillary sinus floor (MSF). This report provides a comprehensive revision of a clinical case series involving seven patients (ages 17-50) treated with multimodal surgical and regenerative protocols. The patients were stratified into five distinct anatomical r

Artificial Intelligence in the Diagnosis of Odontogenous Cysts and Ameloblastomas-A Systematic Review and Meta-Analysis.

Background/Objectives: Odontogenic cysts and ameloblastomas (AB) are mostly asymptomatic, often discovered later due to severe symptoms, and only histopathological examination provides definitive diagnosis. AI-assisted diagnostics offer a fast, noninvasive, painless diagnostic tool. To our knowledge, this is the first meta-analysis aiming to evaluate the classification, detection, and segmentation performance of artificial intelligence (AI) for odontogenic cysts and ABs as distinct entities and to determine if it can achieve clinically acceptable accuracy. Methods: Our systematic search was co

Tooth Transplantation and a Root Submergence Technique After Enucleation of Multiple Continuous Radicular Cysts for Implant Treatment: A Case Report.

When a radicular cyst-involved tooth is challenging to save, tooth extraction and cyst enucleation are required. Since only thin facial bone is present on the coronal side of cystic defects, bundle bone loss and continuity of the cystic cavity after tooth extraction may result in extensive vertical bone defects. To prevent major bone augmentation, the following procedure was performed: the left maxillary incisor (#9), which exhibited apical lesions was extracted, while the right maxillary incisor (#8), despite having a healthy periodontal ligament but being difficult to save, was transplanted

External Cervical Resorption Secondary to Orthodontic Treatment in a Previously Traumatized Tooth: A Case Report.

A history of dental trauma and orthodontic treatment can directly or indirectly cause external root resorption (ERR), which may significantly compromise the structural integrity, pulp vitality, and periapical health of affected teeth. External cervical resorption (ECR) is an invasive form of ERR that originates at the cementoenamel junction and progresses three-dimensionally along the root surface. In this case, a 20-year-old woman presented with discoloration of the maxillary right central incisor following orthodontic treatment, with a history of prior dental trauma. The maxillary right cent

Analysis of CD4+ and CD8+ T Lymphocytes and Expression of Immune Checkpoint Proteins (PD-1 and PD-L1) in Primary and Persistent Periapical Lesions.

INTRODUCTION: Persistent periapical lesions (PePL) represent a clinical challenge because of their inadequate response to conventional endodontic therapies. Studies indicate differences in immunoinflammatory mechanisms between primary periapical lesions (PrPL) and PePL but the findings remain conflicting. This study aimed to evaluate CD4+ and CD8+ T lymphocytes and the expression of immune checkpoint proteins (programmed cell death protein 1 [PD-1] and its ligand [PD-L1]) in PrPL and PePL, correlating the findings with the intensity of the inflammatory infiltrate and pattern of the cystic epit
