Management of oral stomatitis associated with BRAFi-MEKi therapy using intralesional corticosteroid and local anesthetic: a case report.

This case documents the clinical pattern of a fibrinopurulent ulcer with a hyperplastic lesion that developed after the initiation of two classes of targeted therapies acting on different components of the mitogen-activated protein kinase signaling pathway (BRAF inhibitors [BRAFi] and MEK inhibitors [MEKi]). A 66-year-old woman was diagnosed with choroidal melanoma in the right eye in 2012 and successfully treated by choroidal melanoma resection. Tumor recurrence was diagnosed 8 years later. Nivolumab and ipilimumab were prescribed, but this combination was replaced with dabrafenib-trametinib

Carrier-free nanoparticles in microneedles disrupt endosomal acidification suppress ferroptosis in radiation-induced oral mucositis.

Radiation-induced oral mucositis (RIOM) is the most common complication in patients receiving radiotherapy for head and neck, to which ferroptosis is an important contributor. Transferrin receptor (TFRC) -mediated endocytosis is a critical source of iron. Accordingly, we proposed that inhibiting ferroptosis by disrupting TFRC mediated-iron uptake. In this study, we developed endosome-targeting self-assembling nanoparticles (CB NPs) integrated into a detachable microneedle patch (CBT@MNs) for precise and synergistic RIOM therapy. This system integrated the clinically conventional drugs chloroqu

An in vitro evaluation of the antibacterial efficacy, cytocompatibility, and pH modulation of aqueous extract of Ocimum sanctum (Tulsi) on oral streptococci and human gingival fibroblasts.

BACKGROUND: Ocimum sanctum Linn. (Tulsi/Holy Basil) possesses documented antibacterial, anti-inflammatory, and wound-healing properties. Recurrent aphthous stomatitis (RAS), the most common form of oral mucosal ulceration, is primarily an immune-mediated condition, with secondary microbial colonization potentially influencing lesion persistence; current therapies remain largely symptomatic. OBJECTIVE: To evaluate the antibacterial efficacy, cytocompatibility, and pH-modulating effects of an aqueous extract of Ocimum sanctum Linn. against oral streptococci and human gingival fibroblasts (HGF)

Bridging Resource Gaps in Toxic Epidermal Necrolysis Management: A Case Report Highlighting Innovative Care.

Toxic epidermal necrolysis (TEN) is a rare but potentially life-threatening dermatological condition. It is characterised by diffuse exfoliation of >30% of the skin and mucous membranes due to immune-mediated destruction of the epidermis. It is thought to be due to a hypersensitivity reaction to a variety of medications. The mainstay of treatment involves withdrawal of the potential offending agent, supportive care and use of non-adherent dressings. Pre-packaged dressings are expensive or not readily available in our locality; hence, the need for improvisation, to manage the condition and impr

Cutaneous Toxicities of Small Molecules in Targeted Cancer Therapy Part II - Approach to Management and Treatment.

With the rapid expansion of small molecule inhibitors (SMIs) in oncology, the incidence and complexity of dermatologic adverse events (dAEs) have significantly risen1. While SMIs target essential oncogenic pathways, they frequently result in dermatologic toxicities that can impact patient quality of life and lead to dose reduction, treatment interruption, or discontinuation of life-prolonging therapy1. Effective management of these toxicities is critical not only for preserving skin integrity but also for ensuring ongoing oncologic treatment. Part II of this CME discusses guidance on oncoderma

Cutaneous Toxicities of Small Molecules in Targeted Cancer Therapy Part I - Mechanisms of Toxicity.

Advances in the understanding of cancer biology have led to a shift toward more precise, targeted anticancer therapies. Traditional chemotherapy acts as an intracellular poison to disrupt cell division or induce DNA damage resulting in cytotoxicity1. However, the effects of traditional chemotherapy are not relegated to neoplastic tissue, resulting in significant off-target cytotoxicity2. Targeted therapies were developed to provide a more specific approach to cancer treatment, focused on inhibiting the pathways involved in carcinogenesis and tumor growth. Small molecule inhibitors (SMIs) and a

Desquamative Gingivitis: A Narrative Review of Practical Diagnostic Approach for General Dental Practitioners.

Desquamative gingivitis (DG) is a clinical sign characterized by gingival erythema, epithelial desquamation, and erosion rather than a discrete diagnosis. It most commonly reflects underlying oral lichen planus, mucous membrane pemphigoid, or pemphigus vulgaris. As affected patients typically present to general dental practitioners, timely recognition and structured triage are essential for effective management. However, diagnostic delays remain common and can extend the management period by several months. This narrative review synthesizes the current evidence on the clinical presentation, ep

Prevalence of oral recurrent aphthous stomatitis and its association with stress among a sample of Egyptian dental students: a cross-sectional study.

BACKGROUND: Recurrent aphthous stomatitis (RAS) is one of the most common inflammatory ulcerative disorders that impact the oral mucosa. The etiology of aphthous ulcers is complex and multifaceted; however, emotional stress is considered one of the primary risk factors contributing to their development in younger populations. OBJECTIVES: This study aimed to determine the prevalence of RAS among a sample of dental students, its correlation with their stress levels, and to identify possible risk factors linked to these oral ulcers. METHODS: This cross-sectional study involved 4th level dental

Integrating network pharmacology to investigate the mechanism of esculetin from Nyctanthes arbor-tristis leaves against breast cancer.

UNLABELLED: The development of efficient treatment medicines with multitarget modes of action is necessary because breast cancer continues to be one of the leading causes of cancer-related mortality among women worldwide. Although esculetin (6,7-dihydroxycoumarin), a naturally occurring phenolic compound, has demonstrated promising anticancer activity, its molecular mechanism against breast cancer remains poorly understood. Therefore, this study aimed to investigate the therapeutic potential and underlying molecular mechanisms of esculetin against breast cancer using an integrated computationa

A Clinical Perspective on Herpetiform Aphthous Stomatitis: A Case Report on the Findings of 126 Oral Ulcers in a 21-Year-Old Male.

Herpetiform aphthous ulcer (HAU), a rare clinical variant of aphthous ulcer, presents in the oral cavity as multiple, grouped tiny ulcers. These ulcers occur in the mobile, non-keratinized labial mucosa, soft palate, buccal mucosa, and ventral surface of the tongue and are known for their severe and debilitating pain. This article presents a unique and intriguing case of herpetiform aphthous ulcers in a 21-year-old male who reported severe soreness and burning sensations during the consumption of food. The intraoral clinical findings of this rare case of herpetiform aphthous ulcers revealed a

Unusual denture-associated oral squamous papilloma with koilocytosis: a case report and literature review.

BACKGROUND: Oral squamous papilloma (OSP) is a benign epithelial proliferation commonly linked to human papillomavirus (HPV) and usually affects the palate, tongue and lips. Anterior mandibular alveolar mucosa involvement is uncommon, and overlap between koilocytic HPV-suggestive change and chronic denture-related trauma is rarely documented. A review of the literature indicates limited documentation of such overlapping clinicopathological features at this anatomical site. CASE PRESENTATION: A 49-year-old non-smoking male with absence of alcohol consumption history, presented with a 12-month

How Nutrient Deficiencies Impact the Oral Mucosa and How to Manage Them: A Narrative Review.

BACKGROUND: Malnutrition and micronutrient deficiencies may impact the oral mucosa even before any other symptoms become clinically obvious. This makes the oral cavity an important site for early detection of nutritional imbalance. Across the literature review, the most documented vitamin is vitamin B12. Other widely reported nutrients are iron, folate, and zinc. Less consistently documented are vitamins A, C, and D. Their correlation with recurrent aphthous stomatitis (RAS), atrophic glossitis, angular cheilitis, burning mouth symptoms (BMS), mucosal erythema, and oral candidiasis (OC) is bro

Reactive infectious mucocutaneous eruption secondary to Chlamydia pneumoniae infection in a 19-year-old: a case report.

Introduction. Reactive infectious mucocutaneous eruption (RIME) is a phenomenon that occurs predominantly in children and young adults following a bacterial or viral respiratory infection. RIME is most associated with Mycoplasma pneumoniae and generally presents with extensive mucosal and limited cutaneous involvement. Less commonly, infection with Chlamydia pneumoniae can trigger cutaneous eruptions but seldom leads to isolated mucosal involvement. Here, we describe a rare case of RIME mucositis secondary to C. pneumoniae respiratory infection. Case report. An otherwise healthy 19-year-old ma
