Fixed-distance pH probe placement produces systematic bias in reflux detection: the role of esophageal length.

PURPOSE: Ambulatory esophageal pH monitoring is the reference standard for diagnosing gastroesophageal reflux disease, yet a subset of symptomatic patients demonstrate physiologically nonpositive studies. To evaluate whether variation in esophageal length and relative probe position influences measured reflux exposure and contributes to symptom-physiology discordance during wireless pH monitoring. METHODS: We performed a retrospective analysis of data from 63 patients who underwent Bravo pH monitoring system (Medtronic). Relative probe position was defined as the ratio of capsule placement di

An App-Based Proton Pump Inhibitor Discontinuation Tool: A Multicentre, Open-Label, Randomised Superiority Trial.

BACKGROUND: Proton pump inhibitors (PPIs) are frequently used without long-term indication. Discontinuation remains challenging because of rebound symptoms. Whether digital guidance tools improve discontinuation is unknown. We evaluated whether an app improves PPI discontinuation. METHODS: In this open-label adaptive randomised controlled superiority trial, adults using daily PPI for ≥ 4 weeks without chronic PPI indication, according to the Dutch College of General Practitioners guideline, were recruited from outpatient clinics of Internal Medicine, Gastroenterology, Rheumatology and Nephrol

[Distal esophageal spasm as a rare cause of progressive physical decline with malnutrition, sarcopenia, and frailty].

MEDICAL HISTORY: A 93-year-old sarcopenic female patient was admitted to the geriatric ward for further treatment and diagnostic evaluation following a fall and a sacral fracture that had been treated conservatively. Clinically, she primarily presented with loss of appetite accompanied by nausea and recurrent vomiting. A diagnosis of a traction diverticulum had already been made during a previous hospital stay. Subsequently, the patient experienced progressive weight loss with weakness, culminating in a fall and the resulting sacral fracture. EXAMINATION FINDINGS: The clinical-neurological ex

Diagnosis and Management of Actionable Primary Motor Esophagogastric Junction Outflow Obstruction: Clinical Practice Update of the European Society of Neurogastroenterology and Motility.

BACKGROUND: Primary motor esophagogastric junction outflow obstruction (EGJOO) is an esophageal motility disorder identified by high-resolution manometry (HRM) in the absence of an underlying structural or mechanical cause of obstruction. The clinical significance, management, and prognosis of primary motor EGJOO remain uncertain. This clinical practice update provides expert guidance on the evaluation and management of EGJOO, with particular emphasis on determining when the condition is "actionable"-that is, likely to benefit from lower esophageal sphincter (LES)-directed therapies. METHODS:

Prevalence of Gastroesophageal Reflux Disease among Postgraduate Medical Students in North India: A Cross-sectional Study.

BACKGROUND AND OBJECTIVES: Gastroesophageal reflux disease (GERD) is a persistent state due to the retrograde flow of acidic gastric contents, leading to symptoms such as heartburn, regurgitation, chest pain, and dysphagia. Postgraduate medical students are exposed to high stress and lifestyle factors that predispose them to gastrointestinal symptoms. Limited Indian data exist on GERD prevalence in this cohort. MATERIALS AND METHODS: A questionnaire-based cross-sectional study was conducted using the GERD-Q tool. Data were collected via an online Google Form and analyzed using Microsoft Excel

Redefining Acid Suppression in Acid Peptic Disorders: The Emerging Role of Prazans.

ABSTRACT: Acid peptic disorders (APDs) affect approximately 38% of the population in India and commonly present with overlapping phenotypes, including gastroesophageal reflux disease (GERD), dyspepsia, and peptic ulcer disease (PUD), necessitating uniform and sustained acid suppression. Proton pump inhibitors (PPIs) have several clinical limitations, including a delayed onset of action of 3-5 days, incomplete 24-hour acid control with nocturnal acid breakthrough, high nonresponse rates of approximately 40% in GERD and 37% in nonerosive reflux disease (NERD), and variability related to cytochro

Swallowed dental prosthesis causing late oesophageal obstruction and tracheo-oesophageal fistula in a young patient.

A 25-year-old man presented to the emergency department after a rugby accident having swallowed a dental plate. Initial assessment at the time of ingestion, including a chest radiograph, was unremarkable and the plate was presumed to have been expectorated. Five years later he presented with progressive dysphagia, recurrent food bolus obstruction, and haemoptysis. Computed tomography of the thorax performed 5 years later revealed circumferential oesophageal thickening and a large inflammatory mass around a retained radio-opaque foreign body at the level of the carina, with an associated trache

Eosinophilic Oesophagitis and Incidence of Gastrointestinal Cancer: A Population-Based Cohort Study.

BACKGROUND: Evidence concerning gastrointestinal cancer risk after eosinophilic oesophagitis (EoE) diagnosis is limited. AIMS: To examine the association between EoE and incident gastrointestinal cancer in a population-based cohort. METHODS: Using nationwide Danish health registries, we identified all patients diagnosed with EoE between 1997 and 2024, using a validated algorithm. We computed standardised incidence ratios (SIRs) for gastrointestinal cancers overall and site-specific cancers compared with the general population. To distinguish short-term diagnostic activity from longer-term ri

The Dual Roles of Esophageal Submucosal Glands: Protection and Pathogenesis in Benign and Malignant Disease: A Scoping Review.

Background: Esophageal submucosal glands (SMGs) contribute to mucosal defense through bicarbonate, mucin, and reparative-factor secretion. Chronic reflux may impair these functions and remodel the glandular compartment, potentially linking SMG injury to reflux esophagitis, Barrett's esophagus (BE), and esophageal adenocarcinoma (EAC). Objective: To map the human, animal, and in vitro literature on the physiological and pathological roles of esophageal SMGs and to identify evidence gaps relevant to reflux injury, BE, EAC, and endoscopic therapy. Methods: This scoping review was reported in acco

Esophagogastric junction outflow obstruction: advances in diagnosis and clinical management.

Esophagogastric junction outflow obstruction (EGJOO) is an esophageal motility disorder characterized by incomplete lower esophageal sphincter relaxation, with at least partially preserved peristalsis. Recognition of this entity has evolved with the Chicago Classification version 4.0, which has refined diagnostic criteria by incorporating clinical correlation and confirmatory testing, thereby reducing the overdiagnosis observed with previous versions. Diagnosis is based on high-resolution esophageal manometry, identifying elevated integrated relaxation pressure of the lower esophageal sphincte

Long-Term Proton Pump Inhibitors in Lung Transplant Recipients: Near-Universal Use Without Clear Impact on Clinical Outcomes Over a 20-Year Observational Study.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a known risk factor for chronic lung allograft dysfunction (CLAD), the main barrier to long-term survival in lung transplant recipients (LTR). While proton pump inhibitors (PPI) are effective for symptomatic treatment of GERD, data are sparse on the prevalence of long-term PPI use and association with clinical outcomes. METHODS: Single-center retrospective cohort of adult first-time LTR between 1999 to 2018 who were alive and CLAD-free at 1-year post-transplant. PPI exposure was defined as ≥8 continuous weeks in the first year, started wit
