RedoPOEM: efficacy and safety after failure of a first POEM for esophageal motility disorders.

INTRODUCTION: Per-oral endoscopic myotomy (POEM) achieves an 80-90% success rate for the treatment of esophageal motility disorders, but approximately 10% of patients experience persistent symptoms. This study evaluated the results of a repeat POEM procedure, or redoPOEM, as a second-line therapy. METHODS: We conducted a retrospective multicenter study involving 15 French tertiary centers, including all patients undergoing redoPOEM between April 2015 and September 2023. Short and long-term success rates were defined as an Eckardt score < 4 at 3 months, and at the last follow-up visit with no

Association between Gastroesophageal Reflux and Sleep Disorder: A Systematic Review and Meta-analysis.

An association between gastroesophageal reflux disease (GERD) and sleep disorder has been reported. However, the magnitude of this association remains poorly understood. Therefore, we aimed to systematically evaluate this relationship. We conducted a comprehensive search for observational studies in eight databases from their inception to February 28, 2025. The characteristics of each study and pooled risk ratio (RR) with corresponding confidence intervals were calculated. Subgroup analyses were performed to explore sources of heterogeneity. A total of eighteen studies involving 110,417 partic

Association of pre-intubation bag-valve-mask ventilation with hypoxemia and regurgitation in the ED: A multicenter cohort study.

BACKGROUND: The effectiveness of pre-intubation bag-valve-mask (BVM) ventilation for rapid sequence intubation (RSI) in the emergency department (ED) remains uncertain. In this study, we investigated whether pre-intubation BVM ventilation was associated with a lower risk of peri-intubation hypoxemia. METHODS: We analyzed data from a multicenter prospective observational study conducted in 14 EDs across Japan. Adult (≥18 years) patients who underwent RSI between 2020 and 2023 were included. The primary outcome was peri-intubation hypoxemia (SpO₂ <90%), and the secondary outcome was regurgitati

Dual-suture fundoplication for anti-reflux reconstruction after proximal gastrectomy: a single-center retrospective case series.

BACKGROUND: Esophagogastrostomy (EG) after proximal gastrectomy (PG) is widely used but often complicated by reflux. Current anti-reflux procedures, such as double-tract and double-flap reconstructions, are effective but technically demanding. We developed a simplified, device-free anti-reflux EG that uses two sutures to approximate the angle of His, the gastric angle, and a neofundus-like contour. METHODS: We retrospectively analyzed 11 consecutive patients with upper-third gastric cancer who underwent laparoscopic PG followed by dual-suture fundoplication between May 2023 and November 2024.

Ultrasonography for the positioning of esophageal multichannel intraluminal impedance/pH-metry catheter in newborns.

OBJECTIVES: To assess the use of ultrasound (US) to visualize and determine the correct position of a multichannel intraluminal impedance-pH probe (MII-pH). METHODS: This is a monocenter study recruiting newborns who underwent MII-pH for suspicious of gastroesopahegal reflux (GER). Catheter position was based on US and was confirmed by X-ray. Moreover, the depth of the probe was compared with those calculated with several mathematical formula used for pH impedance. The US was performed using (US) system Alpinion-eCube-i7® in B-mode with linear and convex probe. US was used to evaluate the cor

Can Esophageal Anatomy Help to Choose Between Functional Luminal Imaging Probe (FLIP) and Barium Esophagogram in Achalasia?

BACKGROUND: Functional luminal imaging probe (FLIP) and barium esophagogram (BE) are complementary to high-resolution manometry in achalasia. Anatomical changes and achalasia treatment could alter their interpretation. Our objective was to compare FLIP and BE for achalasia diagnosis and follow up and to evaluate the impact of anatomical changes. METHODS: One hundred and thirty patients who underwent FLIP were included. The indication of FLIP was a suspicion of achalasia (naive group, n = 68 patients) or symptoms after achalasia treatment (treated group, n = 62 patients). Esophago-gastric junc

pHoenix score discriminative ability in suspected gastroesophageal reflux disease: a high-volume single-center external validation.

The pHoenix score was recently developed to reduce the proportion of inconclusive diagnoses associated with using total acid exposure time (AET) alone. The aim of this study was to compare the discriminative performance of the pHoenix score to total AET and DeMeester score (DMS) in patients undergoing 24-hour transnasal pH-monitoring (24-h pH). This cross-sectional study included consecutive patients (2017-2024) undergoing 24-h pH for suspected gastroesophageal reflux disease. Exclusions criteria were prior foregut/bariatric procedures, outflow obstruction disorders and inadequate pH-studies (

The American Foregut Society endoscopic classification outperforms Hill in phenotyping the esophago-gastric junction to guide gastroesophageal reflux disease management.

Gastroesophageal reflux disease (GERD) is driven by anti-reflux barrier (ARB) disruption, requiring precise endoscopic tools to guide diagnosis and management. The Hill classification, based solely on flap valve (FV), lacks precision. The American Foregut Society (AFS) classification, integrating hiatal hernia length (L), hiatal opening diameter (D), and FV (F), offers a comprehensive approach to phenotype esophago-gastric junction (EGJ) dysfunction. This study aims to assess the superiority of the AFS classification over the Hill and to weight the single AFS components contribution. A retrosp

Laparoscopic Revision of Sleeve Gastrectomy with Only Hiatal Hernia Repair for Gastroesophageal Reflux Disease: A Retrospective Study.

INTRODUCTION: Occurrence of gastroesophageal reflux disease (GERD) after laparoscopic sleeve gastrectomy (LSG) has been the subject of numerous studies and is certainly multifactorial in origin. We believe that conversion to Roux-en-Y gastric bypass (RYGB) is the gold standard, but it may not be suitable for all patients. METHODS: We retrospectively examined 6500 patients who underwent LSG between January 2015 and February 2023 who developed 6 months of GERD unresponsive to medical therapy with descriptive analysis. 240 patients met the inclusion criteria for the study. Association of GERD with Sleeve Gastrectomy: An Unintended Consequence.

PURPOSE OF REVIEW: Bariatric surgery is a highly effective treatment for obesity that yields durable weight loss with significant improvement or resolution of T2D and other weight-related chronic cardiometabolic diseases. While the advantages of laparoscopic sleeve gastrectomy (LSG), the most performed bariatric surgery procedure, include procedural simplicity, short operating time, lower complication rate, durable weight loss, and significant improvement including remission of type 2 diabetes, a major drawback is gastroesophageal reflux disease (GERD). The purpose of this review is to summari

Adjunctive probiotic therapy sustains symptom relief in gastroesophageal reflux disease through gut microbiome-metabolome remodeling.

Proton pump inhibitors (PPIs) are standard therapy for gastroesophageal reflux disease (GERD), but long-term use causes dysbiosis, gastrointestinal side effects, and symptom relapse after discontinuation. Probiotics may offer adjunctive benefits by modulating the gut ecosystem. The study aimed to evaluate the efficacy of a multi-strain probiotic (Lihuo) with rabeprazole in GERD and its impact on gut microbiota and metabolome. A randomized, double-blind, placebo-controlled trial was conducted in 120 GERD patients assigned to receive rabeprazole with either Lihuo (n = 64) or placebo (n = 56) for

Infant Reflux Trial: study protocol for a multicentre randomized controlled trial on the treatment of gastroesophageal reflux disease in infants.

BACKGROUND: Gastroesophageal reflux disease (GERD) is a common condition in infants below 1 year of age. Symptoms include frequent regurgitation, failure to thrive, food refusal, irritability, and back arching. While many infants experience some degree of physiological reflux, persistent or severe symptoms may indicate GERD. One important underlying cause of GERD in infants is allergy to cow's milk protein. Current international guidelines recommend a trial of a cow's milk protein-free diet prior to initiating medical therapy with a proton-pump inhibitor. However, the efficacy of both the diet

CYP2C19 genotype-guided dosing of proton pump inhibitors: progress in clinical trials and real-world use.

INTRODUCTION: Proton pump inhibitors (PPIs) are widely used to treat acid-related disorders; however, treatment response varies significantly due to Cytochrome P450 2C19 (CYP2C19) genetic polymorphisms that alter individual drug metabolism. Such variation can lead to insufficient acid suppression, resulting in treatment failure or adverse events. Genotype-guided PPI therapy represents an important step toward personalized gastroenterology by optimizing drug efficacy and safety. AREAS COVERED: This review summarizes evidence from clinical trials and meta-analyses examining CYP2C19-mediated dif

Development of a diagnostic model for esophageal achalasia assessed by esophageal high-resolution manometry using artificial intelligence.

Esophageal high-resolution manometry (HRM) is an important tool for diagnosing and assessing esophageal achalasia. Artificial intelligence (AI)-assisted HRM image processing has the potential to aid in the diagnosis of esophageal achalasia. However, addressing the challenges associated with the 'black-box' problem is important. In the present study, an automated system that utilizes AI with class-activation maps to highlight diagnostic areas in HRM images was developed. A total of 211 HRM images, which led to the diagnosis of controls and patients with achalasia, were used to train the system

Investigating the utility of EndoFLIP™ as a screening tool for pathologic reflux EndoFLIP™ as a screening tool for GERD.

BACKGROUND: The Endoluminal Functional Lumen Imaging Probe (EndoFLIP™) is increasingly utilized in the diagnostic evaluation of gastroesophageal reflux disease (GERD). At our Center, EndoFLIP™ is routinely performed before Bravo™ pH monitoring during preoperative esophagogastroduodenoscopy to exclude major motility disorders in patients being considered for antireflux surgery. According to Lyon Consensus 2.0, the presence of Los Angeles (LA) grade B or higher esophagitis is diagnostic for GERD, obviating the need for pH testing. In select cases, bypassing pH monitoring reduces costs, improves
