Treatment of eosinophilic esophagitis with immune modulating agents.

Background: Eosinophilic esophagitis (EoE) is a chronic inflammatory and an immune-mediated condition diagnosed based on both clinical and pathologic findings. EoE has been identified as one of the leading causes of esophageal dysfunction worldwide. If untreated, EoE often follows a progressive course that leads to fibrosis and esophageal stricture and perforation. The use of biologic therapies that target specific pathways will help us understand the essential pathophysiologic steps in EoE, in addition to providing symptomatic treatment. In this narrative review, we review the potential targe

A multidisciplinary Delphi consensus on the formulation of an optimized oral diet for dysphagia and malnutrition (FOOD-DM).

BACKGROUND & AIMS: Dysphagia and malnutrition frequently co-occur and are associated with aspiration pneumonia, sarcopenia, increased mortality, reduced quality of life and restricted social participation. Yet foods and drinks for individuals with dysphagia and malnutrition are inconsistently specified and lack objective standards. This study aimed to develop a consensus-based, expert-derived framework for evaluating foods and drinks perceived suitable for individuals with dysphagia and malnutrition (FOOD-DM). METHODS: A two-round online modified Delphi study was conducted (October 2024-May 2

Using the EVendo Score to Predict Varices and Varices Needing Treatment in Hepatocellular Carcinoma Patients Planned for Atezolizumab-Bevacizumab.

PURPOSE: Atezolizumab-Bevacizumab (AB) is first-line treatment for patients with hepatocellular carcinoma (HCC), and Bevacizumab is associated with increased gastrointestinal bleeding; hence IMBrave150 mandated esophago-gastroduodenoscopy (OGD) before therapy initiation. Universal pre-treatment OGD potentially delays treatment. Non-invasive scores have been developed to risk stratify patients for variceal screening to avoid unnecessary OGD. EVendo Score (ES) was developed with laboratory and clinical data, and demonstrated reliability in identifying patients with cirrhosis with gastroesophagea

Three-Year Outcomes of Reflux Esophagitis and Late Complications After FundoRing Versus Standard One-Anastomosis Gastric Bypass: a Randomized Controlled Trial.

BACKGROUND/OBJECTIVES: The FundoRing gastric bypass method has suggested improved antireflux effects after one year of follow-up compared to standard one-anastomosis gastric bypass (OAGB). However, longer-term impact on adverse effects remains unclear. This study aimed to compare three-year outcomes of reflux esophagitis and other late postoperative complications between the FundoRing method and standard OAGB. This manuscript presents a pre-specified secondary analysis of these endpoints from a single-center RCT. This article is structured as an original research report with detailed technical

Years of Life Lost of Patients With Early Adenocarcinoma of the Esophagogastric Junction Compared to Esophageal or Gastric Adenocarcinoma. A Registry-Based Study.

OBJECTIVE: This registry-based study investigates patients with resected esophageal (EAC), cardiac (CAC), and gastric (GAC) pT1 adenocarcinoma (AC) concerning epidemiological, tumor-related, and prognostic factors, analyzing especially the impact on survival and years of life lost (YLL). BACKGROUND: As CAC is classified as esophageal cancer in the 7th and more specified in the 8th edition of the UICC-TNM classification, it remains unclear if significant differences exist for pT1 CAC in comparison to EAC and GAC. METHODS: We used the ICD-O-3 code comparing tumors in the esophagus (C15.0-C15.9

Prevalence of Claudin 18.2 Expression in Gastric and Gastroesophageal Junction Adenocarcinoma: A Systematic Review and Meta-Analysis.

BACKGROUND: Claudin 18 isoform 2 (CLDN18.2) has emerged as a clinically validated therapeutic target in gastric and gastroesophageal junction (GEJ) adenocarcinoma following the regulatory approval of zolbetuximab in combination with first-line chemotherapy. Accurate prevalence data at the clinically validated immunohistochemical threshold are essential for patient selection, healthcare resource planning, and treatment strategy. Reported prevalence estimates vary widely across studies due to differences in populations, methodologies, and immunohistochemical protocols. This systematic review and

Gaps in Guidelines on Long-Term Management Strategies for Post-Stroke Dysphagia.

BACKGROUND: Post-stroke dysphagia (PSD) is a frequent yet overlooked complication of stroke with significant implications for rehabilitation. While international guidelines provide structured recommendations for early screening and management, guidance on long-term care remains inconsistent. This position paper synthesizes existing guidelines, identifies critical gaps, and highlights the need for standardized long-term management strategies. METHODS: Guidelines on PSD management were identified through a two-step approach. Fourteen guidelines were included from a previous systematic review (2

Camrelizumab plus chemotherapy versus chemoradiotherapy as neoadjuvant therapy for resectable esophageal squamous cell carcinoma: Phase 2 randomized trial (REVO).

Several studies have evaluated PD-1 inhibitors plus chemotherapy as neoadjuvant treatment for locally advanced esophageal squamous cell carcinoma (ESCC), but comparative data with chemoradiotherapy (CRT) remain limited. This multicenter, randomized, open-label, phase 2 non-inferiority trial (REVO, NCT05007145) assessed the efficacy and safety of neoadjuvant camrelizumab plus chemotherapy (ICT) versus CRT in patients with resectable, locally advanced ESCC. A total of 104 patients were randomized to ICT (camrelizumab, nab-paclitaxel, cisplatin) or CRT (nab-paclitaxel, cisplatin, radiotherapy). T

Factors Affecting Treatment Resilience in Patients With Oesophago-gastric Cancers Undergoing Palliative Chemotherapy: A Rapid Review.

AIMS: Oesophago-gastric cancers are the fifth most common in the UK. Most patients present with advanced disease and are unsuitable for curative surgery, instead receiving palliative treatment to improve prognosis and symptom burden. Treatment resilience refers to the ability of patients to tolerate their anti-cancer treatment. Palliative chemotherapy can result in significant toxicity; almost 40% of patients are unable to complete their chemotherapy regimen, with this proportion rising significantly in older and frailer patients. Despite most cases occurring in patients over 70, older and fra

Fluoroscopy Time of Radiologist-Monitored Modified Barium Swallow Exams in Oncology: A 3-year Single Institution Retrospective Study.

Fluoroscopy time is an important metric for radiation safety, but how it is related to dysphagia severity as graded by Dynamic Grade of Swallowing Toxicity (DIGEST) criteria and other factors in oncology practice is undocumented. We evaluated the fluoroscopy time for the bolus protocol used at the originating institution of the DIGEST method and assessed the relationship between fluoroscopy time and DIGEST grade, exam indication, and additional parameters. Eleven trials, including specified VARIBAR® barium volumes, were included in the standard bolus protocol. Electronic health record (EHR) 20

Competing risks multi-state model for time-to-event data analysis of HIV/AIDS: a retrospective cohort national datasets, Ethiopia.

INTRODUCTION: Tuberculosis (TB) remains the most common opportunistic infection and leading cause of death among individuals living with HIV/AIDS in Ethiopia. Its significant impact on morbidity and mortality underscores the crucial link between these two diseases. While the advent of antiretroviral therapy (ART) has led to a dramatic decline in mortality rates among HIV/AIDS patients, TB continues to pose a substantial threat. This study aims to estimate the probability of death due to TB among HIV/AIDS patients on ART, considering the presence of various competing risks, including diarrhea,

Multi-omic features of oesophageal adenocarcinoma in patients treated with preoperative neoadjuvant therapy.

Oesophageal adenocarcinoma is a poor prognosis cancer and the molecular features underpinning response to treatment remain unclear. We investigate whole genome, transcriptomic and methylation data from 115 oesophageal adenocarcinoma patients mostly from the DOCTOR phase II clinical trial (Australian New Zealand Clinical Trials Registry-ACTRN12609000665235), with exploratory analysis pre-specified in the study protocol of the trial. We report genomic features associated with poorer overall survival, such as the APOBEC mutational and RS3-like rearrangement signatures. We also show that positron

Worldwide trends in esophageal cancer survival, by sub-site, morphology, and sex: an analysis of 696,974 adults diagnosed in 60 countries during 2000-2014 (CONCORD-3).

BACKGROUND: Esophageal cancer survival is poor worldwide, though there is some variation. Differences in the distribution of anatomical sub-site and morphological sub-type may help explain international differences in survival for all esophageal cancers combined. We estimated survival by anatomic sub-site and morphological sub-type to understand further the impact of topography and morphology on international comparisons of esophageal cancer survival. METHODS: We estimated age-standardized one-year and five-year net survival among adults (15-99 years) diagnosed with esophageal cancer in each

A qualitative exploration of how oral trials are used in dysphagia management in one inpatient hospital.

BACKGROUND: The lack of high-quality evidence to support specific treatment approaches has been widely documented in the existing literature, with evidence suggesting speech and language therapists (SLTs) frequently rely on experience and expert opinion to inform treatment. One approach that is commonly used within dysphagia management, in spite of a lack of existing evidence to support its efficacy, is the use of oral trials, otherwise known as swallow trials or tasters. This approach involves offering specified, limited amounts of oral diet or fluids for those at risk of aspiration or chokin
