Efficacy and safety of upper gastrointestinal endoscopy in the neonatal intensive care unit.

BACKGROUND: Upper gastrointestinal (GI) disorders frequently lead to neonatal intensive care unit (NICU) admissions; however, conventional imaging often fails to detect atypical presentations. The development of miniaturized endoscopes has progressively expanded the feasibility of upper GI endoscopy in neonates. Evidence regarding the diagnostic utility and safety of upper GI endoscopy in neonates remains limited, and neonatal-specific guidelines are currently absent in China. This study aimed to evaluate the diagnostic value and safety of upper GI endoscopy in NICU infants. METHODS: We retros

A Case of Refractory Peptic Ulcer with Choledochoduodenal Fistula Successfully Treated by Biliary Stent Placement Using an Ultrathin Endoscopic Rendezvous Technique: A Case Report.

Choledochoduodenal fistula (CDF) is an abnormal communication between the common bile duct and the duodenum. While gallstones are the primary cause, peptic ulcer-related CDF has become rare due to the widespread antiulcer therapy. Nevertheless, CDF remains clinically relevant as symptoms are often vague and diagnosis is frequently incidental. We report the case of a 79-year-old woman with vomiting, epigastric pain, and back pain due to a refractory duodenal ulcer complicated by CDF. Endoscopic retrograde cholangiopancreatography (ERCP) using a standard duodenoscope was attempted to facilitate

Genome-Wide Dissection of the Neutrophil-to-Lymphocyte Ratio Uncovers Polygenic Determinants Linked to Inflammatory Gastrointestinal Disorder Susceptibility.

Background: The neutrophil-to-lymphocyte ratio (NLR) is a simple biomarker that reflects the balance between innate immune response and adaptive immunity. Currently, the genetic basis and clinical implications of NLR in relation to inflammatory gastrointestinal diseases have not been extensively explored. Methods: We carried out a genome-wide association study (GWAS) on European individuals from the UK Biobank to detect genetic variants related to NLR, followed by post-GWAS analyses including colocalization analysis, transcriptome-wide association studies (TWAS), and LD score regression. Logis

Novel factors associated with surgical conversion for duodenal perforation: case control study.

BACKGROUND: Based on initial evaluation using indication criteria, conservative treatment or surgery is selected for duodenal ulcer perforation. However, some patients fail conservative management and require surgical intervention. This study aimed to identify novel predictive factors during initial assessment that signal the need for conversion to surgery. METHODS: This retrospective cohort study analyzed 27 patients who underwent initial conservative management for duodenal ulcer perforation at a single institution between January 2012 and July 2023. The surgical conversion rate was evaluat

Association between intraoperative irrigation volume and postoperative abdominal abscess in perforated peptic ulcer.

BACKGROUND: Perforated peptic ulcer is a critical emergency disease requiring surgery, and postoperative intra-abdominal abscess (IAA), an organ/space surgical site infection (SSI), is a common complication. Intraoperative irrigation is a common IAA prevention approach; however, scientific evidence supporting its effectiveness is lacking. We aimed to investigate the association between intraoperative irrigation volume and IAA using multicentre data. METHODS: This multicentre retrospective cohort study was conducted between January 2011 and December 2023. The study included patients aged ≥ 18

Perforated Duodenal Ulcer in the Immediate Postpartum Period: Diagnostic Pitfalls Following Caesarean Delivery.

Postpartum abdominal pain is frequently encountered and is most often related to obstetric causes such as uterine involution, infection, or postoperative discomfort following cesarean delivery; however, more atypical etiologies can also occur. Perforated peptic ulcer disease in the immediate postpartum period is uncommon and may be overlooked, particularly when symptoms overlap with expected postoperative findings and recent surgical history. We report a woman in her early 30s who developed severe upper abdominal pain four days after a primary cesarean delivery performed for maternal exhaustio

Clinical outcomes of transcatheter arterial embolization in patients with angiography-negative duodenal ulcer bleeding after failed endoscopic hemostasis.

BACKGROUND: Duodenal ulcer bleeding is a common cause of non-variceal upper gastrointestinal bleeding. Although endoscopic hemostasis is recommended as the first-line treatment, a subset of patients experience failure of endoscopic therapy and require further intervention, for which transcatheter arterial embolization (TAE) has become a commonly used salvage treatment. In clinical practice, however, angiography may fail to demonstrate definite active bleeding, posing challenges for subsequent therapeutic decision-making. Under such circumstances, empiric embolization is frequently performed, y

Ischemic Duodenal Ulcers Secondary to Superior Mesenteric Artery Stenosis in a Patient With an Aberrant Common Hepatic Artery: A Case Report and Review of the Literature.

A 77-year-old male patient presented with persistent abdominal pain, nausea, vomiting, and diarrhea. Esophagogastroduodenoscopy (EGD) revealed duodenal ulcers suggestive of ischemia. Vascular imaging demonstrated a replaced common hepatic artery arising from the superior mesenteric artery (SMA), with the gastroduodenal artery originating from this SMA-dependent circulation, consistent with Michels' type IX anatomy. Significant stenosis at the SMA origin was identified. Endovascular stenting of the SMA resulted in symptom resolution and complete endoscopic healing of the ulcers. This case highl

Perforated marginal ulcers - what are the surgical options - simple closure or is definitive surgery needed? A case report.

INTRODUCTION AND IMPORTANCE: Marginal ulceration is an uncommon complication following gastrojejunostomy. Perforation of such ulcers is rare but potentially life-threatening, particularly in the era of widespread proton pump inhibitor use. CASE PRESENTATION: We report a 54-year-old man and heavy smoker who presented with acute abdominal pain and vomiting, 3 years after an open gastrojejunostomy performed for a complicated duodenal ulcer. Clinical examination revealed diffuse epigastric tenderness and signs of peritonitis. Laboratory tests showed leukocytosis. Abdominal CT revealed pneumoperit

[A Case of Gastric Remnant Cancer with Multiple Liver Metastases Treated with SOX + Nivolumab Therapy and Conversion Surgery].

A male patient in his 60s with a history of distal gastrectomy (Billroth Ⅱ) for duodenal ulcer 40 years ago. Fecal occult blood was positive, and endoscopy revealed a type 3 tumor at the anastomotic site. He was referred to our hospital and diagnosed with remnant gastric cancer and multiple liver metastases (cT4aN1M1, Stage ⅣB). He received 18 courses of SOX + nivolumab chemotherapy. Liver metastases disappeared, but PET-CT showed residual uptake at the primary site. Laparoscopic total remnant gastrectomy with D2 lymph node dissection was performed as a conversion surgery. Pathology showed no

Silent Aggressor: Unveiling Large-Cell Duodenal Neuroendocrine Carcinoma.

Duodenal neuroendocrine neoplasms (D-NENs) are uncommon tumors that account for a fraction of duodenal tumors. Although often incidentally discovered, those in the ampullary or peri-ampullary regions may present with symptoms due to biliary or gastrointestinal obstruction, including obstruction or bleeding. We describe a 65-year-old male who presented with fatigue, altered mentation, and melena. His evaluation was notable for severe anemia, metabolic derangements, and laboratory findings with concern for tumor lysis syndrome. Emergent management with hemodialysis and supportive care stabilized

Narrative review of extracorporeal membrane oxygenation utilization in surgical management of tracheal intubation injuries.

BACKGROUND AND OBJECTIVE: Tracheobronchial injuries are a rare complication of intubation but carry a high mortality. When surgical airway repair is necessary, veno-venous extracorporeal membrane oxygenation (VV-ECMO) may be required to maintain gas exchange. To better understand contemporary utilization of VV-ECMO in surgical airway repairs, we undertook a comprehensive literature review. METHODS: Using the PubMed database, we identified 1,013 potentially relevant publications and 13 of these, describing 14 patient cases, met criteria. KEY CONTENT AND FINDINGS: Demographics and histories we

Ethnobotanical Evidence of Medicinal Plants Used for Peptic Ulcers in Tanzania: A Systematic Review.

Medicinal plants (MPs) are essential to rural communities in low- and middle-income countries. However, knowledge of the use of MPs for peptic ulcer disease (PUD) in Tanzania remains limited. This review aims to document the traditional knowledge and MPs used by Tanzanians to treat and manage PUD. A comprehensive search was conducted across databases and grey literature, adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data from 26 ethnobotanical studies were analysed using descriptive statistics (means, graphs and charts) in Microsoft Exc

Postoperative Findings Mimicking Duodenal Leak After Graham Patch Repair: Intact Repair With Retroperitoneal Abscess.

Perforated duodenal ulcers represent a surgical emergency requiring prompt operative intervention. The Graham omental patch is the most commonly performed procedure for emergent repair; however, postoperative findings suggestive of leakage remain a serious concern and are associated with significant morbidity and mortality. Traditionally, suspected postoperative leaks have prompted early surgical re-exploration due to the risk of peritonitis and sepsis. However, select patients with concerning postoperative findings may be managed successfully with appropriate drainage, decompression, and nutr

Helicobacter pylori eradication in the face of escalating anti-microbial resistance: Geographic heterogeneity and emerging treatment strategies.

Helicobacter pylori (H. pylori) infection is a global health problem that affects more than half of the world population and leads to gastrointestinal disorders such as gastric ulcer, duodenal ulcer and gastric cancer. The available treatment options are inefficient because of patient non-adherence to the regimen due to the polypharmacy approach of treating H. pylori over longer periods. The bio-availability of antibiotics at the gastric mucosa, where the organism resides, is also a major concern. The continuous adaptation of bacteria such as genetic mutation toward beta-lactamase, macrolide a

Duodenal ulcer perforation presenting as acute appendicitis in a child: a case report and literature review.

INTRODUCTION: Duodenal ulcer perforation (DUP) is exceedingly rare in the pediatric population. Its clinical presentation frequently mimics acute appendicitis (AA), leading to a high risk of preoperative misdiagnosis and posing substantial diagnostic and therapeutic challenges. CASE PRESENTATION: We describe a 12-year-old boy who presented with signs and symptoms suggestive of acute appendicitis. Emergency diagnostic laparoscopy revealed a 0.6-cm perforation on the anterior wall of the duodenal bulb. The perforation was successfully repaired laparoscopically. The patient had an uneventful pos
