tRNA regulatory ecosystem states shape tissue and circulating small-RNA landscapes in gastric cancer.

BACKGROUND: Gastric cancer is a heterogeneous disease shaped by the co-evolution of malignant epithelial programs, stromal remodeling, and immune contexture. Transfer RNA (tRNA) regulation sits at a strategic junction in this biology: it governs translational capacity and selectivity while generating tRNA-derived fragments (tRFs) and tRNA halves (tiRNAs)-a family of regulatory small RNAs detectable in both tissue and blood. OBJECTIVE: We asked whether the coordinated machinery responsible for tRNA transcription, maturation, modification, charging, and cleavage defines biologically meaningful

Successful conservative management of hepatic portal venous gas complicating severe immune-related gastrointestinal injury during pembrolizumab-based concurrent chemoradiotherapy for cervical cancer: a mechanistic case report.

Concurrent chemoradiotherapy (CCRT) combined with pembrolizumab has become the standard treatment for high-risk locally advanced cervical cancer, increasing the need for prompt recognition of immune-related adverse events (irAEs). We report a rare case of severe gastrointestinal irAE complicated by hepatic portal venous gas (HPVG) during pembrolizumab-based CCRT. A 72-year-old woman with FIGO 2018 stage IIIC1r cervical squamous cell carcinoma developed persistent diarrhea and anorexia during treatment, followed by sudden hematemesis after the first brachytherapy session. Upper gastrointestinal

Cross-organ microbes modulate host upper gastrointestinal health and disease via the oral-gastric microbiome axis.

The human body harbors highly diverse and complex microbial communities across various regions. Among these, the oral cavity and stomach are directly connected and engage in dynamic microbial interactions. Microorganisms capable of migrating between these two distinct niches are referred to as trans-organ microbes, which constitute a core component of the oral-gastric microbiome axis. In this review, we elucidate the mechanisms underlying microbial translocation along the oral-gastric axis. We further discuss how these cross-organ microbes exert bidirectional influences on health and disease i

Thyroid storm revealing suspected evolving autoimmune polyglandular syndrome type 2 with steroid-limited assessment of autoimmune adrenal insufficiency: a case report.

BACKGROUND: Autoimmune polyglandular syndrome type 2 (APS-2) is a rare autoimmune disorder characterized by the coexistence of autoimmune adrenal insufficiency with autoimmune thyroid disease and/or type 1 diabetes mellitus. Its heterogeneous and progressive presentation frequently delays diagnosis, particularly during acute endocrine emergencies where one dominant manifestation may obscure concurrent glandular dysfunction. Recognition of evolving autoimmune disease before overt adrenal insufficiency develops remains particularly challenging, especially when emergency corticosteroid therapy is

Eosinophilic Gastroenteritis during Tocilizumab Treatment for Castleman Disease.

Eosinophilic gastroenteritis (EGE) is a rare inflammatory disorder characterized by eosinophilic infiltration of the gastrointestinal tract and its associated symptoms. We report a case of EGE in a patient with idiopathic multicentric Castleman disease and asthma, who developed gastrointestinal symptoms and eosinophilia during tocilizumab therapy. Endoscopy revealed marked gastric mucosal injury and biopsy confirmed eosinophilic infiltration. Despite normal intestinal endoscopic findings, the biopsy specimens revealed eosinophilic infiltration. The discontinuation of tocilizumab and initiation

Beyond seropositivity: quantitative parietal cell antibody titers predict gastric intestinal metaplasia in a high helicobacter pylori prevalence setting.

PURPOSE: Anti-gastric parietal cell antibody (APA) positivity is a recognized serological biomarker for autoimmune gastritis, but whether quantitative APA titer levels correlate with the severity of gastric intestinal metaplasia (GIM) in high Helicobacter pylori (H. pylori) prevalence settings remains unclear. This study aimed to evaluate the association between quantitative APA titer dilutions and histopathological GIM in a large endoscopic cohort. METHODS: In this retrospective cohort study, medical records of adult patients who underwent APA testing at a tertiary referral center between No

Synergistic effect of gastric microbiome and host genetic factor predispose to gastric cancer.

OBJECTIVE: Complex interaction between host genetic and environmental factors has a role in Helicobacter pylori (H. pylori)-related gastric tumorigenesis, while involvement of non-Helicobacter pylori gastric microbiome may be also relevant to this process. We aimed to evaluate whether combining the non-Helicobacter pylori gastric microbiome signatures and the gastric cancer (GC) predisposing genetic factors (PSCA rs2294008 and MUC1 rs4072037 polymorphisms) stratify a high GC risk subpopulation in a Japanese cohort. METHODS: In 620 subjects including 219 GC patients and 410 non-cancers, microb

Gastric dysplasia: an integrated classification of surface epithelium-derived precursor lesions.

Gastric dysplasia comprises a heterogeneous group of non-invasive epithelial neoplasms with distinct clinicopathological and biological characteristics. This review focuses on surface epithelium-derived lesions, including intestinal-type, foveolar-type, and serrated dysplasia/adenoma. Conventional low-grade intestinal-type dysplasias/adenomas arise mainly in atrophic and metaplastic mucosa associated with Helicobacter pylori infection or, much less frequently, autoimmune gastritis. Small, pale, flat-elevated lesions with orderly small-intestinal differentiation generally behave indolently, whe

Loss of UBE3C-mediated PIM2 ubiquitination drives Helicobacter pylori-induced gastritis via JAK2/STAT3 activation.

Helicobacter pylori (H. pylori) infection is a major cause of chronic gastritis, yet its molecular mechanisms remain incompletely understood. In this study, we found that the proviral integration site for Moloney murine leukemia virus kinase 2 (PIM2) was significantly upregulated in H. pylori-infected gastric tissues and cells. Functionally, PIM2 knockdown not only attenuated inflammatory responses both in vivo and in vitro but also significantly reduced gastric H. pylori colonization in mice. Mechanistically, H. pylori infection downregulated the expression of the E3 ubiquitin ligase UBE3C, l

From inflammation to carcinogenesis: MiRNA regulates epithelial-mesenchymal transition to facilitate malignant progression of gastric precancerous lesions.

The process of chronic gastritis transforming into gastric cancer (GC) follows the classic Correa cascade, with persistent inflammation driving the stepwise transition from normal gastric mucosa through superficial gastritis, atrophic gastritis, intestinal metaplasia, dysplasia, and ultimately carcinoma. Among these stages, intestinal metaplasia and dysplasia occurring on an atrophic mucosal background are widely regarded as the most common gastric precancerous lesions (GPL) because of their pronounced carcinogenic risk. Chronic inflammation can induce epithelial-mesenchymal transition (EMT),

[ASSOCIATION BETWEEN PEDIATRIC EOSINOPHILIC GASTROINTESTINAL DISORDERS AND NEURODEVELOPMENTAL DISORDERS: A CLINICAL INVESTIGATION].

BACKGROUND: Eosinophilic gastrointestinal disorders (EGIDs) are classified into eosinophilic esophagitis (EoE) and non-EoE EGIDs, with the latter being more common in Japan. Although recent studies have suggested an association between EGIDs and neurodevelopmental disorders (ND), domestic data remain limited. This study examined the relationship between ND and non-EoE EGIDs. METHODS: We evaluated 33 children diagnosed with non-EoE EGIDs and determined the prevalence of comorbid ND. Clinical characteristics-including sex, age at onset, treatment patterns, and relapse-were examined according to

Targeting the Correa's cascade of gastric cancer: the potential role of phytochemicals and autophagy modulation.

Gastric cancer (GC) development follows a sequential and complex pathophysiological process known as Correa's cascade, progressing from chronic gastritis (active or inactive) to precancerous lesions (intestinal metaplasia (IM) and dysplasia) and ultimately to adenocarcinoma. Current GC therapies have limited efficacy and high recurrence rates. Autophagy, a cellular self-degradation mechanism that removes damaged or harmful components, plays a critical role in maintaining cellular homeostasis. Phytochemicals, owing to their low toxicity and multi-target pharmacological properties, exhibit anti-

Diagnostic value of a serum four-biomarker panel for atrophic gastritis in women under 60 years of age.

BACKGROUND: While endoscopy is considered the gold standard for diagnosing chronic atrophic gastritis (CAG), serum biomarkers such as pepsinogen I (PG1), pepsinogen Ⅱ (PG2), the PGⅠ/PGⅡ ratio, and gastrin-17 (G-17) have been investigated for their diagnostic value in CAG. However, studies to date have reported inconsistent results regarding their accuracy and clinical utility. METHODS: All 567 participants underwent upper gastrointestinal endoscopy and serum biomarkers were examined for PG1, PG2 and G-17 using Gastric Function Triple-Parameter test kit (Biohan Biotechnology Co., Ltd).
