Complete Resolution of Unprovoked Left Renal Vein Thrombosis and Pulmonary Embolism Associated With Severe Iron Deficiency Anemia: A Five-Year Follow-Up Case Report and Dual-Energy CT Appraisal.

Combined renal vein thrombosis (RVT) and pulmonary embolism (PE) in young adults without nephrotic syndrome, malignancy, or an identified major thrombophilia is uncommon. Severe iron deficiency anemia (IDA) has been associated with increased thrombotic risk, although a causal relationship cannot be established from an individual case. A 21-year-old male patient presented with flank and chest pain. Diagnostic workup confirmed acute left RVT and an acute embolism of the posterior basal segmental artery of the right lower lobe with pulmonary infarction, concurrent with profound microcytic IDA (he

Acute Phlegmonous Gastritis: Insights Into Etiology, Diagnosis, and Clinical Management.

Acute phlegmonous gastritis is a rare and life-threatening form of gastritis characterized by suppurative inflammation of the gastric wall. We present the case of a 66-year-old female with a history of diabetes and hypertension who presented with abdominal pain, vomiting, and diarrhea, progressing to refractory shock and severe metabolic acidosis. Abdominal imaging revealed diffuse thickening of the gastric wall with surrounding fat strands, suggesting acute phlegmonous gastritis. Despite aggressive resuscitation and critical care management, the patient's condition continued to deteriorate, a

Epigastric Pain Caused by Left Upper Quadrant Acute Appendicitis in the Setting of Undiagnosed Intestinal Malrotation.

Acute appendicitis is a common surgical emergency that may present atypically due to anatomical variations. We report a case of a previously healthy male in his forties presenting with epigastric abdominal pain, vomiting, and fever, initially managed as gastritis. Contrast-enhanced computed tomography (CT) of the abdomen revealed acute appendicitis in the left upper quadrant on a background of previously undiagnosed intestinal malrotation. The patient underwent emergency laparoscopic appendectomy, with intensive perioperative care for septic shock. He ultimately made a full recovery. This case

A stroke of bubbles: Cerebral air embolism following accidental hydrogen peroxide ingestion in a patient with incidental atrial fibrillation.

BACKGROUND: Cerebral air embolism (CAE) is a rare but potentially devastating complication of concentrated hydrogen peroxide (H₂O₂) ingestion. Distinguishing CAE from other embolic sources is critical for appropriate acute management, especially when alternative etiologies coexist. CASE DESCRIPTION: An 82-year-old woman presented with acute left-sided weakness after accidentally ingesting 50 mL of 35% H₂O₂. While 48-hour Holter monitoring incidentally detected paroxysmal atrial fibrillation (AFib) during the workup, brain MRI revealed a characteristic "scattered cortical pattern" of punctate

Corticosteroid-Associated Adverse Events in Chronic Spontaneous Urticaria: A US Claims Data Study.

INTRODUCTION: Chronic spontaneous urticaria (CSU) is a skin disorder for which short-course systemic corticosteroids are recommended only as rescue medication during acute exacerbations. Here, we describe real-world corticosteroid use, adverse events (AEs) occurring post-corticosteroid initiation, and health care resource utilization (HCRU) of patients with CSU in the USA. METHODS: This retrospective cohort study used data from the US HealthVerity claims database of adults with diagnosed CSU (January 2016-March 2023). Only prescription and non-antihistamine claims were included in the analysi

Diagnostic Overlap Between Uremia and Severe Hyperkalemia in Chronic Kidney Disease: Emphasizing Laboratory-Guided Urgency Despite Absent EKG Changes.

Hyperkalemia is a potentially life-threatening electrolyte disturbance often associated with characteristic electrocardiographic (EKG) changes and neuromuscular manifestations. However, clinical presentation does not always correlate with biochemical severity. We present the case of a 53-year-old female with chronic kidney disease (CKD) who developed acute-on-chronic kidney injury with severe hyperkalemia (serum potassium 8.7 mmol/L) and elevated urea (94 mg/dL), yet without EKG abnormalities or overt symptoms. The patient reported persistent nausea for 24 hours without abdominal pain or hemod

The Puncture, Aspiration, Injection and Re-aspiration (PAIR) Technique of Hepatic Hydatid Cyst: A Case Report and Review of Literature.

Hydatid disease, caused by Echinococcus granulosus, commonly affects the liver and can lead to complications such as rupture, secondary infection, and anaphylaxis. While surgery is the traditional treatment for complicated cases, minimally invasive techniques are emerging as viable alternatives. We report a case of a 27-year-old female with a history of gastritis and a pancreatic cyst who presented with acute diffuse abdominal pain and nausea. Imaging revealed an 11.8×8.8×5.2 cm cystic structure in the left hepatic lobe, consistent with a hydatid cyst, with free intraperitoneal fluid concernin

Intestinal Malrotation Through Acute Obstructive Abdomen in an Adolescent From Northeastern Brazil: A Case Report.

Intestinal malrotation is a congenital anomaly of the midgut resulting from abnormal embryonic rotation. Although predominantly a neonatal diagnosis, it is an extremely rare, isolated finding in adolescents, often presenting with nonspecific symptoms that lead to diagnostic delays and increased morbidity. We report a 17-year-old male presenting with acute abdominal pain, distension, and vomiting. Diagnosis was established via contrast-enhanced computed tomography (CT), which identified spiral rotation of mesenteric vessels and colonic displacement. The patient initially underwent a laparoscopi

Massive Fecaloma Dissolved by Coca-Cola: A Case Report.

Fecalomas are hardened masses of fecal material that are a rare complication of chronic constipation and require surgical resection when conservative measures fail. Recent reports suggest that Coca-Cola, traditionally used to dissolve gastric phytobezoars, may also aid in the dissolution of fecalomas. We report the case of an 82-year-old woman with dementia, type 2 diabetes, hypertension, hypothyroidism, chronic gastritis, and depression who presented with acute abdominal pain. She had not produced stool or passed gas per rectum for several days prior to admission. Physical examination reveale

Vitamin B6 Ameliorates Lipopolysaccharide-Induced Inflammation in Mouse Gastric Organoids.

Gastric mucosal inflammation is a critical precondition of various gastric diseases, and development of safe and effective anti-inflammatory agents for prompt resolution of acute inflammation at early stage is vital to preventive medicine. B vitamins are essential water-soluble nutrients with reported anti-inflammatory properties, yet their potent regulatory effects in gastric mucosal inflammation are not systemically assessed. In this study, we adopted a lipopolysaccharide (LPS)-induced inflammatory mouse gastric organoid model to screen for anti-inflammatory B vitamin family members. With th

Traditional herbal medicine Hyangsapyeongwi-san inhibits alcohol-induced gastric injury in mice by regulating NLRP3 inflammasome signaling.

ETHNOPHARMACOLOGICAL RELEVANCE: Hyangsapyeongwi-san (HPS), known as Xiangsha Pingwei San in Chinese medicine, is a classical herbal formula composed of ten medicinal herbs that has been traditionally used in traditional Korean medicine and East Asian medicine for centuries to treat gastrointestinal disorders associated with digestive impairment, including functional dyspepsia and gastritis. In traditional Korean medicine theory, these conditions correspond to "dampness-heat accumulation" and "qi stagnation" patterns affecting the spleen and stomach. HPS is formulated to "eliminate dampness, st

Duration-Dependent Adverse Events of Systemic Glucocorticosteroids in Atopic Dermatitis: A Nationwide Claims Cohort Study.

INTRODUCTION: Systemic glucocorticosteroids (SGCs) are recommended only for short-term treatment of atopic dermatitis (AD) because of their side effect profile. Nevertheless, prolonged and repeated use remains common in routine care. Evidence on whether treatment duration or cumulative dose is the main driver of adverse events is limited. Therefore, the aim of this study is to investigate the longitudinal association between SGC treatment duration and side effects in persons with AD and to compare its impact with cumulative dose. METHODS: This was a retrospective cohort study based on German

Geriatric Patient Successfully Managed With Conservative Therapy Despite Portal Venous Gas.

Intramural gastric air with concomitant portal venous gas is an uncommon radiographic finding that presents a significant diagnostic challenge. The primary differential diagnoses include gastric emphysema (GE), a generally benign and self-limiting condition, and emphysematous gastritis (EG), a rare but potentially fatal infectious process associated with high mortality rates. Although these conditions may appear similar on computed tomography (CT), they differ considerably in pathophysiology, clinical presentation, management, and prognosis. Accurate distinction between the two entities is ess

A Broad Scope of Differentials: Helicobacter pylori-Associated Chronic Gastritis Mimicking a Gastrointestinal Stromal Tumor.

Chronic gastritis is a common manifestation of Helicobacter pylori infection. Clinical presentations can range from epigastric discomfort to gastrointestinal bleeding. The clinical course and gross endoscopic findings can mimic alternative diagnoses such as gastric adenocarcinoma and gastrointestinal stromal tumors (GISTs). We report a case of H. pylori-associated chronic gastritis that initially presented with frank melena. Subsequent blood tests demonstrated a drop in hemoglobin and a rise in serum urea. The case was managed according to the hospital's upper gastrointestinal bleed protocols.

Eosinophilic Colitis Presenting as Hemoperitoneum: A Report of a Rare Case.

Eosinophilic colitis (EC) is a rare clinical entity. Advanced cases with severe inflammation may develop severe abdominal pain and distension, malabsorption, intestinal obstruction, and even GI hemorrhage. Once diagnosed early, EC responds to steroid therapy. We report the case of a 46-year-old male who presented with acute-onset left upper abdominal pain, enlargement of the right scrotal sac, and hemoperitoneum. A contrast-enhanced CT scan of the abdomen revealed an irregular hypoechoic lesion measuring approximately 6.5 × 4.8 cm, likely a transverse mesocolon hematoma in the epigastric regio

Spatial proteomics reveals four-stage molecular evolution in cancer immunotherapy-related gastritis.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer treatment, yet immune-related adverse events (irAEs) including immunotherapy-related gastritis (IRAEG) pose significant clinical challenges-often necessitating treatment interruption that may compromise antitumor efficacy. IRAEG presents with atypical symptoms, lacks specific biomarkers, and shows histopathological overlap with other forms of gastritis, complicating diagnosis and management. Despite increasing clinical recognition, a systematic understanding of spatial molecular alterations across the full disease course r

When Dysphagia Is Not Psychogenic: An Atypical Presentation of Schatzki Ring in a Patient With Schizophrenia.

Schatzki rings are benign distal esophageal rings that are often asymptomatic but may cause intermittent solid-food dysphagia or acute food bolus impaction. Symptoms are classically localized to the lower chest; however, atypical throat-localized symptoms may obscure the diagnosis, particularly in patients with psychiatric comorbidity. We report the case of a 74-year-old woman with schizophrenia, gastroesophageal reflux disease, chronic kidney disease stage 3, type 2 diabetes mellitus, hypertension, and hyperlipidemia who presented with three days of globus sensation and progressive inability
