Immunological mechanisms of autoimmune gastritis.

Autoimmune gastritis (AIG) is a chronic disease characterized by specific immune damage to the gastric mucosa. Previous studies have mostly focused on the single immune pathway mainly mediated by T cells, but the synergistic role of humoral immunity in disease progression cannot be ignored. This article systematically reviews the immunological mechanism of AIG, and analyzes the inflammatory cascade immune mechanism centered on the self-attack of gastric parietal cells mediated by CD4+ T, with the pro-inflammatory roles of Th1/Th17 cells and defective suppressive function of Tregs as a suppleme

A Ppb-level MIR-PAS for 12CO2/13CO2 isotope analysis toward on-line breath-based H. pylori sensing.

Helicobacter pylori infection is closely associated with chronic gastritis, peptic ulcers, and gastric cancer, rendering rapid and noninvasive diagnostic technologies clinically essential. Current breath tests, such as the 13C-urea breath test (13C-UBT), typically rely on breath collection bags followed by offline analysis, which limits real-time monitoring capabilities. To overcome this constraint, we presented a mid-infrared photoacoustic spectroscopy (MIR-PAS) system for real-time detection of CO2 isotopes and evaluation of 13C-UBT responses. A dual-channel differential resonant photoacoust

Skin Hyperpigmentation as Initial Manifestation of Vitamin B12 Deficiency: A Case of Delayed Diagnosis With Late-Onset Subacute Combined Degeneration of the Cord.

Vitamin B12 deficiency may present with a myriad of symptoms. Rarely, skin hyperpigmentation may be the sole initial manifestation of this micronutrient deficiency, which can easily be overlooked, leading to delayed diagnosis. This case report describes a 32-year-old Ghanaian male who presented with a 4-year history of progressive darkening of the dorsal aspects of the interphalangeal joints of both hands, the palms, and soles of the feet, for which he did not seek immediate medical attention. He later developed a worsening unsteady gait. Neurological assessment revealed mild weakness in both

Taurine suppresses gastric intestinal metaplasia in patient-derived organoids and Atp4a -/- mice.

BACKGROUND: Gastric intestinal metaplasia (GIM) represents a critical precancerous condition in the progression from chronic gastritis to gastric cancer, with limited therapeutic options. Emerging evidence suggests that taurine, a cytoprotective amino acid, may modulate gastric epithelial dysfunction. However, its application and efficiency in the context of GIM remain poorly understood. AIM: To investigate the therapeutic effects of taurine on GIM using patient-derived organoids and Atp4a -/- mouse models. METHODS: Patient-derived GIM organoids (n = 3) and Atp4a -/- mice, which spontaneousl

Spurious Elevations of Chromogranin A in the Setting of Autoimmune Metaplastic Atrophic Gastritis.

Autoimmune metaplastic gastritis (AMAG) is a form of autoimmune gastritis that is characterized by the immune system's attack on gastric parietal cells, leading to chronic inflammation. Gastric neuroendocrine tumors (GNETs) are rare neoplasms that can develop in the gastrointestinal tract in the presence of AMAG. This case presents a 69-year-old female who presented with dyspepsia, and on subsequent endoscopic evaluation, she was found to have AMAG in the context of elevated levels of serum chromogranin A (CgA) and gastrin, suggestive of a GNET. Despite an extensive diagnostic workup, includin

Artificial Intelligence for Single-Image Diagnosis of Autoimmune Gastritis Using Gastric Corpus Atrophy Images: A Multicenter Retrospective Diagnostic Study.

BACKGROUND AND AIMS: Endoscopic differentiation between autoimmune gastritis (AIG) and Helicobacter pylori-associated atrophic gastritis (HpAG) remains clinically challenging, often leading to misdiagnosis and inappropriate treatment. We aimed to develop and validate an AI system for AIG diagnosis using single white-light endoscopic images of gastric corpus atrophy. METHODS: This multicenter retrospective study included 2137 patients (1064 AIG and 1073 HpAG) from five tertiary hospitals in China. Data from two centers were used for training and internal validation, and three centers for exter

Integrated single-cell and spatial transcriptomics reveal the differentiation drivers of gastric epithelial lineage progression.

Gastric cancer (GC) develops through a sequence from chronic gastritis to intestinal metaplasia (IM) and carcinoma, with Helicobacter pylori (HP) as a key driver; however, the molecular mediators linking inflammation to malignant transformation remain unclear. We integrated single-cell RNA sequencing and spatial transcriptomics of gastric mucosal samples from atrophic gastritis, IM, and GC, including HP positive (+) and HP negative (-) cases, to map cellular heterogeneity, differentiation trajectories, and pathway activities. Our analyses revealed that IM epithelium represents a transitional s

Role of Nod-like Receptors in Helicobacter pylori Infection: Insights into Innate Immune Signaling Pathways.

Helicobacter pylori is a prevalent gastric pathogen that establishes chronic infection and contributes to gastritis, peptic ulcer disease, and gastric cancer. Its persistence depends on immune evasion strategies that promote sustained low-grade inflammation in the gastric mucosa. Nucleotide-binding oligomerization domain-like receptors (NLRs) are cytosolic pattern recognition receptors that play key roles in innate immune responses against H. pylori. Nod1 and Nod2 detect bacterial peptidoglycan delivered via the type IV secretion system or outer membrane vesicles, activating NF-κB, MAPK, and i

Accuracy of a new innovative column-free miniaturized gas-mass spectrometer compared with a classic gas-mass spectrometer to diagnose and monitor Helicobacter pylori infection: a prospective single blind study.

BACKGROUND: Helicobacter pylori (H. pylori) is one of the most common bacterial infections, and it causes chronic gastritis, peptic ulcers, gastric mucosa-associated lymphoid tissue lymphoma, and gastric cancer. H. pylori infection can be essentially detected by invasive [i.e. requiring an upper gastrointestinal endoscopy (UGE)], and noninvasive techniques. The principle of the urea breath test exploits the abundant quantities of urease produced by H. pylori. Recently, advances in the field of micro-electromechanical systems and nano-electromechanical systems have been proposed. This study aim

Inverse expression of ubiquitin-specific peptidase 19 and caspase 7 correlates with gastric neoplastic transformation.

BACKGROUND: Ubiquitin-specific protease 19 (USP19) is a deubiquitinylase (DUB) that is part of the USP family, the largest group of DUBs in humans. Growing evidence has indicated that USP19 is involved in tumor progression and serves as a new prognostic marker for various malignant disorders. Interestingly, USP19 has been shown to have both promoting and inhibiting effects on the onset and development of different neoplasms, depending on the specific tissue type. DUBs including USP19 affect a variety of cell functions including apoptotic cell death. Herein, caspase 7 acts as a key executioner
