INTRODUCTION: Chronic gastroduodenal disorders remain challenging to manage, and new diagnostic approaches are needed to better delineate underlying causes and guide therapeutic decisions. Body Surface Gastric Mapping (BSGM) technologies combine high-resolution gastric myoelectrical activity measurements with symptom and psychological profiling to provide mechanistic insights into gastric motor and sensory dysfunction. An International Working Group convened to derive the first consensus classification of BSGM phenotypes (the "Auckland Classification").

Functional dyspepsia (FD) is a common chronic gastrointestinal disorder characterized by persistent or recurrent epigastric symptoms in the absence of detectable structural abnormalities. In this pilot study, we explored whether a Lacticaseibacillus paracasei PC-01 (PC-01) fermented milk beverage alleviates FD symptoms. Fifty-five patients with FD were randomized into an experimental group (EP, n = 37) receiving the PC-01 fermented milk beverage (5.0 × 108 CFU/mL, 200 mL/day) or a control group (CP, n = 18) receiving the active comparator, an acidified milk beverage (non-fermented, without PC-

Disorders of gut-brain interaction (DGBIs), such as irritable bowel syndrome and functional dyspepsia, affect a substantial portion of the global population. These disorders are best conceptualized through a biopsychosocial framework, reflecting the complex interplay and bidirectional communication among biological, psychological, and social factors. While conventional pharmaceutical treatments remain limited in efficacy and patient satisfaction, interest in integrative approaches to care continues to grow. However, despite the increasing use of integrative interventions for DGBIs, their thera

BACKGROUND: Functional dyspepsia (FD) is characterised by postprandial fullness, early satiation, epigastric pain, or burning without structural abnormalities on upper endoscopy. Proton pump inhibitors (PPIs) are the preferred first-line therapy. Given that the diagnosis of FD is largely based on symptom assessment, it remains unclear whether acid-suppression meaningfully influences symptom presentation.
OBJECTIVE: This exploratory study aimed to determine the prevalence of acid-suppressive therapy (PPIs or H2 receptor antagonists (H2RAs)) among FD patients and to compare symptom patterns bet

In the young population, a persistent trend toward an increase in functional gastrointestinal disorders (FGIDs) is observed, with Generation Z representatives showing higher rates. The phenomenon of increased referrals of Generation Z individuals to gastroenterologists for FGIDs is indicative: these conditions account for nearly half of all initial consultations in this age group, driven by a combination of high medical awareness, a tendency toward self-diagnosis, and heightened sensitivity to somatic manifestations of psychoemotional stress. Emotional burnout, resulting from academic and prof

Disorders of gut-brain interaction, such as irritable bowel syndrome and functional dyspepsia, are increasingly linked to defects in gut barrier function. Mucosal disruption, encompassing alterations in the epithelial and mucus layers, leads to enhanced intestinal permeability, microbial translocation, and aberrant immune and neuronal signalling, potentially contributing to symptom severity. Despite growing recognition of barrier dysfunction in disorders of gut-brain interaction, clinical interventions remain largely symptom-based, with few therapies designed to directly restore epithelial int

Circadian rhythms exert fundamental control over gastrointestinal and metabolic physiology, governing 24-hour patterns of motility, secretion, nutrient absorption, microbial activity, immune regulation, and hepatic metabolism. Accumulating evidence indicates that the gastrointestinal tract is not an isolated system but is tightly integrated with systemic metabolic and neuroendocrine networks, forming a coordinated gastro-circadian metabolic axis (GCMA). This axis links molecular clocks in the gut, liver, adipose tissue, and skeletal muscle with rhythmic inputs from the gut microbiome, feeding-

This letter comments on a recent article published in the Revista Española de Enfermedades Digestivas (REED) addressing the management of functional dyspepsia in primary care. In the context of a stepwise treatment framework, we suggest that earlier incorporation of gut-brain-targeted and phenotype-based approaches may be considered in selected patients. A more individualized and mechanism-informed strategy may help optimize symptom control and reduce delays in effective therapy.

Jing-Si Herbal Tea (JSHT) is a traditional multi-herbal preparation composed of flavonoids, polyphenols, triterpenoid saponins, glycyrrhizin, and other bioactive constituents that collectively contribute to a wide spectrum of biological activities. Emerging laboratory and clinical studies indicate that JSHT is associated with modulation of oxidative stress, inflammatory responses, and immune-related pathways, with reported antiviral and cytoprotective effects primarily observed in experimental models and exploratory clinical settings. This review synthesizes current evidence describing the div

OBJECTIVE: Functional dyspepsia (FD) often overlaps with GERD-, IBS-, and FC-like symptoms, complicating treatment. This study evaluated overlap patterns and their association with cinitapride response in Chinese FD patients.
METHODS: In this multicenter prospective observational study, 1297 Chinese FD patients meeting Rome IV criteria received cinitapride 1 mg three times daily for 4 weeks. Overlapping symptoms were classified as GERD-like, IBS-like, or FC-like. Patients were grouped by overlap complexity: no overlap, one overlapping symptom domain, or two overlapping symptom domains. Sympto

PURPOSE OF REVIEW: Chronic gastroduodenal symptoms affect > 7% of adults, yet current diagnostic frameworks are challenged by limited mechanistic specificity, weak symptom correlation, and variable reproducibility. This review evaluates how body surface gastric mapping (BSGM), a non-invasive technology combining high-resolution gastric myoelectrical recording with validated symptom profiling, may improve the diagnosis and management of gastric motility disorders.
RECENT FINDINGS: BSGM employs a high-resolution 64-electrode array to derive validated biomarkers of gastric motor function, includ

UNLABELLED: Functional abdominal pain disorders (FAPDs), currently referred to as abdominal pain-related disorders of gut-brain interaction (AP-DGBIs), are characterized by recurrent abdominal discomfort in children that cannot be fully explained by an identifiable organic disorder. Probiotics are increasingly used as microbiota-targeted treatments, although their true clinical benefit remains uncertain. The objective of this study is to evaluate the efficacy and safety of probiotic supplementation compared with placebo in children with AP-DGBIs. PubMed, Embase, Web of Science and Scopus were

The accurate diagnosis of disorders of gut-brain interaction (DGBI), such as functional dyspepsia and irritable bowel syndrome, is crucial for improving patient symptoms, outcomes, and, ultimately, quality of life. However, busy clinicians often face challenges in diagnosing and managing these conditions, particularly when patients present with nonspecific gastrointestinal symptoms together with psychosocial distress and/or social issues. DGBI are among the most common reasons for gastroenterology referral, with a global prevalence of approximately 40% in adults based on large-scale epidemiolo

Serotonin (5-hydroxytryptamine [5-HT]) is a key regulator of gastrointestinal function and exerts its effects through a diversity of 5-HT receptors. In the gut, 5-HT is released primarily by enterochromaffin cells, which act as specialized epithelial chemo- and mechanosensors. In humans, 5-HT modulates gastric sensorimotor function, and animal studies suggest that it is involved in enteric neurogenesis. Altered serotonergic signaling is increasingly recognized as an important contributor to the pathophysiology of gastric motility disorders and, therefore, represents an important therapeutic ta

BACKGROUND: Functional dyspepsia (FD) is a prevalent yet challenging gastrointestinal disorder described as a group of upper gastrointestinal symptoms such as epigastric pain or burning, bloating, belching among many others, in the absence of an identifiable organic etiology. Its diagnosis is established according to the Rome IV criteria, which define the disorder by bothersome postprandial fullness, early satiety, epigastric pain and/or epigastric burning present for the last 3 months, with symptom onset at least 6 months before diagnosis. Despite its benign nature, functional dyspepsia can c

Tirzepatide, a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, induces significant weight loss and glycemic improvement. Dopaminergic pathways are critically involved in appetite regulation and reward processing, and the pharmacological modulation of dopamine signaling may influence metabolic responses to incretin-based therapies. We report the case of a 54-year-old woman with obesity and newly diagnosed type 2 diabetes mellitus (T2DM) who initiated tirzepatide therapy while receiving levosulpiride for functional dyspepsia.
