Primary HIV-1 infection presenting with nephrotic-range proteinuria and severe acute kidney injury mimicking imported Lassa fever.

PURPOSE: Primary HIV-1 infection (PHI) can present with protean clinical manifestations. We report a rare presentation of PHI that underscores that a high index of suspicion is required for diagnosis of PHI. METHODS: We report on a 54-yearold previously healthy woman of African descent who presented with sudden-onset nephrotic-range proteinuria and acute kidney injury (AKI) requiring hemodialysis in the setting of febrile multiple organ dysfunction syndrome. Both the epidemiological and clinical features initially pointed to imported Lassa fever, but this was ruled out. She was eventually dia

Adaptive Design for Phase II/III Platform Trial of Lassa Fever Therapeutics.

The current recommendation for treating Lassa fever with ribavirin is supported only by weak evidence. Given the persistent effects in areas with endemic transmission and epidemic potential, there is an urgent need to reassess ribavirin and investigate other potential therapeutic candidates; however, a robust clinical trial method adapted to Lassa fever epidemiology has not yet been established. We propose an adaptive phase II/III multicenter randomized controlled platform trial that uses a superiority framework with an equal allocation ratio and accounts for challenges selecting the primary e

Structure-guided design of a prefusion GPC trimer induces neutralizing responses against LASV.

Lassa virus (LASV) belongs to the Arenaviridae family and causes severe hemorrhagic fever in humans. Although many vaccine candidates for Lassa fever exist, no vaccines have been approved for clinical use currently. The precursor glycoprotein complex (GPC), which is expressed as a trimer on the viral surface, is the main target for vaccine development. However, it has been a significant challenge to elicit effective neutralizing antibodies against LASV. In this study, we designed and produced a prefusion GPC trimer antigen of LASV, named GPCv2. Based on the structural information of GPC, we ma

Lassa virus protein-protein interactions as mediators of Lassa fever pathogenesis.

Viral hemorrhagic Lassa fever (LF), caused by Lassa virus (LASV), is a significant public health concern endemic in West Africa with high morbidity and mortality rates, limited treatment options, and potential for international spread. Despite advances in interrogating its epidemiology and clinical manifestations, the molecular mechanisms driving pathogenesis of LASV and other arenaviruses remain incompletely understood. This review synthesizes current knowledge regarding the role of LASV host-virus interactions in mediating the pathogenesis of LF, with emphasis on interactions between viral a

Knowledge, perception and preventive practices of Lassa fever among mothers of under-five children in an endemic community in Edo State, Nigeria.

BACKGROUND: Lassa fever is an acute viral hemorrhagic disease endemic to West Africa, particularly Nigeria. Despite efforts to control its spread, gaps in knowledge and preventive behaviours remain, particularly among mothers of young children. This study assessed the knowledge, perception, and preventive practices regarding Lassa fever among mothers of under-five children in Esan Central Local Government Area, Edo State, Nigeria. METHODS: A descriptive cross-sectional study was conducted among 130 mothers with children under five using a structured interviewer-administered questionnaire. Par

Advancing the frontiers of virology in Africa: The African Virologists Network's Mission and Progress.

Africa has long been a hotspot for emerging and re-emerging viral infections such as Ebola, Mpox, Lassa fever, HIV, and arboviruses. Despite the continent's critical role in virology, African virologists have historically lacked a dedicated platform for collaboration and influence in global health policies. The African Virologists Network (AVN) was established in 2019 to address these gaps, fostering research collaboration, knowledge exchange, and capacity building. This article details the history of AVN, its initiatives, and its impact, including the African Virology Congress (AVC), the AVN

Building climate-resilient health systems in Sierra Leone: addressing the dual burden of infectious and climate-related diseases.

Climate change presents a profound challenge to global health, disproportionately affecting low-income countries like Sierra Leone. This opinion examines the dual burden of infectious and climate-related diseases and their implications for Sierra Leone's health systems. Rising temperatures and changing rainfall patterns intensify vector-borne diseases such as malaria and Lassa fever, while flooding exacerbates waterborne diseases in overcrowded urban areas. Concurrently, climate-driven food insecurity worsens malnutrition, particularly among children, and heat stress contributes to the growing

Preclinical development of a replication-competent vesicular stomatitis virus-based Lassa virus vaccine candidate advanced into human clinical trials.

BACKGROUND: Lassa fever (LF) is a zoonotic haemorrhagic disease caused by Lassa virus (LASV), which is endemic in West African countries. The multimammate rat is the main animal reservoir and its geographic range is expected to expand due to influences like climate change and land usage, and this will place larger parts of Africa at risk. We conducted preclinical development on a promising experimental vaccine that allowed its advancement into human trials. METHODS: The LF vaccine is based on a vesicular stomatitis virus (VSV) vector in which the VSV glycoprotein (G) was replaced with the LAS

Mapping the antibody response to Lassa virus vaccination of non-human primates.

BACKGROUND: Lassa fever, caused by Lassa virus, is a severe disease, endemic in Western Africa, for which no vaccines or therapeutics are yet approved. Understanding the immune responses elicited by candidate vaccines is key for approval, including characterisation of antibody epitopes recognised and capacity for neutralisation. METHODS: Here we used negative-stain electron microscopy polyclonal antibody epitope mapping (EMPEM), in-vitro pseudovirus neutralisation assays, and biophysical antibody competition assays to uncover components of polyclonal antibody responses elicited in nonhuman pr

Safety, immunogenicity, efficacy, and effectiveness of Lassa fever vaccines in pregnant persons, children, and adolescents: a protocol for a living systematic review and meta-analysis.

BACKGROUND: Lassa fever (LF), caused by the Lassa virus (LASV), is a zoonotic viral hemorrhagic disease endemic to West Africa, primarily transmitted through rodent excreta and infected bodily fluids. It poses significant public health challenges due to its high morbidity and mortality rates, particularly among at-risk populations like pregnant persons and children. Despite decades of research, vaccine development has been hindered by the virus's genetic diversity and complex epidemiology. While several vaccine candidates have been developed, none have received regulatory approval. Given the r

Laboratory Diagnosis and Molecular Epidemiological Characterization of the First Imported Case of Lassa Fever in China.

OBJECTIVE: This study reports the first imported case of Lassa fever (LF) in China. Laboratory detection and molecular epidemiological analysis of the Lassa virus (LASV) from this case offer valuable insights for the prevention and control of LF. METHODS: Samples of cerebrospinal fluid (CSF), blood, urine, saliva, and environmental materials were collected from the patient and their close contacts for LASV nucleotide detection. Whole-genome sequencing was performed on positive samples to analyze the genetic characteristics of the virus. RESULTS: LASV was detected in the patient's CSF, blood,

Expansion of myeloid suppressor cells and suppression of Lassa virus-specific T cells during fatal Lassa fever.

Lassa fever is a highly lethal hemorrhagic fever endemic to West Africa. In the absence of efficient prophylactic or therapeutic countermeasures, it poses a substantial threat to public health in this region. The pathophysiological mechanisms underlying the severity of the disease are poorly known because Lassa virus (LASV), its causative agent, has to be handled in BSL-4 laboratories and access to clinical samples is difficult. The control of Lassa fever is associated with a rapid and well-balanced immune response and viral clearance. However, severe disease is characterized by uncontrolled i

Evaluation of the diagnostic accuracy of the ReLASV Pan-Lassa Antigen Rapid Test for Lassa Fever in Nigeria.

Lassa fever is a zoonotic disease found in several countries across West Africa, with estimates of up to 300,000 infections and 10,000 deaths yearly. The highest incidence is in Nigeria. Suspected cases are often seen in areas with limited infrastructure and diagnostics capacity, hence the availability of an accurate rapid diagnostic test (RDT) that could be used in the community would be an important public health tool. Unfortunately, few RDTs for Lassa fever exist and have not been thoroughly validated. Toward that end, we conducted a Phase 2 performance evaluation to assess the diagnostic a

Assessing the Environmental Drivers of Lassa Fever in West Africa: A Systematic Review.

The spread of Lassa virus in West Africa is reliant on the abundance and distribution of its rodent host reservoirs. While the impact of environmental change on viral spread has been studied for many zoonotic viruses, there is still a limited understanding of how seasonal impacts, land-use conversion, and biodiversity loss influence the expansion of Lassa virus among reservoirs. This systematic review synthesizes existing research on the association between environmental variables and Lassa virus circulation in West Africa to inform future research, public health interventions, and One Health

Detection of Immunoglobulin G and/or IgM antibodies specific for Lassa virus among HIV patients in the Northwestern region of Cameroon.

BACKGROUND: Persons with HIV are prone to other infections. Lassa virus (LASV)coinfection with HIV is a public health concern. Viral hemorrhagic fever caused by LASV has been endemic in parts of West Africa. Clinical diagnosis has been a major challenge for effective management and control because the majority of patients are asymptomatic. As such, rapid diagnosis is desirable for prompt therapeutic intervention and the implementation of control measures. The high prevalence of LASV recorded in Nigeria, a neighboring country, places Cameroon at risk. However, the detection of LASV infection am
