Fournier's Gangrene Caused by Actinomyces europaeus in a Young Male With Poorly Controlled Diabetes Mellitus: A Case Report.

Fournier's gangrene (FG) is a rapidly progressive necrotizing infection involving the perineal and genital regions and is associated with significant morbidity and mortality. It is uncommon among adolescents and is typically linked to underlying comorbidities such as diabetes mellitus, immunosuppression, or trauma. We report a case of FG in a 19-year-old male with poorly controlled diabetes who required multiple surgical debridements. Wound cultures identified Actinomyces europaeus, an emerging but uncommon pathogen in necrotizing soft-tissue infections. Early initiation of broad-spectrum anti

A case of mesenteric lipoma of the small intestine in an adult patient.

INTRODUCTION: Lipomas are the most common benign tumors of adipose tissue, often presenting as slow-growing, painless masses. Although typically harmless, they can occasionally mimic more serious conditions, requiring careful evaluation and differentiation in clinical practice. PRESENTATION OF CASE: We report on a 74-year-old man who underwent surgery for an abscess in the right lobe of the liver, gangrenous-perforated acalculous cholecystitis, and a perivesical abscess. During the revision of the abdominal cavity, a dense formation with enlarged lymph nodes was found on the small intestine n

Gangrenous cystitis presenting as an abdominal wall collection in an old frail patient: a case report.

INTRODUCTION AND IMPORTANCE: Gangrenous cystitis (GC) is an extremely rare yet life threatening condition. It commonly presents as urosepsis. We report an unusual case of GC in a frail patient that initially manifested as an abdominal wall collection. CASE PRESENTATION: We report a 70-year-old female, frail bedridden patient undergoing urinary catheterization with history of secondary recurrent urinary tract infection, presenting with an abdominal wall reddish and necrotized lesion. CT scan revealed a 14×8×5 cm collection involving the left anterolateral abdominal wall with a thickening of th

Ileosigmoid knotting in early pregnancy: a case report.

INTRODUCTION: Ileosigmoid knotting (ISK) is a rare cause of intestinal obstruction, occurring when the ileum wraps around the colon and its mesentery. Its occurrence during pregnancy, especially before the third trimester, is extremely uncommon. CASE REPORT: A 31-year-old woman at 12 weeks' gestation presented with 1 day of acute, severe abdominal pain, obstipation, vomiting, and abdominal distention. Physical examination revealed abdominal distention and severe rebound tenderness. Abdominal ultrasound showed a viable 12-week singleton pregnancy, colonic wall thickening, and free intraperiton

Fournier's gangrene caused by accidental ingestion of a date pit: a case report.

INTRODUCTION AND IMPORTANCE: Fournier's gangrene is a rare yet rapidly progressive necrotizing fasciitis that primarily affects the perineal, genital, and perianal regions. Mixed aerobic and anaerobic bacterial infection characterizes it. Urogenital tract infections, intestinal inflammation, or local trauma commonly trigger the condition. However, gastrointestinal foreign bodies seldom directly trigger cases in clinical practice. This paper describes such a case. CASE REPORT: A 65-year-old male patient was admitted, presenting with "perianal pain for one week." Initial examination revealed er

Cystic Artery Hemorrhage as a Rare Complication of Acute Gangrenous Cholecystitis.

This report presents a case of cystic artery hemorrhage resulting from acute gangrenous cholecystitis. During an emergency laparoscopic cholecystectomy, the surgical team unexpectedly encountered hemorrhage from the cystic artery. Subsequent pathological examination confirmed the diagnosis of acute gangrenous cholecystitis. As a severe infectious condition of the gallbladder, acute gangrenous cholecystitis is typically associated with a high risk of complications; however, cystic artery hemorrhage, as a direct complication, is exceedingly rare. This article seeks to investigate both common and

Shotgun metagenomic analysis of the oral microbiomes of children with noma.

Noma is a rapidly progressive orofacial gangrene that predominantly affects children living in extreme poverty. Despite its documentation since antiquity and its designation as a World Health Organisation Neglected Tropical Disease in 2023, the microbiological cause of noma remains poorly understood, with no specific organisms confidently identified as definitive aetiological agents. Here, we present the first deep shotgun metagenomic profiling of oral saliva microbiomes from 19 Nigerian children with acute noma. Our analyses of this preliminary study reveal marked microbial dysbiosis in noma

Incidence of diabetes-related foot disease: results from 2 prospective cohort studies and meta-analysis.

We aimed to estimate the incidence for overall and individual components of diabetes-related foot disease (DFD), and explore potential heterogeneity across studies. We included patients with diabetes who were free of DFD from UK Biobank and Chongqing Diabetes Registry (CDR). We defined DFD according to the International Working Group on the Diabetic Foot 2023 criteria. We further performed meta-analyses by integrating results from the two cohorts and 64 cohorts identified from a systematic review of literature, and quantifying the extent of variation in reported incidence rates. The incidence

Expert consensus on acupuncture for diabetic foot.

Diabetic foot (DF), is called "foot gangrene" (gangrene) or "nerve gangrene" (gangrene of the nerve area) in traditional Chinese medicine. It is a severe complication of diabetes, whose incidence rate is high, and the consequences is serious. Acupuncture is a promising therapy, it is effective, it can alleviate symptoms, accelerate the wound healing and reduce the risk of amputation. This consensus includes clinical evidence and expert practical experience, it shows acupuncture is an effective supportive treatment methods. Its main purpose is to provide practical, standardized, and safe instru

Adult male Fournier gangrene hospitalizations with and without end-stage kidney disease: A 5-year nationwide analysis.

INTRODUCTION: Fournier gangrene (FG) is a severe acute necrotizing fasciitis affecting the genitalia and carries a high mortality. Patients with end-stage kidney disease (ESKD) are an overlooked but potentially vulnerable subgroup given their high burden of comorbidities. As such, we aimed to study FG in those with and without ESKD. MATERIALS AND METHODS: We conducted a retrospective analysis of all adult male FG hospitalizations from 2017 - 2021 in the National Inpatient Sample (NIS) database. We sought to identify independent predictors of in-hospital mortality. Univariable logistic regress

[Chinese expert consensus on endovascular treatment of ischemic lesions in the diabetic foot].

Ischemic diabetic foot is one of the severe chronic complications of diabetes mellitus. Without standardized treatment, it may progressively lead to ulceration, gangrene, and subsequent amputation, and may even result in patient mortality. Patients with ischemic diabetic foot often present with complex lower extremity vascular pathologies, such as diffuse calcification and occlusion of below-the-knee small arteries. In clinical practice, there is significant variability among diagnostic and treatment centers in the selection of therapeutic strategies and technical procedures. To standardize th

Prognostic value of the inflammatory burden index for disease-specific mortality and intensive care need in Fournier's gangrene.

BACKGROUND: Fournier's gangrene (FG) is a rapidly progressive necrotizing infection with high mortality. Early recognition of high-risk patients is critical for improving outcomes. OBJECTIVES: To evaluate the prognostic value of the Inflammatory Burden Index (IBI)-a composite marker derived from C-reactive protein, neutrophil, and lymphocyte counts-for predicting mortality, intensive care requirements, and clinical course in FG. DESIGN AND METHODS: This retrospective cohort included 130 patients diagnosed with FG and treated at a tertiary referral center between 2015 and 2025. Demographic, c

Saphenous Vein Arterialization for Limb Salvage in Unreconstructable Peripheral Arterial Disease: Long-Term Results.

BACKGROUND: Chronic limb-threatening ischemia (CLTI) is the end stage of peripheral arterial disease, characterized by ischemic rest pain, nonhealing ulcers, or gangrene. Up to 40% of patients lack distal targets for revascularization, often resulting in major amputation. Great saphenous vein arterialization (GSVA) with distal pedal vein valve disruption is a promising limb salvage technique. We report 12-month amputation, pain, and wound healing for patients with CLTI treated with GSVA between 2019 and 2024. METHODS: From October 2019 to August 2024, 20 limbs in 15 patients with unreconstruc

Strangulated Amyand's hernia containing gangrenous appendix and cecum in a preterm neonate.

Amyand's hernia defined as the presence of the appendix within an inguinal hernia sac, is rare in neonates and can deteriorate quickly. We report a preterm male twin (33 + 6 weeks; 1460 g) presenting at 1 month with 24 hours of progressive right inguinoscrotal swelling, erythema, irritability, and absent stooling. Exam showed a tender, nonreducible, non-transilluminating mass with abdominal distension. X-ray revealed dilated bowel loops; ultrasound suggested an indirect right inguinal hernia containing an inflamed appendix. Urgent inguinal exploration found incarcerated viable cecum and a gang

Assessing Mortality Prediction: Comparing the Effectiveness of the Fournier Gangrene Severity Index and the Uludag Index.

Background Fournier's gangrene (FG) is a fulminant necrotizing infection associated with substantial morbidity and high mortality. Early and accurate risk stratification is essential to guide clinical decision-making. The Fournier Gangrene Severity Index (FGSI) and the Uludag Fournier Gangrene Severity Index (UFGSI) are widely used prognostic tools; however, their comparative performance in Mexican populations remains insufficiently characterized. This study aimed to assess the association of FGSI and UFGSI with in-hospital mortality and to explore their comparative prognostic performance in a

Fournier's Gangrene in a Young, Morbidly Obese Patient Without Diabetes.

Fournier's gangrene (FG) is a rapidly progressive form of necrotizing fasciitis involving the external genitalia, perineum, and perianal region. We present a case of FG in a morbidly obese patient (BMI > 40) without a known history of diabetes, malignancy, or immunodeficiency. The patient was treated with aggressive surgical debridement, broad-spectrum antibiotics, and early closure, achieving full recovery. This case highlights obesity as an independent risk factor for FG, which has significant ramifications for the future, given obesity rates in young Americans.

Comparative Outcomes of Resection: Primary Anastomosis and Hartmann's Procedure in the Management of Sigmoid Volvulus.

One major cause of large bowel blockage is sigmoid volvulus, which in more complex instances necessitates surgical intervention. Despite the widespread use of two basic surgical techniques-resection-primary anastomosis and Hartmann's procedure- comparative results in settings with limited resources, such as Bangladesh, are still not well understood. This study compared the outcomes of Hartmann's procedure and resection-primary anastomosis in sigmoid volvulus. From September 2018 to August 2019, the Department of Surgery at Mymensingh Medical College Hospital in Bangladesh was the site of this
