Strangulated Amyand's hernia containing gangrenous appendix and cecum in a preterm neonate.

Amyand's hernia defined as the presence of the appendix within an inguinal hernia sac, is rare in neonates and can deteriorate quickly. We report a preterm male twin (33 + 6 weeks; 1460 g) presenting at 1 month with 24 hours of progressive right inguinoscrotal swelling, erythema, irritability, and absent stooling. Exam showed a tender, nonreducible, non-transilluminating mass with abdominal distension. X-ray revealed dilated bowel loops; ultrasound suggested an indirect right inguinal hernia containing an inflamed appendix. Urgent inguinal exploration found incarcerated viable cecum and a gang

Assessing Mortality Prediction: Comparing the Effectiveness of the Fournier Gangrene Severity Index and the Uludag Index.

Background Fournier's gangrene (FG) is a fulminant necrotizing infection associated with substantial morbidity and high mortality. Early and accurate risk stratification is essential to guide clinical decision-making. The Fournier Gangrene Severity Index (FGSI) and the Uludag Fournier Gangrene Severity Index (UFGSI) are widely used prognostic tools; however, their comparative performance in Mexican populations remains insufficiently characterized. This study aimed to assess the association of FGSI and UFGSI with in-hospital mortality and to explore their comparative prognostic performance in a

Fournier's Gangrene in a Young, Morbidly Obese Patient Without Diabetes.

Fournier's gangrene (FG) is a rapidly progressive form of necrotizing fasciitis involving the external genitalia, perineum, and perianal region. We present a case of FG in a morbidly obese patient (BMI > 40) without a known history of diabetes, malignancy, or immunodeficiency. The patient was treated with aggressive surgical debridement, broad-spectrum antibiotics, and early closure, achieving full recovery. This case highlights obesity as an independent risk factor for FG, which has significant ramifications for the future, given obesity rates in young Americans.

Comparative Outcomes of Resection: Primary Anastomosis and Hartmann's Procedure in the Management of Sigmoid Volvulus.

One major cause of large bowel blockage is sigmoid volvulus, which in more complex instances necessitates surgical intervention. Despite the widespread use of two basic surgical techniques-resection-primary anastomosis and Hartmann's procedure- comparative results in settings with limited resources, such as Bangladesh, are still not well understood. This study compared the outcomes of Hartmann's procedure and resection-primary anastomosis in sigmoid volvulus. From September 2018 to August 2019, the Department of Surgery at Mymensingh Medical College Hospital in Bangladesh was the site of this

Appendicitis inflammatory response versus pediatric appendicitis score for grading disease severity in children.

Acute appendicitis is a frequent surgical emergency in children, and early recognition of severe forms remains challenging. This prospective observational study compared the Appendicitis Inflammatory Response (AIR) and Pediatric Appendicitis Score (PAS) for grading disease severity in pediatric acute appendicitis. Among 542 children assessed, 138 with suspected appendicitis were included, and 136 underwent appendectomy. All included patients were prospectively scored with AIR and PAS, and clinical, radiologic, intraoperative, and histopathologic findings were recorded. Associations between sco

Comparison of diverting colostomy and fecal management catheter in fournier gangrene: a retrospective cohort study.

OBJECTIVE: This study aimed to compare the effectiveness of colostomy and fecal management catheterization, two fecal diversion methods, in the treatment of Fournier gangrene (FG) and to determine which method offers greater advantages. MATERIALS AND METHODS: A retrospective study was conducted on 54 patients who underwent surgical debridement and fecal diversion for FG at our hospital's General Surgery Clinic between January 2014 and October 2024. Patients were divided into colostomy (n = 28) and fecal management catheter (Flexi-Seal) (n = 26) groups for comparison. The primary outcome was i

Strangulated small bowel hernia through the foramen of Winslow: a rare surgical emergency.

INTRODUCTION: A rare cause of small bowel obstruction (SBO), an internal hernia via the foramen of Winslow offers a high risk of strangulation because of its delayed identification. We present a 33-year-old male patient who developed gangrenous SBO as a result of incarceration in the lesser sac through the Winslow foramen. CASE PRESENTATION: A 33-year-old male presented with acute abdominal pain, vomiting, and obstipation. Imaging revealed dilated small bowel loops and a transition point near the foramen of Winslow. Emergency laparotomy confirmed a hernia with ischemic bowel, necessitating re

Gallbladder volvulus in an elderly patient presenting as acute calculous cholecystitis: a case report.

INTRODUCTION AND IMPORTANCE: Gallbladder volvulus is a rare, life-threatening emergency resulting from torsion of the gallbladder on its vascular pedicle. Its rarity and clinical similarity to acute cholecystitis make diagnosis difficult. This is the first documented case in the Philippines, highlighting its significance for local surgical practice. PRESENTATION OF CASE: A 74-year-old woman presented with acute right upper quadrant pain, nausea, vomiting, and bloating. Laboratory workup revealed leukocytosis with neutrophilia, and imaging suggested acute calculous cholecystitis. At surgery, c

A rare case of midgut reverse rotation presenting with cecal volvulus in a young adult.

INTRODUCTION AND IMPORTANCE: Congenital malrotation is a disease that almost exclusively affects newborns. When it is discovered in an adult, which happens seldom, it is usually linked to a protracted history of gastrointestinal problems. Reversed rotation of the midgut is the rarest form of midgut malrotations. Small intestinal obstruction caused by volvulus or peritoneal bands is a common consequence of these conditions that typically manifest in the early stages of infancy. CASE PRESENTATION: Here, we present a case of 20-year-old young adult male presented with clinical signs and symptoms

Laparoscopic Versus Open Cholecystectomy in Gangrenous Cholecystitis: A Retrospective Comparative Study.

INTRODUCTION: Gangrenous cholecystitis (GC) is a severe form of acute cholecystitis associated with increased operative difficulty and postoperative morbidity. Although open cholecystectomy is traditionally preferred, advances in minimally invasive surgery have expanded the role of laparoscopy. However, the optimal surgical approach to GC remains controversial. This study aimed to compare the perioperative and postoperative outcomes of laparoscopic and open cholecystectomy in patients with GC. MATERIALS AND METHODS: This retrospective observational study was conducted at the Department of Gen

The Iron Grip: A Rare Case of Penile Strangulation by a Metallic Ring.

Penile strangulation by foreign objects is a rare but serious urological emergency, most often associated with attempts to enhance sexual performance. Metallic rings are common offenders, and delayed intervention can lead to vascular compromise, ischemia, necrosis, and even penile loss. Management is particularly challenging when standard equipment is insufficient. We report the case of a 23-year-old male who presented with penile swelling and mild pain 5 h after symptom onset, following self-application of a metallic ring 12 h earlier. Manual removal attempts using lubrication were unsuccessf

Clinical Significance of HBOT in Hypoxia-Induced Pathophysiological Conditions: A Comprehensive Investigative Study.

Hyperbaric oxygen therapy involves breathing pure oxygen at high pressure (around 2-2.5 atmospheres), leading to increased blood oxygen levels in tissues. The therapeutic efficacy of hyperbaric oxygen therapy (HBOT) is often dose-dependent, correlating with the number of applications, particularly within hypoxic tissues. HBOT provides neuroprotection following central nervous system trauma and serves as a critical intervention for non-neurological conditions, including Fournier's gangrene, chronic osteomyelitis, and carbon monoxide poisoning. It has also been applied as an adjunctive treatment

Pure single-incision laparoscopic cholecystectomy using an articulating instrument: the Ewha Method in a retrospective observational study.

PURPOSE: Conventional multiport laparoscopic cholecystectomy (CMLC) is the gold standard for benign gallbladder disease. Single-incision laparoscopic cholecystectomy (SILC) offers superior cosmesis but has limited adoption due to technical challenges. We introduce the Ewha Method, a modified SILC technique using an articulating instrument to improve visualization, ergonomics, and safety. METHODS: We retrospectively analyzed 530 patients who underwent SILC between November 2023 and August 2024. Surgical outcomes were compared between senior (n = 338) and junior (n = 192) surgeons. Case Report: Acute acalculous cholecystitis due to Salmonella Typhi: a case of conservative treatment failure and review of the literature.

BACKGROUND: Acute acalculous cholecystitis (AAC) is an uncommon but potentially life-threatening inflammation of the gallbladder in the absence of gallstones, accounting for 5-10% of all cases of acute cholecystitis. While most cases occur in critically ill or postoperative patients, infectious etiologies-particularly Salmonella Typhi-are increasingly recognized. Typhoid-related AAC is well described in children from endemic regions but remains rare in adults, with limited evidence to guide management. CASE PRESENTATION: We report the case of a 67-year-old man presenting with fever, chills, a

Gallbladder Volvulus, A Surgical Challenge in Inflammatory Pathology of the Gallbladder: A Case Report.

INTRODUCTION: Gallbladder volvulus (GV) is a rare surgical entity marked by torsion of the gallbladder on its axis secondary to mesenteric anomalies. Due to its clinical similarity to acute cholecystitis and preoperative diagnosis is difficult. CLINICAL CASE: A 79-year-old female with cardiovascular comorbidities presented with sudden abdominal pain and emesis. Initial laboratory examination tests showed leukocytosis and hyperbilirubinemia, while ultrasound suggested acute cholecystitis. Due to persistent symptoms and the development of a palpable mass, the patient underwent laparoscopic chol

Clostridium septicum Alpha Toxin-Based Recombinant Subunit Vaccine Protects Broiler Chickens Against Clostridial Dermatitis.

Clostridial dermatitis (CD), caused by Clostridium septicum, is an economically important disease of broiler chickens, specifically in flocks raised without antibiotics. Our previous work in turkeys identified a nontoxic domain no. 2 of C. septicum alpha-toxin (ntATX-D2) as a protective vaccine antigen. In the present study, we used a prime-boost immunization regimen to vaccinate broiler chickens subcutaneously with recombinant ntATX-D2 protein followed by a challenge with virulent C. septicum to assess protective immunity. Immunization led to clinical protection, as determined by a significan
