Heritability of Crohn's disease and ulcerative colitis: a Swedish nationwide population-based twin study.

BACKGROUND AND AIMS: Limited statistical power has hampered previous estimates of concordance between relatives and heritability in inflammatory bowel diseases (IBD). To examine the genetic component of Crohn's disease and ulcerative colitis, we established the largest nationwide IBD twin cohort to date and assessed estimates of concordance and heritability. METHODS: We used the Swedish Twin Registry to identify all twins from complete pairs with known zygosity born between 1886 and 2004. The Swedish National Patient Register was used to identify all patients diagnosed with IBD. We calculated

Stricturing Diseases of the Gastrointestinal Tract-Current Clinical Practice.

Gastrointestinal stricture can occur in the esophagus, stomach, small intestine, colon, and anorectum. Most of the strictures are benign. The prevalence of esophageal strictures (ES) secondary to eosinophilic esophagitis has increased. Pyloric stenosis (PS) is rarely seen in clinical practice. Sleeve stenosis (SS) and stomal stenosis or gastrojejunal anastomotic stricture (GJAS) are increasingly seen due to bariatric surgery. Crohn's disease (CD) remains a significant challenge even in the biologic era. The symptomatology of strictures depends on the location, severity, and underlying cause. I

A Narrative Review on Integrative Bioinformatics Approaches for microRNA Research in Familial Mediterranean Fever: Current Insights and Future Directions.

BACKGROUND AND AIMS: Familial Mediterranean Fever (FMF) is a monogenic autoinflammatory disease caused by mutations in the MEFV gene, resulting in recurrent inflammatory episodes and a risk of developing amyloidosis. Although its pathophysiology is well described, FMF still lacks specific biomarkers and personalized treatment strategies. MicroRNAs (miRNAs), which regulate gene expression posttranscriptionally, have emerged as promising diagnostic, prognostic, and therapeutic biomarkers. Given their complex expression patterns, bioinformatics approaches are essential for their identification an

Macrophage MERTK restrains GRAMD1A-driven mitochondrial oxysterol trafficking to limit colitis.

Impaired macrophage efferocytosis drives chronic inflammation, yet the underlying immunometabolic mechanisms remain poorly defined. In this study, we identify that impaired macrophage efferocytosis triggers excessive mitochondrial oxysterol accumulation, which drives mitochondrial ROS-associated inflammation in Crohn's disease (CD). We report an inverse correlation between CD activity and the abundance of MERTK+ macrophages. Mechanistically, MERTK loss enhances cholesterol biosynthesis in the endoplasmic reticulum (ER) and stabilizes the cholesterol transporter GRAMD1A. This promotes excessive

Inflammation significantly alters mucosal transcriptomic signatures in pediatric inflammatory bowel disease.

OBJECTIVES AND STUDY: Crohn's disease (CD) and ulcerative colitis (UC) share common features of inflammation to a greater extent in children than in adults. However, histopathological scoring systems of mucosal inflammation are usually available only for either one of these entities. The IBD-DCA score is the first of its kind to incorporate both into one scoring system but still lacks validation in pediatric IBD. METHODS: An existing data of a multiplexed gene expression analysis of mucosal biopsies of 25 patients diagnosed with either CD (n = 16) or UC (n = 9) were evaluated according to the

CD64 binding potential does not translate into enhanced therapeutic efficacy for anti-IL-23 antibodies under physiologically relevant conditions.

BACKGROUND: Interleukin (IL)-23 neutralizing antibodies are clinically efficacious but differ in the functionality of their Fc portion. Guselkumab (GUS) and ustekinumab (UST) have a wild-type (WT) Fc portion, permitting native binding to Fc gamma receptors (FcγRs), while risankizumab (RZB) lacks FcγR binding due to the L234A to L235A (LALA) mutation. Recently, GUS was reported to neutralize IL-23 more effectively than RZB in vitro, owing to GUS-mediated binding of FcγRI (CD64) on macrophages. However, these findings do not account for the fact that FcγRI (CD64) is competitively occupied by end

Increased remodeling of type XXII collagen is associated with an inflammatory phenotype in Crohn's disease.

In Crohn's disease, extracellular matrix (ECM) dysfunction and loss of tissue integrity are key pathological features. Fibril-associated collagens with interrupted triple helices (FACIT) are known to regulate ECM assembly and mediate biophysical cues in their microenvironment but remain largely understudied. This study aimed to investigate a novel biomarker reflecting remodelling of the FACIT collagen type XXII (PRO-C22) and explore its association with differing phenotypes of CD and treatment outcomes. Two cohorts of patients with CD were included in the study [cohort 1 (n = 65), cohort 2 (n

Integrating Intestinal Ultrasound in the Personalized Management of IBD.

Personalized medicine is increasingly shaping the management of inflammatory bowel disease (IBD), with the goal of tailoring diagnostic and therapeutic strategies to individual patients. Intestinal ultrasound (IUS) has emerged as a pivotal, non-invasive, and repeatable tool for assessing disease activity, treatment response, and complications in both Crohn's disease and ulcerative colitis. Beyond its role in routine monitoring, IUS enables real-time decision-making and facilitates tight control strategies, aligning with the principles of precision medicine. By combining morphological assessmen

Spatial transcriptomics atlas of inflammatory bowel disease to guide implementation in research consortiums and clinical trials.

Using over 100 intestinal tissue sections from non-diseased controls and patients with ulcerative colitis or Crohn's disease across multiple inflammatory bowel disease consortia, we construct a spatially resolved atlas containing over three million cells and systematically evaluate two imaging-based spatial transcriptomics platforms. Here we show that CosMx tends to achieve higher detection efficiency than Xenium across commercially available panels in both ulcerative colitis and Crohn's disease, whereas Xenium shows reduced performance associated with tissue type, block quality, and panel siz

Prophylactic effects of Acanthopanax senticosus Harms on the development of colitis in mice.

Inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis (UC), comprises chronic relapsing inflammatory disorders of the gastrointestinal tract. Although the acute dextran sodium sulfate (DSS)-induced murine colitis model does not reproduce chronic human IBD, it reflects acute colonic epithelial injury and superficial mucosal inflammation, which are more characteristic of UC than of Crohn's disease. Therefore, the present study investigated whether the root extract of Acanthopanax senticosus Harms (ASH) exerts a prophylactic effect in an acute DSS-induced experim

Integrative eQTL and Mendelian Randomization Analyses with Experimental Validation Prioritize Genetic Candidate Biomarkers for Crohn's Disease.

INTRODUCTION: Crohn's disease (CD) is a chronic, idiopathic inflammatory bowel disease. Understanding the genetic and immunological basis of CD can lead to better therapeutic strategies. METHODS: We employed publicly available datasets from the Gene Expression Omnibus (GEO) database to identify instrumental variables through expression quantitative trait loci (eQTL) analysis and conducted two-sample Mendelian randomization (MR) analyses. Additionally, we investigated the functions and pathways of co-expressed genes using GO, KEGG, and GSEA analyses. The association between these genes and imm

Escherichia coli pathobionts and Crohn's disease: varied genetic paths leading to similar phenotypes.

Crohn's disease (CD), an inflammatory bowel disease that arises from an immune attack on the gastrointestinal tract, affects roughly 1.6 million Americans. The etiology of CD and the other major irritable bowel disease, ulcerative colitis, is not known, but host genetics and immunology, the gut microbiome, and environmental factors are all thought to be involved. In addition, adherent-invasive Escherichia coli (AIEC) strains, which are able to adhere to and invade epithelial cells and macrophages, are frequently found to be associated with CD. Besides their adherence and invasion properties, k

From Cost to Value: A Comparative Analysis of Medical and Surgical Management of Inflammatory Bowel Disease in a Public Referral Hospital in Mexico.

BACKGROUND AND OBJECTIVE: Inflammatory bowel disease (IBD) represents a growing clinical and economic challenge for healthcare systems, particularly in settings that provide free access to specialised care. Advances in medical therapy have modified disease outcomes but have also reshaped healthcare expenditure, while surgical management continues to represent high-impact cost events. Integrated analyses comparing medical and surgical costs from the perspective of public healthcare systems remain scarce in Latin America. This study aims to compare the economic burden of medical versus surgical-

Exclusive enteral nutrition for induction of remission in Crohn's disease in adults: A systematic review and meta-analysis of randomized trials and real-world studies.

BACKGROUND: Exclusive enteral nutrition (EEN) is an established induction therapy in pediatric Crohn's disease (CD); however, its role in adults remains less well defined. METHODS: A systematic search was performed through December 2025. Eligible studies included randomized controlled trials (RCTs) and non-randomized studies evaluating EEN in adults (≥ 18 years) with active CD. The primary outcome was clinical remission, defined by validated disease activity indices. Risk of bias was assessed using RoB 2 and the Newcastle-Ottawa Scale. Random-effects models were applied to estimate pooled rem

Determinants of fecal diversion prior to rectovaginal fistula surgery.

BACKGROUND: Temporary fecal diversion (FD) is performed selectively in patients with rectovaginal fistulas (RVFs). This study aimed to identify the factors associated with FD in the treatment of RVF and determine whether FD is associated with increased recurrence-free survival. METHODS: A retrospective review of females who underwent repair of an RVF was performed. Patients were divided into 2 groups based on the use of FD. Recurrence was defined as evidence of RVF on clinical examination or imaging after at least 2 previous follow-ups, with healing noted. RESULTS: A total of 158 patients un

Cyclophilin A Induces Mechanisms of Cell Rearrangement and Fusion During Differentiation and Maturation of Early Macrophage Precursors.

Macrophages are a heterogeneous cell population whose functional diversity is formed during their maturation and depends on factors of the microenvironment after their migration into the bloodstream or tissues. One such factor is the pro-inflammatory protein cyclophilin A (CypA, 18 kDa). Using a model of early human monocytic THP-1 cells, it was shown that recombinant human CypA (rhCypA) exerts a differentiating effect on these cells, inducing their maturation, adhesion, and spreading. Under the effect of rhCypA, the THP-1 cells developed an actin cytoskeleton characteristic of motile cells wi

Intraoperative Autologous Adipose-Derived Therapies and PRP as Add-On in the Surgical Treatment of Cryptoglandular and Crohn's Disease-Related Perianal Fistula-A Systematic Review.

BACKGROUND: The treatment of perianal fistulas remains challenging, with low healing and high recurrence rates. Autologous adipose-derived regenerative therapies and platelet-rich plasma (PRP) have emerged as adjuncts to surgical intervention for cryptoglandular and Crohn's disease (CD)-related perianal fistulas (PAF). This systematic review evaluates the outcomes of these therapies as an add-on to surgical intervention. METHODS: A systematic search was conducted in several online databases up to December 2025.
