Ulcerative colitis is a chronic inflammatory bowel disease usually confined to the colon and rectum. Although extraintestinal manifestations are common, ulcerative colitis-like lesions of the upper gastrointestinal tract, referred to as gastroduodenal ulcerative colitis, are rare and typically observed in patients with pancolitis or after total proctocolectomy. We report a 43-year-old woman with steroid-dependent pancolitis-type ulcerative colitis who developed recurrent duodenal lesions without prior total proctocolectomy. She subsequently presented with nausea and epigastric pain; endoscopy

Cytomegalovirus (CMV) is an infrequently reported etiology of secondary pouchitis in patients who have undergone total proctocolectomy with ileal pouch-anal anastomosis. We present the case of a patient with chronic antibiotic-dependent pouchitis, ulcerative pancolitis, primary sclerosing cholangitis, and orthotopic liver transplant who presented with hematochezia. Pouchoscopy showed ulcerated mucosa, and histopathology with immunohistochemistry confirmed CMV pouchitis. The patient was treated with antiviral therapy with improvement in bleeding and viremia. However, persistent refractory pouch

INTRODUCTION: Ulcerative colitis (UC) is a chronic, idiopathic inflammatory bowel disease (IBD) characterised by relapsing-remitting mucosal inflammation commencing in the rectum and extending proximally towards the intestine. The global incidence of UC is on the rise presenting a complex therapeutic challenge. Conventional management ranging from aminosalicylates and corticosteroids to biologics and surgical management using colectomy is effective but is often limited by secondary non-response, significant adverse events and high costs. This case report explores the clinical and endoscopic im

INTRODUCTION: Ulcerative colitis (UC) is a chronic relapsing inflammatory bowel disease limited to the colonic mucosa, with rising incidence and prevalence in many regions of the world, including Latin America. A subset of patients requires colectomy for medically refractory disease, acute severe colitis, dysplasia, or malignancy, for whom restorative proctocolectomy with ileal pouch-anal anastomosis (RPC-IPAA) is the preferred restorative procedure. Pouch-related complications -- including pouchitis, chronic antibiotic-refractory pouchitis (CARP), pouch failure, and diagnostic reclassificatio

BACKGROUND: Inflammatory bowel disease (IBD), comprising Crohn's disease, ulcerative colitis, and IBD-unclassified, is a chronic relapsing inflammatory disorder of the gastrointestinal tract, with up to 25% of cases presenting in childhood or adolescence. Although historically uncommon in Asia, the incidence of pediatric IBD in the region has been increasing, and disease phenotype appears to differ from Western populations. In the Philippines, data on pediatric IBD remain limited, with no comprehensive local studies describing its clinical features, subtypes, and outcomes.

Ulcerative colitis (UC) is a chronic, relapsing inflammatory bowel disease characterized by mucosal inflammation limited to the colon, which may progress to pancolitis in severe cases. This debilitating condition significantly impairs patients' quality of life and may result in long-term disability, representing a growing global public health concern. Despite extensive research, the precise pathogenesis of UC remains incompletely understood. Current evidence implicates a multifactorial etiology involving genetic susceptibility, immune dysregulation, intestinal barrier dysfunction, and gut micr

INTRODUCTION: Ulcerative colitis (UC) has a complex multifocal etiology that includes immune dysregulation of the gut microbiome leading to pro-inflammatory cytokine release. UC may present initially through subclinical manifestations such as sacroiliitis due to underlying inflammation, known as the gut-joint axis. JAK inhibition provides an effective treatment method for overlapping intestinal and articular inflammation.

BACKGROUND: Autoimmune sclerosing cholangitis (ASC) is a rare clinical entity characterized by overlapping features of autoimmune hepatitis and primary sclerosing cholangitis. It predominantly affects pediatric patients. Therapeutic management is often complex, requiring a multidisciplinary and individualized approach, especially in the context of associated autoimmune diseases.
CASE PRESENTATION: We present the case of a female patient diagnosed at the age of 10 with ASC, for which immunosuppressive therapy with prednisone, azathioprine (AZA), and ursodeoxycholic acid (UDCA) was initiated, w

Immunosuppression from human immunodeficiency virus (HIV) or other comorbidities increases susceptibility to gastrointestinal infections such as Entamoeba histolytica and cytomegalovirus (CMV). Although each pathogen can independently cause severe colitis, co-infection is rare and may lead to rapid clinical deterioration if not recognized early. We report two cases of concurrent amebic and CMV colitis in immunocompromised patients. The first patient, a 56-year-old man with HIV, Kaposi sarcoma, and hepatitis C, presented with bloody diarrhea and hypotension. Colonoscopy revealed pancolitis with

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

BACKGROUND: Common variable immunodeficiency (CVID) predisposes patients to inflammatory bowel disease (IBD). There are limited studies characterizing adults with concomitant CVID and IBD (CVID-IBD).
METHODS: Demographics, clinical courses, endoscopic findings, and disease-specific therapies were obtained in adults ≥18 years with CVID-IBD over a 5-year period.
RESULTS: We identified 11 patients with CVID-IBD, 6 with Crohn's disease (CD), and 5 with ulcerative colitis (UC). The presenting symptoms in 10 (91%) were abdominal pain and diarrhea. A majority of patients were diagnosed with IBD bef

We present an unusual case of complement-mediated thrombotic microangiopathy (formerly known as atypical hemolytic uremic syndrome) associated with inflammatory disease in a young patient. A 26-year-old male patient with no significant past medical history presented to our emergency department with a four-week history of diffuse, moderate, cramping, non-radiating abdominal pain with no known aggravating or relieving factors. Abdominal pain was associated with nausea, vomiting, and bloody stools. His physical examination revealed pale conjunctiva, tachycardia, and mild tenderness in the lower a

Ulcerative Colitis (UC) is a chronic inflammatory condition resulting from an abnormal immune response to gut microbiota, leading to cytokine dysregulation, including elevated interleukin-23 (IL-23) levels. Emerging evidence suggests that vitamin D (VD) plays a crucial role in immune modulation. However, its correlation with IL-23 in UC is not well addressed. This study aims to elucidate the relationship between serum VD and IL-23 levels in UC patients. We included forty-four UC patients and forty-four healthy controls. VD insufficiency was more common in UC patients (n = 14) compared to contr

Inflammatory bowel diseases (IBD), including ulcerative colitis (UC) and Crohn's disease, are chronic disorders relating to gut microbiota dysbiosis. Despite severe pancolitis being more prevalent in pediatric UC than in adults, alterations in the colon mucosa-associated microbiota (MAM) and their association with disease severity remain to be elucidated. The present study aimed to compare the gut microbiota in colon lavage fluids (CLFs) and fecal samples from 19 pediatric UC and 19 non-IBD patients. The community structure of MAM inferred by 16S metagenomic analysis was similar throughout the

BACKGROUND: Ulcerative colitis (UC) is a chronic inflammatory bowel disease treated with immunosuppressants to control inflammation. Drugs like azathioprine (AZA) and anti-tumor necrosis factor agents increase the risk of extraintestinal malignancies. However, no association has been established between these therapies and endometrial stromal sarcoma. This report presents a rare case of endometrial stromal sarcoma in a patient with UC undergoing immunosuppressive treatment and includes a literature review to explore any possible correlation between the disease, the therapies used, and the deve

Background/Objectives: Ulcerative colitis (UC) is a chronic inflammatory bowel disease that significantly increases the colorectal cancer (CRC) risk. This study used nationwide data on intractable diseases to clarify the clinical epidemiology of UC-related CRC in Japan. Methods: Patients diagnosed with UC between FY 2003 and 2011 were included. The relative incidence ratio (RR) was calculated using the standardized incidence ratio from the National Cancer Registry. To compare prognostic factors, outcomes were evaluated using the Cox proportional hazards model analysis for cancer occurrence, an

Cytomegalovirus (CMV) colitis, a complication in patients with inflammatory bowel disease (IBD), particularly ulcerative colitis (UC), is a significant diagnostic and therapeutic challenge due to its overlap with IBD flares. CMV reactivation in IBD is driven by chronic inflammation, compromised immune function, and use of immunosuppressive agents like corticosteroids. Risk factors include older age, pancolitis, and severe disease. Diagnosis hinges on endoscopy and histology, with tissue biopsy and immunohistochemistry as the gold standard. Quantitative tissue PCR may aid in differentiating lat

Amoebic colitis is a common mimic of inflammatory bowel disease (IBD), primarily encountered in developing countries. We present a case of a 73-year-old British male with no travel history to any amoebic endemic regions, who presented with a three-month history of per rectal (PR) bleeding, diarrhoea, and a positive faecal immunochemical test (FIT). Prior to this, he reported no history of experiencing any gastrointestinal symptoms. Colonoscopy revealed patchy pan-colitis, most marked in the ascending colon, and histology confirmed chronic inflammation. A diagnosis of IBD was made, and the pati

Ulcerative colitis (UC), one of the two major inflammatory bowel diseases (IBD), is a chronic immune-mediated inflammatory disorder with varying degrees of colonic mucosal involvement. Patients often present with inflammation limited to the rectum, also known as ulcerative proctitis, proximal colonic involvement, or pancolitis which affects the entire colon. Clinical manifestations of UC flare-ups include hematochezia, diarrhea, and abdominal pain. Yersinia enterocolitica, an acute cause of infectious diarrhea, is usually caused by the ingestion of food products contaminated with toxins and pa
