Unmasking Semaglutide-Induced Gastroparesis: The Dangers of Rapid Dose Escalation in a Diabetic Patient.

Gastroparesis is a chronic disorder characterized by delayed gastric emptying without mechanical obstruction, causing symptoms such as nausea, vomiting, and abdominal discomfort. Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist used for type 2 diabetes, can slow gastric motility and induce gastroparesis. We present a case of a 48-year-old woman with type 2 diabetes, who developed persistent nausea and vomiting after resuming semaglutide at 2 mg subcutaneously once weekly without following the recommended stepwise titration schedule (typically initiated at 0.25 mg weekly with gra

Clinical and Genetic Characteristics of Pediatric Patients with Inflammatory Bowel Disease Transitioning to Adult Medicine: A Single-Center Ten-Year Experience.

Background/Objectives: Inflammatory bowel diseases (IBDs) comprise a group of chronic idiopathic disorders, including ulcerative colitis (UC), Crohn's disease (CD), and indeterminate colitis (IC). Complex genetic factors, in addition to environmental triggers, have been shown to play a fundamental role in the pathogenesis of IBD, contributing to disease susceptibility. The transition of adolescents with inflammatory bowel disease (IBD) to adult care represents a significant challenge for patients, their families, and healthcare providers. Approximately 25% of individuals with IBD receive a dia

Long-term remission of severe refractory scleritis associated with ulcerative colitis after proctocolectomy: a case report and literature review.

Extraintestinal manifestations occur in up to 50% of patients with inflammatory bowel disease and can affect various organs, including the joints, skin, and eyes. Scleritis, one such extraintestinal manifestation, is associated with severe eye pain, vision impairment, and potential permanent vision loss. However, it is rare, and detailed descriptions of its characteristics and management are limited. We report the case of a 55-year-old man initially diagnosed with proctitis ulcerative colitis who simultaneously developed scleritis as the disease progressed to left-sided colitis. Treatment with

Effectiveness of Tofacitinib in Ulcerative Proctitis Compared to Left Sided Colitis and Pancolitis.

BACKGROUND: Ulcerative proctitis (UP), though associated with high symptom burden and poor quality of life, is excluded from most of the randomized controlled trials in UC, including the OCTAVE trials. We aimed to analyse the effectiveness of tofacitinib in UP, and compare it to that in left sided colitis (LSC) and pancolitis (PC). METHODS: This was a prospective cohort study. Patients with either steroid-dependent or refractory ulcerative colitis, who received tofacitinib, were divided into three groups based on the disease extent [UP, LSC and PC]. The primary outcome was comparison of propo

Formulation and optimization of a single-layer coat for targeting budesonide pellets to the descending Colon.

The current budesonide formulations are inadequate for addressing left-sided colitis, and patients might hesitate to use an enema for a prolonged time. This study focuses on developing a single-layer coating for budesonide pellets targeting the descending colon. Pellets containing budesonide (1.5%w/w), PVP K30 (5%w/w), lactose monohydrate (25%w/w) and Avicel pH 102 (68.5%w/w) were prepared using extrusion spheronization technique. Coating formulations were designed using response surface methodology with pH and time-dependent Eudragits. Dissolution tests were conducted at different pH levels (

Level of interleukin 17 in inflammatory bowel disease and its relation with disease activity.

BACKGROUND: Inflammatory bowel disease (IBD) is a chronic relapsing inflammatory disorder of the gastrointestinal tract (GIT).It results in progressive intestinal epithelium structural and functional damage that necessitates lifetime medication.Thereis imbalance in the production of T helper 1 (Th1), Th2 and Th17 cytokines. This plays a crucial role in the chronic inflammatory process and the defective immune response to pathogenic agents; thus promoting the recurrence of the disease. Our aim of this study was to detect serum IL-17 levels in IBD patients and its relation with disease activity.

Need for therapeutic escalation in patients with refractory ulcerative proctitis: Results from the PROCU study of the ENEIDA registry.

BACKGROUND: Ulcerative proctitis (UP) can have a milder, less aggressive course than left-sided colitis or extensive colitis. Therefore, immunosuppressants tend to be used less in patients with this condition. Evidence, however, is scarce because these patients are excluded from randomised controlled clinical trials. Our aim was to describe the characteristics of patients with refractory UP and their disease-related complications, and to identify the need for immunosuppressive therapies. METHODS: We identified patients with UP from the prospective ENEIDA registry sponsored by the GETECCU. We

[Efficient treatment of mild Crohn's disease and mild ulcerative colitis].

The cornerstone of treatment for mild ulcerative colitis is still the oral or topical (rectal) application of aminosalicylates (5-ASA). 5‑ASA preparations are often only administered orally in mild ulcerative colitis. Study data show that in ulcerative proctitis and left-sided colitis, rectal 5‑ASA preparations are even more effective than oral administration. In a next step, steroid-containing topical therapies should be used. Topical steroids such as budesonide are also primarily used in mild Crohn's disease. However, it is controversial whether treatment is necessary in symptom-free patient
