Epithelial TMPRSS2 impairs glucose homeostasis in obese mice by regulating ghrelin-GLP-1 receptor signaling pathway.

Glucagon-like peptide-1 (GLP-1) and glucose-induced insulinotropic polypeptide (GIP) receptor agonists have revolutionized obesity therapy, but causes of obesity-associated dysregulation of endogenous incretin production remain incompletely understood. Here we show that intestinal transmembrane serine protease 2 (TMPRSS2) plays a pivotal role in deregulating anti-diabetic GLP-1 production in obesity. TMPRSS2 is widely coexpressed in intestinal epithelial cells along with its signaling target protease-activated receptor 2 (PAR2). In addition to its role in regulating coagulation protease-mediat

Idiopathic Mesenteric Phlebosclerosis Presenting as Recurrent Diarrhea.

Our manuscript presents a rare case of idiopathic mesenteric phlebosclerosis (IMP) in a 73-year-old woman who experienced recurrent abdominal pain and diarrhea. Through a detailed clinical investigation, including laboratory tests, colonoscopy, biopsy, and enhanced abdominal CTA, we diagnosed the patient with IMP. Notably, we identified a potential link between the patient's long-term intake of a traditional Chinese herb containing Gardenia Fructus and the development of IMP. This finding is supported by existing literature, which suggests that genipin, a component of Gardenia Fructus, may cau

Intersecting Molecular Pathways in Cardiovascular Disease and Diabetes Mellitus: Emerging Roles of Inflammation and Therapeutics.

Diabetes mellitus (DM) and cardiovascular diseases (CVD) remain leading contributors to global morbidity and mortality, imposing a substantial burden on healthcare systems worldwide. The pathophysiological mechanisms underlying these conditions are complex and closely interconnected, with chronic low-grade inflammation, oxidative stress, endothelial dysfunction, insulin resistance and dysregulated lipid metabolism serving as pivotal shared pathways. Persistent hyperglycaemia and metabolic imbalance in DM accelerate vascular injury and atherosclerotic progression, thereby significantly increasi

Rare and fatal late-term complication of endovascular aneurysm repair: Migration of the endograft into the duodenum.

Endovascular aneurysm repair (EVAR) is a treatment method that has become increasingly popular for abdominal aortic aneurysms (AAA) due to its ease of application, reduced hospital stay, and its suitability as an alternative for patients who cannot tolerate open surgery. Although the early outcomes of EVAR are better than those of open surgery, complications such as endoleak, migration, thrombosis or kinking of the endograft limbs, graft infection, and secondary rupture may occur. In this study, we present a patient who underwent EVAR at another institution approximately 10 years earlier and w

New Advances in the Mechanisms and Therapeutic Strategies of Gut Microbiota in Regulating Glycolipid Metabolism to Combat Atherosclerosis.

Atherosclerosis is the major pathological basis of cardiovascular diseases, characterized by chronic inflammation and disturbed glucose and lipid metabolism. Despite the widespread use of statins, the global clinical burden continues to rise, and effective therapeutic strategies remain limited. As the "second genome," gut microbiota participates in the progression of atherosclerosis by regulating host glucose and lipid metabolism, inflammatory responses, and metabolite production. Natural plant components exert multiple effects, including regulating glucose and lipid metabolism, modulating gut

Dangshen formula Shengmai-Yin suppresses atherosclerosis through restoring the gut microbiota and homeostatic efferocytosis.

BACKGROUND & AIMS: Atherosclerosis (AS) is a chronic progressive vascular disease characterized by lipid deposition and aortic inflammation. On the basis of traditional Chinese medicine (TCM) theory, the Dangshen Formula Shengmai-Yin (DS-SMY) has demonstrated unique advantages in treating AS. However, the mechanism by which DS-SMY affects AS has not been revealed. METHODS: In this study, high-fat diet-induced apolipoprotein E knockout (ApoE-/-) mice were used to construct AS models. The effects of DS-SMY on aortic collagen deposition and inflammatory infiltration were investigated, and atorva

A pathogenic gut lipoglycan drives systemic thromboinflammation in lupus nephritis.

OBJECTIVES: The gut microbiome plays a crucial role in regulating systemic immunity and has been implicated in several chronic inflammatory diseases. Intestinal expansions of Ruminococcus gnavus (RG), a dominant gut commensal, correlate with disease flares in lupus nephritis (LN), but the underlying mechanism remains unknown. METHODS: In a Pilot cohort of patients with biopsy-proven LN, subsetted by gut microbiota community, immune status was characterised using bulk-blood RNA sequencing libraries, serum levels of representative host proteins, and levels of immunoglobulin (Ig)G antibodies to

Remote mobilization of gut Tregs exosomes serves as nanoshuttles for targeted cardioprotection against ischemia/reperfusion injury by ginseng polysaccharides.

Myocardial ischemia-reperfusion injury (MIRI) remains a critical clinical condition with limited preventive and therapeutic strategies, underscoring the need for novel interventions. The gut-heart axis and exosomal crosstalk hold therapeutic promise for cardioprotection, yet the fundamental question of which specific gut cells release the beneficial exosomes is unanswered. This study aimed to elucidate whether ginseng polysaccharides WGPA exert remote cardiac protection by regulating intestinal immune homeostasis, specifically through promoting the release of exosomes from regulatory T cells (

Oral alginate microspheres deliver Rg3/aspirin liposomes to modulate foam cells and gut microbiota in atherosclerosis.

Atherosclerosis is a complex pathology driven by chronic inflammation, lipid accumulation, and thrombosis, necessitating multi-pathway therapeutic strategies. Herein, we engineered an oral multi-stage platform (RALM) comprising aspirin (Asp) and ginsenoside Rg3 co-loaded phosphatidylserine (PS) liposomes (RA-Lipo) encapsulated within sodium alginate microspheres. Physicochemical and biological evaluations revealed that the incorporation of Rg3 into the lipid bilayer as a cholesterol substitute yielded uniform RA-Lipo, which exhibited colloidal stability, intestinal epithelial transport, and pr

Mycotic Aneurysms in Intestinal Transplantation: A Case Series.

INTRODUCTION: Mycotic aneurysms (MA) are a potentially fatal complication following intestinal transplantation (ITx). Their occurrence arises from several factors, such as chronic abdominal sepsis, potential for contamination of the vascular grafts during enteric anastomoses and the heightened burden of immunosuppression. METHODS: A retrospective review of a prospectively maintained database of all adult ITx from December 2007 to June 2025 was conducted. RESULTS: Among 168 ITx, six patients (3.6%) experienced eight MA episodes. MA occurred bimodally, early post-transplant (2-8 weeks) or in a

Sleep Deprivation Exacerbates Ischemic Stroke Outcomes via Akkermansia Depletion and Metabolic Dysregulation.

AIMS: Sleep disturbances are a potential risk factor for stroke. Our clinical cohort shows that insomnia is associated with a higher 90-day stroke recurrence rate, prompting an investigation into the underlying mechanisms, particularly the role of the gut microbiota. METHODS: We utilized a rat MCAO model of sleep deprivation (SD) combined with antibiotic (ABX)-induced gut dysbiosis. Neurological deficits were assessed through behavioral tests, and cerebral infarct volume was measured using TTC staining. Structural changes in brain and intestinal tissues were observed by H&E staining, while th

[Angioedema 2025 - Advancements in Diagnosis and Treatment].

Angioedema is a pathophysiologic process characterized by increased vascular endothelial hyperpermeability, leading to fluid accumulation and tissue swelling. This process is reversible in most cases; therefore, it is clinically exhibited by exacerbations and remissions. Patients experience paroxysmal, unpredictable, and slowly progressing episodes of swelling of the face, extremities, genitals, tongue, and larynx, and severe abdominal pains. Attacks affecting the oropharyngeal and upper airways impose a considerable life-threatening risk. Intestinal wall edema can cause bowel obstruction, mim

The Gut-Oral-Skin-Heart Microbiota Axis and Oxidative Stress: Where Have We Been and Where Are We Going.

Scientific interest in the relationship between cardiovascular disease and the human microbiome has grown substantially in recent years, with particular emphasis placed on the gut microbiota. Emerging evidence suggests, however, that distinct microbial niches-including the skin, oral cavity, and gut-may interact to influence cardiovascular homeostasis through shared pathogenic pathways, most notably oxidative stress and systemic inflammation. Intestinal dysbiosis has been strongly linked to the increased production of microbial metabolites such as trimethylamine N-oxide, which promotes endothe

The gut-lung axis in ARDS: beyond microbial translocation.

The gut-lung axis has emerged as a pivotal pathway in the pathogenesis of acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Disruption of the intestinal barrier, a common event in critical illness, facilitates the systemic dissemination of live microbiota, their pathogen-associated molecular patterns (PAMPs), and bioactive metabolites. This process critically depends on the integrity of the gut vascular barrier (GVB). The GVB is the endothelial layer underlying the gut epithelium. It serves as the final gatekeeper, restricting microbial products from entering the systemic

Gut microbiota dysbiosis-induced chronic inflammation as a driver of atherosclerosis: cellular crosstalk and host-microbe interactions.

Gut microbiota dysbiosis is increasingly recognized as an upstream contributor to chronic low-grade inflammation and atherosclerosis (AS). Disruption of microbial homeostasis may impair intestinal barrier integrity, increase exposure to pro-inflammatory microbial products and metabolites, and reduce protective metabolites such as short-chain fatty acids (SCFAs), thereby activating innate immune signaling and sustaining vascular inflammation. Current evidence indicates that gut dysbiosis promotes atherosclerosis mainly through three interconnected processes: metabolite imbalance, barrier dysfun

Hepatotoxic mechanisms of functionalized nanopolystyrene: decoding the role of ionic surface groups.

BACKGROUND: With annual global plastic production exceeding 400 million tons, nanoscale polystyrene particles (nPS) have become a major health concern due to their bioaccumulation capacity and ability to cross biological barriers. Surface-charged nPS variants (cationic, anionic, and neutral) show distinct biodistribution patterns, yet the mechanisms underlying their systemic damage remain incompletely understood. This study aimed to investigate the systemic injury mechanisms of nPS with different surface charges. METHODS: Mice were exposed to fluorescently labeled cationic (amino-modified), a

Hypertension-associated gut dysbiosis drives target organ damage through impaired polyunsaturated fatty acids metabolism and immune activation.

Hypertension (HTN) is a major risk factor for cardiovascular diseases, with chronic low-grade inflammation emerging as a critical contributor to its development and target organ damage. Emerging evidence implies gut microbiota in blood pressure regulation. However, the long-term impact of patient-derived gut dysbiosis on the chronic progression of HTN remains insufficiently characterized. This study aimed to determine how HTN-associated gut microbiota contributes to sustained blood pressure elevation and target organ damage during long-term colonization, and to elucidate underlying immune-meta
