[Diagnostic and therapeutic strategies for small-bowel gastrointestinal bleeding when initial upper endoscopy and colonoscopy are unremarkable].

Suspected small bowel bleeding is defined as a gastrointestinal bleeding, overt or occult (iron-deficiency anemia), with unremarkable high-quality upper endoscopy and colonoscopy. The main causes are vascular lesions (angiodysplasia) and inflammatory lesions (Crohn's disease). Diagnosis is primarily based on capsule endoscopy, which has an optimal yield if performed within 48 hours on bleeding onset. Deep enteroscopy often allows targeted treatment, but its availability is limited. If the capsule endoscopy is not contributive as well, the bleeding is referred to as obscure gastrointestinal ble

Regulation of Inducible Nitric Oxide Synthase (NOS2) Expression in Healthy and Inflamed Bowel: A Narrative Review.

Crohn's disease and ulcerative colitis, the principal forms of inflammatory bowel disease (IBD), are chronic inflammatory disorders characterized by recurrent intestinal injury, impaired mucosal healing, and substantial disease burden. Despite significant therapeutic advances, many patients fail to achieve sustained remission, highlighting the need for a better understanding of the molecular mechanisms driving intestinal inflammation. Nitric oxide (NO) is a key regulator of intestinal homeostasis, influencing epithelial barrier integrity, vascular function, host defense, and immune responses.

Undetected Gastric Angiodysplastic Lesion Causing Obscure Gastrointestinal Bleeding.

Obscure gastrointestinal bleeding (OGIB) has traditionally been defined to as bleeding originating from the small intestine that is beyond the reach of esophagogastroduodenoscopy and colonoscopy. However, gastric lesions associated with chronic intermittent bleeding may be undetected if there is no overt bleeding during initial endoscopic examination, resulting in a diagnosis of OGIB and repeated complex follow-up investigations. Vascular lesions, including angiodysplasia, Dieulafoy's lesion, and small vascular ectasias, are major causes of chronic intermittent gastrointestinal bleeding. Hemat

Cholesterol: Facts and Opinions by a Non-lipid Cardiologist.

Plasma low-density lipoprotein cholesterol (LDL-C) is the main cause of coronary artery disease (CAD). All cells synthesize their own cholesterol when needed to form membranes for the cell wall and other cellular structures, including the synthesis of vitamin D. Cholesterol in excess of the cell's needs is dumped into the bloodstream to be metabolized by the liver and secreted into the intestine. Of the circulating cholesterol, 80% is derived from this excessive endogenous cholesterol, with only a small proportion from diet. Thus, it is not surprising that lowering plasma LDL-C to levels as lo

Takayasu Arteritis in a Child with Chronic Active Proctitis of Unspecified Etiology: A Case Report with Diagnostic Challenges and Literature Review.

Takayasu arteritis (TAK) is a rare large-vessel vasculitis in children. A 14-year-old girl presented with intermittent fever, elevated inflammatory markers (C-reactive protein up to 117.63 mg/L and erythrocyte sedimentation rate of 93 mm/h), and computed tomography angiography demonstrating diffuse wall thickening of the aortic arch and its major branches. She had a previous diagnosis of ulcerative colitis; however, colonoscopy and biopsy revealed severe chronic active proctitis with glandular architectural irregularity and occasional cryptitis, but without crypt abscesses or granulomas, and t

Hemosuccus Pancreaticus from Large Inferior Pancreaticoduodenal Artery Pseudoaneurysm Complicating Chronic Pancreatitis.

Hemosuccus pancreaticus is an uncommon cause of upper gastrointestinal bleeding characterized by hemorrhage entering the pancreatic duct and draining into the duodenum. Although pseudoaneurysms may arise from several peripancreatic vessels in the setting of chronic pancreatitis, involvement of the inferior pancreaticoduodenal artery (IPDA) is particularly uncommon and carries a high risk of rupture. A man in his late 30s with a known history of chronic calcific pancreatitis presented with melena, severe anemia, and hemodynamic instability. Side-viewing duodenoscopy revealed intermittent bleedi

Short-chain fatty acids as key mediators of the microbiota-gut-brain axis in chronic cerebral hypoperfusion: mechanisms and therapeutics.

Chronic cerebral hypoperfusion (CCH) is an important contributor to vascular cognitive impairment and neurodegenerative disorders, characterized by neuronal injury, blood-brain barrier (BBB) disruption, neuroinflammation, mitochondrial dysfunction, and synaptic impairment. Increasing evidence suggests that the microbiota-gut-brain axis plays a crucial role in ischemic brain injury. CCH is associated with gut microbiota dysbiosis, intestinal barrier dysfunction, and reduced production of microbiota-derived short-chain fatty acids (SCFAs), which may be linked to cognitive decline through bidirec

Fitness Training for γδ T cells in mouse and human atherosclerosis takes place in plaques and artery tertiary lymphoid organs.

BACKGROUND: γδ T cells represent a heterogeneous family of innate-like lymphocytes with adaptive immunity features. Although abundant in barrier tissues such as skin and intestine, γδ T cells are rare in the cardiovascular system, which severely limits exploration of their roles in atherosclerosis. Consequently, the spatial localization, functional states, and antigen-driven responses of γδ T cells within atherosclerotic lesions remain poorly defined. METHODS: To nevertheless examine γδ T cells in atherosclerosis, we employed an integrative, three-pronged strategy combining γδ T cell single-ce

PLCγ2 deficiency compromises systemic immune tolerance and erodes myelin homeostasis while enhancing oxidative metabolism in the mouse brain.

BACKGROUND: Phospholipase C gamma-2 (PLCγ2) catalyzes the hydrolysis of the membrane phosphatidylinositol-4,5-bisphosphate (PIP2) to form diacylglycerol (DAG) and inositol trisphosphate (IP3), feeding into diverse downstream signaling pathways. PLCG2 polymorphisms have been associated with reduced and/or increased risk of Alzheimer's disease (AD) and related dementias, longevity, autoinflammation, and immune disorders. In the brain, PLCγ2 is expressed in microglia, and other neuroimmune and vascular interface populations, yet its role in brain homeostasis remains incompletely defined. METHODS

Sodium cyclamate aggravates DSS-induced colitis in mice with associated microbial, metabolic, and epithelial alterations.

Sodium cyclamate is widely used as a non-nutritive sweetener, but its effects during intestinal inflammation remain poorly defined. We investigated whether chronic cyclamate exposure aggravated dextran sulfate sodium (DSS)-induced colitis in mice using disease phenotyping, 16S rRNA gene sequencing, targeted short-chain fatty acid and bile acid measurements, metabolomics, lipidomics, discovery proteomics, and independent western blot validation. Relative to DSS alone, DSS + Cyclamate mice exhibited greater disease severity and higher abundances of Bacteroides and Klebsiella; Lactobacillus and M

High cholesterol absorption efficiency enhances proatherogenic properties of low-density lipoprotein particles.

BACKGROUND AND AIMS: High cholesterol absorption efficiency is determined by genetic variation in small intestinal sterol transporters and affects one-third of individuals. Their risk for atherosclerotic cardiovascular disease (ASCVD) is increased compared with low cholesterol absorbers, despite similar serum lipid concentrations. We investigated the association of cholesterol absorption efficiency with proatherogenic properties of low-density lipoproteins (LDLs). METHODS: The study cohort consisted of 90 middle-aged participants, 56 females and 34 males, without lipid-lowering therapy or ASC

Palmitoylethanolamide for Nickel Allergy: Plausible, Untested, and Worth Considering.

Nickel allergy remains the most prevalent cause of allergic contact dermatitis worldwide, imposing a substantial socio-epidemiological and economic burden. Beyond its classical cutaneous presentation, systemic nickel allergy syndrome highlights the systemic dimension of Nickel hypersensitivity, wherein dietary nickel intake may provoke both gastrointestinal and cutaneous symptoms through mechanisms involving gut barrier impairment and mucosal immune priming. Recent evidence highlights the contribution of angiogenesis and lymph-angiogenesis to Nickel-induced allergic contact dermatitis, through

Vascular Ehlers-Danlos syndrome: A multidisciplinary clinical framework for early suspicion, emergency-safe management and lifelong care.

Vascular Ehlers-Danlos syndrome (vEDS), caused by likely pathogenic COL3A1 variants, carries a high risk of arterial dissection or rupture, intestinal perforation and uterine rupture, and a median survival near 51 years. It is often recognized only at a catastrophe, and its acute management is deliberately counter-intuitive. The therapeutic landscape changed in 2025 with the first randomized evidence for an angiotensin-receptor blocker. We provide a clinical, practice-oriented review for internists and the multidisciplinary team. As is appropriate for a rare disease, case-level evidence was co

The gut microbiome-cardiometabolic axis: insights into obesity, type 2 diabetes, and hypertension.

Alterations in gut microbial ecology have been linked to obesity, type 2 diabetes (T2D), and hypertension, but their biological significance remains difficult to separate from diet, medication use, adiposity, and other host factors. We synthesize evidence on intestinal barrier dysfunction, microbial translocation, low-grade inflammation, and microbiota-derived metabolites as interconnected mechanisms across these disorders. SCFAs, bile acids, trimethylamine N-oxide, tryptophan derivatives, branched-chain amino acid metabolites, and phenylacetylglutamine influence epithelial function, immune ac

Cardiovascular-kidney-metabolic syndrome through the lens of gut‑derived uremic toxins.

Cardiovascular-Kidney-Metabolic (CKM) syndrome represents a complex, interconnected cluster of cardiovascular disease, chronic kidney disease, and metabolic disorders such as obesity and type 2 diabetes. These conditions share overlapping metabolic, inflammatory, and vascular pathways, with the gut microbiome increasingly recognised as a key contributor and common underlying risk factor. Uremic toxins, traditionally considered waste products of host and microbial metabolism, are now recognised as active mediators of tissue damage across the CKM spectrum, particularly in the context of impaired

Verifying genetic diagnoses: Hereditary hemorrhagic telangiectasia - juvenile polyposis syndrome (HHT-JPS) or familial adenomatous polyposis (FAP)?

A 26-year-old woman was diagnosed with hereditary hemorrhagic telangiectasia - juvenile polyposis syndrome (HHT-JPS) after an incidental finding of a pulmonary arteriovenous malformation. Molecular genetic testing confirmed the presence of a pathogenic SMAD4 variant, later found to be present in several other family members who were all previously labelled as having familial adenomatous polyposis. This short report contrasts and compares the conditions and highlights the importance of using genetic testing to distinguish similar presentations of gastrointestinal polyps in association with gene

Risk of intestinal involvement in mucocutaneous-onset Behçet's disease: data from the AIDA network registry.

OBJECTIVE: Behçet's disease (BD) may initially manifest solely with mucocutaneous involvement. This study aimed to identify demographic and clinical factors associated with subsequent intestinal involvement in patients presenting exclusively with mucocutaneous manifestations during early disease stages. METHODS: Data were obtained from the International AutoInflammatory Disease Alliance Network registry dedicated to BD; a Bayesian statistical approach was employed to address the limited sample size resulting from subgroup stratifications. RESULTS: In total, 328 BD patients with exclusively m

Mass-forming ischemic colitis mimicking colon cancer: A case report of early repeat colonoscopy.

RATIONALE: Ischemic colitis is the most common form of intestinal ischemia, typically affecting elderly individuals with vascular risk factors such as hypertension, diabetes mellitus, atherosclerosis, and chronic kidney disease. In rare cases, a mass-forming variant may mimic malignancy or inflammatory bowel disease, creating a diagnostic challenge. We report a case of mass-forming ischemic colitis, initially suspected to be colon cancer, in which the correct diagnosis was established through short-term follow-up colonoscopy with biopsy, thereby preventing unnecessary surgical intervention.
