Genetic evidence for a causal protective effect of GLP-1 receptor agonists on paralytic ileus but not on other gastrointestinal events.

The association between glucagon-like peptide-1 receptor agonists (GLP-1RAs) and gastrointestinal events remains controversial in observational studies. To address this, our study employed drug-target Mendelian randomization (MR) to investigate the causal relationship between GLP-1RAs and 24 gastrointestinal disorders, supplemented by sensitivity analyses, Cox proportional hazards regression analysis and mediation testing. Using single-nucleotide polymorphisms significantly associated with GLP1R gene expression as instrumental variables, we mimicked the lifelong pharmacological effects of GLP-

Postoperative ileus following radical cystectomy in current urological practice: Incidence and risk factors.

INTRODUCTION: Postoperative Ileus (POI) is a common complication after Radical Cystectomy (RC), often leading to delayed recovery and extended hospital stay. Given the developments in perioperative techniques and the implementation of enhanced recovery protocols, it is essential to evaluate the risk factors contributing to POI. Our objective was to analyze various risk factors that impact the incidence of POI after RC in contemporary practice. METHODS: A retrospective study of bladder cancer patients who underwent RC with ileal conduit or neobladder was conducted. Baseline characteristics and

Postoperative Ileus After Rectal Cancer Surgery: Assessing Incidence, Severity, and Impact Across Open, Laparoscopic, and Robotic Approaches.

Background/Objectives: Postoperative paralytic ileus (POI) is a common complication after rectal resections. Although it is often argued that laparoscopic or robotic surgery reduces ileus compared to open surgery, research indicates that the incidence rates remain considerably high after minimally invasive surgery (MIS), and it is unclear whether laparoscopy or robotic surgery confers lower ileus rates. Furthermore, the literature lacks consistency in defining ileus and does not adequately address the severity levels of this complication. This study aims to compare the incidence and severity o

Adverse Events Associated with Over-the-counter Antidiarrheals in Children: An Observational Real-world FAERS-based Disproportionality Analysis Study.

BACKGROUND: Over-the-counter (OTC) antidiarrheal medications are commonly used despite potential risks for diarrheal management in children. We aimed to assess the reported suspected adverse events associated with OTC antidiarrheals from the FDA Adverse Event Reporting System (FAERS). METHODS: We conducted a disproportionality analysis of suspected adverse event reports pertaining to commonly used antidiarrheals in the pediatric age group (aged < 16 years) as follows: bismuth subsalicylate, diphenoxylate, atropine/diphenoxylate, kaolin, pectin, and loperamide. Spontaneous reports from the FAE

Feasibility and safety of surgical strategy based on tumor characteristics and location in laparoscopic and endoscopic cooperative surgery for non-ampullary duodenal tumors.

BACKGROUND AND AIM: Recently, several studies have demonstrated the safety and feasibility of laparoscopic and endoscopic cooperative surgery for duodenal tumors (D-LECS). However, the standard procedure for D-LECS has not been established. Herein, we introduce the feasibility and safety of a surgical strategy based on tumor characteristics and location in D-LECS. METHODS: This retrospective single-center study included 17 consecutive patients with duodenal tumors who underwent D-LECS between October 2017 and November 2023. RESULTS: Two, 13 and 2 tumors were located in the first, second and

Impact of early enteral nutrition after transit reconstruction in pediatrics.

INTRODUCTION: Restoration of intestinal continuity is a common procedure in pediatric surgery. However, controversy persists regarding the optimal timing for initiating postoperative enteral nutrition. Although traditionally a delayed start has been recommended to reduce the risk of complications such as anastomotic leak or paralytic ileus, recent studies in adult populations suggest benefits of early initiation. Evidence in the pediatric population is limited. This study evaluates the safety and clinical outcomes of early versus late enteral nutrition in children undergoing intestinal continu

Source-stratified gut-extraintestinal organ crosstalk in sepsis-associated acute gastrointestinal injury and paralytic ileus: the gut as both driver and target.

Sepsis-associated acute gastrointestinal injury and paralytic ileus are common but underrecognized manifestations of systemic critical illness and are closely linked to feeding intolerance, barrier failure, secondary infection, and progression to multiorgan dysfunction. However, the gut in sepsis should not be viewed solely as a passive target of distant organ injury. Owing to its unique microbial burden, highly specialized epithelial-immune barrier, and central role in host-microbe and metabolic homeostasis, the gut may function either as an initiator of injury amplification or as a vulnerabl

Case Report: Acute pancreatitis obscured by paralytic ileus in a patient with extensive burns.

Extensive burns are associated with numerous severe complications. While paralytic ileus is relatively common, the co-occurrence of acute pancreatitis (AP) is rare, clinically insidious, and highly fatal. Diagnosis is particularly challenging due to masking by critical illness and coexisting ileus. We present a 32-year-old male with 65% TBSA flame burns and inhalation injury, admitted in hypovolemic shock. Resuscitation followed Parkland protocol, and escharotomy was performed. On day 2, abdominal distension and reduced bowel sounds suggested paralytic ileus, managed with nasogastric decompres

Establishment and verification of influencing factors and prediction model of early intestinal paralysis after radical operation of endometrial cancer.

Radical operation is a primary treatment modality for endometrial carcinoma (EC). While effective, it is associated with postoperative complications, including postoperative paralytic ileus (POI). In this retrospective study, clinicopathological data from 587 EC patients who received radical surgery were analyzed. First, logistic regression analysis was used to identify influencing factors for postoperative POI in EC patients. Subsequently, a risk prediction model was developed based on the identified risk factors. The results showed that a history of cesarean section, prior abdominal surgery,

A Case Report of Acute Intermittent Porphyria Accompanied by Severe Peripheral Neuropathy.

Background: Acute intermittent porphyria (AIP) is the most common and severe form of acute hepatic porphyria, caused by heterozygous mutations in the HMBS gene. Due to its non-specific clinical manifestations and low clinical awareness among clinicians, AIP is frequently misdiagnosed, leading to significant diagnostic delays and potentially fatal complications. Case presentation: We report a 20-year-old female patient who presented with a 9-month history of recurrent abdominal pain, paralytic ileus, unexplained liver injury, and hyponatremia, followed by progressive limb weakness.
