Short-term clinical outcomes of nasoduodenal tube versus feeding jejunostomy after minimally invasive esophagectomy: a retrospective cohort study with exploratory microbiome analysis.

BACKGROUND: Esophagectomy remains an effective treatment for esophageal cancer but is associated with severe postoperative complications, in which nutritional status plays a critical role. The optimal route for enteral feeding remains uncertain. To address this uncertainty, we compared the short-term clinical outcomes of the two approaches and exploratorily analyzed the differences in gut microbiota. METHODS: This retrospective cohort study enrolled patients who underwent minimally invasive esophagectomy (MIE) for esophageal squamous cell carcinoma (ESCC) between July 2018 and June 2021. Prim

Neonatal appendicitis with rectal bleeding: an unusual presentation.

BACKGROUND: Neonatal appendicitis is an exceptionally rare condition with a high risk of delayed diagnosis and perforation. Its nonspecific clinical presentation may mimic other causes of acute abdomen and gastrointestinal bleeding in newborns, particularly necrotizing enterocolitis. CASE PRESENTATION: A 36-week, 2500-g male neonate developed abdominal distension, irritability, decreased oral intake, and bright red rectal bleeding on the second day of life. Abdominal radiography demonstrated multiple air-fluid levels, while ultrasonography revealed dilated bowel loops and free intraperitoneal

Long-term trends in pediatric intussusception in Hokkaido, Japan, 2007-2024 across the rotavirus vaccination era.

OBJECTIVES: Pediatric intussusception is the most common cause of acute intestinal obstruction in young children. We investigated long-term epidemiological trends in pediatric intussusception before and after rotavirus vaccine introduction and during the COVID-19 pandemic in Hokkaido, Japan. METHODS: A retrospective multicenter surveillance study was conducted at 17 hospitals in Hokkaido between 2007 and 2024. Clinical characteristics, management, outcomes, and viral antigen test results were collected. Only cases fulfilling Brighton Level 1 diagnostic certainty were included. Annual incidenc

Langerhans cell histiocytosis presenting as a cecal mass in an adolescent: a rare surgical case report.

INTRODUCTION AND IMPORTANCE: Langerhans cell histiocytosis (LCH) is a rare, idiopathic, multifocal disease that commonly occurs in the skin and skeletal system, although involvement of the gastrointestinal tract is exceptional. Only a limited number of cases have been reported in the literature, particularly at the intestinal level, often with incidental diagnosis after colonoscopic polypectomy, and presentation as a cecal mass is extremely uncommon. CASE PRESENTATION: A 15-year-old boy presented with abdominal pain and a non-tender right iliac fossa mass. Hematological and biochemical tests

A rare case of lesser omental hernia in a male patient following total colectomy.

INTRODUCTION AND IMPORTANCE: A lesser omental hernia is a rare internal hernia in which intestinal loops become entrapped through a defect in the lesser omentum, potentially causing bowel obstruction. Its diagnosis is challenging because of nonspecific symptoms that resemble those of other gastrointestinal diseases. This report presents a case of lesser omental hernia following total colectomy. CASE PRESENTATION: A 65-year-old male with a history of laparoscopic total colectomy for ulcerative colitis 4 years prior presented with acute abdominal pain and distension. Computed tomography (CT) re

The vicious cycle of nonoperatively managed adhesive small bowel obstruction: does weekend admission play a role?

BACKGROUND: Data on the influence of nonpatient factors, such as day of admission, on adhesive small bowel obstruction (aSBO) management are limited. This study assesses the association between weekend admission, management approach, and postdischarge outcomes. METHODS: We conducted a retrospective cohort study using the 2020 and 2021 National Readmission Database. Adult patients admitted with aSBO between January and March of each study year were followed for 9 months after discharge. Patients were stratified by admission day (weekend vs weekday) and management (nonoperative [NOM] vs operati

A Retrospective Multicenter Study Examining Clinical and Imaging Predictors of Emergent Surgery in Small Bowel Obstruction.

BACKGROUND: Operative management for small bowel obstruction (SBO) is indicated for patients with concerns about bowel ischemia or failure of conservative therapy; however, most patients respond to nonoperative management. OBJECTIVE: Our primary objective was to identify clinical and imaging predictors of operative management in SBO, stratified by the timing of intervention, with an emphasis on emergent operative management within 24 h of presentation. Secondarily, we sought to explore which patient populations may be candidates for future prospective evaluation of computed tomography (CT)-av

Report of Meckel Diverticulum Mimicking Stricturing Crohn's Disease in an Adolescent Boy.

Inflammatory bowel disease, specifically Crohn's disease (CD), can present with hematochezia, iron deficiency anemia, intestinal obstruction, elevated fecal calprotectin often overlapping with other conditions. We report a 16-year-old adolescent boy with presumed stricturing CD whose anti-tumor necrosis factor therapy failed to respond. Persistent iron deficiency anemia and elevated fecal calprotectin prompted reevaluation imaging, raising concerns for the Meckel diverticulum. Surgical resection resulted in resolution of anemia. This case highlights the importance of reassessing a diagnosis in

Mechanosensitive ion channels in intestinal homeostasis and disease.

Mechanical deformation is integral to intestinal physiology, linking luminal transit to epithelial adaptation and motor control. These physical cues engage multiple mechanotransduction pathways that coordinate cellular adaptation and help preserve intestinal homeostasis. Inflammation, obstruction and matrix remodelling alter both the physical input and the state of the responding cell, allowing normally transient signals to persist or acquire pathological consequences. Ion channels act at several levels of this process. PIEZO1 and PIEZO2 provide a direct route for force-gated cation entry; mec

Intestinal cystic lymphangioma.

A 7-year-old girl, coming from a rural area in Cameroon, presented to the emergency department with a 3-months history of abdominal pain. Her family also reported vomiting and minimal food intake for two weeks. Physical examination showed a palpable and mobile abdominal mass. An ultrasound showed a large intrabdominal multicystic lesion of about 10cm, close to the intestine and with no solid lesions in other organs. A laparotomy was scheduled and a mobile mass dependent on the jejunum was found. The mass caused an intestinal obstruction and was composed of several large cysts with whitish flui

A Twisted Presentation: Adult Intestinal Malrotation Complicated by Caecal Volvulus and Perforation.

Intestinal malrotation is a congenital anomaly caused by incomplete rotation and fixation of the midgut during embryological development. Although it is usually diagnosed in infancy, rare adult cases may present with acute intestinal obstruction or perforation. Cecal volvulus is an uncommon but potentially life-threatening complication of malrotation. We report a 35-year-old woman who presented with severe abdominal pain, progressive distension, and absolute constipation for two days. Clinical examination revealed generalized peritonitis with absent bowel sounds, and an erect radiograph demons

Adult Onset Hirschsprung's Disease With Overlapping Presentation of Sigmoid Volvulus: Two Case Reports With Literature Review at Tikur Ambessa Specialized Hospital, AAU, Ethiopia.

Adult-onset Hirschsprung's disease (HSD) is uncommon and frequently leads to diagnostic difficulty after infancy. It is a congenital illness that causes intestinal obstruction due to a lack of ganglion cells. Approximately 1 in 5000 live newborns have this illness, and 95% of HSD cases are identified in infants. This case report will fill a knowledge gap at a low-resource setting where published papers on this topic are extremely rare, and will provide insight into diagnosis and surgical management options that are feasible in resource-limited environments with good outcomes. In this case repo

A jejunal adenoma with high-grade dysplasia in the excluded limb 25 years after biliopancreatic diversion: A case report.

Biliopancreatic diversion (BPD) creates a biliopancreatic limb that is inaccessible to conventional endoscopic surveillance, and its long-term neoplastic risk is unknown. A woman in her late 40 s presented 25 years after BPD with recurrent protein malnutrition and subocclusive symptoms. Computed tomography showed jejunal intussusception. At surgery, the intussusception was localized to the proximal biliopancreatic limb, 10-20 cm from the ligament of Treitz and remote from the entero-enterostomy. A 2.6-cm tubulovillous adenoma with focal high-grade dysplasia was found at the corresponding site

Gastrointestinal Manifestations of Systemic Sclerosis: Screening and Management to Improve Outcomes.

Gastrointestinal (GI) involvement is among the most common and clinically important manifestations of systemic sclerosis (SSc), affecting up to 90% of patients and substantially contributing to morbidity and reduced quality of life. SSc-related GI disease results from microvascular injury, enteric nervous system dysfunction, and progressive smooth muscle atrophy with fibrosis, producing widespread dysmotility throughout the gastrointestinal tract. Clinical involvement can extend from the oral cavity to the anorectum, with esophageal dysfunction, gastroesophageal reflux disease (GERD), gastropa

Current Status of PSMA-Targeted Agents Based on Small Molecules for Prostate Cancer in Preclinical Studies.

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC). METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

Initial Adhesive Small Bowel Obstruction Management Pathway Drives Infectious Complication Occurrence.

INTRODUCTION: The Bologna guideline outlines three small bowel obstruction (SBO) management pathways. It remains unclear how pathway selection influences post-operative infections. METHODS: A multi-national, prospective, observational, audit of SBO management (November 1, 2023-May 31, 2024) captured demographics, care, and outcomes. Patients were grouped by pathway (successful non-operative management [NOM], NOM followed by surgery [NOM-T], direct to surgery [DTS]). Intergroup comparisons by chi-square or Fisher exact test, significance for p < 0.05. RESULTS: A total of 1,737 patients were a

Transverse blowhole colostomy versus Hartmann's for urgent management of large bowel obstruction secondary to diverticular stricture.

PURPOSE: Sigmoid resection with end colostomy (Hartmann's procedure) is the procedure of choice when a large bowel obstruction secondary to diverticular disease requires surgery. Less morbid alternatives are less common. This study compares a transverse blowhole colostomy as a first-stage intervention in diverticular-associated obstruction. Our primary aim was to compare colostomy reversal rates and postoperative outcomes between blowhole colostomy and Hartmann's procedure. METHODS: This is a single-center, retrospective cohort study.
