Recurrent Jejunojejunal Intussusception of the Common Limb in a Patient With Roux-en-Y Gastric Bypass: A Report of an Unusual Case and Literature Review.

Intestinal intussusception is a rare late complication of Roux-en-Y gastric bypass (RYGB), typically presenting without a lead point, often in a retrograde pattern, and most commonly involving the jejunojejunal anastomosis and common limb. We report the case of a 49-year-old female with a history of RYGB performed in 2000 and prior intussusception in 2010, who presented with an acute abdomen. Contrast-enhanced computed tomography demonstrated intestinal obstruction secondary to recurrent jejunojejunal intussusception, with associated signs of vascular compromise. The patient underwent explorat

Unmasking the "Braided" Bowel: Chronic Intestinal Pseudo-Obstruction-A Case Report.

Background and Clinical Significance: Chronic intestinal pseudo-obstruction (CIPO) clinically presents as a mechanical obstruction. When patients present with signs of impending ischemia, emergency exploratory laparotomy is indicated. Differentiating CIPO from other etiologies preoperatively is highly challenging, making intraoperative findings and definitive histopathology crucial for an accurate diagnosis. This report aims to elucidate a novel macroscopic presentation of myopathic CIPO and explore the potential biomechanical mechanisms underlying the observed structural anomaly. Case Present

Bowel endometriosis: from pathogenesis to clinical management.

Bowel involvement is a severe manifestation of deep endometriosis that affects approximately 8-12% of women with endometriosis. Bowel endometriosis is most commonly localised in the rectosigmoid colon and frequently coexists with other pelvic lesions. The pathogenesis of bowel endometriosis is multifactorial, involving hormonal, inflammatory, immune, genetic, and anatomical factors. Clinical presentation ranges from asymptomatic disease to severe gastrointestinal and pelvic symptoms, which can mimic other digestive disorders such as irritable bowel syndrome and inflammatory bowel disease. Strangulated Spigelian Hernia With Small Bowel Obstruction Requiring Intestinal Resection: An Uncommon Cause of Acute Abdomen.

Spigelian hernia is a rare abdominal wall hernia. It occurs more commonly in women between the fifth and seventh decades of life and is typically located within the Spigelian belt. Because of its interparietal location and small fascial defect, diagnosis may be challenging, and the risk of incarceration and strangulation is high. We present the case of a 64-year-old woman who presented to the emergency department with abdominal pain, nausea, and obstipation. Clinical evaluation was consistent with an acute abdomen secondary to intestinal obstruction. Emergency exploratory laparotomy revealed a

Diagnostic Pitfalls in Obturator Hernia: A Case Report and Literature Review.

Obturator hernia is a rare clinical entity that predominantly affects elderly, thin women due to anatomical factors such as a wider pelvis and the loss of preperitoneal fat. Despite its classic description, the preoperative diagnosis remains a major challenge due to its deep location and non-specific presentation as a mechanical small bowel obstruction. This report describes the case of a 66-year-old female whose history of prior abdominal surgeries created a diagnostic bias toward adhesive obstruction, resulting in an initial period of conservative management. The most significant clinical si

Adult Ileocolic Intussusception Secondary to an Inflammatory Fibroid Polyp of the Terminal Ileum Presenting as Acute Small Bowel Obstruction: A Case Report.

Intussusception in adults is an uncommon cause of intestinal obstruction and is usually associated with an underlying structural lesion, unlike pediatric cases. In most adult patients, a pathological lead point is identified, frequently neoplastic in origin, making surgical management the standard treatment. We present the case of a 66-year-old man with a history of type 2 diabetes mellitus who presented with colicky abdominal pain, vomiting, and hematochezia. Physical examination revealed abdominal distension, tympany, diffuse tenderness, rebound tenderness, and hyperactive bowel sounds. Comp

Double, double toil and trouble: a case of combined esophageal and gastrointestinal duplication cysts in infancy and a review of the literature.

INTRODUCTION AND IMPORTANCE: Gastrointestinal duplication cysts are rare congenital anomalies, usually solitary and most often involving the ileum and esophagus. Multiple non-contiguous duplications are exceptionally uncommon and may mimic more frequent neonatal thoracic or abdominal conditions. CASE PRESENTATION: We report a term female neonate with a prenatal suspicion of congenital pulmonary airway malformation. Postnatally, she developed respiratory distress due to a large thoracic cyst, which was resected thoracoscopically and confirmed to be an esophageal duplication cyst. Weeks later,

Factors associated with acute intestinal ischemia after cardiac surgery: A propensity score-matched study.

OBJECTIVES: Acute intestinal ischemia from cardiac surgery is rare but highly lethal. Conventional risk models lack preoperative markers reflecting splanchnic arterial capacity. We hypothesized that subclinical abdominal atherosclerosis predisposes to perioperative intestinal ischemia and introduced a novel radiologic measurement-the total enteric cross-sectional artery-area (TECSA)-to quantify anatomical obstruction of intestinal flow capacity. METHODS: In this retrospective, propensity score-matched study, we identified 53 cases of acute intestinal ischemia among 9301 patients who underwent

The rare case of carcinoid tumor in mid ileal segment causing intestinal obstruction.

Carcinoids are rare neuroendocrine tumors (NETs) that are thought to arise from the enterochromaffin (Kulchitsky) cells found in the Lieberkuehn crypts of the intestinal gut. The term enterochromaffin refers to the ability to stain with chromium salts, a common feature of 5-HT-containing cells. The median age is 63 years. Using barium examinations, carcinoid tumors of the small intestine and colon can be found as polypoidal or ulcerated masses, submucosal filling deficiencies, or both. As the main indicator of a neuroendocrine tumor's potential for malignancy, consider tumor differentiation. I

Management of Colonic Intramural Hematoma Complicated by Intestinal Obstruction in a Patient With Hemophilia: A Case Report and Literature Review.

Colonic intramural hematoma is an exceptional complication of hemophilia. We report the case of an 18-year-old adolescent with severe hemophilia A who presented with a large colonic intramural hematoma complicated by intestinal obstruction following minor abdominal trauma. Conservative management, combining factor VIII replacement therapy, initial resuscitation, and bowel rest, resulted in a favorable outcome, with complete resolution of the hematoma and no need for surgical intervention. This case highlights the effectiveness of early medical management in this rare clinical situation.

Intestinal Endometriosis in a 38-Year-Old Woman.

Endometriosis is a common gynecological disorder that can involve the intestinal tract and may present with bleeding, bowel obstruction, and, rarely, perforation or malignant transformation. Intestinal endometriosis can mimic malignant tumors or other conditions, which may lead to unnecessary aggressive surgical resections. It continues to be a challenging diagnosis to make preoperatively. We report a case of intestinal endometriosis with endoscopic features of lymphatic dilatation-like changes, which correlated with the pathological findings, aiming to provide a reference for the endoscopic r

Acute intestinal bleeding due to enteric duplication cyst (EDC) and visceral malrotation in a young male adult: A case report.

Enteric duplication cysts (EDCs) are rare congenital abnormalities of the digestive tract. They may occur with different clinical presentations-including acute intestinal obstruction, perforation or bleeding. The aim of this educational case report is to present the diagnostic and therapeutic process of a young male patient admitted for intestinal bleeding. The peculiarity of this case lies in the association between a rare pathology, EDC, and an equally unusual anatomical condition such as intestinal malrotation. A 27-year-old Italian Caucasian man was admitted with diarrhea and currant jelly

Urological Causes Mimicking Acute Abdomen: Diagnostic Challenges in General Surgical Emergency Practice.

Background Urological disorders are important differential diagnoses in patients presenting with an acute abdomen to a general surgical emergency unit. Conditions such as ureteric colic, obstructive uropathy, acute pyelonephritis, urinary retention, renal trauma, and testicular torsion may simulate appendicitis, intestinal obstruction, biliary colic, or perforation peritonitis. This clinical overlap may delay correct diagnosis, particularly when urinary symptoms are subtle or absent. This study aimed to assess the pattern of urological conditions mimicking acute abdomen and the diagnostic diff

Invasive Urothelial Carcinoma Presenting With Combined Obstructive Uropathy and Ischemic Small- and Large-Bowel Obstruction.

Intestinal involvement by urothelial malignancy is uncommon and can obscure the primary diagnosis. We report a woman in her 70s who presented with shock, profound anemia, acute kidney injury, and bilateral hydronephrosis caused by a necrotic pelvic mass suspected to arise from the bladder. Despite resuscitation and urinary decompression, she developed abdominal distention, rising lactate levels, and respiratory decline. Exploratory laparotomy revealed combined small- and large-bowel obstruction with ischemic bowel caused by an invasive pelvic mass. Pathology from the resected small bowel showe

Double Closed-Loop Obstruction Caused by Right Paraduodenal Hernia With Intestinal Malrotation.

Double closed-loop (DCL) obstruction is a rare but diagnostically important computed tomography (CT) finding observed in a subset of strangulated intestinal obstructions. A man in his 40s presented with sudden abdominal pain. CT demonstrated two simultaneous closed loop obstructions with a sac-like appearance, along with the superior mesenteric vein (SMV) rotation sign and absence of the duodenal horizontal segment, leading to a preoperative diagnosis of right paraduodenal hernia (RPDH) with intestinal malrotation. Emergency laparotomy was performed within three hours. Two closed loops of smal

Primary Colonic Diffuse Large B-cell Lymphoma: A Diagnostic Challenge.

Primary colonic diffuse large B-cell lymphoma (DLBCL) is a rare extranodal malignancy that accounts for a small percentage of colonic malignancies. With nonspecific gastrointestinal symptoms such as weight loss, abdominal pain, and bloody stools, it may mimic the presentation of inflammatory bowel disease. Symptoms usually present in advanced stages of the disease. We present the case of a 19-year-old man with a history of one year of intermittent abdominal pain, unintentional weight loss, diarrhea, and bloody stools. Initial abdominal ultrasound reported distended bowel loops with no evidence

Adult intestinal malrotation with Ladd's bands causing acute small-bowel obstruction in a virgin abdomen: a case report.

INTRODUCTION AND IMPORTANCE: Intestinal malrotation is a rare congenital anomaly that results from incomplete midgut rotation. Although 90% of cases present in infancy, adult presentations account for only 0.2%-0.5% of cases. Congenital peritoneal bands causing obstruction in a virgin abdomen are rarely considered, making this a clinically important and easily overlooked diagnosis.
