Regional and Temporal Dynamics of Dengue in Mexico (2020-2024): A Comprehensive Epidemiological Analysis.

Dengue remains a major public health concern in Mexico, with incidence and clinical burden increasing substantially in recent years. Using national surveillance data from 2020 to 2024, this study provides a comprehensive overview of the changing epidemiological landscape, with an emphasis on geographic distribution, temporal trends, serotype dynamics, and disease severity. Over this period, Mexico saw an almost fivefold increase in dengue cases, reaching over 123,000 confirmed infections and 426 deaths in 2024, the highest annual number recorded. Seasonal patterns shifted, with sustained trans

Monocyte-associated C1q upregulation and complement activation contribute to vascular endothelial and multi-organ injury in severe dengue.

Dengue virus (DENV) infection causes a broad spectrum of clinical manifestations, ranging from self-limiting dengue fever (DF) to life-threatening severe dengue (SD). Increased vascular permeability and endothelial dysfunction are hallmarks of SD and contribute to plasma leakage, shock, and multi-organ injury, yet the underlying mechanisms remain incompletely understood. Here, we integrated dynamic bulk and single-cell RNA sequencing data from peripheral blood mononuclear cells (PBMCs) of dengue patients at different disease stages to investigate the mechanisms associated with disease progress

Association between vitamin D level and dengue severity in children: A prospective study in an Eastern Indian City.

BACKGROUND: Dengue fever, a significant health concern in tropical regions, including India, presents in healthcare settings with a disease spectrum ranging from mild to severe illness. Anti-body dependent enhancement (ADE) and non-ADE mechanism such as nutrition (vitamin D, etc.) have been suggested for dengue severity. The relationship between vitamin D, a known immune modulator, and the severity of dengue fever in pediatric patients was investigated. METHOD: This prospective study, enrolled 99 children aged 1-18 years with confirmed dengue fever between April 2023 and September 2024. Patie

Longitudinal Trajectories of Platelet Indices Across Dengue Severity in Children: A Linear Mixed-Model Analysis.

Background Dengue has a high disease burden, and some affected children progress to severe dengue (SD), making early identification of deterioration a clinical priority. Automated platelet indices such as mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT) are generated at no additional cost by routine hematology analyzers. However, they are usually interpreted as static measurements and cannot distinguish peripheral platelet destruction from impaired bone marrow production. Linear mixed modeling (LMM) is well suited to correlated repeated measurements but has

Aberrant systemic acute-phase complement responses in conjunction with soluble CR1 attribute to varying grades of dengue disease severity.

BACKGROUND: Dengue virus (DENV) infection poses a serious health burden across the tropical and sub-tropical regions. The role of complement factors and acute-phase reactants in clinical dengue remains ambiguous. METHODS: The cross-sectional study enrolled 156 participants, with 114 confirmed clinical dengue cases and 42 healthy controls. Serological profiling (NS1, anti-DENV IgM, and anti-DENV IgG), estimation of serum acute-phase reactants, clinico-laboratory parameters, and viral load were performed to classify dengue patients under dengue with warning signs (DWS+, n = 35), dengue without

Long-term efficacy and safety of the single-dose tetravalent Butantan dengue vaccine.

In individuals with prior dengue virus (DENV) exposure, subsequent heterotypic infection can increase the risk of severe disease. A single-dose dengue vaccine that protects against the four DENV serotypes across a wide age range and regardless of DENV serostatus is needed. Here we report the long-term safety and efficacy of Butantan dengue vaccine (Butantan-DV), a live, attenuated, tetravalent dengue vaccine, among participants ages 2-59 who were randomized 2:1 in a double-blind, placebo-controlled, phase 3 trial in Brazil. A primary objective was to evaluate vaccine efficacy (VE) against reve

Evaluation of Serum Ferritin Levels and Their Correlation With Clinical Outcomes in Dengue Virus Infection in a Population of Older Adults.

OBJECTIVES: Prior studies have associated elevated ferritin levels with worse dengue outcomes but data from older adults remain limited. The aim of this study is to evaluate the serum ferritin levels and their correlation with disease severity in a population of older adults. METHODS: We included 284 patients aged 60 years or older with a diagnosis of dengue attended during a high dengue fever outbreak. We analysed demographic variables, the presence of comorbidities, hospitalisation, death and correlated the serum ferritin level with three categories of dengue. Patients' ferritin levels were

Association of Viraemic Phase Viral Load, Antibody Responses, and Immune Biomarkers With Severe Dengue.

Severe dengue is influenced by the virus serotype, viral load, host exposure status, immune response, host genetics, and other host factors. The objective of the present study was to identify a panel of prognostic markers for severe dengue during the viraemic phase. We investigated 326 real-time RT-PCR positive dengue cases to find out the association of viral load, antibody titers, and immune status. Plasma levels of 27 cytokines, chemokines, and endothelial cell-related factors were estimated in a subset of 206 patients. Viral load was lower in DENV-3 cases compared to DENV-1 and DENV-2 case

Evidence that dengue virus envelope protein in patient platelets originates from infection of megakaryocytic precursors.

Dengue virus infection is a major global health problem with clinical outcomes ranging from mild febrile illness to severe disease. Although a reduction in platelet counts is a common feature of dengue, the mechanisms underlying the presence of viral antigens in circulating platelets remain incompletely understood. In this study, we investigated whether infection of megakaryocytic precursors contributes to the generation of platelets carrying dengue virus proteins. Using the human megakaryocytic precursor cell lines K562 and MEG-01 cell lines as in vitro models, we show that reference strains

Effectiveness of Montelukast in Reducing the Risk of Severe Dengue in Dengue Fever Patients: An Evidence-Based Case Report.

BACKGROUND: Dengue fever continues to spread worldwide, particularly in tropical regions. Some patients with dengue fever may progress to severe dengue, which is associated with significantly higher morbidity and mortality. Despite this, no definitive treatment has been found to prevent its progression. Montelukast, a leukotriene receptor antagonist, has shown potential in reducing plasma leakage, a key factor in the pathophysiology of severe dengue. Therefore, this study aimed to evaluate the effectiveness of montelukast in reducing the risk of severe dengue. METHODS: A systematic literature

Profiling of plasma exosome cytokines as biomarkers of severe dengue.

INTRODUCTION: Dengue is a global health threat, with severe cases causing significant complications. Cytokines have been proposed as potential indicators of disease severity; however, the short plasma half-life and pre-analytical instability of free-form cytokines limit their clinical applicability. Exosome-encapsulated cytokines are protected by a lipid bilayer and may provide a more stable and integrated representation of host immune responses. METHODS: In this cross-sectional study, we analyzed single time-point plasma samples collected from patients during clinical evaluation between July

Profiling of Secretory mRNAs in Dengue Virus Infection: Possible Implications in Thrombocytopenia.

Dengue virus is a global health concern resulting in approximately 100 million infections and 21 000 deaths annually. The disease progresses from dengue without warning signs (PLAN A) to dengue with warning signs (PLAN B) and then to severe dengue (PLAN C). However, the molecular mechanisms underlying this transition are not known. In the absence of effective antivirals and vaccines, predicting disease severity early is the only option for better disease management. Extracellular vesicles (EVs) are membrane-bound vesicles secreted from cells and contain miRNA, proteins, and mRNA, which play a

Integrated Surveillance Strategies and Clinical Characteristics of Arboviral Diseases in a Pediatric Cohort in Merida, Mexico, 2021-2023.

Dengue, chikungunya, and Zika are Aedes-borne diseases (ABDs) of global health significance. Epidemiological studies with sensitive case detection are critical for evaluating vector control strategies to prevent ABDs; however, data on which surveillance method is most effective are limited. The performance of five surveillance methods (home visits, phone calls, SMS reminders, toll-free phone line (TF) and the Ministry of Health surveillance platform) was assessed, and the clinical characteristics of ABD cases in the targeted indoor residual spraying trial, which quantified the efficacy of prev

DENV-Specific IL-10-Producing Regulatory T Cells Are Associated With Mild Clinical Outcomes in Dengue.

Dengue is classified into mild dengue, which is treated at outpatient clinics, dengue with warning signs (WS+), which requires hospitalization, and severe dengue, which can be fatal. It is assumed that severe manifestations result from uncontrolled immune activation. Regulatory T cells (Tregs) are crucial for controlling inflammation in various diseases, but are poorly studied in dengue. This study aimed to investigate the Tregs phenotype in different clinical forms of dengue infection. Plasma and peripheral blood mononuclear cells (PBMCs) were collected from healthy donors and dengue patients

Severe Dengue-Associated Hepatitis in an Adolescent Treated With Oral Acetylcysteine: A Case Report.

Dengue infection may lead to liver involvement ranging from mild transaminase elevation to severe hepatitis and, rarely, acute liver failure. Management is mainly supportive, and no specific therapy exists. We report the case of a 13-year-old adolescent with dengue with warning signs who developed severe hepatitis (aspartate aminotransferase (AST) 2034 U/L, alanine transaminase (ALT) 532 U/L) and thrombocytopenia. Oral N-acetylcysteine (NAC) was initiated as adjuvant therapy. During hospitalisation, a reduction in aminotransferase levels, clinical improvement, and recovery of platelet count we
