Spontaneous Renal Hemorrhage Secondary to MRSA Renal Abscess Requiring Nephrectomy After Failed Percutaneous Drainage: A Case Report.

INTRODUCTION: Spontaneous renal hemorrhage (SRH) caused by renal infection is extremely rare. Furthermore, sepsis caused by methicillin-resistant Staphylococcus aureus (MRSA) has a high mortality rate and requires prompt source control. CASE PRESENTATION: A 63-year-old man with severe diabetes presented to the emergency department in shock. Computed tomography revealed a left renal subcapsular hematoma. Based on his clinical course and history, infection-induced SRH was suspected. Blood and urine cultures yielded MRSA. Although percutaneous drainage was performed after the initial bleeding st

Five-Biomarker Definition for Hypervirulent Klebsiella pneumoniae Bloodstream Infection and Abscess Complications, Japan, 2022-2024.

Hypervirulent Klebsiella pneumoniae (hvKp) is an emerging cause of invasive infection, but clinical relevance of a 5-biomarker genotypic definition remains unclear. We analyzed 207 episodes of K. pneumoniae bloodstream infection at a tertiary hospital in Japan during 2022-2024. We defined hvKp by PCR detection of all 5 biomarkers: rmpA, rmpA2, iucA, iroB, and peg-344. Overall, hvKp accounted for 14% (28/207) of episodes. Abscess complications occurred in 61% (17/28) of hvKp episodes and 13% (23/179) of classical K. pneumoniae episodes. hvKp case-patients had longer median hospitalization (28 v

Spontaneous Mesenteric Hematoma Following Severe Coughing in a Patient on Dual Antiplatelet Therapy: A Case Report.

Spontaneous mesenteric hematoma (SMH) is a rare cause of acute abdominal pain and may mimic more common intra-abdominal conditions, creating diagnostic challenges. A 67-year-old man with hypertension, hyperlipidemia, coronary artery disease status post four-vessel coronary artery bypass grafting, and chronic dual antiplatelet therapy with aspirin and clopidogrel presented with acute worsening lower abdominal pain. He reported a severe coughing episode during a recent bronchitis-like illness approximately two weeks before presentation, followed by lower abdominal pain in the same general region

G-CSF-Associated Superficial and Deep Neutrophilic Folliculitis of the Scalp with Folliculitis Decalvans-Like Features.

Granulocyte colony-stimulating factor (G-CSF) is known to induce several cutaneous neutrophilic reactions, including Sweet syndrome-like eruptions, neutrophilic panniculitis, neutrophilic eccrine hidradenitis, and sterile folliculitis. We report the case of a male patient with an unclassified malformative syndrome and chronic neutropenia who developed acute painful suppurative nodular scalp lesions while receiving long-term G-CSF therapy and during a period of marked neutrophilia. Histopathological examination showed superficial and deep suppurative neutrophilic folliculitis with follicular ab

[Branchial cleft anomalies of the neck : Embryology, clinical presentation, and management].

Branchial cleft anomalies are the second most common congenital neck lesion. They present as cysts, sinuses, or fistulas arising from remnants of the pharyngeal apparatus; second arch anomalies predominate. The embryologic origin determines their anatomical relationships to nerves, vessels, and the pharynx and guides diagnosis and surgical planning. Typical findings are cervical swelling, cutaneous discharge, or recurrent infection. Third and fourth branchial anomalies may cause recurrent neck abscesses or acute suppurative thyroiditis, particularly in children. Ultrasonography is the primary

Community-Acquired Klebsiella pneumoniae Meningitis With Liver Abscess and Neurological Sequelae in an Immunocompetent Adult: A Case Report.

Klebsiella pneumoniae meningitis (KPM) is a rare but severe form of community-acquired bacterial meningitis that is often associated with high mortality and neurological sequelae. We report a 46-year-old previously healthy male who presented with acute altered mental status and high-grade fever. Cerebrospinal fluid analysis was consistent with bacterial meningitis, and cultures from the cerebrospinal fluid and blood showed Klebsiella pneumoniae (K. pneumoniae) growth. The clinical course of the patient was complicated with seizures and acute right-sided hemiplegia. Persistent fever (despite ap

Recurrent severe pyogenic infections revealing neutrophil-specific granule deficiency caused by a novel CEBPEsplice-region variant: a case report.

Neutrophil-specific granule deficiency is an exceptionally rare autosomal recessive primary immunodeficiency caused by pathogenic variants in the CCAAT/enhancer-binding protein epsilon (CEBPE) gene. Defective neutrophil maturation results in impaired bactericidal activity and recurrent pyogenic infections, predominantly affecting the skin and soft tissues. We describe two affected siblings from a family of four carrying a previously unreported CEBPE missense substitution in the splice region (p.K170N; c.510G>C). A 37-year-old woman presented with lifelong recurrent pyogenic skin infections, in

CNS Neoscytalidium dimidiatum: A Rare Cause of Fungal Brain Abscess.

Neoscytalidium dimidiatum is a dematiaceous fungus that primarily affects plants but can rarely act as an opportunistic pathogen in humans, particularly in immunocompromised patients. It typically causes superficial skin and nail infections in tropical regions; however, central nervous system (CNS) infections are extremely rare. The neurotropism of this fungus is not well understood, but hematogenous spread from cutaneous or pulmonary foci has been hypothesized. A 60-year-old male with chronic lymphocytic leukemia (CLL) treated with acalabrutinib, a second-generation Bruton's tyrosine kinase (

Duplicated Appendix With Mucocele Mimicking an Appendiceal Neoplasm in a 45-Year-Old Woman: A Case Report.

BACKGROUND Appendiceal duplication is an extremely rare congenital anomaly, with an incidence of 0.004% to 0.009%. An association with an appendiceal mucocele is even rarer. It can complicate the diagnosis and management of right iliac fossa pathology and remains difficult to identify preoperatively. Failure to recognize it can result in recurrent symptoms, missed pathology, intraoperative challenges, and medicolegal consequences. This report describes the case of a 45-year-old woman with history of a loculated peri-appendiceal abscess managed with percutaneous drainage and intravenous antibio

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Thirty-Day Outcomes in Ultrasound Versus CT-Only Imaging in Obese Adults With Acute Appendicitis.

Background Acute appendicitis is one of the most common surgical emergencies affecting people worldwide. Current guidelines suggest imaging with an immediate computed tomography (CT) scan as the gold standard imaging pathway. However, emerging research has suggested that the addition of an ultrasound (US) can play a role in the diagnosis of acute appendicitis. In this study, we wanted to see if US as an adjunct to the existing imaging pathway of a CT scan had any improvement in 30-day outcomes in adult obese patients. Method Using data gathered from real patients using the TriNetX Global Colla

Impact of Bedside Versus Operating Room Drainage on Outcomes of Perianal Abscess: A Two-Center Retrospective Cohort Study With Propensity Score Matching.

ObjectiveTo compare 1-year outcomes of bedside and operating-room drainage of perianal abscesses using propensity score methods to address confounding by indication.MethodsThis two-center retrospective cohort included 303 adults undergoing drainage of perianal or anorectal abscesses. Because all bedside procedures involved perianal abscesses, comparative analyses were restricted to 271 patients with perianal abscesses who were potentially eligible for either drainage approach (183 bedside and 88 operating room). We performed 1:1 propensity score matching, inverse probability of treatment weigh

Trans-Cervical Ultrasound Imaging of the Adult Palatine Tonsils.

Trans-cervical ultrasound of the palatine tonsils provides a non-invasive technique to image the adult tonsils and the associated peritonsilar space to investigate intratonsillar and peritonsillar abscesses. The peritonsilar space, where peritonsillar cellulitis and abscesses are located is readily identified sonographically between the palatine tonsil and the superior constrictor muscle, which can be used as a helpful sonographic landmark. An understanding of the sonographic anatomy, technique, landmarks and imaging appearances is essential to correctly perform and interpret ultrasound imagin

A case report: axillary ALK-positive anaplastic large-cell lymphoma mimicking an infectious abscess in an adolescent.

BACKGROUND: Abscess-like presentations are common in surgical practice, but malignant soft-tissue and lymphoid disorders can occasionally imitate infection and delay definitive treatment. Anaplastic large-cell lymphoma (ALCL) is a CD30-positive T-cell lymphoma that may involve skin, lymph nodes, or soft tissue and can show extensive necrosis, inflammatory change, and systemic symptoms. CASE PRESENTATION: A 17-year-old female student presented with a 3-week history of progressive painful erythema and swelling of the left axilla after ineffective intravenous antibiotic treatment elsewhere. Staged Omental Wrapping for Perigraft Fluid Collection after Total Aortic Arch Replacement: A Case Report.

INTRODUCTION: Perigraft fluid collection after aortic graft replacement is often self-limiting; however, progressive enlargement or the development of symptoms warrants careful evaluation. The differential diagnosis includes hematoma, abscess, seroma, and lymphatic leakage. However, differentiating among these conditions can be challenging based solely on clinical symptoms and imaging findings. CASE PRESENTATION: A 51-year-old man underwent total aortic arch replacement (TAR) for chronic Stanford type A aortic dissection. Nine months later, he developed rapidly progressive swelling and erythe

ST20 Staphylococcus aureus causing suppurative cervical lymphadenitis in an extremely low-birth-weight infant.

Neonatal suppurative cervical lymphadenitis is uncommon, particularly in extremely low-birth-weight infants. We report a 772 g infant born at 28 weeks of gestation who developed rapidly progressive unilateral cervical lymphadenitis caused by methicillin-susceptible Staphylococcus aureus (MSSA) on postnatal day 31. The infant remained afebrile and blood cultures were negative despite progression requiring repeated drainage. The cervical isolate was genotyped as ST20/spa-t3929 and shared an identical genetic profile with an earlier pharyngeal isolate, suggesting that the infection arose from pre
