Multivariate Genome-Wide Analysis Delineates Shared Genetic Determinants Across Periodontal and Oral Microbial Phenotypes.

BACKGROUND: Periodontitis affects approximately 10% of the global population, posing a serious threat to tooth retention and quality of life. The disease emerges from bidirectional host-microbiome interactions that can shift from homeostasis to dysbiosis and chronic inflammation, yet conventional single-phenotype analyses cannot resolve pleiotropic loci acting across interrelated periodontal and microbial traits. We aimed to jointly model the shared genetic structure of six periodontitis-related phenotypes and to prioritise pleiotropic loci with translational relevance. METHODS AND MATERIALS:

Group A Streptococcus in an Unexpected Site: Temporal Pyomyositis in 2 Children.

BACKGROUND: Pyomyositis is an uncommon bacterial infection of skeletal muscle. Group A Streptococcus (GAS) accounts for a smaller proportion of pediatric cases, and temporal muscle involvement is exceptionally rare. CASE DESCRIPTION: We report 2 previously healthy children, a 7-year-old boy and a 6-year-old girl, who presented with acute otitis media (AOM) and painful unilateral temporomandibular swelling. Computed tomography revealed AOM with temporal myositis, complicated by mastoiditis in the first case. Both were treated with intravenous antibiotics, and the first case required drainage a

Borrelia garinii spirochetemia in a previously healthy child presenting with Streptococcus anginosus group brain abscess: a case report.

BACKGROUND: The clinical significance of transient Borrelia burgdorferi sensu lato complex (hereafter referred to as B. burgdorferi) spirochetemia remains poorly understood and has rarely been documented. We report the isolation of viable B. garinii genospecies from the blood of a previously healthy child presenting with a Streptococcus anginosus group (SAG) brain abscess. CASE PRESENTATION: An eight-year-old girl presented with four days of headache, fever, nausea, slightly reduced general condition, and mild facial asymmetry. Initial evaluation, including lumbar puncture, raised suspicion o

Skeletal muscle metastasis from gastric carcinoma: a systematic review of published case reports and case series over six decades.

BACKGROUND: Skeletal muscle metastasis (SMM) from gastric carcinoma is exceptionally rare despite the extensive vascularity and mass of skeletal muscles. The true prevalence remains unknown, and existing knowledge is limited to isolated case reports. OBJECTIVE: To systematically review all reported cases of SMM arising from gastric carcinoma, summarize clinical features, diagnostic approaches, imaging characteristics, treatments, and outcomes, and highlight gaps in existing literature. METHODS: A comprehensive literature search was conducted across PubMed, Embase, Scopus, and Google Scholar

Optimizing Catheter Removal in Amebic Liver Abscess: A Simplified Protocol for Faster Recovery.

INTRODUCTION: Percutaneous catheter drainage (PCD) is widely used in the management of large amebic liver abscesses (ALA). However, considerable variation exists in catheter removal practices, and concerns regarding residual cavities often result in prolonged catheterization and hospital stay. This study assessed whether an early catheter removal strategy could improve clinical outcomes without increasing complications. METHODS: In this prospective observational study, 40 adult patients with uncomplicated ALA requiring percutaneous drainage were enrolled. Catheter removal was attempted after

[Multiple neonatal staphylococcal cold abscesses in large skin folds].

Multiple neonatal staphylococcal cold abscesses in large skin folds (MNSCA) is a benign infection in newborns. It is characterised by abscess formation in large skin folds with mild local signs of inflammation but no systemic symptoms. Diagnosis is based on clinical presentation and culture of Staphylococcus aureus. In this case report, a 9-day-old girl with MNSCA is presented. MNSCA has an excellent prognosis, with or without treatment. Recognising this condition can prevent unnecessary diagnostic work-ups and antibiotic use.

Severe Hyponatremia as an Early Manifestation of Peritransplant Abscess After Living Donor Kidney Transplantation: A Case Report.

Electrolyte abnormalities are common after kidney transplantation, but severe hyponatremia as an early sign of occult peritransplant infection is rare. A 68-year-old woman underwent living unrelated kidney transplantation and was discharged on postoperative day 4 with a drain in place. On postoperative day 8, she developed acute kidney injury and profound hyponatremia, with a serum sodium level of 109 mmol/L, despite only mild fatigue and no fever, abdominal pain, allograft tenderness, or wound symptoms. Sodium improved with hypertonic saline, nutritional support, and fluid restriction. On pos

Multiple Cold Abscesses Without Respiratory Symptoms Revealing Multifocal Tuberculosis: A Case Report.

Soft-tissue tuberculosis is an uncommon extrapulmonary manifestation and may be difficult to recognize when respiratory symptoms are absent. We report a 33-year-old, HIV-negative Sri Lankan woman without diabetes or known pharmacological immunosuppression who presented with three months of progressively enlarging, minimally tender superficial collections over the left wrist, right knee, and right ankle, accompanied by anorexia and unquantified weight loss. The swellings had minimal local inflammatory signs. She was anemic and hypoalbuminemic, with elevated inflammatory markers. Separate draina

Non-traumatic acute mediastinitis secondary to acute phlegmonous esophagitis: a rare case report.

Acute mediastinitis secondary to acute phlegmonous esophagitis (APE) is an exceptionally rare, life-threatening condition with high morbidity. We report a 75-year-old diabetic female with recent severe pneumonia who presented with pleuritic chest pain, dyspnea, and elevated inflammatory markers. Computed tomography pulmonary angiography excluded embolism but revealed diffuse long-segment esophageal wall thickening with mediastinal fat stranding, aortic encasement, and pleural effusion, features consistent with APE and secondary non-traumatic mediastinitis without overt perforation. A Procedural Faculty Development Course for Emergency Physicians Practicing in Community and Academic Settings.

INTRODUCTION: Maintaining competence in high-acuity, low-occurrence (HALO) procedures poses a challenge for emergency physicians as they progress past residency training or practice in lower-acuity settings. Simulation-based deliberate practice offers a feasible method to sustain procedural competence. We developed and evaluated a blended, faculty development procedural curriculum designed to improve confidence and clinical application of HALO skills across a five-hospital emergency medicine (EM) network. METHODS: A faculty needs assessment and departmental quality data informed the creation

Giant infected splenic pseudocyst complicated by rupture in a patient with chronic Q fever.

Giant infected splenic pseudocysts are rare and may be difficult to diagnose due to overlapping clinical and imaging features with other cystic splenic lesions, including hydatid cysts and abscesses. At our institution, a 59-year-old male with no reported history of splenic trauma presented with weight loss, early satiety, and left upper quadrant pain. Computed tomography demonstrated a large calcified splenic cyst measuring up to 14.6 cm with internal septations and an air-fluid level, concerning for infection and rupture. The cyst fluid measured a median of 12 Hounsfield units, without perce

Septic arthritis attributed to a spider bite - a case report and review of the literature.

Individuals who travel to tropical or subtropical areas are exposed to various infectious, environ-mental, and climatic risk factors, as well as traffic accidents, security problems, natural disasters, and direct contact with potentially dangerous plants or animals. Accidental bites by venomous spiders during long journeys are always dangerous to health and life, but it is not usually recognized that nonvenomous spiders also pose such a substantial threat to the lives of local inhabitants and travelers. Here, we present an unusual case of a 41-year-old male patient who was admitted to the Univ

Brain Abscess Caused by Bifidobacterium spp. in a Previously Healthy Adolescent: A Case Report.

Brain abscesses in children are uncommon but serious conditions. Bifidobacterium species, typically considered commensal organisms of the oral cavity, gastrointestinal tract, and female genital tract, are rarely implicated in invasive infections. We present a case of multiple brain abscesses caused by Bifidobacterium spp. in a previously healthy 11-year-old male, secondary to contiguous spread from chronic left-sided paranasal sinusitis with ethmoid bone erosion. Neuroimaging demonstrated left frontal abscesses with marked perilesional edema, ring enhancement and midline shift, together with o

Periapical Granuloma: A Case Report.

A periapical granuloma is a fleshy mass of tissue invaded by varied numbers of chronic inflammatory cells, namely, epitheloid cells (modified tissue macrophages), mast cells, lymphocytes, and plasma cells, usually attached to the apex of a nonvital carious tooth as a sequelae of periapical infection from the diseased pulp. The growth of periapical granulomas develops in spurts amidst recurrent acute exacerbations. Periapical granulomas may form as the initial periapical pathosis or as a result of a quiescent periapical abscess. These lesions might not always be static. Radiographic changes are
