Heavy metal exposures and constipation among United States adults: A cross-sectional study.

Constipation is a common gastrointestinal disorder that can substantially impair quality of life. Environmental exposure to heavy metals has been implicated in gastrointestinal dysfunction; however, population-based evidence regarding their associations with constipation remains limited. This study aimed to evaluate the associations of blood lead, mercury, and cadmium levels with constipation among US adults. This cross-sectional study used data from the National Health and Nutrition Examination Survey (NHANES) 2005-2010 and included 11,014 adults aged ≥20 years. Survey-weighted multivariable

Shared genetic architecture between constipation and aging-related phenotypes: Insights from large-scale genome-wide cross-trait analysis.

While the frequency of constipation increases sharply with age, the genetic links between constipation and aging-related phenotypes remain largely unexplored. So far, research has typically focused only on individual characteristics while neglecting broader genetic connections. In this study, we evaluated the shared genetic architecture of constipation and aging-related phenotypes in order to better understand the causes of aging-related intestinal dysfunction. Using publicly available genome-wide association study summary statistics, we developed a multi-stage analysis pipeline to identify co

Whole Brain Cellular Activation After Mu and NOP Receptor Agonism Identifies Differential Regional and Network Consequences.

Mu opioid agonists, the most widely used opioids, produce analgesia, respiratory depression, constipation, and reward leading to substance abuse. Like all opioid receptor family members, mu receptors are Gi/o-coupled and inhibitory, yet mu-mediated disinhibition of GABAergic neurons propagates downstream activation that differs between naive and dependent animals and across receptor subtypes. Here we used TRAP2/Ai9 reporter mice, which deposit tdTomato in active neurons via the c-Fos locus, with tissue clearing and light sheet microscopy to map whole-brain neuronal activation after acute morph

Extensive Perianal Basal Cell Carcinoma Presenting With Severe Anorectal Dysfunction.

Perianal basal cell carcinoma is an exceedingly rare malignancy that may mimic benign anorectal conditions or anal squamous cell carcinoma. We report an 84-year-old man with a several-year history of a progressively enlarging circumferential perianal lesion, complicated by constipation, chronic fecal incontinence, bleeding, and significant weight loss. Examination revealed a 15 cm ulcerated tumor involving the entire anal verge, with biopsy confirming basal cell carcinoma and sparing of the anal canal mucosa. Imaging showed fecal impaction and perianal soft tissue thickening without metastasis

An unusual lead point: jejunal leiomyosarcoma causing jejuno-jejunal intussusception - a case report.

INTRODUCTION: Adult intussusception is rare. Unlike in pediatric cases, adult intussusception typically arises from an underlying pathology. Primary jejunal leiomyosarcoma is an exceptionally rare malignant tumor originating from the smooth muscle cells of the small intestine. CASE PRESENTATION: A 52-year-old man presented with signs and symptoms of acute intestinal obstruction, including abdominal pain, constipation, vomiting, and abdominal distension of 2 days duration. Contrast-enhanced computed tomography revealed a jejunojejunal intussusception with a lead-point intraluminal mass. Explor

Giant hydronephrosis due to neglected congenital pelvi-ureteric junction obstruction: a case report and literature review.

INTRODUCTION: Giant hydronephrosis is defined as having 1.5 L or 1.6% of body weight of fluid in the renal pelvis. In the exception of a few, the preponderance of giant hydronephrosis is attributable to congenital anomalies, mostly pelvi-ureteric junction obstruction (PUJO). However, giant hydronephrosis could also be attributed to ureteric calculi, ureteral tumors or trauma. We report the case of a giant hydronephrosis in a 14-year-old African female, with a review of the etiology and peculiarities of management in an African context. CASE PRESENTATION: A 14-year-old female presented with a

Add-on oral amisulpride for prevention of chemotherapy-induced nausea and vomiting during highly emetogenic chemotherapy: A randomized double-blind placebo-controlled group sequential trial.

IntroductionGuideline-based prophylaxis prevents most vomiting during highly emetogenic chemotherapy (HEC), but nausea remains incompletely controlled, especially where NK1 receptor antagonists and olanzapine are inaccessible or ineffective. Amisulpride, a dopamine D2/D3 antagonist, may add benefit with less sedation than olanzapine.MethodsAdults receiving cisplatin-based or anthracycline-cyclophosphamide HEC were studied during a single cycle in this randomized, double-blind, placebo-controlled group sequential trial. All received intravenous ondansetron and dexamethasone on day 1 plus oral d

Association of Fecal Calprotectin with Motor and Nonmotor Features in Idiopathic Parkinson's Disease and Atypical Parkinsonism.

BACKGROUND AND OBJECTIVES: Gut inflammation and microbiota dysregulation are increasingly implicated in Parkinson's disease (PD). Fecal calprotectin, a neutrophil-derived protein and marker of intestinal inflammation, may serve as a non-invasive indicator of gut-brain axis dysfunction in parkinsonian disorders. To evaluate fecal calprotectin levels in idiopathic Parkinson's disease (IPD) and atypical parkinsonian syndromes-progressive supranuclear palsy (PSP) and multiple system atrophy (MSA)-and examine their relationship with motor and non-motor features. METHODS: In this case-control study
