Myofascial pelvic floor dysfunction as a hidden contributor to urinary symptoms.

Lower urinary tract symptoms (LUTS) such as frequency, urgency, and sensation of incomplete emptying are frequently attributed to bladder or urothelial disorders, but pelvic floor myofascial dysfunction is underrecognized as an etiologic factor. To present a conceptual framework for myofascial pelvic floor dysfunction (MPFD) as a contributor to urinary symptoms, review its urologic manifestations, discuss, and explore occult contributing factors, and propose integrated diagnostic and interventional therapeutic strategies. This is a narrative review and conceptual synthesis anchored on recent o

Laparoscopic-Assisted Anorectoplasty for Anorectal Malformations Without Rectourinary Fistula Ligation: Two Decades of Experience at a Tertiary Center.

AIM: To report our experience with laparoscopic-assisted anorectoplasty (LAARP) for anorectal malformations (ARM) performed without ligation of the rectourinary fistula (RUF). METHODS: In this single-center study, we retrospectively analyzed all consecutive primary LAARP procedures for ARM without RUF ligation performed between 2004 and 2022. Forty patients were included (19 rectobulbar, 16 rectoprostatic, 5 rectobladder). Colostomy was performed at a median age of 1 day and LAARP at 147 days (median weight 6 kg). Median operative time was 180 minutes, and 1 patient required conversion. No ea

Inflammation rewires the enteric nervous system through neurogenic monocyte recruitment.

Proper organization of the enteric nervous system (ENS) is critical for normal gastrointestinal (GI) physiology. Inflammatory bowel disease (IBD) disrupts key GI functions, including bowel motility. However, in many IBD patients, motility disorders persist even during remission, suggesting an irreversible ENS defect secondary to IBD. Here, we show that postinflammatory GI motility dysfunction arises from structural remodeling of the ENS, driven by a combination of neuronal loss and neurogenesis. During mucosal inflammation, enteric neurons upregulate CCL2 expression, facilitating the recruitme

Bowel continence in adult individuals with spina bifida: Who takes ownership of care?

INTRODUCTION: Neurogenic bowel dysfunction is a significant health and quality-of-life concern for individuals with spina bifida. To date, little is known about how these individuals manage continence issues once they reach independence in adulthood. The purpose of this study is to better understand how adults with spina bifida manage their bowel continence, surgical complications, and coordination of care. STUDY DESIGN: An anonymous web-based survey was distributed through the Spina Bifida Association (SBA) e-mail listserv and various social media groups. Data regarding demographics, bowel m

Abnormal enteric nervous system organization and gastrointestinal motility in mice with valproic acid-induced neural tube defects.

BACKGROUND: Neurogenic bowel is a major cause of morbidity in patients affected by neural tube defects (NTDs) such as spina bifida, but the underlying reasons for bowel dysfunction are unknown. An absolute requirement for gastrointestinal (GI) motility is the enteric nervous system (ENS) located within the walls of the GI tract. Enteric neurons coalesce into circumferential stripes throughout embryonic and early postnatal development, and this gradual organization of the ENS into enteric neuronal stripes correlates with the emergence of neurogenic GI motility. We hypothesized that NTDs are ass

Case Report: A rare but critical complication in patients with lumbar infection combined with cauda equina syndrome-sacrococcygeal pressure sores.

This report presents a complex case of a 65-year-old male patient with a lumbar spine infection (T12-L1) caused by Staphylococcus aureus, leading to cauda equina syndrome, and subsequently developing a massive infectious pressure sore in the sacrococcygeal region. Back pain due to spinal infections, analgesic use, neurological deficits, and systemic infections masked the early signs of the pressure sore. Prolonged immobilization and neurogenic bladder and bowel incontinence led to a 5 cm × 5 cm deep pressure sore in the sacrococcygeal area. Involving a single-stage surgery for debriding the sp

When Lupus Hits the Spine: A Case of Extensive Transverse Myelitis.

PRESENTATION: The patient presented with acute urinary retention, fever (39°C), and rapidly progressive bilateral lower limb weakness progressing to flaccid paraplegia (0/5 MRC) with a T6 sensory level. DIAGNOSIS: MRI spine demonstrated longitudinally extensive T2-hyperintense intramedullary lesions from T6 to the conus. CSF analysis showed lymphocytic pleocytosis and elevated protein; autoimmune serology confirmed active systemic lupus erythematosus (SLE). SLE-associated longitudinally extensive transverse myelitis (LETM) was diagnosed following exclusion of other differential diagnoses. TR

The Effect of Traumatic Brain Injury on the Gastrointestinal System: A Comprehensive Review.

BACKGROUND/OBJECTIVES: Traumatic brain injury (TBI) is a significant public health concern resulting in physical, cognitive, and behavioral impairments. Emerging evidence highlights a bidirectional relationship between brain injury and gut health, known as the brain-gut axis. This paper provides a comprehensive review of current literature exploring the relationship between TBI and various gastrointestinal (GI) pathologies, examining how brain injuries contribute to GI dysfunction and how gut health influences neurorecovery. METHODS: A comprehensive search of peer-reviewed articles was conduc

Safety and Efficacy of Manual and Automated Abdominal Colonic Massage for Chronic Constipation: A Systematic Review and Meta-Analysis.

INTRODUCTION: Abdominal colonic massage shows benefits against chronic constipation (CC). While both manual and automated versions of this therapy show potential, there is currently no comprehensive synthesis comparing their effectiveness and safety. METHODS: Studies on manual and device-assisted abdominal massage for CC were identified from major bibliographic databases (1990-2025). Risk of bias was assessed. Outcomes included bowel movements per week (BM/w), quality of life (QoL), and adverse events. Meta-analyses were stratified by patient profile and intervention type. Where available, co

Asymptomatic multisegmental epidural hematoma following unilateral biportal endoscopic lumbar decompression: a case report.

BACKGROUND: This case is noteworthy as it presents an asymptomatic multisegmental epidural hematoma spanning five spinal segments (T12-L4) following unilateral biportal endoscopic (UBE) decompression, with the patient achieving favorable recovery without surgical intervention. This phenomenon has rarely been documented in existing literature and may serve as a reference for the management of epidural hematoma after UBE procedures. CASE PRESENTATION: An 82-year-old female patient (Chinese, the Han nationality) presenting with refractory left L5 radiculopathy and neurogenic claudication was dia

Neurogenic irritable bowel syndrome? Long-term resolution of chronic gastrointestinal stress following vagus nerve microvascular decompression: illustrative case.

BACKGROUND: Microvascular decompression (MVD) is a well-established treatment for compressive cranial neuralgias such as trigeminal neuralgia and hemifacial spasm. The vagus nerve (cranial nerve [CN] X), with its extensive parasympathetic innervation to the gastrointestinal (GI) system, has less clearly defined neuropathic symptomatology, and MVD of CN X has only been reported once. The authors report a unique case of treatment-refractory GI distress that resolved completely following CN X MVD. OBSERVATIONS: A 32-year-old male with a 13-year history of hemifacial spasm also experienced chroni

Validation of MENTOR Tool to Describe Bowel Dysfunction in Inpatient Rehabilitation Setting.

BACKGROUND: People living with spinal cord injury (SCI) rate neurogenic bowel dysfunction (NBD) as one of the biggest daily challenges. Currently, no validated clinical decision-making tool exists to guide NBD management during the acute/subacute period following SCI, a time highlighted by frequent complications and rapid clinical changes. Previously, the Monitoring Efficacy of NBD Treatment On Response (MENTOR) tool was developed to facilitate shared decision-making between the clinician and the person living with chronic SCI in bowel management. OBJECTIVES: To assess MENTOR tool validity in

Review and assessment of policies and practices for spinal cord injury-related health care supplies, services, and mobility equipment reveal wide inequities and inadequacies in Canada.

INTRODUCTION: Canada maintains a universal, publicly funded health care system, although provinces and territories retain flexibility and independence in determining eligibility and funding levels for services and supplies. This flexibility and independence can lead to variability in health care delivery and therefore downstream health outcomes between provinces for the same health condition. Over 80,000 Canadians live with spinal cord injury (SCI), which causes motor paralysis and dysfunctions in multiple body systems. These dysfunctions are managed as multiple chronic health conditions to ma
