Concurrent sigmoid volvulus and caecal adenocarcinoma in a paraplegic patient with neurogenic bowel: a case report.

A 76-year-old man with paraplegia and neurogenic bowel dysfunction presented with progressive abdominal distension and altered bowel habits. Contrast-enhanced computed tomography demonstrated sigmoid volvulus and an incidental 5 × 4 cm ileocaecal mass. After endoscopic detorsion, multidisciplinary review recommended definitive resection during the same admission. He underwent right hemicolectomy with ileocolic anastomosis and sigmoidectomy with colorectal anastomosis. Histopathology confirmed moderately differentiated caecal adenocarcinoma (pT3N1a, R0). The postoperative course was complicated

Transient receptor potential vanilloid channels in inflammatory bowel diseases: linking inflammation, angiogenesis, and oxidative stress.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is characterized by chronic relapsing intestinal inflammation driven by complex interactions among immune dysregulation, epithelial barrier disruption, vascular dysfunction, oxidative stress, and gut microbiota imbalance. Despite advances in biologic and small-molecule therapies, a substantial proportion of patients remain refractory to treatment or experience significant adverse effects, underscoring the need for novel mechanistic insights and therapeutic targets. Emerging evidence suggests that the

Distinct enteric nervous system pattern in obstructed defecation syndrome and slow-transit constipation: a controlled digital pathology study.

PURPOSE: Chronic constipation comprises heterogeneous clinical entities, including slow-transit constipation (STC) and functional defecation disorders such as obstructed defecation syndrome (ODS). Although neuroenteric mechanisms are implicated, reproducible histomorphological correlates remain incompletely defined, partly due to methodological heterogeneity and lack of standardized quantification. The study characterizes the myenteric plexus morphology and neuroimmune cell distribution across clinically defined subgroups of functional bowel disorders using a standardized digital pathology app

Clinical presentation, diagnosis and management of chronic intestinal and colonic pseudo-obstruction in neurological diseases.

Chronic intestinal and colonic pseudo-obstruction (CIPO) represent a severe and heterogeneous group of gastrointestinal motility disorders with symptoms of bowel obstruction in the absence of a mechanical cause. Neurological and neuromuscular diseases are among the most important etiological factors, caused by dysfunction of the enteric nervous system, extrinsic autonomic pathways and intestinal smooth muscle. However, intestinal dysmotility in these conditions remains frequently under-recognized leading to delayed diagnosis and substantial morbidity. In this review, we detailed the CIPO-relat

Clinical and Physiological Characterization of Neurogenic Bowel Dysfunction in Motor-Incomplete Spinal Cord Injury.

BACKGROUND: Neurogenic bowel dysfunction is highly prevalent after spinal cord injury and significantly affects quality of life. The pathophysiology in motor-incomplete spinal cord injury remains poorly characterized despite the availability of physiological tests, such as high-resolution anorectal manometry and colonic transit time. OBJECTIVE: This study aims to characterize clinical presentation, high-resolution anorectal manometry findings, and colonic transit time in motor-incomplete spinal cord injury and to compare patients with neurological level at or above T6 versus below T6. Predictive value of anal sphincter electromyography for sacral neuromodulation test-phase outcomes.

BACKGROUND: Sacral neuromodulation (SNM) is an established therapy for refractory pelvic organ dysfunction. Anal sphincter electromyography (EMG) is commonly used preoperatively to assess sacral and peripheral nerve integrity. However, the prognostic significance of chronic neurogenic EMG changes for SNM outcomes remains unclear. OBJECTIVE: To evaluate whether chronic neurogenic changes on preoperative anal sphincter EMG predict the outcome of the SNM test phase. METHODS: We retrospectively analysed 62 consecutive patients with bladder and/or bowel dysfunction or pelvic pain who were candida

At-home feasibility trial of genital nerve stimulation to modulate neurogenic bowel dysfunction in individuals living with spinal cord injury: a protocol.

INTRODUCTION: Neurogenic bowel dysfunction is a consequence of almost all spinal injuries. Primary management is via conservative treatment methods with few intermediary interventions available prior to surgical management. This protocol describes a trial to determine if genital nerve stimulation (GNS) can reliably be delivered at-home for the purpose of modulating neurogenic bowel dysfunction in people living with spinal cord injury (SCI). METHODS AND ANALYSIS: A total of 12 participants, aged 18 years or older with any severity of a traumatic SCI (at least 6 months post-injury) at or above

Case Report: Benign colonic schwannoma with restricted diffusion mimicking malignancy.

INTRODUCTION: Colonic schwannoma is a rare gastrointestinal mesenchymal tumor that is usually benign but may be difficult to distinguish from malignancy before surgery. This case is notable because a benign colonic schwannoma showed both pronounced restricted diffusion on MRI and prominent mesocolic lymphadenopathy, creating a strong radiologic impression of malignancy. CASE PRESENTATION: A 43-year-old woman presented with a 2-day history of abdominal pain accompanied by altered bowel habits. Colonoscopy revealed a large spherical submucosal mass in the transverse colon. Biopsy suggested a ne

Concurrent Central and Autonomic Nervous System Involvement in Varicella-Zoster Virus Infection in an Immunocompetent Patient: A Case-Based Mechanistic Analysis.

BACKGROUND: Varicella-zoster virus (VZV) is a neurotropic alphaherpesvirus capable of causing a broad spectrum of neurologic complications beyond classic dermatomal herpes zoster. Although meningitis and encephalitis are well recognized manifestations of neuroinvasive VZV infection, associated autonomic dysfunction remains comparatively underreported, particularly in immunocompetent individuals. CASE PRESENTATION: We describe a 66-year-old immunocompetent man who developed VZV meningoencephalitis associated with sacral dermatomal herpes zoster, urinary retention, and bowel dysmotility. Tumour distribution and characteristics associated with poor surgical outcomes in patients with sporadic spinal schwannomas.

PURPOSE: Spinal schwannomas are benign tumours that can compress the spinal cord or nerve roots, causing neurological symptoms. Despite successful surgical resection, some patients experience suboptimal functional recovery. Several risk factors for poor prognosis have been identified, but limited research has explored the influence of tumour distribution and characteristics. In this study, we aimed to identify prognostic variables associated with residual neurological deficit in patients undergoing surgical resection for sporadic spinal schwannomas. METHODS: Clinical and radiological data of
