Goldberg-Shprintzen Megacolon Syndrome Diagnosed in the Neonatal Period: A Case Report With Molecular Confirmation.

Goldberg-Shprintzen megacolon syndrome (GOSHS) is a rare autosomal recessive neurodevelopmental disorder characterized by Hirschsprung disease, microcephaly, neurodevelopmental impairment, and craniofacial dysmorphism. We report a male neonate born at 35+4 weeks of gestation who presented on day 4 of life with abdominal distension and delayed passage of meconium. Clinical examination revealed microcephaly, generalized hypotonia, craniosynostosis, and dysmorphic facial features, with a positive family history of GOSHS. Hirschsprung disease was confirmed by rectal biopsy, and cranial imaging dem

Association of small intestinal atresia and Hirschsprung disease in a multi-institutional colorectal registry.

PURPOSE: Unlike colonic atresia, concomitant jejuno-ileal atresia (JIA) and Hirschsprung disease (HSCR) is an under-recognized association. We used a multi-institutional patient registry to describe the incidence, clinical characteristics, management, and outcomes of patients with both conditions. METHODS: We performed a retrospective review of patients with HSCR in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) registry. Additional data was collected from participating sites regarding atresia management and outcomes. RESULTS: Of 1459 eligible patients with HSCR, 15 (1.0%) a

Impact of Trisomy 21 on post pull-through enterocolitis and bowel function in children with Hirschsprung disease: A PCPLC analysis.

INTRODUCTION: We evaluated whether Trisomy 21 (T21) affected bowel management use, ability to toilet train for stool, and rates of Hirschsprung-Associated Enterocolitis (HAEC) in children with Hirschsprung disease (HD). We hypothesized that T21 would be associated with increased rates of HAEC, bowel management program (BMP) use, and delayed toilet training for stool. METHODS: Children with HD in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) database who underwent primary pull-through were included and stratified by T21 status: HD only versus HD + T21. Children requiring a re

Cytomegalovirus (CMV) Colitis Complicated by a Stricture in an Immunocompetent Adult.

Although cytomegalovirus (CMV) seropositivity is common in the general population, primary CMV infection in immunocompetent adults is usually asymptomatic or causes only a mild mononucleosis-like illness, and severe symptomatic or tissue-invasive disease is uncommon. CMV colitis may present with nonspecific symptoms, delaying recognition, particularly in immunocompetent patients in whom clinical suspicion is often low. Delayed diagnosis may allow progression to serious complications, including gastrointestinal bleeding, perforation, toxic megacolon, and, less commonly, stricture formation. We

Single-cell RNA Sequencing for Profiling Ganglionic and Aganglionic Colonic Segments from Patients with Hirschsprung Disease.

Hirschsprung disease (HSCR) is a congenital intestinal motility disorder characterized by the absence of enteric neurons in the distal bowel. Compared with bulk RNA sequencing, single-cell RNA sequencing (scRNA-seq) enables gene expression profiling at the single-cell level, resolving cellular heterogeneity that is masked in population-averaged analyses. A standardized scRNA-seq workflow was established to characterize the transcriptional landscape of ganglionic and aganglionic segments from patients with HSCR. Colon tissues were collected, and hematoxylin and eosin staining was performed to i

Regulatory gene analysis in enteric neural crest cells stratified by GDNF activation status: An insight into Hirschsprung disease pathophysiology.

BACKGROUND: Incomplete migration of enteric neural crest cells (ENCCs) into the colon underlies the pathogenesis of Hirschsprung disease, resulting in aganglionosis and disordered intestinal motility. Although activation of the glial cell line-derived neurotrophic factor (GDNF) pathway has been implicated in neurosphere differentiation, specific regulatory genes governing ENCC differentiation and proliferation remain poorly characterized. This study seeks to elucidate regulatory genes and pathways involved in neural crest cell differentiation, furthering our understanding of GDNF's role in ent

Megacolon Associated with Multiple Endocrine Neoplasia Type 2B: A Case Successfully Managed with Ileostomy.

INTRODUCTION: Multiple endocrine neoplasia type 2B (MEN2B) is a rare genetic disorder characterized by medullary thyroid carcinoma, pheochromocytoma, mucosal neuromas, and Marfanoid body habitus. Megacolon is a common non-endocrine manifestation, with approximately 75% of patients experiencing gastrointestinal symptoms, such as constipation or diarrhea, often as early clinical signs. Although these symptoms may be important diagnostic signs, there are currently no distinct therapeutic guidelines. Here, we report a case of MEN2B-associated megacolon that was managed surgically, resulting in a f

circRET-mediated regulation of RET influences enteric neural crest cell development in Hirschsprung disease.

Hirschsprung disease (HSCR) is a life-threatening congenital disorder characterized by defective migration and proliferation of enteric neural crest cells (ENCCs), a process in which the receptor tyrosine kinase RET plays a central regulatory role. However, the regulatory mechanisms governing RET expression remain incompletely understood. In the present study, we identify the circular RNA circRET (hsa_circ_0093651) as a critical translational regulator of its parental gene RET and explore its role in HSCR pathogenesis. We demonstrate that circRET is significantly downregulated in the colonic t

Haddad Syndrome - more than don't forget to breathe.

Congenital central hypoventilation syndrome (CCHS) and Hirschsprung's disease (HD) belong to neurocristopathies - autonomic nervous system defects caused by abnormal neural crest cells migration. Their co-occurrence defines Haddad Syndrome (HS) associated also with increased risk of neural crest origin tumors, mainly neuroblastoma (NBL), and PHOX2B gene mutation. This case concerns a male newborn presenting respiratory failure minutes after birth. Due to sleep apnea and increasing hypercapnia, he required intubation. Genetic testing revealed an anomaly in PHOX2B gene. In subsequent days, the p

The EMB/ATG7-mediated autophagic process regulates neurite outgrowth and promotes enteric neuronal development.

The formation of the enteric nervous system (ENS) primarily involves the migration of enteric neural crest-derived cells (ENCCs) and the subsequent maturation of enteric neurons. The developmental dysfunction of ENCCs and enteric neurons can result in ENS disorders, such as hypoganglionosis (HG). Although neurite outgrowth is fundamental to neuronal maturation, the mechanisms by which neurite outgrowth influences neuronal maturation remain poorly defined. Here, we identified EMB as a critical regulator of enteric neuronal maturation. In EMB mutant patients, the expression of EMB is reduced in

Dysregulation of GDNF and GFRα1 expression in multifactorial Hirschsprung disease.

OBJECTIVE: HSCR is caused by disruption of complex signaling pathways within the gene regulatory network (GRN) during enteric nervous system (ENS) development, including glial cell line-derived neurotrophic factor (GDNF) and GDNF family receptor alpha-1 (GFRα1). However, pathogenic variants in all GRN genes account for only ~ 80% cases; therefore, the epigenetic role remains to be elucidated. We compared GDNF and GFRα1 expression between HSCR patients and controls. RESULTS: qPCR revealed an upregulated GDNF expression in both ganglionic (206.37-fold) and aganglionic (126.35-fold) HSCR compare

Fulminant Clostridioides difficile pseudomembranous colitis requiring subtotal colectomy after pancreaticoduodenectomy: A case report and mini-review.

Pseudomembranous colitis due to Clostridioides difficile infection (CDI) is a serious complication of antibiotic therapy and hospitalization that can progress to toxic megacolon, necessitating emergency colectomy. Patients undergoing pancreaticoduodenectomy (PD) are particularly at high risk due to biliary obstruction, recurrent cholangitis, broad-spectrum antibiotic exposure, and prolonged postoperative stays. We report the case of an elderly woman with multiple cardiovascular comorbidities and recent CDI who underwent PD for pancreatic ductal adenocarcinoma (PDAC). Prior to surgery, she had

Preoperative underweight is associated with a more complicated perioperative course and impairs recovery in Hirschsprung's disease: The pivotal role of weight-for-age z-score.

BACKGROUND & AIMS: Low weight-for-age z-score (WAZ) is prevalent in children with Hirschsprung's disease (HD), yet its prognostic value for surgical outcomes is unclear. This study evaluated the impact of preoperative WAZ on perioperative course complexity and postoperative recovery in children undergoing pull-through surgery. METHODS: A retrospective cohort study was conducted on 256 children aged ≤10 years who underwent pull-through. Patients were stratified by preoperative WAZ: underweight (WAZ < -2), at risk of underweight (-2 ≤ WAZ < -1), and well-nourished (WAZ ≥ -1). Demographic, clini

Age and seasonality in Hirschsprung-associated enterocolitis risk: a longitudinal cohort study.

PURPOSE: Hirschsprung-associated enterocolitis (HAEC) remains a major cause of morbidity, with poorly defined triggers and limited modifiable risk factors. Given the roles of immune immaturity and mucosal dysfunction in HAEC pathogenesis, this study evaluated whether age is associated with HAEC risk, and whether seasonal variation consistent with viral exposure is present. METHODS: A retrospective cohort study was conducted including patients with HD treated at Children's Hospital Colorado (2008-2021). A piecewise exponential Poisson regression model estimated age-specific HAEC incidence rate

Economic evaluation of Hirschsprung disease testing strategies for children with medically-refractory chronic constipation: A cost-effectiveness analysis.

BACKGROUND: Early and accurate diagnosis of Hirschsprung disease (HSCR) prevents life-threatening complications, improves patient outcomes, and reduces healthcare utilization. Contrast enema (CE), anorectal manometry (AMAN), and rectal biopsy (RB) are commonly used diagnostic tests, with RB being the gold standard. However, the most cost-effective initial diagnostic strategy remains unclear. METHODS: A decision-analytic model was used to compare CE-first, AMAN-first, and RB-first strategies for the evaluation of HSCR in children between 1 and 18 years of age presenting to specialty care for c

Endogenous Peptide Derived from c-Cbl-Associated Protein Counteracts Its Inhibitory Effect on Enteric Neural Crest Cell Colonization in Hirschsprung Disease.

Hirschsprung disease (HSCR) is a congenital malformation characterized by the absence of the enteric nervous system (ENS) in the distal colon, resulting from defective colonization of enteric neural crest cells (ENCCs). The underlying pathogenesis of HSCR remains incompletely understood. Here, we report that c-Cbl-associated protein (CAP), also known as sorbin and SH3 domain-containing protein 1 (SORBS1), is upregulated in the aganglionic colon tissues of children with HSCR. Functional studies revealed that CAP overexpression suppresses ENCC colonization by binding the lipid raft protein floti

Endoscopic-Assisted Division of Septal Formation After Duhamel Procedure for Hirschsprung Disease: Two Case Reports.

The management of septal formation after the Duhamel procedure in Hirschsprung disease remains unclear. We report two cases treated with an endoscopic-assisted approach. Case 1 is a 5-year-old girl with Hirschsprung disease who underwent the Duhamel procedure. Persistent constipation led to a contrast enema, confirming septal formation. Transanal endoscopic-assisted division with an endoscopic linear stapler improved symptoms initially. However, constipation and enterocolitis with bleeding recurred 5 months later, requiring redo surgery. Case 2 is a 21-year-old female with a history of Hirschs
