Non-standardized surgery lateral internal sphincterotomy: Is there a consensus?

OBJECTIVE: Lateral internal sphincterotomy (LIS) is considered the gold standard surgical treatment for chronic anal fissures. However, substantial variation exists in the surgical techniques applied. This study aims to evaluate practice differences among surgeons performing LIS and to assess whether a consensus has been established. MATERIAL AND METHODS: An anonymous online survey was conducted to assess surgeons' technical approaches to LIS. Data were collected using a 24-question survey targeting surgeons from various countries. RESULTS: A total of 207 surgeons (131 from Türkiye, 76 from

Three Years of Experience Using a Diode Laser in the Management of Common Anal Benign Pathologies: A Retrospective Study.

Background The management of anorectal conditions continues to evolve with newer techniques for colorectal surgeons. Recently, diode laser technology has gained attention in proctology as a less invasive alternative to traditional surgery. The mechanism involves tissue shrinkage and degeneration and depends on the power of the laser and the duration of exposure. This method is associated with reductions in postoperative pain, bleeding, discharge, and hospital stays. Patients and methods This retrospective study involved 456 patients with hemorrhoids, chronic anal fissure, and perianal fistula.

The Effect of Sphincter Pressure and Anal Length on Surgical Decision in Chronic Anal Fissure.

Background/Objectives: Chronic anal fissure, an anorectal condition, is characterized by internal anal sphincter fibers at the base of the fissure. The lifetime prevalence is 7-8%. Medical management and lifestyle modifications are the first-line treatments, whereas chemical or surgical internal anal sphincterectomy is an option for refractory cases. The aim of this study was to evaluate the impact of surgical anal canal length and sphincter pressure on postoperative symptom relief and continence outcomes. Methods: A total of 26 patients diagnosed with chronic anal fissures were selected for s

Ultrasound-guided botulinum toxin for the treatment of chronic anal fissure: technical notes.

We are thrilled to present a new technique for treating chronic anal fissures using ultrasound-guided botulinum toxin injections. Our approach involves injecting botulinum toxin into the internal anal sphincter (IAS) guided by ultrasound for maximum effectiveness. We believe that our technique has significant potential to improve outcomes. We could not find any studies where ultrasound-guided TB puncture was used to treat chronic anal fissures.

Management of chronic anal fissures: should fissurectomy be performed with botulinum toxin injection?

BACKGROUND: Despite the superior success rate of lateral internal sphincterotomy for managing chronic anal fissure (CAF), there is a trend towards sphincter-preserving treatment due to the increased risk of incontinence. Botulinum toxin (BT) and fissurectomy are two sphincter-preserving options for CAF. We aim to assess if combining fissurectomy with botox treatment is superior to botulinum toxin alone in the management of CAF. METHODS: This retrospective cohort study was conducted across two Sydney hospitals over 7 years. All patients with a CAF managed with either BT and fissurectomy (group

The 5-year outcomes and predictors of healing in chronic anal fissure treated with botulinum toxin: a retrospective analysis of 199 cases.

BACKGROUND: Botulinum toxin (BT) is a nonsurgical alternative to lateral internal sphincterotomy (LIS). While there are promising results, there is still a gap in knowledge regarding long-term outcomes and the predictors for healing after BT. METHODS: Chronic anal fissure (CAF) patients treated with 100 IU BT with a minimum 5-year follow-up were analyzed retrospectively. Patients with persistent or recurrent fissures after their first BT injection were offered either a second BT injection or LIS. Healing was defined as complete symptom resolution with fissure epithelization. The primary outco

Early Outcome of Closed Lateral Internal Sphincterotomy in the Treatment of Chronic Anal Fissure Compared to Open Method.

Lateral internal sphincterotomy (LIS) has long been regarded as the standard treatment for chronic anal fissure (CAF). Both open and closed methods are practiced for LIS. In this study evaluated and compared the early clinical outcome in terms of patient morbidity and procedure related complications of both the methods. This was a quasi-experimental study conducted in the department of Surgery, Sir Salimullah Medical College Mitford Hospital, Dhaka, Bangladesh (SSMCMH), Dhaka, Bangladesh from May 2018 to April 2019. Fifty (50) patients of chronic anal fissure had been chosen purposively and di

[Chronic anal fissure-Cremes or surgery?].

An anal fissure is a painful lesion of the anal canal. The acute form often heals spontaneously and should be treated conservatively. The treatment of chronic anal fissures is the subject of many studies but the treatment recommendations of international guidelines are contradictory. Conservative treatment consists of chemical sphincterotomy using sphincter-relaxing drugs, such as calcium channel antagonists, nitrates or botulinum toxin and leads to healing in only about 50% of patients with a prolonged healing time and a high rate of recurrence. In contrast surgical treatment is much more eff

Exploring the therapeutic potential of L-arginine in chronic anal fissure management: a comprehensive review.

Anal fissure causes pain and bleeding during or after bowel movements, significantly impacting individuals' quality of life. Current treatments aim to interrupt this cycle but have associated risks and limitations. The emergence of arginine, crucial for protein creation and nitric oxide (NO) production, presents an intriguing therapeutic avenue by the impact on reducing anal sphincter pressure and enhancing anoderm blood flow, due to its roles in vasodilation, anti-inflammatory responses, and collagen synthesis, which can promote wound healing and highlighting its potential as an alternative t

Botulinum toxin injections vs lateral internal sphincterotomy in chronic anal fissure management.

Among the various treatment modalities for chronic anal fissures, lateral internal sphincterotomy, acknowledged as the gold standard, is associated with superior healing rates but entails a higher risk of complications due to its invasive nature. Conversely, botulinum toxin injection, a minimally invasive alternative, remains a viable therapeutic option for select patient populations despite its relatively higher recurrence rate, owing to its favorable safety profile. The objective of this study was to compare the botulinum toxin and sphincterotomy efficacy in the management of chronic anal fi

A Comparative Study of Topical 2% Diltiazem Versus Lateral Internal Sphincterotomy in the Treatment of Fissure-in-Ano.

INTRODUCTION: Chronic anal fissure is a common clinical entity seen in surgical practice. Lateral internal sphincterotomy is considered the gold standard for treating chronic fissures. But recent developments in understanding the pathophysiology of anal fissures have helped to develop better conservative treatment options. In this study, we compare the healing, symptomatic relief, and side effects of topical 2% Diltiazem gel versus lateral internal sphincterotomy in the treatment of chronic anal fissures. Efficacy of botulinum toxin in the treatment of chronic anal fissure: a comprehensive systematic review.

Acute anal fissure maintains a debilitating state that produces enduring wound damage on the anal mucosa while the internal anal sphincter demonstrates increased muscle tone. This study aims to conduct a comprehensive systematic review to evaluate the efficacy of botulinum toxin (BT) in the treatment of chronic anal fissure, synthesizing current evidence from clinical trials and observational studies to guide evidence-based practice. A review of systematic research investigates the performance and security along with extended outcomes linked to BT therapy for treating chronic anal fissure by a

Drugs in focus: Botulinum toxin in the therapy of gastrointestinal disorders in children.

WHAT IS KNOWN: Botulinum toxin (BoNT) causes muscle relaxation by inhibiting acetylcholine release from presynaptic motor neurons at the neuromuscular junction. WHAT IS NEW: In children with achalasia, BoNT can be considered only in patients in whom rapid weight gain is important to improve surgical outcomes. BoNT has been suggested for treating cricopharyngeal achalasia and delayed gastric emptying. Anal achalasia and constipation after Hirschsprung disease corrective surgery are very promising indications for BoNT use. In selected children with resistant type of functional constipation and
