The Re-Emergence of Bundibugyo Ebolavirus in Uganda and the Democratic Republic of Congo: Epidemiological Drivers, Response Strategies, and Implications for Global Health Security.

Bundibugyo ebolavirus (BDBV) is one of the least studied species within the genus Orthoebolavirus (family Filoviridae), despite its capacity to cause severe Ebola virus disease (EVD) with substantial mortality. First identified during a 2007-2008 outbreak in Bundibugyo District, western Uganda (149 reported cases, 37 deaths; case-fatality rate [CFR] approximately 25-36%), BDBV re-emerged in 2012 in Orientale Province, Democratic Republic of the Congo (DRC) (57-59 cases, 29-34 deaths; CFR 34-58%), before resurfacing in Ituri Province, DRC, in April-May 2026. By 11 August 2026, this third outbre

Community event-based surveillance as trust infrastructure for Bundibugyo Ebola response.

The 2026 outbreak of Ebola caused by Bundibugyo virus disease in the Democratic Republic of the Congo and Uganda highlights persistent gaps in centralized outbreak surveillance, particularly in settings affected by insecurity, displacement, cross-border mobility, community mistrust, and limited disease-specific countermeasures. Community event-based surveillance (CEBS) offers a practical approach for strengthening case-finding while building trust. CEBS relies on trained, equipped, and compensated community members to report trigger events that may indicate hidden transmission, linking local a

A generalized multi-population model for Ebola virus transmission incorporating environmental reservoir dynamics.

BACKGROUND: Ebola virus disease (EVD) remains a serious public health hazard due to high mortality, multiple modes of transmission, and persistence in the environment. The existing Ebola transmission models have been constrained to a single or two-population framework, which are insufficient to capture multiple-population interaction transmission structures. METHODS: This study proposes a multi-population autonomous Ebola transmission model with Susceptible, Exposed, Infectious, Hospitalized, Recovered, and Deceased infected corpse compartments and a shared viral reservoir in the environment.

Validation of a Bayesian Ebola Diagnostic Model Using Data from the Tenth Epidemic of 2018 to 2020 in the Democratic Republic of Congo.

Background/Objectives: Diagnosing Ebola virus disease (EVD) based solely on clinical signs remains challenging in settings where its manifestations overlap with other endemic infections. This study aimed to validate a Bayesian clinical diagnostic model to support decision-making for suspected EVD cases in resource-limited settings without access to laboratory confirmation. Methods: We conducted a retrospective analysis using data from the 10th Ebola outbreak in the Democratic Republic of the Congo (2018-2020). Clinical information from 450 suspected and laboratory-confirmed EVD cases was evalu

Mapping the research landscape of antibody therapy for Ebola virus disease: a bibliometric analysis.

BACKGROUND: Antibody-based therapies have become a cornerstone of Ebola virus disease (EVD) treatment and prevention, particularly following the successful clinical development of monoclonal antibody therapeutics against Zaire ebolavirus. However, despite substantial advances, the global research landscape, collaboration patterns, and evolving thematic trends of Ebola antibody therapy have not been systematically characterized. The ongoing 2026 Bundibugyo ebolavirus outbreak, for which no licensed vaccines or therapeutics currently exist, further underscores the importance of understanding the

Caesarean section rates and obstetric practices in Sierra Leone before, during, and after the Ebola epidemic: Results from a nationwide household study.

BackgroundThrough health system reforms Sierra Leone has achieved reductions from 1,603 to 354 maternal deaths per 100,000 live births between 2000 and 2023, but progress has been uneven and severely disrupted by the 2013-2016 Ebola virus disease (EVD) outbreak. National surveys track key maternal health indicators but provide limited insight into assisted vaginal delivery and women's access to emergency obstetric care, including caesarean section, during and after health system shocks.ObjectivesTo describe patterns of caesarean section and assisted vaginal delivery, place and mode of transpor

Ebola Virus Disease in the Era of One Health and Global Preparedness: Evolving Epidemiology, Genomic Surveillance, and Future Challenges.

Ebola virus disease (EVD) remains one of the most severe viral hemorrhagic fevers, with recurrent outbreaks challenging global healthcare systems. This narrative review traces the evolving epidemiology of EVD from the first recognized outbreaks in 1976 to the ongoing 2026 Bundibugyo virus public health emergency in the Democratic Republic of the Congo (DRC) and Uganda. Over nearly five decades, the recognized range of Ebola outbreak contexts and amplification mechanisms has broadened considerably: rural zoonotic spillovers remain the predominant mode of emergence, but the scale and reach of su

Field Investigation of the Bundibugyo Ebola Virus Disease (BDBV) Outbreak in Ituri Province, Democratic Republic of the Congo: Challenges, Strategies, and Priority Actions for Outbreak Control-An Outbreak Investigation Review.

BACKGROUND: The 2026 outbreak of Bundibugyo Ebola virus disease (BDBV) in eastern Democratic Republic of the Congo (DRC), centered in Ituri Province, represents the largest documented outbreak caused by Bundibugyo ebolavirus since its discovery in Uganda in 2007. The outbreak evolved within a complex humanitarian setting characterized by armed conflict, population displacement, mining-related migration, weak health systems, extensive population mobility, and an infodemic environment marked by misinformation and reduced public trust. We conducted a field investigation to assess epidemiological,

Ebola Virus RNA Persists in Reproductive Tissues and Sperm of Male Ifnar1-/- Mice After Resolution of Viremia.

Ebola virus (EBOV) has been shown to persist in the reproductive tracts of male Ebola virus disease (EVD) survivors ranging from months to years following recovery, with shedding of EBOV in semen leading to chains of disease transmission. In the current study, we used a mouse model of EBOV infection to investigate the timing of virus infiltration of the male reproductive tissues, the extent of virus persistence in these tissues, and the impact of persistence on tissue health. We challenged male Ifnar-/- mice with wild-type EBOV and collected blood, liver, testis, epididymis, and sperm samples

Public Information Demand During the 2026 Bundibugyo Virus Outbreak: A Dual Infodemiology Analysis Across Six Ebola Virus Disease Outbreaks.

The 2026 Bundibugyo virus disease outbreak in the Democratic Republic of the Congo and Uganda, the first Ebola-related PHEIC caused by a species lacking a licensed vaccine, has been accompanied by an information crisis marked by conspiracy-driven violence against response teams and destruction of treatment centres. Understanding what populations actually seek to know during such events is essential for designing effective communication strategies, yet the content of public information demand during Ebola outbreaks has not been systematically characterised. We combined Google Autocomplete query

Characterization of Mouse-Adapted Marburg and Ravn Viruses in Inbred BALB/c and Outbred CD-1 Mice.

Marburgviruses cause severe hemorrhagic disease in humans, yet characterization of rodent models for pathogenesis and medical countermeasure development remains limited. Here, we investigated clinical course, blood chemistry, viral dissemination, and mucosal viral detection in inbred BALB/c and outbred CD-1 mice infected with mouse-adapted (MA)-Marburg virus (MARV)/Angola or MA-Ravn virus (RAVV) to further mouse model development. Regardless of virus or mouse strain, infection resulted in widespread viral dissemination in animals sampled up to 5 days post-infection. However, in animals followe

Genomic Insights into Marburg Virus Strains from 2023 and 2025 Outbreaks in Kagera, Tanzania.

Marburg virus (MARV) is the primary cause of Marburg virus disease (MVD), a severe hemorrhagic fever with a high case-fatality rate. The first reported MVD outbreak in Tanzania occurred in 2023, followed by a second outbreak in 2025, both within the Kagera region. During those MVD outbreaks, 174 suspected cases were identified; of those, 10 were laboratory confirmed. After complete genome assembly and bioinformatic analyses, we found the MARV strains of the 2023 and 2025 outbreaks to be closely related and clustered with MARV strains that caused outbreaks in Rwanda (2024) and Uganda (2014). Machine Learning-Enabled Quantification of Hepatocellular Necrosis in the Liver After Lethal Marburg and Ebola Virus Exposures.

The liver is a key target of pathogenic Marburg and Ebola viruses (MARV and EBOV, respectively), with injury common in severe filovirid disease, yet high-resolution histopathologic quantification is lacking. We addressed this by deploying deep learning (DL) models for pixel-level quantitative analysis of digitized liver pathology slides in lethal rhesus monkey (RM) models of MARV and 2 EBOV variants, Makona and Kikwit. Our DL model segmented arteries, veins, bile ducts, and hepatic necrosis, achieving interobserver variability in necrosis segmentation comparable to that of 3 pathologists. DL-q

The critical role of emergency departments in managing infectious disease outbreaks: Lessons from the Rwanda Marburg virus response.

The 2024 Marburg Virus Disease outbreak in Rwanda underscored the indispensable role of Emergency Departments in the early detection, containment, and management of novel infectious diseases. As public health crises continue to grow in complexity, the Emergency Department becomes uniquely positioned to bridge the gap between clinical care and public health responses. Using lessons learned from King Faisal Hospital, Rwanda during the outbreak, this report outlines how Emergency Departments serve as frontline defenses during crisis management and identifies strategies to strengthen and enhance t

Immunoinformatics-guided design of a universal chimeric multi-epitope subunit vaccine against Marburg virus disease and Ravn virus co-infection.

Marburg Virus Disease (MVD) is a life-threatening hemorrhagic fever, caused by two viruses-Marburg virus (MARV) and Ravn virus (RAVV)-that belong to the family Filoviridae. Due to its high mortality rate and the lack of licensed vaccines, MVD remains a major global health concern. In this study, a comprehensive immunoinformatics workflow was used to design a universal chimeric multi-epitope subunit vaccine targeting conserved antigenic regions of both viruses to reduce MVD-related mortality. Antigenicity analysis identified glycoproteins (GP) of MARV and RAVV as the most immunogenic viral prot
