Zbtb44-mediated transcriptional repression critically regulates colonic inflammation.

The regulatory mechanisms that maintain colonic immune homeostasis remain incompletely understood. Here we show that genetic deletion of Zbtb44, a previously uncharacterized member of the ZBTB transcription factor family, leads to spontaneous colitis and rectal prolapse. Zbtb44-/- T cells expressed increased IFN-γ, a pro-inflammatory cytokine strongly associated with human inflammatory bowel disease. Mechanistically, Zbtb44 undergoes SUMOylation, which facilitates its interaction with a conserved SUMO-interacting motif in T-bet, enabling binding to the IFN-γ promoter and enhancer to restrain c

Robinsoniella Bacteremia in a Patient With Radiation Proctitis and Prior Colon Cancer: A Rare Case.

Robinsoniella species are strictly anaerobic, Gram-positive, spore-forming bacilli infrequently associated with human bloodstream infections. We present a case of a 49-year-old woman with a history of colon and anal carcinoma treated with radiation and chemotherapy. She presented with fever, dyspnea, and worsening diarrhea. Blood cultures grew Serratia marcescens, Robinsoniella species, and Clostridium species, and she was initially treated with intravenous piperacillin-tazobactam and vancomycin, followed by cefepime, before ultimately transitioning to ertapenem. Although no treatment is curre

Solitary Rectal Ulcer Syndrome Mimicking Ulcerative Colitis: A Case Report.

Solitary rectal ulcer syndrome (SRUS) is a benign rectal disorder associated with abnormal defecation and mucosal prolapse. Its clinical and endoscopic features may mimic inflammatory bowel disease (IBD). We report the case of a 28-year-old woman with longstanding obstructed defecation who presented with recurrent bloody mucus discharge and severe iron-deficiency anemia (hemoglobin 6.6 g/dL), which was treated with intravenous iron infusions. C-reactive protein was normal, whereas fecal calprotectin was markedly elevated. Initial colonoscopy showed erythematous and congested mucosa extending f

Schistosomal Rectal Polyposis Mimicking Colorectal Cancer: A Case Report of a 60-mm Polyp in a Young Ethiopian Man Treated at Firoomsis General Hospital.

BACKGROUND: Schistosomiasis is a major public health problem in endemic regions, affecting over 250 million individuals globally, with Schistosoma mansoni frequently causing intestinal and hepatosplenic disease. Chronic infection may lead to rectal polyposis, which can mimic colorectal malignancy and create diagnostic and therapeutic challenges. Addressing these intersecting diagnostic and therapeutic issues necessitates a comprehensive multidisciplinary approach that integrates increased clinical vigilance in endemic populations, accurate histopathological assessment, focused endoscopic treat

Public support for novel carbon removal hinges on procedural and distributive fairness.

Scientifically valid data on the safety of drug use during pregnancy are a significant public health need. Data are rarely available on the fetal effects of in utero exposure in human pregnancies, particularly when a drug is first marketed. Data from animal reproductive toxicology studies, which function as a screen for potential human teratogenicity, are usually all that is available in a product's labelling. For practising clinicians, translating known animal risks into an accurate assessment of teratogenic risks in their patients is very difficult, if not impossible. Without human data on t

Transcription start site aware catalogue of microRNA encoded peptides in tomato (Solanum lycopersicum).

UNLABELLED: Plant microRNAs (miRNAs) are key regulators of gene expression, and primary miRNA transcripts (pri-miRNAs) can encode short regulatory peptides known as microRNA-encoded peptides (miPEPs), which enhance transcription of their associated miRNAs. However, comprehensive and accurately defined miPEP catalogues remain limited in crop species, including tomato (Solanum lycopersicum). Here, we generated a curated tomato miPEP catalogue using a transcription start site (TSS)-aware pipeline that integrates upstream sequence extension, promoter prediction, and stringent open reading frame (O

Recurrent Pregnancy Loss: Novel Insights and Future Directions.

In the last 15 years, the US and UK have fought two major wars in the Persian Gulf region. Controversy has arisen over the nature and causes of health problems among military veterans of these two wars. Toxic exposures have been hypothesised to cause the majority of the long-term health problems experienced by veterans of the 1991 Gulf War. The assessment of these toxic exposures and the resolution of controversy about their health effects provide a unique case study for understanding how toxicological disputes are settled in the US. Neither clinical examination of ill war veterans nor scienti

Surgeon preferences and practice patterns in rectopexy: Results of an international survey.

AIM: Rectopexy is the preferred abdominal intervention for rectal prolapse. Despite similar procedural steps - rectal mobilisation, prolapse reduction, and fixation - techniques vary widely, and onsensus on the optimal approach is lacking. This study aimed to assess global surgeon preferences and practices in rectopexy. METHODS: An international 28-item online survey was distributed between November 2023 and March 2024 through professional networks and social media. Questions addressed surgeon demographics, perioperative strategies, and technical approaches to rectopexy. Responses were analys

The Levator Hiatus Area detected by 3D-TPUS as an indicator of rectal prolapse severity.

PURPOSE: Rectal prolapse (RP) is a clinically significant condition with vaginal delivery as a major risk factor, especially in elderly females, needs precise evaluation for guiding treatment. Given the limitations of current diagnostic methods in terms of convenience, this study aims to develop an improved measure for RP. METHODS: A retrospective analysis of 181 female patients undergoing both dynamic three-dimensional transperineal ultrasound (3D-TPUS) and radiographic (X-ray or MRI) defecography (X-ray and MRI) was conducted to investigate the correlation between 3D-TPUS parameters and RP

Severe fecal incontinence predicts higher recurrence risk in complete rectal prolapse: a retrospective cohort study.

BACKGROUND: Postoperative recurrence poses a major challenge in the management of complete rectal prolapse (CRP). However, the association between fecal incontinence (FI) and recurrence risk remains unclear. In this study, we aimed to examine the relationship between FI severity and postoperative recurrence in patients with CRP. METHODS: Patients with CRP who underwent surgical treatment at Guang’anmen Hospital, China Academy of Traditional Chinese Medicine, between January 1, 2013, and August 31, 2025, were enrolled. Multivariate survival analysis was conducted to investigate the relationship

Development of Rectal Prolapse After Surgical Management of Pelvic Organ Prolapse: Obliterative Versus Reconstructive.

INTRODUCTION AND HYPOTHESIS: Pelvic organ prolapse (POP) is a common condition among aging women, whereas rectal prolapse (RP) is uncommon. The change of anatomy and vector of downward force after surgical repair of POP may be a risk factor for RP. The objective of this study is to examine the occurrence of RP after surgical repair of POP and associated clinical factors. METHODS: A retrospective chart review was performed from 2013 to 2024 for women who had undergone surgery for POP at a single institution. Demographic variables were analyzed. Correct diagnoses were validated, and clinical co

Laparoscopic Resection Rectopexy with Transanal Specimen Extraction for Complete Rectal Prolapse: Retrospective Cohort Study of Functional Outcomes.

Background: Complete rectal prolapse (RP) is a debilitating pelvic floor disorder often accompanied by obstructed defecation syndrome (ODS), fecal incontinence, and LARS-like bowel dysfunction. Laparoscopic resection rectopexy (LRR) is an established abdominal approach; however, functional outcomes after LRR with transanal specimen extraction (LRR-TSE) are incompletely defined. Aim: To evaluate short- and long-term functional outcomes-ODS, Wexner incontinence score (WIS), and LARS-in patients undergoing LRR-TSE. Methods: This single-center cohort included 53 consecutive patients who underwent

A comprehensive scoring system for defecation disorders derived by merging various validated patient-reported outcome measures for fecal incontinence, chronic constipation, and obstructed defecation.

PURPOSE: Currently, too many Patient-Reported Outcome Measures (PROMs) with redundant and repetitive domains are adopted to assess defecation disorders, resulting in more extended clinical visits and increased patient burden. The aim of this study was to develop a new comprehensive Defecation Disorders Scoring System (DDSS) by incorporating all items of the most commonly used and validated PROMs. METHODS: This is a prospective observational study on patients waiting for rectal prolapse and defecation disorders surgery. Preoperatively, each patient completed seven different authoritative PROMs
