OBJECTIVE: A novel modification of the cutback procedure, a posterior rectal advancement anoplasty (PRAA) for select male patients with an anorectal malformation and a rectoperineal fistula was recently described, which incised only within the limits of the sphincteric ellipse and eliminated an anterior rectal dissection and thus avoiding any possibility of a urethral injury. This report provides longer-term postoperative outcomes after PRAA.
METHOD: A retrospective, single-institution study was performed examining male patients with a rectoperineal fistula between January 2020 and December 2

Rectal prolapse and intussusception are rare in adults and particularly unusual in the postpartum period. We present the case of a 39-year-old woman, one month postpartum following cesarean delivery of twins, who developed rectal prolapse during pregnancy associated with persistent constipation. She later presented with rectal bleeding and mucus discharge. Computed tomography of the abdomen and pelvis demonstrated rectosigmoid intussusception, while colonoscopy revealed a large fungating and friable mass at 25 cm from the anal verge. Biopsies confirmed adenocarcinoma. Magnetic resonance imagin

BACKGROUND: Autophagy is a self-degradative process that plays a role in removing misfolded or aggregated proteins located in the cytosol. Autophagy induction is initiated by the mammalian target of rapamycin (mTOR). Beclin-1, and microtubule-associated protein 1A/1B-light chain 3 (LC3) are proteins involved in the regulation of autophagy. Cystic fibrosis (CF) cells suffer from defective autophagy, related to altered proteostasis and chronic inflammation. The study aimed to assess the genetic expression of mTOR, Beclin-1, and LC3b in CF children and to correlate them with genotypes, phenotypes

PURPOSE: There have been few reports that have focused on the combination of the laparoscopic approach and natural orifice specimen extraction (NOSE) surgery for complete rectal prolapse (CRP). The aim of this study was to compare the feasibility, safety and short-term clinical efficacy of NOSE surgery with those of conventional laparoscopic resection rectopexy in the treatment of CRP. METHODS: In a group of 45 patients selected between January 2019 and October 2023, 28 patients underwent conventional laparoscopic resection rectopexy (LRR), and 17 patients underwent transanal NOSE laparoscopic

Rectal prolapse is a complex, multifactorial condition with significant functional and psychosocial impact. This review outlines current understanding of its pathophysiology, clinical presentation, diagnostic evaluation, and evidence-based management. Emphasis is placed on a patient-centered approach that incorporates anatomic and functional factors, as well as multidisciplinary correction of coexisting pelvic organ prolapse. Both perineal and abdominal surgical approaches offer effective treatment. Recent data support individualized operative planning based on patient characteristics, surgica

Literature has previously compared the outcomes amongst the different approaches of operative management of rectal prolapse. There is no national dataset comparing robotic rectopexy to other approaches. This study aims to compare the outcomes of patients undergoing rectopexy with and without resection via a laparoscopic versus robotic approach. A retrospective study of the American College of Surgeons National Surgical Quality Improvement Program database (2022-2023) was performed to identify patients who underwent minimally invasive surgery for rectal prolapse. UNLABELLED: Herein the authors discuss their approach of using resection rectopexy to select cases of external and high grade internal rectal prolapse resistant to conservative therapy. They describe a scientific, anatomical and physiological approach to this complex clinical problem. They present a methodical framework based on the latest scientific understanding, but also the importance of tailoring this to individual patients. The authors describe, for the first time in the literature, different clinical phenotypes of prolapse that they have identified and include a high quality surgical vi

BACKGROUND: Ileal pouch-anal anastomosis (IPAA) is a key procedure to master in colorectal surgery. It is one task of the Colorectal Objective Structured Assessment of Technical Skill (COSATS) of the American Board of Colon and Rectal Surgery. The virtual colorectal surgical trainer-IPAA (VCoST-IPAA) was designed as an innovative platform for training and assessing performance in this procedure. Our aim was to establish the validity of the simulator by demonstrating its ability to distinguish between levels of surgical expertise.
METHODS: In this IRB-approved study, general surgery residents

Anorectal manometry (ARM) is a valuable diagnostic modality for evaluating anorectal disorders. While rare, complications can occur including rectal perforation. We report a case of iatrogenic rectal perforation following ARM in a 77-year-old female with multiple prior rectal prolapse surgeries who was receiving chronic anticoagulation therapy. The patient developed rectal bleeding immediately post-procedure with subsequent imaging demonstrating retroperitoneal and mediastinal air. Flexible sigmoidoscopy revealed a 30% circumference rectal wall defect. Despite the extensive injury, the patient

INTRODUCTION: Limited data exists in literature regarding concomitant ventral rectopexy (VRP) and sacrocolpo/hysteropexy (SCP/SHP), with existing studies being predominantly retrospective. The aim of this meta-analysis is to assess the anatomical and functional outcomes of combined VRP and SCP/SHP for the treatment of multicompartmental pelvic organ prolapse (POP).
METHODS: We performed systematic research and meta-analysis from PubMed/MEDLINE and EMBASE, according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines, until 15 January 2025. Women subm

BACKGROUND: Full-thickness rectal prolapse (FTRP) is heterogeneous, with outcomes varying despite similar anatomy. Cooper et al. proposed a radiological classification based on prolapse 'take-off' level on defecating proctography (DPG), hypothesising that high take-off (HTO) and low take-off (LTO) phenotypes may respond differently to abdominal versus perineal repair. This hypothesis has not previously been tested in a clinical cohort.
METHODS: A retrospective observational cohort study was performed across three hospitals over 12 years (2012-2024). Patients undergoing laparoscopic ventral me

BACKGROUND: Rectourethral fistulae (RUFs) are common in boys with anorectal malformations (ARMs). Laparoscopically assisted anorectal pull-through (LAARP) is the preferred approach for treating high/intermediate imperforate anus RUF; however, its application for rectourethral bulbar fistula (RUBF) is contentious due to worries about the remnant of the original fistula (ROOF). Our primary objective is to present the surgical technique for transanal ligation of RUBF and to discuss its merits in mitigating urethral complications related to a ROOF for LAARP surgery.

Rectal prolapse is a common pelvic floor disorder. Its pathogenesis primarily involves the degeneration and impairment of the supporting tissues of the rectum, leading to their laxity and consequent displacement of the rectum. As the pelvic cavity is a complex system consisting of gynecology, urology, and coloproctology, rectal prolapse often coexists with other pelvic relaxation disorders, such as perineal descent, pelvic floor hernias, and uterovaginal prolapse. Traditionally, pelvic-related disciplines have operated independently, creating disciplinary boundaries that restricted perspective

In recent years, significant progress has been made in the treatment of chronic constipation, with high-quality studies emerging in areas such as fecal microbiota transplantation (FMT), sacral neuromodulation (SNM), acupuncture, and surgical techniques. The therapeutic approach is shifting from a traditional "medication and surgery" model toward an integrated strategy that includes dietary and defecation habit adjustment, FMT, SNM, acupuncture, pharmacotherapy, and surgery. Although FMT can partially improve stool frequency and consistency, its standardization and long-term efficacy require fu
