Adherent-invasive Escherichia coli pduB drives vitamin B12-dependent colitis in mice.

UNLABELLED: Adherent-invasive Escherichia coli (AIEC) are strongly implicated in the pathogenesis of Crohn's disease, but the virulence factors driving their invasive and pro-inflammatory behavior remain incompletely defined. Here, we demonstrate that vitamin B12 availability modulates pdu-dependent AIEC virulence. The 1,2-propanediol utilization (pdu) operon, which requires vitamin B12 as a cofactor, has previously been associated with bacterial virulence. Here, we validated this association and demonstrated its functional relevance in mouse models of colitis. Deletion of pduB, a structural c

Clinical Outcomes and Recurrence Prevention in Laparoscopic Wells Rectopexy for Rectal Prolapse through Mesh Trimming and Fixation Techniques.

OBJECTIVES: Laparoscopic Wells rectopexy (LWR) is a standard procedure for rectal prolapse; however, recurrence and mesh-related complications remain clinical challenges, particularly in elderly patients with small pelvic anatomy. This study aimed to evaluate the clinical outcomes of a modified LWR technique featuring intraoperative mesh tailoring and a hybrid fixation method. METHODS: We retrospectively analyzed 29 patients who underwent modified LWR between October 2014 and June 2025. Technical modifications included intraoperative trimming of the polypropylene mesh to fit each patient's pe

Early Post-Operative Diagnosis of Anastomotic Leak by Local Electrophysiological Parameters: Multicenter Pilot Study.

Background: Morbidity associated with anastomotic leak (AL) can be reduced by timely detection. Local electrophysiological parameters are highly sensitive to pathophysiological processes (inflammation/ischemia) underlying AL and may enable early post-op monitoring ahead of clinical signs. A new approach to early leak detection includes post-operative measurement of bioelectric activity of the GI tract. The method has been implemented in a medical device that uses electrodes embedded in a standard surgical drain to continuously monitor local bowel myoelectric activity following GI surgery. Meth

Benign lesions prolapsing through the anus: a differential diagnostic algorithm and evidence-based surgical management-a narrative review.

Masses prolapsing through the anus represent a frequent diagnostic challenge in coloproctological practice. Misidentification may delay the management of malignant disease or surgical emergencies. This narrative review integrates current evidence on seven benign entities that may prolapse through the anus-complete rectal prolapse, mucosal rectal prolapse, internal hemorrhoids, thrombosed hemorrhoids, anal condylomas, hypertrophied anal papilla, and anal canal polyps-and proposes a clinical decision algorithm based on findings from physical examination. Particular emphasis is placed on the iden

The First Evidence of Intermittent Rectal Prolapse in a Wild Chimpanzee (Pan troglodytes).

BACKGROUND: Rectal prolapse in wild chimpanzees has been rarely reported. We document a case of rectal prolapse in a wild young male chimpanzee at Mahale Mountains National Park, Tanzania. METHODS: An affected young male in the Mahale M group was observed with photographs/video and diagnosed according to operational criteria used in human (Oxford grading system). Cause and course of the prolapse were estimated by a set of longitudinal field data. RESULTS: An intermittently self-reducing full-thickness rectal prolapse was diagnosed. Protrusion fluctuated within minutes; tenesmus and diarrhea

Case Report: Sigmoidorectal intussusception mimicking rectal prolapse-a diagnostic pitfall with surgical implications.

Adult intussusception is a rare condition typically associated with a structural lead point and often requiring surgical management, particularly in colonic forms where malignancy risk is high. Sigmoidorectal intussusception represents an exceptionally uncommon subtype that may clinically mimic rectal prolapse, posing a significant diagnostic challenge. We report the case of a 57-year-old female presenting with an irreducible anorectal mass, initially suggestive of prolapse, in whom computed tomography suggested the diagnosis of sigmoidorectal intussusception, which was subsequently confirmed

Rectal Prolapse Associated With Chronic Ketamine Misuse: A Report of Two Cases.

Chronic ketamine misuse is a well-recognized cause of ketamine-induced uropathy (KIU), characterized by severe lower urinary tract symptoms and markedly reduced bladder capacity. Its potential association with anorectal dysfunction remains poorly described. We report two young adults with KIU who developed progressive pelvic floor dysfunction and anorectal pathology ranging from pelvic floor dyssynergia and internal rectal intussusception to full-thickness rectal prolapse (RP). Both patients experienced severe urinary frequency, suprapubic pain, and chronic straining to void. One patient devel

Thickening of the internal anal sphincter: correlation with external rectal intussusception and associated features at MR proctography and anal manometry.

OBJECTIVES: To determine if internal anal sphincter (IAS) thickness can be reproducibly measured with endoanal MRI in conjunction with MR defecography, and if IAS thickening is associated with external rectal intussusception (i.e., external rectal prolapse) at MR defecography or high-resolution anal manometry (HRM) measurements. METHODS: In this case-control study, 119 patients underwent clinical endoanal MRI, dynamic MR defecography, and HRM. Patients with intra-anal or external rectal intussusception (cases) were matched to those who had neither (controls). Five radiologists, blinded to all

Manometric and Sonographic Examination of Anal Sphincter Complex, Fecal Incontinence and Quality of Life Outcomes in Fournier's Gangrene Survivors.

Fournier's gangrene (FG) often requires repeated aggressive debridement. Its long-term effects on continence, anal sphincter integrity, and quality of life remain insufficiently defined. Among 188 patients treated for FG between 2000 and 2021 and assessed for eligibility, 21 survivors who had perianal debridement within 5 cm of the anal verge, healed without permanent stoma, and completed the study protocol were included. Participants underwent standardized physical examination, anorectal manometry, endoanal ultrasonography, and questionnaire-based assessment of fecal incontinence and quality

Comparing Preoperative Healthcare Utilization Between Immigrants and Non-Immigrants Undergoing Emergency General Surgery in British Columbia.

Immigrants to Canada face unique barriers that result in them accessing publicly funded healthcare services differently than non-immigrants. These barriers are particularly critical in the context of Emergency General Surgery (EGS) where timely access to healthcare is essential for better health outcomes. This study compares preoperative healthcare utilization between immigrants undergoing EGS with non-immigrants. EGS procedures in British Columbia between 2013 and 2021 were analyzed using population-based linked immigration, physician billing, and hospital discharge summary data. Preoperative

Clinical and genetic analysis of a family with 16p11.2 microduplication syndrome and variable multisystem manifestations.

16p11.2 microduplication syndrome (OMIM #614671) is a pathogenic recurrent copy-number gain at the 16p11.2 locus and is associated with variable expressivity across neurodevelopmental, growth, and medical phenotypes. Gastrointestinal symptoms have been reported in carrier cohorts, but detailed documentation of gastrointestinal motility and neuromuscular findings remains limited. We performed clinical and genetic analyses in a multigenerational family in which the proband (III1) presented with limb muscle pain, exercise intolerance, and chronic gastrointestinal symptoms. Next-generation sequenc

Simulation of errors related to the VLBI transmitter on Genesis and their impact on the terrestrial reference frame.

BACKGROUND: During the second wave of COVID-19 pandemic, there was surge in reporting of COVID-19-associated mucormycosis cases. Government of India declared rhino-orbito-cerebral mucormycosis as a notifiable disease. Managing this condition posed a new challenge to clinicians. Management includes therapeutic drugs and aggressive surgical debridement. Even though prosthetic rehabilitation with zygomatic implants is one of the most conservative and effective options, owing to unpredictability of disease prognosis and financial burden for patients, rehabilitation with interim obturators is benef

Connective tissue and genetics in the pathogenesis of rectal prolapse: A scoping review.

AIM: Rectal prolapse can occur in individuals who lack the established risk factors (constipation, obstetric trauma and advanced age). A recognised association with hereditary connective tissue diseases may indicate a role for variants in connective tissue-related genes in the pathogenesis of the disease. This scoping review evaluates the evidence supporting this. METHODS: A scoping review was performed using a systematic search of Medline, Embase, CINAHL and The Cochrane Library in March 2026. Studies describing any of the following in the context of rectal prolapse were included: epidemiolo

Long-term performance evaluation and seasonal field calibration of a low-cost particulate matter sensor.

UNLABELLED: Low-cost air quality sensors (LCS) provide high-resolution monitoring with affordability and real-time data but require performance evaluation and calibration due to probable lower accuracy compared to reference instruments and sensitivity to meteorological factors such as air relative humidity and temperature. Although most studies on low-cost sensors have been conducted in Europe and America, we did not find any study in regions like Iran, where air pollution levels and meteorological conditions differ significantly. In this study, performance evaluation and calibration of a low-

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm
