Over a period of seven years, 474 patients with acquired immune deficiency syndrome (AIDS) or AIDS-related complex were admitted on 782 occasions to the St. Francis Medical Center, Trenton, New Jersey. Abdominal surgery was performed on 16 (3.4 percent) patients, 14 (88 percent) of whom were heterosexual intravenous drug users. Anorectal surgery was performed on 20 (4.2 percent), 14 (70 percent) of whom were intravenous drug users. Intravenous drug users undergoing abdominal surgery had the same types of surgical abdominal conditions that occur in the general population. None required surgery

Anorectal manometry and electrophysiology studies were conducted in 25 men with non-prolapsing haemorrhoids, ten men with prolapsing haemorrhoids, and 20 age-matched normal men. In our population of trauma patients abdomino-perineal impalement injuries are very rare. With reference to our own patients we demonstrate the problems that arise with impalement injuries: infection, hemorrhage, intraabdominal organ lesions and difficulty in recognizing these. The necessity for elicitation of a precise history and meticulous diagnostic examination is undisputed. In our own patients we have always tried to achieve definitive therapy as a primary procedure. There have been no lethal complications.

In 100 preparations it has been stated that with reference to the operation of sphincteroplasty, the main vascular-nervous fasciculus should be considered the one, which composition includes posterior branches of the obturative arteries, veins and nerve. The intramuscular course of the arterial and nervous branches coincides. For sphincteroplasty it is possible to use one type of transplants cut out of the posterior part of the muscle.

The present study compared the outcome of a small series of patients (7 cases) who underwent total proctocolectomy without mucosal proctectomy and stapled ileal pouch-anal anastomosis made at the apex of the anal transitional zone, with our previous experience (17 cases) in which the ileal pouch was anastomosed at the dentate line after mucosectomy. Eighteen patients with severe constipation after undergoing the Ripstein operation for rectal prolapse (n = 11) or internal rectal procidentia (n = 7) were studied with defecography, anorectal manometry, electromyography of the external anal sphincter and the puborectalis muscle, colonic transit time, and blood tests. The results of an investigation of plasma levels of gastrointestinal hormones in patients after the construction of a pelvic reservoir are reported. We report on 3 cases in which ano-rectal lesions were associated with chronic administration of suppositories containing non-steroidal anti-inflammatory drugs (NSAID). Lesions were characterized by erosions or ulcer in the rectum and by stenosis of the anal verge. Erosions or ulcers were observed in patients taking the drugs for a relatively short time. The lesions remitted after suspension of the treatment. The cases observed indicate that suppositories containing NSAID could induce ano-rectal lesions similar to those observed after the use of suppositories containing ergotamine.

In a prospective study of 100 consecutive patients with cirrhosis, 44% had anorectal varices. Conservative proctocolectomy was performed for ulcerative colitis in 19 patients, Crohn's disease in three and familial adenomatous polyposis in one. Healing was uncomplicated in only three patients (13 per cent). Eleven developed an anal discharge and nine an infected pelvic haematoma despite peranal drainage. Fourteen patients developed pelvic sepsis and, despite surgical curettage in 11, none healed. Changes of bile composition in gallbladder bile and serum chemistries were investigated in nine dogs after proctocolectomy and ileoanal anastomosis, using an interposed jejunal segment as a neorectum.
