A case of multiorgan IgG4-related disease with coronary artery vasculitis: Review of the 2019 Rheumatologic Classification Guidelines and what radiologists need to know.

Immunoglobulin G4-related disease (IgG4-RD) is a rare fibroinflammatory disorder with diverse manifestations that can affect nearly any organ system. This case report describes a 58-year-old man presenting with obstructive jaundice due to a biliary stricture concerning for cholangiocarcinoma; however, eventual pathology, serology and imaging findings confirmed IgG4-RD. Over 2 years, the disease progressed to involve multiple organs, including 3-vessel coronary arteritis, a serious but underrecognized manifestation that may present with the ``pigs-in-a-blanket" sign on imaging. Despite treatmen

Clinical phenotypes and prognosis of IgG4-RD patients with allergy history: A retrospective cohort study.

INTRODUCTION: There is a high prevalence of allergic conditions in patients with IgG4-related disease (IgG4-RD), paralleled by shared dysregulation in immune responses. The aim of this study was to explore the clinical features and outcomes of IgG4-RD patients with allergy history. METHODS: 215 IgG4-RD patients were retrospectively enrolled and clinical features were compared between IgG4-RD patients with and without allergy history. Subsequently, 148 patients who had been followed up for ≥ 1 year were further selected to evaluate the outcomes and risk factors of relapse by conventional COX a

IgG4-Related disease: From diagnosis to remission.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder that can affect many organ systems. This chapter provides a comprehensive overview of current knowledge regarding IgG4-RD, addressing key questions about disease mechanisms, diagnostic approaches, and therapeutic strategies. We examine the diverse clinical manifestations ranging from glandular enlargement to life-threatening vascular involvement, emphasizing the importance of recognizing both inflammatory and fibrotic disease phenotypes. Diagnostic approaches integrate clinical symptoms, imaging findings, hist

Improvement of Lymphedema in IgG4-Related Disease with Oral Glucocorticoid Therapy.

An 81-year-old Japanese man with IgG4-related disease (IgG4-RD) presented with marked edema in the right lower leg. He was diagnosed with IgG4-RD with retroperitoneal fibrosis, hilar and mediastinal lymphadenopathy, elevated serum IgG4 levels, and IgG4-positive plasma cell infiltration in a mediastinal lymph node. Lymphatic scintigraphy revealed an obstruction corresponding to edema due to retroperitoneal fibrosis. Glucocorticoid therapy resulted in rapid improvement. Lymphedema due to IgG4-RD with retroperitoneal fibrosis is rare and no standard treatment exists. This case underscores the imp

Relapse Predictors of Idiopathic Retroperitoneal Fibrosis: A Long-Term Cohort Study.

OBJECTIVE: This study aims to identify predictive factors for the relapse of idiopathic retroperitoneal fibrosis (IRF) and provide instructions for the optimization of the maintenance therapy. METHODS: All patients with a clinical diagnosis of IRF were enrolled and followed up every 3-6 months. Their clinical characteristics, laboratory data and treatment strategies were recorded at each visit. RESULTS: 96 IRF patients (77 males and 19 females) with a median age of 55 years (interquartile range [IQR], 50-61) were enrolled. The median follow-up time was 2.50 (IQR, 1.75-4.13) years. A Systematic Review of Erdheim-Chester Disease and IgG4-Related Disease: Building a Diagnostic Framework for the Rheumatologist.

INTRODUCTION: Erdheim-Chester disease (ECD) and immunoglobulin G4-related disease (IgG4-RD) are both rare, multisystem disorders with overlapping clinical, radiologic, and histopathologic features. This overlap leads to delays or misdiagnoses. Early diagnosis with proper distinction is critical for treatment and a better prognosis. METHODS: A review of the literature using PubMed and Google Scholar was conducted to identify distinguishing features between ECD and IgG4-RD. The focus was on clinical presentation, diagnostic imaging, histopathologic findings, immunohistochemistry, and genetic mu

Robotic Bilateral Ileal Interposition: The Total Intracorporeal Approach.

PURPOSE: While grafting procedures can be used to manage upper ureteric strictures (1), ileal interposition still has its role in the more complex cases with long or bilateral ureteric strictures. Ketamine-associated uropathy can mimic retroperitoneal fibrosis and give rise to long bilateral ureteric strictures (2, 3). The objective of the video is to present the total intracorporeal technique for bilateral ileal interposition with a U-configuration of small bowel loop. PATIENT AND METHODS: A 34-year-old lady with bilateral long ureteric stricture secondary to ketamine-associated uropathy was

Encased by Infection: A Rare Case of Retroperitoneal Fibrosis Induced by Disseminated Cysticercosis.

Cysticercosis refers to a disease caused by the larvae of Taenia solium parasitizing various tissues and organs of the human body. It is reported that cysticercosis is most commonly caused by the central nervous system, and retroperitoneal fibrosis is rare. We report a case of retroperitoneal fibrosis caused by cysticercosis and a urinary tract obstruction caused by retroperitoneal fibrosis, which was successfully relieved by transurethral stenting. The complex pathophysiological mechanism of cysticercosis-induced retroperitoneal fibrosis requires further study. In the diagnosis and treatment

Pembrolizumab-Induced Retroperitoneal Fibrosis Detected on 68 Ga-FAPi PET/CT.

Hepatocellular carcinoma is one of the leading causes of cancer-related death worldwide. Immune checkpoint inhibitors (ICI) have improved progression and overall survival in patients progressing on sorafenib therapy. But activation of the immune system can lead to numerous immune-related adverse events. Here we present a case of hepatocellular carcinoma with abdominal pain after initiation of pembrolizumab. 68 Ga-FAPi PET/CT highlighted FAPi avid soft tissue density in the paraaortic and presacral regions with diffusely increased FAPi uptake in the thyroid, suggestive of ICI-induced retroperit

Highlights from the breakout session: other forms of vasculitis.

The session on other forms of vasculitis included a masterful review on IgG4-related disease (IgG4-RD) following which two research studies evaluating therapeutic agents and one study on histopathologic findings of IgG4-RD were presented. Peyronel F., et al. discussed the results of a prospective multicentre randomized clinical trial evaluating methotrexate for patients with retroperitoneal fibrosis. Methotrexate, in combination with a tapering regimen of glucocorticoids, was effective in inducing remission and allows use of lower cumulative prednisone doses. Hobbelink J., et al. evaluated the

Imaging in Renovascular Hypertension: State of the Art.

Renovascular hypertension (RVH) results from activation of the renin-angiotensin system by renal arterial stenosis (RAS). RAS can result from either intrinsic narrowing or extrinsic compression. Intrinsic narrowing may be caused by atherosclerosis, fibromuscular dysplasia, vasculitis, dissection, thrombus, aneurysm, arteriovenous fistula, neurofibromatosis type 1, or midaortic syndrome. Extrinsic compression can be from retroperitoneal fibrosis, neoplasm, or subcapsular hematoma or fluid. Diagnostic evaluation for RVH is triggered by either a high index of suspicion in a patient with hypertens

Clinicopathological Pearls and Diagnostic Pitfalls in IgG4-Related Disease: Challenging Case Series and Literature Review.

Background: IgG4-related disease (IgG4-RD) is a chronic immune-mediated fibroinflammatory disorder characterized by lymphoplasmacytic infiltrates enriched in IgG4-positive plasma cells, storiform fibrosis, and frequently elevated serum IgG4 levels. Classic forms, such as pancreaticobiliary or retroperitoneal involvement, are often recognized early, whereas atypical manifestations mimic malignancy or inflammatory conditions, leading to delayed or inappropriate treatment. Case Series: A 30-year-old man presented with hyperemesis, proptosis, and gait instability. He was found to have colonic sten

Diagnostic Dilemma of a Neuroendocrine Tumour Complicated by Simultaneous Retroperitoneal Fibrosis and Carcinoid Heart Disease in a Perimenopausal Woman.

Carcinoid tumours are rare, slow-growing neuroendocrine neoplasms that often remain asymptomatic until metastatic spread or the development of carcinoid syndrome. Carcinoid syndrome is characterised by flushing, diarrhoea, and bronchospasm due to the secretion of vasoactive hormones. These tumours commonly arise in the gastrointestinal tract but can also occur in other organs, namely, the lungs, genitourinary tract, and pancreas. Retroperitoneal fibrosis (RPF), a rare inflammatory disease, involves chronic inflammation leading to fibrous scarring and compression of surrounding structures like

A Case of Idiopathic Retroperitoneal Fibrosis and Inflammatory Abdominal Aortic Aneurysm in a 73-year-old Patient.

Retroperitoneal fibrosis (RPF) involves an inflammatory fibrous retroperitoneal development that surrounds the abdominal aorta. Oftentimes, extension into the iliac arteries and ureters is seen, which can cause hydronephrosis and lead to renal failure. There are a variety of causes for RPF including an idiopathic presentation, complication of infection such as syphilis or tuberculosis, or a complication of autoimmune disease. A case of idiopathic retroperitoneal fibrosis in a 73-year-old male will be examined. The patient presented with bilateral flank pain, hematuria, and low urine output tha

Retroperitoneal fibrosis and bilateral hydronephrosis secondary to gastric cancer: A case report.

RATIONALE: Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory condition that can lead to ureteral obstruction and renal dysfunction. While most cases are idiopathic, secondary causes including malignancies should be considered. RPF secondary to gastric cancer is extremely rare, with only a few cases reported. PATIENT CONCERNS: A 56-year-old woman presented with generalized edema, decreased urine output, and nausea that had begun 7 days prior. Laboratory tests revealed acute kidney injury, and imaging showed bilateral hydronephrosis. DIAGNOSES: Further evaluation through abdominal-pel

Four-Level Cervical Disc Arthroplasty.

Within the variable course of ankylosing spondylitis (AS), peripheral arthritis, enthesitis, and involvement of other organs can add to the burden of the disease. The primary complaints of patients with spondyloarthritis (SpA) are pain, stiffness, fatigue, and limitation in activities and social participation. Instruments currently available for the assessment of patients with SpA focus predominantly on specific aspects of health such as pain, disease activity, and physical function and measure specific concepts like physical function and health-related quality of life (HR-QoL).
