Advanced Multimodal Imaging in Granulomatous Uveitis: From Differential Diagnosis to Treatment Monitoring and Surgical Integration.

Background/Objectives: Granulomatous uveitis comprises a clinically heterogeneous group of inflammatory disorders, including ocular sarcoidosis, Vogt-Koyanagi-Harada disease, sympathetic ophthalmia, tuberculosis-associated uveitis, and syphilitic uveitis. Because these entities may share overlapping posterior segment findings, clinical examination alone is often insufficient for differential diagnosis, particularly when choroidal, retinal, or retinal vascular involvement predominates. Methods: This review provides a clinically oriented overview of multimodal imaging in granulomatous uveitis, i

Indocyanine green angiography in uveitis.

Indocyanine green angiography (ICGA) represents an indispensable imaging modality for the precise evaluation and management of posterior uveitis by allowing to explore the choroidal compartment. ICGA leverages near-infrared fluorescence and extensive protein binding to provide an unhindered, dynamic assessment of the choriocapillaris and choroidal stroma. This review explores the principles, standard protocols, and clinical applications of ICGA in posterior uveitis. It details ICGA's critical role in classifying and diagnosing primary inflammatory choriocapillaropathies, like multiple evanesce

Membranous nephropathy and hepatitis secondary to syphilis: a case report and literature review.

Syphilis is a re-emerging infectious disease with diverse clinical manifestations; however, renal and hepatic involvement remains rare and under-recognized. Here, we describe the case of a 58-year-old man who presented with nephrotic syndrome. He had noted bilateral leg edema 1 month prior to presentation. Renal function was preserved; however, serum alkaline phosphatase (ALP) and γ-glutamyl transpeptidase (γ-GT) levels were elevated. Renal biopsy confirmed a diagnosis of membranous nephropathy (MN). In addition to positive results for both the Treponema pallidum antibody and rapid plasma reag

The Great Imitator: Syphilitic Hepatitis Presenting as a Mimic to Autoimmune Liver Disease.

Syphilis is known to mimic other disease presentations, but is seldom documented as mimicking autoimmune hepatitis with positive autoantibodies. We present a case of a 34-year-old male on pre-exposure prophylaxis (PrEP) for HIV with prior negative syphilis screening presenting with epigastric pain and constitutional symptoms. He endorsed anal sex practices with a monogamous male partner. Laboratory testing revealed elevated aminotransferases, alkaline phosphatase, and bilirubin, along with positive smooth muscle antibodies. A rapid plasma reagin and treponemal antibodies were reactive. He was

Treponema pallidum lipoprotein Tp0768 promotes H3K18 Lactylation modification to target PTK2 and enhance endothelial permeability.

Syphilis is a systemic sexually transmitted disease, yet the mechanisms underlying the dissemination of its causative pathogen, Treponema pallidum (T.pallidum), within the host remain incompletely understood. Previous studies have shown that T.pallidum can damage vascular endothelial cells, thereby breaching the endothelial barrier to invade lymph nodes at the infection site. This facilitates its systemic dissemination via the bloodstream, leading to persistent infection across multiple vascular and tissue compartments. However, the precise molecular mechanisms governing this process remain el

Crucial Role of Spectral Domain OCT in the Diagnosis and Follow-up of Acute Syphilitic Posterior Placoid Chorioretinitis (ASPPC) - a Case Series.

ABSTRACT: BACKGROUND: Acute syphilitic posterior placoid chorioretinitis (ASPPC) is a pathognomonic manifestation of ocular syphilis. Although formerly considered as being uncommon, its incidence appears to be increasing in recent years. Advances in multimodal imaging, particularly spectral domain optical coherence tomography (SD-OCT), have provided new insights into its pathophysiology and natural course. ABSTRACT: CASE SERIES: We present three cases of ASPPC with multimodal imaging and long-term follow-up. Clinical history, fundus findings, fluorescein and indocyanine green angiography, fun

Beyond placoid: diverse clinical presentations and functional outcomes of acute syphilitic posterior placoid chorioretinopathy.

OBJECTIVES: Acute syphilitic posterior placoid chorioretinopathy (ASPPC) is traditionally viewed as a uniform presentation of syphilitic posterior uveitis, though recent evidence suggests broader phenotypic variability. This study aimed to characterise ASPPC heterogeneity using multimodal imaging (MMI). METHODS: Single-centre retrospective cohort study on patients presenting with syphilitic posterior uveitis or panuveitis. MMI included fundus autofluorescence (FAF), fluorescein angiography (FA), indocyanine green angiography (ICGA), and optical coherence tomography (OCT). Cases were classifie

Alopecia Areata in a Patient Undergoing Treatment for Secondary Syphilis: A Diagnostic Challenge.

A 17-year-old male with a recent history of incarceration and a diagnosis of secondary syphilis presented for follow-up three months after receiving appropriate antibiotic treatment. His venereal disease research laboratory (VDRL) titers showed a favorable decline (from 1:16 to 1:4); however, he reported new-onset patchy scalp alopecia. Clinically, the lesions raised suspicion for syphilitic alopecia (SA). Nevertheless, trichoscopy revealed hallmark findings of alopecia areata (AA), including exclamation mark hairs and black dots, while lacking specific signs of SA, such as an erythematous bac

Case report: thoracic syphilitic gumma masquerading as a malignant tumor in a patient with AIDS.

Syphilis, a systemic infection caused by Treponema pallidum, can present with atypical and severe manifestations in people living with the human immunodeficiency virus (HIV), posing significant diagnostic challenges. We report the case of a patient with acquired immunodeficiency syndrome (AIDS) who presented with a chest wall mass. Imaging findings were initially suggestive of a malignancy. However, subsequent evaluation, guided by positive serum syphilis antibodies despite non-specific inflammatory changes on histology and a negative metagenomic next-generation sequencing (mNGS) result, led t

Research at the Paris Foundling Hospital, Part 1: Until and during the Revolution.

BACKGROUND: The Paris Foundling Hospital was a breeding ground for neonatal medicine, but little is known about its research. During the century preceding the Revolution, admission of newborns rose dramatically, making this institution the largest infant hospital in the world. A total of 3,162 infants were admitted in 1742, of whom 40% died in the hospice and another 20% with the nurses. During the Revolution (1789-1804), monasteries were transformed into jails or hospitals, and the foundling hospital became a teaching institute. Students flocked to the capital, and those believed to qualify f

Case Report: Secondary syphilitic alopecia following non-penetrative sexual contact: a case of initial misdiagnosis.

We describe a case of secondary syphilitic alopecia following non-penetrative sexual contact that was initially misdiagnosed as alopecia areata. A 26-year-old unmarried male presented with moth-eaten patchy alopecia in the occipital region. Prior topical treatment for presumed alopecia areata at another facility was ineffective. The patient reported repeated episodes of mutual masturbation with a female massage worker two months earlier, and he noticed a small erythematous papule on the penile frenulum thereafter. Serological testing revealed a rapid plasma reagin (RPR) titer of 1:32 and a pos

Solitary ulcer at the mons pubis in a heterosexual male patient.

BACKGROUND: Syphilis is a curable, systemic sexually transmitted infection caused by the bacterium Treponema pallidum subsp. pallidum (TP). It is known as the "great imitator" due to its potential to mimic various dermatoses, possibly delaying correct diagnosis, particularly if syphilitic lesions appear in atypical locations. CASE DESCRIPTION: We report the case of a 54-year-old heterosexual white man who presented with a solitary, indurated ulcer on the mons pubis. Patient history revealed minimal trauma to the area 3 weeks prior, resulting from shaving. Initial topical treatment for bacteri

Treponemal Endovasculitis leading to branch artery occlusion: a rare manifestation of ocular syphilis.

Ocular syphilis, a re-emerging cause of uveitis, commonly features retinal vasculitis, but branch retinal artery occlusion (BRAO) is a rare, irreversible damaging complication. We present a 47-year-old woman with bilateral syphilitic panuveitis, retinal vasculitis and an inferior BRAO, emphasising the diagnostic value of multimodal imaging. She presented with progressive bilateral blurred vision, exhibiting bilateral nongranulomatous anterior uveitis, vitritis, peripheral vascular sheathing, and multiple chorioretinal lesions.
