Bedaquiline pharmacokinetics in drug-resistant tuberculosis: from exposure variability to individualized therapy.

Bedaquiline has transformed the treatment of multidrug- and rifampicin-resistant tuberculosis (MDR/RR-TB) and is now a core component of contemporary all-oral regimens. However, translating its distinctive pharmacokinetic properties into individualized clinical management remains challenging. Bedaquiline exhibits food-enhanced absorption, extensive plasma protein binding, broad but heterogeneous tissue distribution, cytochrome P450 3A4-mediated metabolism to the N-monodesmethyl metabolite M2, and prolonged persistence caused by tissue sequestration and slow redistribution. These features contr

Assessing early therapeutic response in extrapulmonary tuberculosis using [18F]FDG-PET/CT imaging.

INTRODUCTION: Monitoring treatment response in extrapulmonary tuberculosis (EPTB) remains challenging due to limited microbiological accessibility and the lack of validated biomarkers. [18F]FDG positron emission tomography/computed tomography [(18F)FDG-PET/CT] enables assessment of metabolic disease activity and has been explored in infectious diseases, but evidence supporting its use in EPTB is limited. METHODS: We conducted a prospective observational study including adults diagnosed with EPTB at a tertiary referral center in Denmark between 2016 and 2018. Participants underwent baseline an

High-throughput screening to identify inhibitors of the MTB CARD•RNA polymerase protein-protein interaction.

In 2023, tuberculosis (TB) once again became the world's deadliest infectious disease, overtaking COVID-19 as the leading cause of death from a single infectious agent, Mycobacterium tuberculosis (MTB). The World Health Organization (WHO) reports that the COVID-19 pandemic has reversed years of progress in providing essential TB services and reducing TB disease burden. In 2025, TB was responsible for an estimated 1.23 million deaths globally (95% uncertainty interval: 1.13-1.33 million), and there were 400 K cases of rifamycin resistant/multidrug resistant TB (RR, RIFR, MDR). While treatment s

Comprehensive genetic screening of the South African Parkinson's disease study collection using the NeuroBooster array.

INTRODUCTION: Although there have been significant advances in the genetic etiology of Parkinson's disease (PD), most of this research has been conducted in European populations. This study aimed to characterize the genetic landscape of PD in South Africa, whose populations are renowned for their extensive genetic diversity and admixture. METHODS: We recruited 689 PD probands across South Africa. These participants were genotyped using the NeuroBooster array in collaboration with the Global Parkinson's Genetics Program (GP2). An in-house computational workflow was used to identify pathogenic

Integrated transcriptomic, machine learning, and network pharmacology analyses reveal shared inflammatory biomarkers between intervertebral disc degeneration and osteoporosis.

BACKGROUND: Intervertebral disc degeneration (IDD) and osteoporosis (OP) frequently coexist, but their shared inflammatory molecular features remain unclear. We investigated whether the two disorders share an immune-extracellular matrix axis and sought candidate inflammation-associated biomarkers. METHODS: Public transcriptomic datasets GSE124272 and GSE35956 were analyzed separately using differential expression analysis and gene set enrichment analysis. Shared genes were prioritized using LASSO regression, random forest, and support vector machine algorithms, with model performance estimate

Social Determinants of Health and Behavioral Risk Factors in the Tuberculosis-Type 2 Diabetes Syndemic: A Narrative Review.

The co-occurrence of tuberculosis (TB) and type 2 diabetes mellitus (T2DM) represents a growing global syndemic that disproportionately affects populations with limited access to healthcare and preventive resources. T2DM is an established independent risk factor for TB, while social determinants of health (SDoH) further contribute to disease susceptibility, delayed diagnosis, and poor clinical outcomes. This narrative literature review examines the impact of SDoH and modifiable lifestyle factors on the TB-T2DM syndemic and highlights opportunities for intervention. A literature review was cond

Structural determinants of communicable disease risk in asylum reception centres.

INTRODUCTION: With over 120 million people forcibly displaced globally, asylum reception systems represent critical epidemiological interfaces. This synthesis is not confined to a single geographic region: evidence was drawn from reception systems across high-income host countries in Europe and, where available, comparable settings elsewhere, reflecting the substantial heterogeneity of reception models worldwide. Within a One Health framework, these socio-spatial environments shape communicable disease dynamics through accommodation density, sanitation capacity, and governance continuity. ARE

Immune Responsiveness Induced by Administration of Heat-Inactivated M. bovis and BCG Prior to Experimental Infection in African Buffalo.

African buffalo (Syncerus caffer) are the primary wildlife reservoir for bovine tuberculosis (BTB) in Africa. Infected buffalo populations spread BTB to numerous species in conservation areas as well as to cattle at park boundaries. Quarantine requirements for BTB complicate wild animal metapopulation management. Considerable interest has arisen in the vaccination of cattle and indigenous wildlife species against BTB as an additional control strategy. This pilot trial aimed to evaluate cell-mediated and humoral immune responses in African buffalo immunised against BTB. Twenty-one sub-adult Afr

Utilization of the CRISPR-Cas system for investigating tuberculosis drug resistance.

Drug-resistant tuberculosis(TB) presents a significant challenge to global efforts to control the disease. Addressing this issue requires a comprehensive understanding of drug resistance in Mycobacterium tuberculosis (M.tb). The clustered regularly interspaced short palindromic repeat (CRISPR) and CRISPR-associated protein (Cas) system are efficient genome-editing tools for various organisms and have facilitated new research directions regarding drug resistance in M.tb. This review summarizes recent progress in applying CRISPR/Cas technology to investigate drug resistance mechanisms in M.tuber

Melioidosis in Asia: Hidden burden, epidemiological distribution, vaccine and therapeutic prospects, and public health priorities.

Melioidosis is an under-recognized tropical infectious disease caused by Burkholderia pseudomallei, an environmental pathogen that persists mainly in soil and surface water across tropical and subtropical regions. In Asia, the burden of melioidosis remains poorly defined because many cases are underdiagnosed, underreported, or clinically misclassified as tuberculosis, pneumonia, sepsis, or other bacterial infections. This narrative review examines the epidemiological distribution, transmission pathways, risk factors, vaccine and therapeutic developments, public health challenges, and future co

Aptamer-CRISPR/Cas as a tool in point-of-care testing of infectious diseases.

Aptamers and the CRISPR/Cas system have become the cornerstones of target detections due to their numerous advantages. Aptamers enable direct recognition of non-nucleic acid targets, such as pathogens and antigens, while CRISPR/Cas system drives signal amplification and rapid detection. The combined aptamer-CRISPR/Cas system can overcome the limitations of traditional target detection by realizing field-deployable, highly sensitive, specific, versatile, and rapid assays. Given the increasing number of different aptamer-CRISPR/Cas systems developed recently, particularly for infectious diseases

A quasi-experimental study evaluating the effectiveness of a male-specific tuberculosis screening and diagnosis intervention in Uganda (IGNITE).

BACKGROUND: Despite men having a higher burden of TB, they are frequently missed by TB services due to social, behavioural, and health system factors that limit healthcare access. In Uganda, where there are no gender-specific guidelines for TB, we implemented and evaluated a gender-specific intervention to find and treat men with TB. METHODS: We conducted a quasi-experimental study comparing TB notifications before (January-June 2023) and after (July-December 2023) the intervention at both intervention (Gombe and Mityana hospitals) and control (Luwero and Kiboga hospitals) sites. The interven

Antimicrobial activity of twelve plant-derived compounds against fish-associated non-tuberculous mycobacteria.

Nontuberculous mycobacteria (NTM) are important pathogens of fish, causing chronic infections that are difficult to control in aquaculture. In this study, we evaluated the in vitro antimicrobial activity of twelve plant-derived compounds against six fish-associated NTM strains using a broth microdilution assay. Minimum inhibitory concentrations (MICs) ranged from 0.25 to >32 µg/mL. Our results demonstrated that cinnamaldehyde exhibited the strongest antimycobacterial effect, followed by sesamol and farnesol, while several other phytochemicals showed moderate activity and salicylic acid was ina

The Potential Role of Gut Microbiota in Combating Mycobacterium tuberculosis Infection.

Tuberculosis (TB) is a chronic infectious disease caused by Mycobacterium tuberculosis (M.TB) infection. Although there have been significant advances in the treatment of M.TB infection, it still faces significant challenges. The gut microbiota is a highly complex ecosystem that plays an important role in regulating metabolism, immune regulation, and maintaining healthy balance in the human body. More and more studies have found that while gut microbiota regulates the immune regulation process of pulmonary TB through the "gut-lung axis," infection with M.TB can also affect the microbial stabil
