Hepatocellular- cholangiocarcinoma collision tumor: a case report.

Hepatocellular-cholangiocarcinoma collision tumor is an extremely rare primary liver malignancy, defined as histologically distinct hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (ICC) growing adjacently with clear demarcation, which is easily misdiagnosed due to overlapping features with combined hepatocellular-cholangiocarcinoma and metastatic liver cancer, and lacks standardized diagnostic and treatment evidence. We report a 54-year-old male with 15-year chronic hepatitis B infection admitted in June 2024 with fatigue, poor appetite and abdominal distension. Preoperative

Effect of Hepatitis B virus X T118N mutant protein in HepG2 cell cycle and apoptosis regulation.

BACKGROUND: Hepatitis B virus (HBV) belongs to Hepadnaviridae family and caused liver infection. HBV infection can develop into hepatocellular carcinoma, one of which is due to presence of HBV X protein (HBx). Some mutations are known to be associated with development of liver carcinoma, for instance HBx K130M/V131I (HBx_KV). In Indonesia, HBx T118N (HBx_T) has been found at similar frequency as the HBx_KV. This study aims to examine the effect of HBx T118N mutant on cell cycle and apoptosis regulation. METHODS AND RESULTS: In this study, analysis of CyclinE, p21, Bax, and Bcl-2 gene expressi

Hepatic Lipid Stress Drives Metabolic Dysfunction-Associated Steatohepatitis Progression through FTO O-GlcNAcylation-Induced m6A Demethylation of HRAS.

Metabolic dysfunction-associated steatohepatitis (MASH) is driven by nutrient stress through incompletely defined epigenetic mechanisms. This study demonstrates that lipid stress activates the hexosamine biosynthesis pathway (HBP), elevating O-linked β-N-acetylglucosamine (O-GlcNAc) modification of the N6-methyladenosine (m6A) demethylase fat mass and obesity-associated protein (FTO) at Ser195. This post-translational modification recruits the deubiquitinase ubiquitin-specific peptidase 20 (USP20), displaces the E3 ligase F-box and WD repeat domain containing 7 (FBXW7), and enhances FTO protei

Chronic alanine aminotransferase elevation in people with HIV: Time trends over 22 years and genetic predisposition.

INTRODUCTION: Chronic elevation of alanine aminotransferase (cALT) is common in people with HIV. Published reports on cALT prevalence time trends have not been uniform. It is unknown whether genetic background is associated with cALT in people with HIV. METHODS: cALT was defined as ALT levels (male, >50 U/L, female, >35 U/L) at ≥2 consecutive, 6-monthly per protocol Swiss HIV Cohort Study (SHCS) visits. We calculated cALT prevalence plus trends in time-updated clinical and HIV-related cALT risk factors per calendar year in SHCS participants without viral hepatitis co-infection. In cALT cases

Macrophage-derived IL-27 sustains autoreactive CD8 + cytotoxic T cells in autoimmune hepatitis.

Chronic or persistent T cell activation is widely thought to culminate in T cell exhaustion, yet how autoreactive CD8⁺ T cells remain functionally intact despite sustained self-antigen exposure is poorly understood. In chronic infection and cancer this durability is attributed to stem-like, TCF1⁺ PD-1⁺ progenitor-exhausted (Tpex) cells that reside in lymphoid tissue and continuously replenish functional effectors; whether such a reservoir operates within a chronically inflamed peripheral organ during sterile autoimmunity is unknown. Here we show that mice lacking the lysosomal nuclease DNase2a

GPR82-mediated activation of TGF-β/SMAD signaling contributes to CD8+ T cell dysfunction in HBV-ACLF.

Patients with acute-on-chronic liver failure (ACLF) have severe CD8+ T cell dysfunction. This study examined G protein-coupled receptor 82 (GPR82) in CD8+ T cell exhaustion in hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). Using GEO analysis, clinical samples, GPR82 manipulation, transforming growth factor beta (TGF-β)/SMAD intervention, dual-luciferase assays, and a mouse hepatitis virus strain 3 (MHV-3) mouse model, we found GPR82 overexpression in ACLF liver and HBV-ACLF CD8+ T cells, correlating with T cell exhaustion. GPR82 knockdown enhanced CD8+ T cell function, wh

4-Diketopyrrole-2-carboxamide derivatives affecting HBV capsid assembly: structure-activity relationship around the GLP-26 scaffold.

Chronic hepatitis B virus (HBV) infection remains a major global health burden with limited curative options. Capsid assembly modulators (CAMs) have emerged as a promising antiviral strategy by disrupting nucleocapsid formation and suppressing cccDNA amplification. In this study, we discuss the structure-activity relationship (SAR) around the 4-diketopyrrole-2-carboxamide scaffold of GLP-26, focusing on modifying the aniline moiety and the oxoacetamide region, and introducing alternative acetamide groups. Compounds were synthesized through efficient coupling strategies, including microwave-ass

Adjuvant pegylated interferon-alpha after nucleos(t)ide analog withdrawal drives a temporally coordinated expansion of regulatory B cells and contraction of HBV-specific T cells.

BACKGROUND AND AIMS: The NUC-B study showed that a 4-month course of pegylated interferon-alpha (PEG-IFNα) enhanced the outcome and reduced hepatic flares following nucleos(t)ide analog (NUC) withdrawal in HBeAg-negative chronic hepatitis B. Here, we investigated differential T- and regulatory B-cell (BREG) changes before and during these interventions in a substudy of 41 representative patients. APPROACH AND RESULTS: Antigen-specific T cells were primarily evaluated using quality-controlled IFN-γ ELISpot with 15 pools of genotype-matched HBV core/small-surface peptides, confirmed by multipar

Microbial cues and T cell immunity: deciphering their interactions in chronic liver disease.

Chronic liver disease (CLD) poses a major global health challenge, as its progression to cirrhosis and hepatocellular carcinoma significantly contributes to worldwide morbidity and mortality. Mounting evidence has identified the gut microbiota as a critical modulator of CLD progression, primarily via the gut-liver axis. The liver is continuously exposed to gut-derived signals through the portal circulation, while reciprocally secreting bioactive mediators that shape intestinal microbial composition and function. This bidirectional network enables intricate crosstalk between the gut microbiota

Low-level of mitochondrial DNA alterations during direct-acting antiviral treatment in leukocytes of people who inject drugs living with HCV.

UNLABELLED: Direct-acting antivirals (DAAs) have revolutionized hepatitis C virus (HCV) treatment, offering high cure rates and favorable safety profiles. However, concerns remain regarding their long-term mitochondrial safety, particularly among vulnerable populations such as people who inject drugs (PWID), who are frequently exposed to multiple mitotoxic agents. This longitudinal study investigated mitochondrial DNA (mtDNA) instability, including deletions and single-nucleotide variants, among PWID living with HCV during DAA treatment, using whole mitochondrial genome sequencing. We analyzed

Impact of histologic hepatic steatosis on virological response to nucleos(t)ide analogue therapy in patients with chronic hepatitis B.

Chronic hepatitis B remains a major cause of cirrhosis and hepatocellular carcinoma worldwide. We investigated whether histologic hepatic steatosis affects hepatitis B virus deoxyribonucleic acid (HBV DNA) suppression in patients receiving antiviral therapy. This retrospective observational study included 307 adult patients with chronic hepatitis B who received tenofovir or entecavir therapy and underwent pretreatment liver biopsy. Demographic, laboratory, virological, radiological, and histopathological data were retrieved from the electronic medical record system. Laboratory parameters inclu
