Electrochemical immunosensors for KIM-1 and NGAL: recent advances toward early and point-of-care diagnosis of acute kidney injury.

Early and reliable detection of acute kidney injury (AKI) still remains an important challenge at the clinic. Kidney injury molecule-1 (KIM-1) and neutrophil gelatinase-associated lipocalin (NGAL), which can be measured in both serum and urine samples, are among the most promising biomarkers for the diagnosis and monitoring of AKI. In the present review, we covered electrochemical immunosensors developed for the analysis of these two biomarker proteins of interest for the diagnosis of acute kidney injury and some other related /associated conditions. We compared these sensors with respect to m

Post-translational control of synaptic adhesion molecules: molecular mechanisms and disease implications.

BACKGROUND AND AIMS: Prospective studies on the simultaneous effects of multiple determinants on objectively assessed mobility are few. The aim of this study was to analyse mobility performance, its stability and sensory, psychomotor and musculoskeletal determinants in an older population from age 75 to age 80. METHODS: Sixty-three men and 121 women aged 75 participated at baseline and, five years later, in the follow-up phase of this population-based prospective study. Maximal walking speed and step-mounting height were assessed at baseline and follow-up. Maximal isometric knee extension str

AI portal tract detection and characterisation for a regional analysis of steatosis and inflammation in MASLD, MASH and AIH.

AIMS: Annotation of liver biopsies for disease staging is increasingly aided by digital pathology; however, existing systems do not quantify inflammation and steatosis within an anatomical framework. We hypothesise that an artificial intelligence (AI) system that quantifies portal tracts (PT) and the anatomical distribution of steatotic vesicles and inflammatory cells will align with manual pathologist scoring and stratify liver diseases. METHODS: In this observational, cross-sectional study, digitised images of haematoxylin and eosin-stained specimens were pooled from four independent cohort

Serum Markers of Fibroinflammatory Activity PRO-C3 and CPa9-HNE Are Associated with Liver-Related Outcomes in Patients with Chronic Hepatitis C.

BACKGROUND: Chronic hepatitis C (CHC) infection remains a global health burden, contributing to the risk of long-term complications such as progressive fibrosis, cirrhosis, and hepatocellular carcinoma, particularly in undiagnosed or untreated individuals. Novel biomarkers for the assessment of disease progression and prognosis are required for fibroinflammatory chronic liver disease. Our study population from the Hepatitis C Antiviral Long Term Treatment Against Cirrhosis (HALT-C) trial was used to investigate the association of neoepitope fibroinflammatory biomarkers with clinical outcomes i

Metabolic Dysfunction-Associated Steatotic Liver Disease in Adults: A Review.

IMPORTANCE: Metabolic dysfunction-associated steatotic liver disease (MASLD) includes a range of liver conditions, progressing from isolated steatosis (characterized by fat accumulation in the liver without inflammation) to metabolic dysfunction-associated steatohepatitis (MASH), which involves fat accumulation and inflammation in the liver. The presence of MASLD is associated with increased morbidity and mortality due to liver-related complications, hepatocellular carcinoma, cardiovascular disease, and certain extrahepatic cancers. OBSERVATIONS: The most common chronic liver disease worldwid

Hepatic Manifestations in Systemic Juvenile Idiopathic Arthritis and Macrophage Activation Syndrome.

OBJECTIVE: Systemic juvenile idiopathic arthritis (sJIA) is a chronic inflammatory disease characterized by systemic features and arthritis. Macrophage activation syndrome (MAS) is a severe complication of sJIA often involving the liver. MAS confined predominantly to the liver, causing severe hepatitis, has been increasingly recognized. When liver MAS is the primary manifestation, significant hepatic injury can occur; therefore, differentiation from other forms of sJIA-related liver involvement, which may warrant distinct treatment approaches, is required. This study examined liver pathology i

Risk of Burdensome Health Care Spending Over Time in the US.

IMPORTANCE: Cross-sectional studies indicate that out-of-pocket (OOP) health care costs strain household finances, but few studies have examined the risk of incurring burdensome health care spending longitudinally among individuals in the US. OBJECTIVE: To assess burdensome OOP health care costs (and care foregone due to cost) over a 4-year period. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, data were analyzed on respondents to the 4-year longitudinal Medical Expenditure Panel Surveys (MEPS), a nationally representative household survey of the US noninstitutionalized population

Targeting T cell exhaustion in colorectal cancer: emerging roles of LAG-3, TIM-3, and TIGIT signaling in overcoming immunotherapy resistance.

Exhaustion of T cells, characterized by their compromised function and cytokine production, was first discovered in chronic infections and later redefined in the context of immunosuppressed tumor microenvironment (TME). Exhaustion markers include the immune checkpoints PD-1, TIM-3, LAG-3, and TIGIT. Colorectal cancer (CRC), which ranks among the highest in global prevalence, has been associated with exhaustion of T cells. While many trials have focused on anti-PD1 therapeutics in clinical trials, results indicate suboptimal efficacy. A robust approach involved dual blockade of other immune che

Silybin for Liver Diseases via Anti-Oxidative Stress: A Comprehensive Review.

Liver diseases pose a significant challenge in global public health, and scientific prevention and treatment strategies have become particularly crucial. Silybin, a flavonoid and active ingredient extracted from the traditional Chinese medicinal herb milk thistle, is the most critical effective component in silymarin, and has clearly demonstrated potent anti-oxidative stress capabilities. This unique attribute makes it a highly promising drug candidate for treating acute hepatitis, chronic hepatitis, cirrhosis, liver cancer, and other liver diseases. In the progression of liver diseases, silyb

Branched-chain amino acids for people with cirrhosis and hepatic encephalopathy.

RATIONALE: Hepatic encephalopathy is a brain dysfunction characterised by neurological and psychiatric changes due to liver insufficiency or portal-systemic shunting. The severity ranges from minor symptoms to coma. The previous version of this updated review meta-analysed data from 16 randomised clinical trials on branched-chain amino acids (BCAAs) versus control interventions, and found that BCAAs did not affect mortality but had a beneficial effect on hepatic encephalopathy. As data on critical outcomes were insufficient and new trials were published, we updated the review again. OBJECTIVE

Cost-Effectiveness of One-Time Universal Childhood Hepatitis C Screening in the United States.

BACKGROUND: Hepatitis C Virus (HCV) in pregnancy has increased, leading to increased perinatally exposed infants. Although universal HCV screening in pregnancy is recommended, pediatric cases remain undiagnosed. We examine the cost-effectiveness of universal HCV screening among children at age 2 and 10, when other routine blood testing is recommended. METHODS: An HCV natural history Markov model evaluated the cost-effectiveness of universal HCV screening independently at ages 2 and 10 compared to the currently recommended risk-based screening of children born to those with HCV. Based on previ

Overcoming barriers to HCV screening in Latin America: From evidence to action.

Chronic hepatitis C virus (HCV) infection remains a major global public health challenge. Despite the availability of highly effective therapies capable of curing the vast majority of patients, millions remain undiagnosed and often present for medical care at advanced stages of disease. This delay not only increases mortality and the burden of cirrhosis and hepatocellular carcinoma but also affects quality of life, productivity, and healthcare system costs. In this context, screening emerges as a cornerstone for achieving HCV elimination. It enables the identification of hidden cases, early tr

Impact of cardiometabolic risk factors on hepatocellular carcinoma incidence in patients with chronic hepatitis B: A retrospective cohort study.

BACKGROUND AND AIMS: Chronic hepatitis B virus (HBV) infection remains a major global health burden and a leading cause of hepatocellular carcinoma (HCC). While cirrhosis is a well-established risk factor, the contributions of metabolic dysfunction-associated steatotic liver disease (MASLD) and cardiometabolic risk factors (CMRFs) are less clearly defined. This study aimed to evaluate the impact of MASLD and CMRFs on HCC risk in patients with chronic hepatitis B (CHB). METHODS: We conducted a retrospective cohort study of CHB patients at Songklanagarind Hospital between 2011 and 2021, excludi
