Noncirrhotic Portopulmonary Hypertension Due to Hepatoportal Sclerosis in Adams-Oliver Syndrome.

A 13-year-old girl presented for evaluation of pulmonary hypertension after symptoms of dyspnea and exercise intolerance. Full evaluation was negative except for abdominal ultrasonography with splenomegaly and esophageal varices suggestive of portal hypertension. Cardiac and hepatic vein catheterization confirmed portopulmonary hypertension. Liver biopsy demonstrated sinusoidal dilatation, nodularity, and minimal fibrosis, which was interpreted as possible nodular regenerative hyperplasia but not cirrhosis. Radiographic imaging, including computed tomography venography, demonstrated an elongat

Autotaxin as a potential biomarker in hepatic encephalopathy: Insights from a mouse model and cirrhotic patients.

Hepatic encephalopathy (HE) is a debilitating neuropsychiatric condition associated with acute liver failure or advanced liver diseases, significantly affecting patient morbidity and mortality. Autotaxin (ATX), a lysophospholipase D that generates lysophosphatidic acid (LPA), has been proposed as a biomarker for liver fibrosis progression, but its role in HE remains poorly defined. This study aimed to explore the potential roles of ATX using in vivo TAA-induced HE models in mouse and clinical evaluations of cirrhotic patients with and without HE. ATX concentrations were quantified in plasma, l

Comorbidities in multiple sclerosis: acquired hepatocerebral degeneration mimicking disease progression.

A 63-year-old patient with relapsing-remitting multiple sclerosis (MS) and comorbid liver cirrhosis presented with progressive cognitive decline, gait disturbance and elevated serum neurofilament light chain (NfL), suggesting disease progression independent of relapse activity (PIRA). Development of acute hepatic encephalopathy led to reconsideration of the cause of previous clinical worsening and MRI revealed progressive basal ganglia manganese accumulation, establishing the diagnosis of acquired hepatocerebral degeneration, retrospectively challenging the PIRA diagnosis. Careful evaluation o

Protein phosphatase 6 regulates metabolic dysfunction-associated steatohepatitis via the mTORC1 pathway.

BACKGROUND & AIMS: Metabolic dysfunction-associated steatohepatitis (MASH) is a serious chronic liver disease for which therapeutic options are limited. Although fibroblast growth factor 21 (FGF21) analogs have shown therapeutic promise for MASH in multiple preclinical and clinical studies, their underlying mechanisms of action remain elusive. METHODS: Liver-specific PPP6C and βKlotho knockout mice and their wild-type littermates were fed an AMLN (Amylin liver NASH) diet for 16 weeks or a CDA-HFD (choline-deficient, L-amino acid-defined, high-fat diet) for 8 weeks, followed by daily subcutane

NADPH Oxidase: A Potential Therapeutic Target to Reduce Primary Sclerosis Cholangitis Following Liver Transplantation.

The molecular mechanisms and causes of primary sclerosis cholangitis (PSC) post-liver transplantation are still unclear. PSC is a progressive cholestatic hepatobiliary disease that happens in about 25% of patients post-liver transplantation and requires re-- transplantation. Nicotinamide adenine dinucleotide phosphate oxidase (NADPH oxidase or Nox) is a family of transmembrane proteins whose main function is producing reactive oxygen species (ROS). ROS generation as a result of NADPH oxidase activity of Kupffer cells and polymorphonuclear leukocytes has been implicated in the pathogenesis of i

Clinical outcomes in patients with autoimmune hepatitis and primary biliary cholangitis overlap syndrome in the United States.

BACKGROUND: There is evidence of overlap between autoimmune hepatitis (AIH) and primary biliary cholangitis (PBC), two autoimmune hepatobiliary diseases. This retrospective analysis aimed to investigate extra-hepatic auto-immune diseases and compare outcomes of hospitalized patients with AIH-PBC overlap with AIH and PBC. We hypothesize that there are differences in rates of major adverse liver outcomes (MALO's) when comparing these groups. METHODS: We conducted a retrospective cohort study using National Inpatient Sample 2016-2020. Patients with AIH, PBC and AIH-PBC were identified using resp

Ginsenoside RK1 Promotes Hepatic Stellate Cell Glycolysis-Mediated Ferroptosis by Activating the HK2/ACSL4/LPCAT3/ALOX5 Signaling Pathway.

Liver fibrosis (LF) is a pathological condition that, if not controlled, can progress into liver sclerosis and malignant tumors. Ferroptosis has been comprehensively studied and found to improve LF by exacerbating hepatic cell ferroptosis. Ginsenoside RK1 (GRK1) is an essential component of ginseng with antifibrotic effects. However, the anti-LF mechanism of GRK1 remains elusive. The impact of GRK1 on LF was evaluated via RNA-sequence analysis of GRK1-treated hepatic stellate cells (HSC). Furthermore, a cellular thermal shift assay, drug affinity-responsive target stability, and molecular dock

Follicular helper T cells and fibrotic diseases.

T follicular helper (Tfh) cells constitute a functionally specialized subset of CD4+ T lymphocytes that orchestrate germinal center (GC) responses, critically regulating B cell destination and the development of long-term humoral immunity. Emerging evidence implicates Tfh cells as key mediators in the pathogenesis of multiple fibrotic disorders across multiple organ systems, including systemic sclerosis (SSc), hepatic fibrosis, chronic kidney disease, idiopathic pulmonary fibrosis (IPF) and IgG4-Related Disease (IgG4-RD). This review synthesizes current understanding of Tfh cell biology in fib

Network Modeling of Biomarker Systems in Liver Steatosis and Fibrosis.

Metabolic dysfunction-associated fatty liver disease causes hepatic fat accumulation (steatosis) and fibrosis, which can be measured with ultrasound elastography imaging. The dependencies of elastography-derived hepatic steatosis and fibrosis measures with chronic inflammation, disease states, and physiological determinants of drug dosing (PDODD) were assessed. Liver elastography data for n = 5494 participants (50% female, 12-80 years) were obtained from the National Health and Nutrition Examination Survey. Controlled attenuation parameter (CAP) and median liver stiffness (LSM) elastography me

Matrix mechanical remodeled carrier-free nanosystem for programmable closed-loop reversal of liver fibrosis via STING alkylation.

Extracellular matrix (ECM) sclerosis represents a prominent feature of fibrotic disorders; however, the macrophage response to changes in matrix stiffness and its impact on fibrotic diseases remain unclear. This study reveals a vicious circle of ECM-cell-ECM, where increased ECM hardness activates the STING pathway in macrophages, in turn activates hepatic stellate cells (HSCs), thus enhancing ECM stiffness again and exacerbating liver fibrosis. To reverse liver fibrosis, an innovative carrier-free nanosystem capable of degrading ECM, specifically blocking the STING pathway in macrophages as w

Anti-centromere antibody positivity with coexisting idiopathic portal hypertension and primary biliary cholangitis progressing to limited cutaneous systemic sclerosis: a case report.

BACKGROUND: Anti-centromere antibodies are autoantibodies that selectively bind to the centromere region of chromosomes. Studies have indicated that anti-centromere antibodies can induce microvascular alterations and tissue remodeling, ultimately leading to fibrosis. They have been implicated in limited cutaneous systemic sclerosis, including calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia syndrome, and primary biliary cholangitis, where anti-centromere antibody positivity can be associated with rapid progression of portal hypertension, although the
